Pinch-Off Syndrome, a Rare Complication of Totally Implantable Venous Access Device Implantation: A Case Series and Literature Review

  • Background: Pinch-off syndrome (POS) is a rare complication after totally implantable venous access device (TIVAD) implantation. In cancer patients, it is important to prevent this rare complication and to recognize it early if it does occur. We present a case series of POS after TIVAD implantation and the results of a literature search about this complication. Methods: From July 2006 to December 2015, 924 permanent implantable central venous catheter implantation procedures were performed. The most common indication was vascular access for chemotherapy. Results: POS occurred in 5 patients in our clinic. Two patients experienced POS within 2 weeks, and the other 3 patients were admitted to department of surgery, Istanbul Faculty of Medicine at 6 to 14 months following implantation. The catheters were found to be occluded during medication administration, and all patients complained of serious pain. The transected fragments of the catheters had migrated to the heart. They were successfully removed under angiography with a single-loop snare. Conclusion: POS is a serious complication after TIVAD implantation. It is important to be aware of this possibility and to make an early diagnosis in order to prevent complications such as drug extravasation and occlusion events.

만성 염증성 폐질환의 전폐적축술 후 합병증에 영향을 미치는 요인 (Factors Affecting Postoperative Complication in Pneumonectomy for Chronic Complicated Inflammatory Lung Disease)

  • Background: this study was designed to estimate whether specific risk factors could increase the postoperative complication rate of pneumonectomy for chronic complicated inf-lammatory lung disease. Material and Method: Eighty-five patients underwent pneumon-ectomy for chronic complicated inflammatory lung disease(tuberculosis, 67 ; bronchiecasis 11; aspergio- losis, 4; others, 3) between January 1991 and August 1998. We performed a univariated statistical analysis to identify preoperative and intraoperative risk factors associated with postoperative complications, Result: There was no operative mortality. There were a total of 18 postoperative complications(22.2%) Bronchopleural fistula(BPF) and empyema occurred in 5(5.9%) and 2(2.4%) respectively. General complication rate was significantly higher in patients with right-sided pneumonectomy(p=.029) extrapleural pneu-monectomy(p=.009) and intraoperative pleural spillage due to cavity or lesion perforation (p=.004). The prevalence of BPF and empyema was higher in patients with right sided pneumonectomy(p=.007) extrapleural pneumonectomy(p=.015) and intraoperative pl- eural spillage due to cavity or lesion perforation(p=.003) which is as the same results as gen-eral complication rate. Conclusion: The postoperative complication rate of pneumone-ctomy for chronic complicated lung disease is accptably low. But it is increase in patients with right sided pneumonectomy extrapleural pneumonectomy and intraoperative pleural spillage due to cavity or lesion perforation. therefore more careful and meticulous intra-operatve management are needed in right sided extrapleural pneumonectomy without intra- pleural spillage.

  • A vertical skin incision with median sternotomy for open heart surgery usually leaves an unsightly hypertrophic scar, and it makes cosmetic and psychic problems especially in young female patients. Since November 1986, we have used a bilateral submammary skin incision with dissection of a fasciocutaneous flap in 5 young female patients with uncomplicated acyanotic congenital heart disease. The exposure of the operative field was excellent. And we had good results without any significant complication associated with this type of incision.

폐결핵(肺結核)의 외과적(外科的) 요법(療法) (Surgery on Pulmonary Tuberculosis: A Review of 19 years's Experience)

  • The author reviewed 263 cases of pulmonary tuberculosis operated on at the Department of Thoracic Surgery, Seoul National University Hospital during the 19 year period, from January, 1957 to December 1975. Following observations were made: 1) The sex ratio was 2.8: 1 in male's favor, and 83.6% of the patients were between 20 and 49 years of age. 2) The trends in operative procedures revealed that surgical collapse as a definitive therapy of pulmonary tuberculosis has virtually disappeared and operations required for residuals of pleural diseases have increased. 3) The overall hospital mortality rate was 3-8%. The highest mortality rate was encountered in the cases of resection and decortication with 28.6%, 4) The rate of overall postoperative complications was 24.7%, the highest being encountered in decortication with 56.4%. 5) The most frequent cause of death was bleeding, while the dead space was the most frequent complication.

