제V급 복합레진 수복물의 표면전색이 미세변연누출에 미치는 효과 (The Effect of Surface Sealing on the Microleakage of Class V Composite Resin Restorations)

  • 본 연구는 다양한 순환 및 시효 조건 하에서 제V급 복합레진 수복물의 표면전색이 미세변연누출에 미치는 효과에 대해 알아보고자 하였다. 100개의 건전한 소구치의 협면에 제V급 와동을 형성하고 제조사의 지시대로 광중합형 복합레진으로 수복하였다. 무작위로 나누어 표면전색을 시행하지 않는 I군과 표면을 산부식 후 표면전색을 시행한 II군으로 설정한 후 열순환(thermocycling)을 하였다. 각 군에서 하위군(A군= 열순환 외 다른 시효조건이 없는 군, B군=칫솔질군, C군=부하순환군, D군=칫솔질과 부하순환을 모두 시행한 군, E군=6개월간 증류수에 보관한 군)으로 나누어 각각에 맞는 시효조건을 시행하였다. 2% methylene blue 용액에 침윤시켜 변연의 미세누출정도를 평가하여 다음과 같은 결과를 얻었다. 1. 표면전색을 시행하지 않은 군의 교합변연과 치은변연에서 시효조건에 따른 미세누출의 유의한 차이는 보이지 않았다(p>0.05). 2. 표면전색을 시행한 군의 교합변연에서는 시효조건에 따른 미세누출의 유의한 차이가 없었다(p>0.05). 3. 표면전색을 시행한 군의 치은변연에서는 부하순환이나 6개월간 증류수에서 보관하는 시효과정을 거친 군에서 미세누출이 유의하게 크게 나타났다(p<0.05). 4. 시효조건이 없는 군과 칫솔질군의 경우, 표면전색을 시행한 군이 표면전색을 시행하지 않은 군에 비해 유의하게 적은 미세누출결과를 보였다(p<0.05). 5. 부하순환을 시행한 군과 6개월간 증류수에서 보관한 경우에는 표면전색을 시행한 군과 표면전색을 시행하지 않은 군 사이에 미세누출의 차이가 있었지만 유의차가 없었다(p>0.05). 따라서 이 연구의 결과는 변연부분의 미세간격으로 침투한 표면전색제가 처음에는 미세누출을 감소하는데 효과를 보여주었으나, 교합시 교두가 휘어져서 생기는 응력과 장기간의 수분흡수로 인한 변화를 견디기에 불충분함을 보여주었다.

Influence of nano-structured alumina coating on shear bond strength between Y-TZP ceramic and various dual-cured resin cements

  • PURPOSE. The purpose of this study was to evaluate the effect of nano-structured alumina surface coating on shear bond strength between Y-TZP ceramic and various dual-cured resin cements. MATERIALS AND METHODS. A total of 90 disk-shaped zirconia specimens (HASS CO., Gangneung, Korea) were divided into three groups by surface treatment method: (1) airborne particle abrasion, (2) tribochemicalsilica coating, and (3) nano-structured alumina coating. Each group was categorized into three subgroups of ten specimens and bonded with three different types of dual-cured resin cements. After thermocycling, shear bond strength was measured and failure modes were observed through FE-SEM. Two-way ANOVA and the Tukey's HSD test were performed to determine the effects of surface treatment method and type of cement on bond strength (P<.05). To confirm the correlation of surface treatment and failure mode, the Chi-square test was used. RESULTS. Groups treated with the nano-structured alumina coating showed significantly higher shear bond strength compared to other groups treated with airborne particle abrasion or tribochemical silica coating. Clearfil SA Luting showed a significantly higher shear bond strength compared to RelyX ARC and RelyX Unicem. The cohesive failure mode was observed to be dominant in the groups treated with nano-structured alumina coating, while the adhesive failure mode was prevalent in the groups treated with either airborne particle abrasion or tribochemical silica coating. CONCLUSION. Nano-structured alumina coating is an effective zirconia surface treatment method for enhancing the bond strength between Y-TZP ceramic and various dual-cured resin cements.

