Primary Repair of Traumatic Aortic Transection with Clamp and Sew Technique -Report of 2 cases- (단순결찰봉합술식을 이용한 외상성 대동맥완전파열의 치료 -2례 보고-)

  • Traumatic aortic transection after blunt chest injury is highly lethal and has high operative mortality. Recently, the diagnostic and therapeutic method of this injury is advanced, especially in spinal cord protection during aortic cross-clamping. We have experienced two cases of traumatic aortic transection with left hemothorax after blunt chest injury, which was diagnosed in operative field. The transected aorta was primarily repaired with clamp and sew method and postoperative paraplegia had not occured. The patients were dischraged without any significant complications. We report these cases with a review of literature.

Trend of Basic Research for Vocal Fold Scar (성대 반흔에 대한 기초연구의 최신 경향)

  • Vocal fold scar disrupts structure of lamina propria and causes significant change in vocal fold tissue biomechanics, resulting in a range of voice problems that often significantly compromise patient quality of life. Although several therapeutic management have been offered in an attempt to improve vocal fold scar, the ideal treatment has not yet been found. Recently, several tissue engineering technique for vocal fold scar using growth factors, several cells, and scaffolds have been described in tissue culture and animal models. Several growth factors such as hepatocyte growth factor, basic fibroblast growth factor, and transforming growth factor beta 3 for therapy and prevention of vocal fold scar have been studied. Cell types to regenerate vocal folds in scarring tissue have been introduced autologous or scarred vocal fold fibroblast and adult mesenchymal stem cells. Decellularized organ matrix and several hyaluronic acid materials have used as scaffolds for vocal fold scar.

  • forty-five claw lamed cows with pododermatitis circumscripta, hyperplasia interdigitalis, and dermatitis verrucosa were treated surgically through resection of necrotic tissues, currettage, antibiotics application, and compressive bandage, under retrograde intravenous regional anesthesia of claw. At 7 days postoperation were showed lameness score 0, absense of swelling of extremitis, without fistulation and purulent exudate in wound. Mean duration time of action of local anesthetic drug was 75 minutes and mean operation time for treatment of pododermatitis circumscripta, hyperplasia interdigitalis, and dermatitis verrucosa was 35 minutes, 20 minutes, and 21 minutes, respectively.

A Study of Constraint-Induced Therapy(CIT) on Stroke Patients (뇌졸중 환자의 건측 상지 제한 치료에 대한 고찰)

  • Constraint-induce therapy(CIT) has been documented to improve motor function in the upper extremity of people with hemiparesis. The exercise program uses the training technique 'shaping'. Shaping involves repetitive exercise that are progressed in small steps, where only positive feedback is given to the patients. It involves 14 days of using a restraint on the unaffected arm after stroke for 90% of waking hours. In conjunction with this, ten days of intensive exercise with the affected arm are undertaken for around six hour daily. CIT produces great improvement of motor function with a period of 2 weeks, that the treatment effect remains stable for many months after the end of therapy, and that it transfers into the everyday lives of patients. The purpose of this study is to introduce about the principles of CIT charactieristic, techniques, therapeutic effects-throughout reference books.

  • Brachytherapy is known to be a good modality to achieve local control as a boost treatment following limited external irradiation, which may reduce the external beam related complication particularly in head and neck cancer. The authors developed iridium-192 ribbons recently to replace the radium needles in the field of brachytherapy. Total of 48 cases of head and neck and pelvic-perineal cancer patients had been treated with Ir-192 ribbons during last two years from October 1986 to September 1988, and the results were analyzed to assess the applicability of the fabricated sources. The conclusion is as follows; 1. Iridium implant resulted excellent tumor control effect in clinical application. 2. Iridium is superior than radium and cecium in brachytherapy because of easier to use and lesser exposure to the personnel. 3. Afterloading technique is useful to modify dose distribution, to expand treatment site and method, and to develop interstitial hyperthermia.

