Effect of Case Management Intervention Program for Registered Clients with Home Health Care of Hypertension (고혈압 대상자의 방문간호 중재프로그램의 효과)

  • Purpose: The study compared the hypertension care efficacy of a case management intervention program for registered clients. Methods: The nonequivalent control group pretest-posttest design involved 53 registered hypertensive patients of a customized visiting health care service in S-city(n=28 experimental group, n=25 control group). Data collection and case management intervention were carried out from April to July, 2009. The experimental group had six home visits and two phone calls, and the control group had two home visits during the 8-week period. Outcome variables for test hypotheses were changes in physiologic index (blood pressure and total cholesterol) and degree of self-management performance and confidence. Results: Repeated measure ANOVA and t-test of means revealed significant differences before and after program for systolic and diastolic blood pressure and self-management performance and confidence, but no significant difference concerning total cholesterol. Conclusion: A case management program can have positive effects on blood pressure control and self management. However, research is needed to create a more effective case management for vulnerable patient populations.

Effects of Self-help Group Program Including Elastic Band Exercise on ADL and ROM of Hemophilia Patient (탄력밴드운동을 포함한 자조관리 프로그램이 혈우인의 일상생활과 관절가동범위에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of Self-help group program including Elastic band exercise on ADL and ROM of hemophilia patient. Method: A quasi -experimental design was used. The subjects were 40 young adults with hemophilia (21 experimental group, 19 control group). The subjects of experimental group were participated self-help group program in which five sessions for 5 weeks. The program consisted of health education on hemophilia, elastic band exercise, and therapeutic recreation and its outcomes have been evaluated on ADL and ROM of extremities. The obtained data were analyzed by using the Mann-Whitney U test and ANCOVA of SPSS. Result: 1) The score of ADL increased significantly in the experimental group as compared to the control group. 2) The degree of range of motion of extremities improved in the experimental group as compared to the control group. Conclusion: Considering these research results, the program including elastic band exercise was effective in improving self-management ability and range of motion of extremities. Therefore this program including elastic band exercise could be implemented it as self-management for the hemophilia clients.

The Effects of a Program on Cancer Symptoms, Self-Efficacy, Self-esteem, and Empowerment in Home-based Cancer Patients (자기역량강화프로그램이 저소득층 재가 암환자의 암증상, 자기효능감, 자아존중감 및 자기역량에 미치는 효과)

  • Purpose: The purpose of this research was to develop and evaluate the effects of an empowerment program. Method: We developed an 8-week empowerment program to decrease cancer symptoms and increase self-efficacy, self-esteem, and empowerment. This program includes aspects concerning self-knowledge and management, self-help groups, self-advocacy, resource network, and laughter therapy. We evaluated the effects of this program on 32 home-based cancer patients (one group pretest-posttest design). The data collection was performed from August 22, 2011 to October 13, 2011. Data were analyzed using the SPSS/WIN 18.0 program. Result: There were significant differences in self-esteem derived from the empowerment program. However, there were no significant differences in cancer symptoms, self-efficacy, and empowerment after the program. Conclusion: More research, using a control group pretest-posttest design that considers an appropriate intervention duration, is needed to more accurately examine the effects of the program. We expect this intervention to improve the empowerment of home-based cancer patients.

Effects of a Fall Prevention Program on Physical Fitness and Psychological Functions in Community Dwelling Elders (낙상예방 프로그램이 재가노인의 체력과 심리기능에 미치는 효과)

  • Purpose: This study was done to identify effects of a fall prevention program on physical fitness and psychological functions in community dwelling elders. Methods: A quasi-experimental study was carried out with a nonequivalent control group pre & post-test design. The program, which included exercises and education, consisted of a 12-week group program and an 8-week self-management program using a health calendar. An experimental group (32) and a control group (21) participated. Results: There were significant differences in SPPB (t=-3.92, p<.001), TUG test (t=4.94, p<.001), standing with right leg (t= -3.60, p=.001), standing with left leg (t=-3.74, p<.001), front and rear maximum step length test (t=-4.34, p<.001), right-left maximum step length test (t=-2.65, p=.011), and fall efficacy (t=-2.42, p=.019). Fall efficacy, fear of falling and depression showed significant differences following the 12-week exercise program and 8-week self-management program in the experimental group. Conclusion: Study findings indicate that the fall prevention program is an effective nursing intervention to enhance physical fitness and psychological functions for elders. Using a health calendar, the self-management program was more effective for psychological functions compared to only the group program. Therefore, health providers should develop diversified fall prevention programs which include motivation plans to encourage clients in participating.