The effect of dentin desensitizers and Nd:YAG laser pre-treatment on microtensile bond strength of self-adhesive resin cement to dentin

  • PURPOSE. The purpose of this study is to evaluate if pre-treatment with desensitizers have a negative effect on microtensile bond strength before cementing a restoration using recently introduced self-adhesive resin cement to dentin. MATERIALS AND METHODS. Thirty-five human molars' occlusal surfaces were ground to expose dentin; and were randomly grouped as (n=5); 1) Gluma-(Glutaraldehyde/HEMA) 2) Aqua-Prep F-(Fluoride), 3) Bisblock-(Oxalate), 4) Cervitec Plus-(Clorhexidine), 5) Smart protect-(Triclosan), 6) Nd:YAG laser, 7) No treatment (control). After applying the selected agent, RelyX U200 self-adhesive resin cement was used to bond composite resin blocks to dentin. All groups were subjected to thermocycling for 1000 cycles between $5-55^{\circ}C$. Each bonded specimen was sectioned to microbars ($6mm{\times}1mm{\times}1mm$) (n=20). Specimens were submitted to microtensile bond strength test at a crosshead speed of 0.5 mm/min. Kolmogorov-Smirnov, Levene's test, Kruskal-Wallis One-way Analysis of Variance, and Conover's nonparametric statistical analysis were used (P<.05). RESULTS. Gluma, Smart Protect and Nd:YAG laser treatments showed comparable microtensile bond strengths compared with the control group (P>.05). The microtensile bond strengths of Aqua-Prep F, and Cervitec Plus were similar to each other but significantly lower than the control group (P<.05). Bisblock showed the lowest microtensile bond strength among all groups (P<.001). Most groups showed adhesive failure. CONCLUSION. Within the limitation of this study, it is not recommended to use Aqua-prep F, Cervitec Plus and Bisblock on dentin when used with a self-adhesive resin cement due to the decrease they cause in bond strength. Beside, pre-treatment of dentin with Gluma, Smart protect, and Nd:YAG laser do not have a negative effect.

Sealed amalgam restoration의 미세누출에 관한 연구 (A STUDY ON MICROLEAKAGE OF SEALED AMALGAM RESTORATION)

  • 아말감은 널리 사용되어 온 대표적 치과재료이지만 치질과의 접착성이 없어 와동의 형태에 의해 유지되는 단점을 가진다. 아말감 수복물은 산화물에 의한 변연봉쇄가 일어나기 전까지 초기에 큰 미세누출을 보이며 수복초기에 이를 적절하게 예방하지 않으면 타액이나 미생물이 와동내에 침투하게 되고 이로 인해 수복 후의 과민반응, 충전물의 용해 및 파괴, 변연 변색과 2차 우식이 유발되어, 결과적으로 수복물의 수명이 단축되며 치수 병변이 발생될 수도 있다. 최근 기존 아말감 수복의 장점을 그대로 유지하면서 치질삭제를 줄이고 변연을 봉쇄하며 소와열구에 예방적 처치를 함께 할 수 있는 수복법이 제안되었는데 이를 이른바 sealed amalgam이라 한다. 이 술식은 예방적 확대 없이 병소만을 제거한 뒤 수복물 변연과 인접 소와열구에 치면열구전색재를 도포함으로서 소와열구의 예방적 충전은 물론 수복물 변연과 치질사이의 틈을 봉쇄하여 미세누출을 감소시킬 수 있다고 하였다. 이에 저자는 아말감 수복후 수복물의 마무리 처리와 전색재 적용시점을 달리 하여 sealed amalgam 수복과 기존의 아말감 수복과의 미세누출의 차이를 비교하고자 30개의 상, 하악 소구치를 준비하여 V급 와동을 소구치의 협, 설측에 형성하고 통상적 아말감 수복을 시행한 후 다음의 각 군으로 나누어 처리하였다. 제 1 군 : 24시간 후 연마 (대조군) 제 2 군 : 연마하지 않음, 즉시 전색재 적용 제 3 군 : 연마하지 않음, 열순환(thermocycling) 500회 후 전색재 적용 위의 처리 후 각 군을 $5^{\circ}C$$55^{\circ}C$의 온도변화를 30초씩 번갈아 주며 총 500회의 열순환을 실시한 후 1% methylene blue 용액에 침윤시켜서 100% 습도가 유지된 $37^{\circ}C$ 항온기에 24시간 보관하였다. 치아를 레진에 매몰한 후 협설측으로 치아 장축에 평행하게 절단하여 stereomicroscope를 사용해 색소침투도를 관찰하여 다음과 같은 결론을 얻었다. 1. 평균 미세누출은 연마하지 않고 바로 치면열구전색재를 도포한 2군이 가장 낮았고, 연마만 시행한 1군이 가장 높았다. 2. 실험군간의 미세누출 비교에서 연마만 시행한 1군은 2군에 비해 미세누출이 켰으며 통계적으로 유의성 있는 차이를 보였다(p<0.05). 3군은 1군보다 평균미세누출이 작았으나 통계적으로 유의한 차이가 없었다(p>0.05). 2군은 3군보다 평균 미세누출이 작았으나 통계적으로 유의한 차이가 없었다(p>0.05).