A Study on Flow Properties of Semisolid Dosage Forms

  • There are a wide variety of semi-solid ointments used for healing the skin diseases, whose therapeutic and skin penetration abililties may greatly differ from one another depending on the compositions of ointment vehicles. A computer optimization technique was applied to obtain the optimum formula of o/w type ointment giving the in vitro maximum absorption rate through hairless rat skin membrane. Some of the formulations were selected to find out a relationship between skin penetration of ointment and its Theological characteristics. The experimental value of absorption rate obtained from the ointment by optimum formula agreed well with the theoretical value obtained from a polynomial regression analysis, Three kinds of ointments selected among 15 formulations were obtained with a concentric cylinder type rheometer (Model; Rheolab SM-HM Physica, Germany) at 20, 30, 40 and $50^{\circ}C$ for rheograms of rhelolgical properties of o/w type ointments. As the temperature was raised, all products showed a decrease in both shear stress and yield values. The higher skin penetration, the lower shear stress showed.

  • Biofeedback devices have been used successfully to improve head control and symmetrical walking in cerebral palsied children. Biofeedback of postural sway was investigated as a therapeutic technique to reduce postural abnormality in 24 selected children with cerebral palsy. Subjects were evaluated their weight distribution of both sides during their standing before and after the visual and auditory feedback training. The effectiveness of biofeedback was compared to conventional physical therapy practices in reestabilishing symmetrical standing in cerebral palsied children. Our study found biofeedback was effective in training symmetrical standing posture.

  • We demonstrated heat generation efficiency of the magnetic hyperthermia system to find optimal condition using gelatin tissue phantom. Magnetic hyperthermia induction can be used to make heat generation with different concentration of $Fe_3O_4$ iron oxide inside tissue phantom and magnetically labeled cells by applying AC magntic field at a frequency of 145 kHz. It was observed that the maximum temperature achieved in the magnetic gelatin tissue phantom increased with the concentration of $Fe_3O_4$ iron oxide and alternating magnetic field intensity. Results were discussed with respect to further optimization of therapeutic technique for biomedical application with modified functional nanoparticles.

  • Analysis of synaptic plasticity together with behavioral and molecular studies have become a popular approach to model autism spectrum disorders in order to gain insight into the pathosphysiological mechanisms and to find therapeutic targets. Abnormalities of specific types of synaptic plasticity have been revealed in numerous genetically modified mice that have molecular construct validity to human autism spectrum disorders. Constrained by the feasibility of technique, the common regions analyzed in most studies are hippocampus and visual cortex. The relevance of the synaptic defects in these regions to the behavioral abnormalities of autistic like behaviors is still a subject of debate. Because the exact regions or circuits responsible for the core features of autistic behaviors in humans are still poorly understood, investigation using region-specific conditional mutant mice may help to provide the insight into the neuroanatomical basis of autism in the future.

Closure of Chronic Postpneumonectomy Bronchopleural Fistula using the Transsternal Transpericardial Approach -A case report- (우측 전페절제술후 발생한 기관지늑막루의 Transsternal transpericardial approach를 이용한 폐쇄치료 -1예보고-)

  • The Bronchial stump disruption in bronchopleural fistula with empyema thoracis after pneumonectomy has remained one of the most dreaded complications of thoracic surgery. Management of chronic bronchopleural fistula still poses a therapeutic dilemma in spite of various surgical techniques that have been attempted to control this complication. Only recently, transsternal transpericardial approach for repair of the postpneumonectomy bronchopleural fistula has been utilized in some cases. The patient was a 31 year-old woman who was admitted to our hospital on August 18th, 1989 due to right postpneumonectomy bronchopleural fistula with empyema thoracis for 5 years since she had undergone right pneumonectomy due to pulmonary tuberculosis at E-hospital in 1984. Transsternal transpericardial closure of the fistula was employed and then the thoracic catheter was removed two months later, after the empyema cavity was sterilized by the Clagett method. So, we think this surgical technique is a relatively simple and effective method to the control of chronic postpneumonectomy bronchopleural fistula with empyema thoracis.
