Hysterosalpingo-contrast sonography: is possible to quantify the therapeutic effect of a diagnostic test?

  • Objective: To evaluate the effect of hysterosalpingo-contrast sonography (HyCoSy) on natural conception in the infertile patient. Methods: We conducted a prospective observational study recruiting 180 patients admitted to Infertility Center of Ferrara University from January 2010 to February 2012. The essential inclusion criteria was the couple's desire to perform only diagnostic evaluation on infertility causes and to wait for natural conception before proceeding with further management. Couples were investigated with hormonal profile, semen analysis and HyCoSy. Expected time for spontaneous pregnancy was 180 days from HyCoSy. First datation sonography of pregnancy was used calculating time elapsed from HyCoSy at conception. Results: Forty patients (22.2%) obtained spontaneous pregnancy within 6 months after HyCoSy. The mean of "conception time" was 75 days. The pregnancy rate was significantly higher in the first 30 days (45%) compared to other the months of observation (p<0.0005). Multiple linear regression analysis showed that maternal age and sterility duration proved independent variables in detecting the "conception time" after HyCoSy (t=3.742, p=0.001, t=2.371, p=0.02, respectively). Conclusion: A possible beneficial effect of HyCoSy is feasible especially in the days following its execution. This temporal correlation supports its therapeutic use.

  • Products of gridded surface wind and windstress vectors over the world ocean have been constructed by satellite scatterometer data with highly temporal and spatial resolutions. Even if the ADEOS-II/SeaWinds has supplied surface wind data only for short duration in Apr. to Oct. 2003 to us, it permits us to construct a product with higher resolution together with the Qscat/SeaWinds. In addition to our basic product with its resolution of $1^{\circ}\times1^{\circ}$ in space and daily in time, we try to construct products with $1/2^{\circ}\times1/2^{\circ}$ and semi- and quarter-daily resolution. These products are validated by inter-comparison with in-situ data (TAO and NDBC buoys), and also compared with numerical weather prediction(NWP) ones (NCEP reanalysis). Result reveals that our product has higher reliability in the study area than the NCEP's. For the open ocean regions in the middle and high latitudes where there are no in-situ data, we find that there are clear differences between them. Especially in the southern westerly region of 400-600S, the' wind-stress magnitudes by the NCEP are significantly larger than the others, suggesting that they are overestimated. We also calculate wind-stress curl field that is an important factor for ocean dynamics and focus its spatial character in the northwestern Pacific around Japan. Positive curl areas are found to cover from southwest to northeast in our focus region and almost correspond to the Kuroshio path. It is suggested that the vorticity field in the lower atmosphere is related to the upper oceanic one, and thus an aspect of air-sea interaction process.

  • This paper studies the activities characteristics of the elderly aged 60-75 years old who can take care of themselves in the community public space, and puts forward the design principles of the community public space in the aging society. Using the methodology of experience design, through naturalistic observation, questionnaires, interviews and shadowing, the spatial and temporal characteristics of daily activities of the elderly are analyzed. In the community public space, the behavior of the elderly is essentially a social activity to get more attention and communication. This determines the needs, time, frequency and duration of activities of the elderly in community public space. In the community public space, the behavior of the elderly has the characteristics of long-term and regularity, and its behavior changes slightly with the space, the objective factors that affect the elderly are gender, age and climate. The subjective factors are lifestyle, family members and income level.

On the Characteristics of Meteorological Drought over the South Korea

  • Meteorologists define a drought as a period of common dry weather. This may sound straightforward, but it is not so in reality. In this study, we attempted to identify meteorological drought conditions over South Korea. To evaluate the temporal and spatial variability of drought, we calculated two commonly used drought indices, the percent of normal precipitation (PNP) and the Palmer drought severity index (PDSI) calculated from fifty-eight meteorological stations below the Korean Meteorological Administration (KMA). The yearly precipitation has been growing gradually, and the amplitude between maximum and minimum also grow more explicitly from 1960's. According to the analysis of percentile anomaly of monthly precipitation, major drought duration was $1927{\sim}1929,\;1937{\sim}1939,\;1942{\sim}1944,\;1967{\sim}1968,\;1976{\sim}1977,\;1982{\sim}1983,\;1988,\;and\;1994{\sim}1995$. The severe drought occurred most frequently in Mokpo, Daegu, Jeonju, Busan, and Gangneung; it tended to occur more frequently in south sector than in mid sector of Korea and in south west sector than in south east sector. According to the analysis of seasonal distribution, extreme droughts occurred frequently in winter at Seoul, Gangneung, Jeonju, Daegu, and Busan. Severe droughts in summer were formed frequently at Seoul, Gangneung, and Mokpo, while that for spring at Jeonju, Daegu, and Busan. The results of PDSI distribution for the $1994{\sim}1995$ drought period were one of the most severe and widely spreaded droughts; it occurred most frequently in the south sector of South Korea. The comparison of time series between PDSI and Normal Percent showed that they exhibit a strong compatibility for the entire study period; it implies that both drought indices are useful method to indicate drought severity.