Silane의 처리시기와 타액오염이 도재-레진 시멘트의 전단 결합강도에 미치는 영향 (The effect of silane treatment timing and saliva contamination on shear bond strength of resin cement to porcelain)

  • 서론: 인접 자연치와 아름답고 자연스런 조화를 이루는 심미적 치료를 위해 도재 수복물은 대중적인 치료방법의 하나로 잡고 있다. 임상에서 도재 수복물이 제작 후 치아에 시멘트로 접착을 하기 전 먼저 구강 내에서 시적할 때 수복물 표면에 타액, 혈액, 시적용 시멘트 등으로 인해 오염될 수 있다. 연구 목적: 이 연구의 목적은 도재 수복물로 사용되고 있는 라미네이트 도재에 실란 결합제의 처리 시기를 달리할 때와 도재 수복물을 치아에 시적 할 때 타액의 오염여부가 실란 결합제의 물리적, 화학적 반응성에 미치는 영향을 관찰하여 도재와 레진 시멘트 사이의 결합력의 안정성을 평가하는 것이다. 연구 재료 및 방법: 라미네이트용 도재를 360개의 원반모형과 5개의 정사각형 모형으로 제작하여 20군으로 나눈다. 각 군에 표면처리를 달리 한 뒤 푸리에 적외선 분광계, 접촉각 측정기와 인스트론 만능 시험기를 이용해 화학적, 물리적 비교를 하였다. 통계는 각 군의 전단 결합 강도의 유의성을 검증하기 위하여 일원분산분석(ANOVA) 후 사후검정은 던넷 다중비교 (Dunnett's multiple comparison)를 시행하였다 (P > .05). 연구 결과: 푸리에 적외선 분광 자료에 의해 본 타액 오염 및 실란의 처리 시기에 따른 화학적 변화는 크지 않았다. 접촉각은 타액 오염 후 낮아졌으나 인산으로 산 부식 후 각도가 증가하였다. 전단 결합강도값은 열 순환후의 군에서 미리 실란으로 보호한 경우에 유의한 차이가 있었다. 결론: 수복물의 내구성에 영향을 미치는 다양한 구강환경과 유사한 조건에서 수복물의 물성평가를 같이 고려해 볼 때 실란 결합제를 이용한 수복물의 접착시 실란 결합제를 수복물에 미리 도포하여 일정시간의 경과 후 수복물을 접착하며, 타액 오염이 발생할 경우 인산으로 수복물을 세척하는 것이 유용한 방법 임을 알 수 있었다

광중합 복합레진 INLAY 수복물의 변연누출에 관한 실험적 연구 (IN VITRO STUDY ON MARGINAL LEAKAGE OF COMPOSITE RESIN INLAY RESTORATIONS)