Effects of Telephone Consulting Program on Self-efficacy and Self-care in NIDDM Patients (전화상담 프로그램이 당뇨병환자의 자기효능과 자가간호에 미치는 효과)

  • Purpose: This study was intended to examine the effects of a telephone consulting program on self-efficacy and self-care in NIDDM patients. Method: Sixty-eight NIDDM patients participated. Thirty-six were assigned to the experimental group and received the telephone consulting program from one to two times per week for 4 weeks. This program was undertaken by base on small booklet relating to diabetic disease summary, diet, drug, exercise, foot management, prevention and treatment of complications, and insulin injection methods. Result: 1. Hypothesis 1 that "the experimental group who receives the telephone consulting program will have higher self-efficacy score than the control group who does not have the telephone consulting" was supported (t=5.12, p= .000). 2. Hypothesis 2 that "the experimental group who receives the telephone consulting program will have higher self-care score than the control group who does not have the telephone consulting" was supported(t=5.29, p= .000). Conclusion: The telephone consulting program improved self-efficacy and self-care in NIDDM patients. Accordingly, this program can be adopted as an effective nursing intervention in the care of the diabetic patients.

Relationship between Self-management, Stress Coping and Physical Self-concept of Healthy Exercise Participants (건강운동참여자의 자기관리, 스트레스 대처 및 신체적 자기개념의 관계)

  • The purpose of this study was to investigate the relationship between self-management, stress coping, and physical self-concept of health exercise participants. To this end, a total of 274 questionnaire results were obtained by visiting 12 fitness center in the metropolitan area that is running a health exercise program. This was analyzed through the SPSS program, and the following results were obtained. It showed a significant explanatory power in the relationship between self-management and stress coping of health exercise participants. In addition, it also showed significant explanatory power in the relationship between self-management and physical self-concept. Finally, it showed significant explanatory power in the relationship between stress coping and physical self-concept. Through this study, it was found that there was a significant causal relationship between self-management, stress coping, and physical self-concept of healthy exercise participants. In follow-up studies, an in-depth analysis of self-management is required through the causal relationship with environmental factors related to self-management.

The Effects of an Admission-Education Program on Knowledge, Self-Efficacy, Self-Care and Glucose Control in Type 2 Diabetes Patients (당뇨입원교육프로그램이 제2형 당뇨병 환자의 지식, 자기효능감, 자가관리 및 당 조절에 미치는 효과)

  • Purpose: This study was performed to explore the effects of an admission-education program on knowledge, self-efficacy, self-care and glucose control in type 2 diabetes patients. Method: A single group pre-post test design was used. Twenty-one patients participated in this program during 6 days which consisted of education and practice about exercise, diet, self-care and prevention of complications. Variables at baseline, 3 months, and 6 months were measured. Results: Knowledge level, self-care level and self-efficacy were significantly increased over time after the admission-education program. The $HbA_1C$ level and glucose level in urine were significantly decreased over time after the admission-education program. Conclusion: The admission-education program can increase levels of knowledge, self-care, self-efficacy and decrease glucose levels. Therefore, this admissioneducation program was shown to have a positive effect on the management of diabetes.

Development and Effects of a Child Health Management Program Based on Cooperative Learning for Mothers of Preschool Children (학령전기 아동 어머니를 위한 협동학습 기반 아동건강관리 프로그램 개발 및 효과)

  • Purpose: This study was conducted to develop and test effects of a child health management program based on cooperative learning for mothers of preschool children. Methods: For this quasi-experimental study, a nonequivalent control group pretest-posttest design was used. Outcome measures were level of child health management self-efficacy, child health management practices, child's health behavior. Participants were 55 mothers (experimental group 28, control group 27). The program was composed of 6 categories of home health management for preschool children, and provided for three 120-minute sessions. Data were collected from September to October, 2015, at 3 times: baseline, 3 and 6 weeks after beginning the intervention. Data were analyzed using SPSS/WIN 21.0 program. Results: After 3 and 6 weeks, there were significant differences between the two groups in child health management self-efficacy (F=18.33, p<.001), child health management practices (F=8.91, p<.001), and child's health behavior (F=9.91, p<.001). Conclusion: Study findings indicate that this child health management program based on cooperative learning is effective and can be recommended as an intervention for mothers of preschool children. It will contribute to enhanced child health management self-efficacy and health management practices and improved child's health behavior.