  • Rib fracture due to intense pain, may restrict patients from inadequate coughing. These conditions may produce varying degrees of complications such as atelectasis, pneumonia and arterial hypoxemia. Thoracic epidural analgesia has been used to treat pain associated multiple rib fractures because of its marked improvement in vital capacity and dynamic lung compliance. However, there are complications related to thoracic epidural analgesia which may include damage to spinal cord, perforation of dura, respiratory depression, decrease heart rate and arterial blood pressure. We experienced such a case of cardiac arrest during thoracic epidural analgesia while treating a patient for multiple rib fractures.

Emergency Quadrido-Bentall Procedure for Aortic Rupture in a Patient with Behcet's Disease

  • Cardiovascular involvement in cases of Behcet's disease is a rare but life-threatening condition, and prosthetic valve detachment is a frequent and serious complication attributable to Behcet's disease following the surgical repair of aortic regurgitation. We report the case of a patient with Behcet's disease presenting with contained aortic rupture around the aortic root. The patient had previously undergone aortic valve surgery three times due to recurrent prosthetic valve detachment. An emergency operation was performed, consisting of aortic root replacement (ARR) using a composite valved conduit and the replacement of the hemiarch. ARR may be an appropriate surgical option for patients with Behcet's disease in order to prevent recurrence of the disease.

Aortopulmonary Fistula Presenting without an Endoleak after Thoracic Endovascular Aortic Repair

  • Herein, we report the case of a 60-year-old man, a smoker with a history of arterial hypertension and diabetes mellitus. After computed tomography (CT) for an episode of hemoptysis, the patient underwent elective thoracic endovascular aortic repair (TEVAR) because of a degenerative aneurysm of the descending thoracic aorta. The area of perianeurysmal pulmonary atelectasis reported on the CT scan was not considered. Three months later, he developed an aortopulmonary fistula without endoleaks. Although TEVAR is a relatively safe procedure, no detail should be overlooked in the preoperative evaluation in order to avoid life-threatening complications. Further, the effectiveness and modality of prolonged antibiotic prophylaxis and/or preoperative respiratory physiotherapy should be assessed in such cases.

  • We studied blood salvage effect of Cell Saver Apparatus[CSA with adult cardiac patients. This study compares blood salvage in two groups of patients undergoing valve replacement or congenital cardiac operation.Group 1 is composed with 60 patients operated during 1991-1992 using CSA. Group 2 is composed with 60 patients operated during 1990-1991. No special effort was made to salvage blood in Group 2. The average requirements in Group 1 were approximately one-fourth the amount of RBC products by Group 2. The hospital course, blood chemistry level were comparable for the two groups. CSA produced blood was concentrated, well oxygenated blood and the average amount was 830 ml. Any complication was not detected in transfusion of CSA produced blood. This study shows that CSA allows safe and practical blood salvage during adult cardiac operations.

  • 중심정맥 삽관은 술기의 안정성과 효용성으로 인해 날로 증가하는 추세이나 그에 의한 합병증도 빈발하게 나타난다. 본 증례는 혈관 천공 등의 여타 합병증 없이 당뇨와 천식을 가진 74세 남자 환자에서 중심정맥 삽관 후 발생한 종격동염을 수술을 통해 성공적으로 치료한 예이다.

Acquired Pharyngeal Diverticulum after Anterior Cervical Fusion Operation Misdiagnosed as Typical Zenker Diverticulum

  • A pharyngeal diverticulum is a rare complication of an anterior cervical discectomy and fusion (ACDF). We present a case of a pharyngeal diverticulum after an ACDF, which was misdiagnosed as a typical Zenker diverticulum. A 54-year-old woman presented with dysphagia and a sense of irritation in the neck following C5 through C7 cervical fusion 3 years prior. The patient underwent open surgery to resect the diverticulum with concurrent cricopharyngeal myotomy. An ACDF-related diverticulum is difficult to distinguish from a typical Zenker diverticulum.