  • The primary aim of this study was to access the degree of marginal leakage in composite resin inlay restorations. Class V cavities were prepared on sixty extracted premolars. They were classified as control group and experimental group 1, 2 and each group was filled with BIS-FIL $I^{(R)}$ and $Silux^{(R)}$ composite resins. In the control group, the composite resin was inserted directly, the experimental group 2 was inserted as composite resin inlay after heat treatment on $125^{\circ}C$, 10 minutes. Then thermocycling was performed 1000 times. After staining with 1% Basic Fuchsin, they were cut in Buccolingual direction and the degree of penetration of the dye was examined under L/M. The following results were obtained : 1. In occlusal margin area, difference in marginal leakage was not observed in all groups. 2. In gingival margin area, cavities filled with composite resin inlay was less marginal leakage than filled directly in BIS-FIL $I^{(R)}$ group, and statistical significant difference was not existed in $Silux^{(R)}$ group. 3. The statistical significance was not existed between composite resin inlay and composite resin inlay heated secondarily. 4. In all groups, gingival margin area reveals more marginal leakage than occlusal margin area and statistical significance was existed.

강화형 간접복합레진과 치과용 합금의 결합강도에 관한 연구 (A STUDY ON THE BOND STRENGTH OF REINFORCED INDIRECT COMPOSITE RESINS TO DENIAL ALLOYS)

  • Indirect composite resins are used as an popular effective esthetic material in prosthetic dentistry, often with metallic substructure that provides support for restorations. Recently, new indirect composite resins as a substitute of ceramic have been developed. These resins provide good esthetics, with a wide range of hue and chroma. And the flexural strength of those is in the range of 120-150MPa, Which is higher than that of feldspathic Ceramic, and similar th that of Dicor. Although it has many merits, one of the major clinical problems of composite resins is the bond failure between metal and resin due to insufficient interfacial bond strength. The purpose of this study was to evaluate shear bond strength of the reinforced indirect composite resin to dental alloys. Three different composite resin systems($Artglass^{(R)},\;Sculpture^{(R)},\;Targis^{(R)}$) as test groups and ceramic($VMK\;68^{(R)}$) as control group were bonded to Ni-Cr-Be alloy($Rexillium\;III^{(R)}$) and gold alloy(Deva 4). All specimens were stored at $^37{\circ}C$ distilled water for 24 hours and the half of specimens were thermocycled 2000 times at temperature from $5^{\circ}C\;to\;60^{\circ}C$. The shear bond strengths of reinforced indirect composite resins to dental alloys were measured by using the universal testing machine, and modes of debonding were observed by stereoscope and scanning electron microscope. The results were as follows: 1 The shear bond strengths of reinforced indirect composite resins to dental alloys were approximately half those of ceramic to dental alloys(P<0.01). 2. There was no significant difference between the shear bond strength of several reinforced indirect composite resins to metal. 3. Alloy type did not affect on the shear bond strengths of resin to metal, but the shear bond strengths of ceramic to gold alloys were higher than those of ceramic to Ni-Cr alloys(P<0.05). 4. The shear bond strengths of Artglass and Targil to gold alloys were significantly decreased after thermocycling treatment(P<0.01). 5. Sculpture showed cohesive, adhesive, and mixed failure modes, but Artglass and Targis showed adhesive or mixed failures. And ceramic showed cohesive and mixed failures.

Evaluation of Marginal and Internal Integrity of Modified Resin-Bonded Fixed Partial Dentures: An In Vitro Study