Effects of Health Management Program on Body composition, Self-efficacy and Health Promotion Behavior in Middle-Aged Women (중년여성의 건강관리 프로그램이 신체조성, 자기효능감 및 건강증진행위에 미치는 효과)

  • The purpose of this study is to determine the effect of a health management program on body composition(body weight, body fat mass, body fat ratio, lean body mass, abdominal fat ratio), self-efficacy and health promotion behavior in middle-aged women. This 6-week long program containing exercise and health education was developed by researchers. The study design was one group pretest-posttest design. Data for the study was collected from August 1 to September 15, 2002. The participants consisted of 15 middle - aged women living in the community. The collected data was analyzed using Wilcoxon Matched - Signed - Ranks Test by SPSS/WIN program The results are as follows : 1. There was a significant difference in body composition at week 6 compared to week 1. The body weight, body fat mass and body fat ratio significantly decreased(Z= -2.533, p=. 011 ; Z= -2.023, p= .043; Z= -2.023, p= .043). But the lean body mass significantly increased(Z= -2.226, p= .026). 2.There was a significant improvement in self-efficacy at week 6 compared to week 1(Z= -3.434, p= .001). 3.There was a significant improvement in health promotion behavior at week 6 compared to week 1(Z = -3.305, p= .001). In conclusion, health management program promoting self - efficacy for middle-aged women was effective in improving health promotion behavior. Further study with a longer follow up period is necessary in order to test the long term effect of the program.

The Development and Test of Self-Efficacy Promotion Program on Self-care of Hemodialysis Patients (혈액투석환자의 자가간호를 위한 자기효능증진 프로그램 개발 및 효과)

  • The purpose of this study is to develop self-efficacy promotion program and to test its effects on self-efficacy, self-care, physiologic index of hemodialysis patients after applying this program to them. Preliminary study was carried out to identify the levels and types of self-care, self-efficacy of hemodialysis patients. To develop self-efficacy promoting program, several discussions with nursing professors and nurse specialists on hemodialysis patients were made after in-depth literature review on the area. Through these processes, the self-efficacy promoting program including 20 minutes long videotape and other counciling documents were completed. This videotape consisted of specific self-care techniques for hemodialysis patients including management of fistula, measurement of blood presure and body weight, special diets, medications, exercise and rest, management of physical problems and social adjustment. Two group equivalent pre and post test quasai-experimental research design was used in this study. The total subjects were 34 hemodialysis patients who received hemodialysis three time per week at 1 university hospital. Seventeen experimental group subjects were matched with control group subjects in sex and age. Data were analysed with the SPSS window program. Homogeniety between experimental and control group pretest data was tested by x$^2$ and t-test. There were no significanct differences in general characteristics, illness history, specific self-efficacy and self-care between the two groups. The differences of general self-efficacy of two groups were tested with the Repeated Measure ANCOVA because of significant differences of pretest data of general self efficacy between two groups. The differences of self-efficacy and self-care of two groups were tested with Repeated Measure ANOVA and the differences of physiologic indecies including blood potassium level and blood phosphorus level and interdialytic weight gain were tested by t-test. The results were as follows: 1. There was no significant difference in general self-efficacy between the two groups over four different time, and no interaction by groups and by time. 2. There was significant difference in specific self-efficacy between the two groups over four different time, and interaction by groups and by time. 3. There was significant difference in self-care between the two groups over four different time, and interaction by groups and by time. 4. There were no significant differences of blood potassium level and blood phosphorus level, but there was significant difference of interdialytic weight gain between the two groups. From the results above, it can be concluded that the self-efficacy promotion program for hemodialysis patients was effective to improve degree of specific self-efficacy and self-care and to decrease interdialytic weight gain. Considering results, the followings are recommended: 1) Repeated studies are needed for another hemodialysis patients. 2) This program can be used for improving degree of self-efficacy and self-care of hemodialysis patients by nurse practitioner and nurse educator.