  • Purpose: The purpose of this study was to evaluate the microleakage of various types of resin-bonded fixed partial dentures (RBFPDs) after artificial aging. Materials and Methods: Forty models with missing first molar were fabricated using artificial resin teeth and were divided into four groups: Group A, conventional RBFPDs design; Group B, modified RBFPDs design; Group C, assembled 3-piece RBFPDs design; and Group D, assembled 3-piece RBFPDs with different occlusal rest positions. Half of the specimens underwent chewing simulation process (240,000 cycles, 50 N load, 1.7 Hz) and thermocycling (temperatures $5^{\circ}C{\sim}55^{\circ}C$, dwelling time 30 seconds) and the remaining 20 specimens didn't receive any treatment. All the specimens were immersed in 2% methylene blue solution for 24 hours to evaluate microleakage, and were sectioned at the middle part of abutment teeth. To evaluate the microleaskage, a dye penetration was calculated. Result: With artificial aging, cyclic loading and thermocycling, a 3-piece RBPFD and a 2-piece RBPFD using original tooth undercuts have significantly lower microleakge (P<0.05) compared to the conventional design of RBPFD and modified RBPFD. Conclusion: Within the limit of this experiment, the assembled RBFPDs exhibited a smaller microleakage than the conventional RBFPDs, implying that the assembled RBFPDs can be more effective for reducing the dislodgement of the RBFPDs.

치과용 금-은-팔라디움 합금에 대한 프라이머 처리가 금속-레진 접착에 미치는 영향 (EFFECT OF METAL PRIMER TREATMENT OF THE Au-Ag-PD ALLOY SURFACE ON THE METAL-RESIN BONDING)

  • The pcf metal primers on the bond strength and durability of 4-META/MMA-TBB resins adhered to an Au-Ag-Pd alloy. For this study, the specimens were divided into 8 groups as follows: Thermocyle 0 : (1) control group : sandblast, (2) Group I : sandblast + Cesead Opaque Primer; (3) Group II : sandblast + Metal Primer; (4) Group III : sandblast + V-Primer; Thermocyle 10,000 (5) control sandblast: (6) Group I : sandblast + Cesead Opaque Primer: (7) Group II : sandblast + Metal Primer; (8) Group III sandblast + V-Primer. The shear bond strength was determined using an Instron were observed with the use of scanning electron microscope. Finally, the strengths of bonded joints were evaluated with regard to their adherence energy using a wedge test. The results obtained were as follows ; (1) The shear bond strength of 4-META/MMA-TBB resin to the Au-Ag-Pd alloy was significantly improved in all the groups treated with the primers (p<0.05). (2) Regardless of the adhesive primers used, a significant difference was observed in the bond strength of the thermocycle 0 groups and 10,000 groups (p<0.05). (3) Both before and after thermocycling, the strongest bond strength between the resin an the alloy was obtained after treatment with a metal primer containing MEPS (p<0.05). (4) In the wedge test, the adherence energies of the control group and Group III decreased more rapidly than those of Group I and II during the 2nd day of storage in water.

Comparative evaluation of shear bond strength of orthodontic brackets bonded to three-dimensionally-printed and milled materials after surface treatment and artificial aging

  • Objective: This study aimed to evaluate the shear bond strength (SBS) of orthodontic brackets bonded to three-dimensionally (3D)-printed materials after various surface treatments and artificial aging compared with that bonded to computer-aided design/computer-aided manufacturing (CAD-CAM) polymethyl methacrylate (PMMA)-milled materials. Methods: Eighty cylindrical specimens were 3D printed and divided into the following four subgroups (n = 20 each) according to the surface treatment and artificial aging procedure. Group A, sandblasted with 50 ㎛ aluminum oxide particles (SA) and aging; group B, sandblasted with 30 ㎛ silica-coated alumina particles (CO) and aging; group C, SA without aging; and group D, CO without aging. For the control group, 20 CAD-CAM PMMA-milled cylindrical specimens were sandblasted with SA and aged. The SBS was measured using a universal testing machine (0.25 mm/min), examined at ×2.5 magnification for failure mode classification, and statistically analyzed (p = 0.05). Results: The retention obtained with the 3D-printed materials (groups A-D) was higher than that obtained with the PMMA-milled materials (control group). However, no significant difference was found between the study and control groups, except for group C (SA without aging), which showed significantly higher retention than the control group (PMMA-SA and thermocycling) (p = 0.037). Study groups A-D predominantly exhibited a cohesive specimen mode, indicating specimen fracture. Conclusions: Orthodontic brackets bonded to 3D-printed materials exhibit acceptable bonding strengths. However, 3D-printed materials are prone to cohesive failure, which may result in crown fractures.