  • An all-reflective, simple noncollinear second harmonic (SH) autocorrelator is described for monitoring the shot-to-shot behavior of ultrashort high-power laser pulses. Two mirrors are used for the dispersion-free splitting of a pulse into two halves. One of the mirrors is able to adjust the delay time and angle between two halves of the laser pulse in a nonlinear crystal. We present the possibility of real-time measurement of the pulse duration, peak intensity (or energy), and the pointing jitters of a laser pulse, by analyzing the spatial profile of the SH autocorrelation signal measured by a CCD camera. The measurement of the shot-to-shot variation of those parameters will be important for the detailed characterization of laser accelerated electrons or protons.

Influence Analysis of Temporal Continuity Change of Flow Data on Load Duration Curve (유량자료의 시간적 연속성 변화가 오염부하지속곡선에 미치는 영향 비교 분석)

  • In korea, TMDL is being implemented to manage nonpoint pollution sources as well as point pollution sources. LDC is being used for the planning of TMDL. In order to analyze the water quality using LDC, it is necessary to prepare FDC using the daily flow data. However, only the daily flow data is measured at the WAMIS branch, and 8days flow data and water quality data are measured at the monitoring Networks. So, in many researches, the water quality is being grasped by deriving the LDC using the 8days flow or the daily flow obtained by various methods. These fluctuations may lead to differences in determining whether the target load is achieved. In this study, each LDC was prepared using the 8day flow and the related daily flow. Then, the effect using different flow data on the achievement of target load was compared according to flow conditions. As a result, the difference ratio in the number of overloads under flow condition was showed 19% in high flows, 42% in moist conditions, 49% in mid-range flows, 41% in dry conditions, and 104% in low flows. In the top ten watershed with the highest difference ratio, the flow became lower the difference ration increases. These differences can cause uncertainty in assessing the achievement of target load using LDC. Therefore, in order to evaluate the water quality accurately and reliably using LDC, accurate daily flow data and water quality data should be secured through the installation of national nonpoint measurement network.

EFFECTS OF ORTHOGNATHIC SURGERY ON THE OCCLUSAL FORCE (악교정술(顎矯正術)이 교합력(咬合力)에 미치는 영향(影響)에 관(關)한 연구(硏究))

  • This study was undertaken to investigate the effect of orthognathic surgery on occlusal force. The maximum bite force was measured in 26 dentofacial deformity patients, aged 14-26(mean age 20.3) years, before surgery and at IMF removal, 3, 6, and 12months postsurgery. To grope the correlation of bite force and skeletal change after orthognathic surgery, the cephalometric headplates were measured, tabulated and statistically analyzed. The results were as follows. 1. The presurgical maximum bite force was 13.7kg in upper first molar(rt. Side 12.7kg, it. Side 14.6kg). There was remarkable difference with that of normal occlusion. 2. The recovery of bite force was very significant in according to the operation method and the duration of IMF that was 7.6kg at IMF removal, 14.2kg at 3 months, 19.7kg at 6 months. 26.1kg at 12 months postsurgery. 3. To fasten the recovery and to increase the bite force after orthognathic surgery, the long IMF time and the injury to the masticatory muscle should be avoided by the internal rigid fixation and early physical exercise. 4. The bite force was positively correlated to the changes of mandibular plane angle, the angle between platatal plane and mandibular plan, the angle between occlusal plane and mandibular plane, and negatively correlated to the changes of mandibular body length in craniofacial structure. 5. There was no correlationship between bit force and mesial inclination of tooth long axis of first molar in this subject. 6. There was no correlation between the changes of bite force and the changes of mechanical advantage of the temporal and masseter muscle.

The Usefulness of Brain Magnetic Resonance Imaging with Mild Head Injury and the Negative Findings of Brain Computed Tomography

  • Objective : To investigate the cases of intracranial abnormal brain MRI findings even in the negative brain CT scan after mild head injury. Methods : During a 2-year period (January 2009-December 2010), we prospectively evaluated both brain CT and brain MRI of 180 patients with mild head injury. Patients were classified into two groups according to presence or absence of abnormal brain MRI finding even in the negative brain CT scan after mild head injury. Two neurosurgeons and one neuroradiologist validated the images from both brain CT scan and brain MRI double blindly. Results : Intracranial injury with negative brain CT scan after mild head injury occurred in 18 patients (10.0%). Headache (51.7%) without neurologic signs was the most common symptom. Locations of intracranial lesions showing abnormal brain MRI were as follows; temporal base (n=8), frontal pole (n=5), falx cerebri (n=2), basal ganglia (n=1), tentorium (n=1), and sylvian fissure (n=1). Intracranial injury was common in patients with a loss of consciousness, symptom duration >2 weeks, or in cases of patients with linear skull fracture (p=0.00013), and also more frequent in multiple associated injury than simple one (35.7%>8.6%) (p=0.105). Conclusion : Our investigation showed that patients with mild head injury even in the negative brain CT scan had a few cases of intracranial injury. These findings indicate that even though the brain CT does not show abnormal findings, they should be thoroughly watched in further study including brain MRI in cases of multiple injuries and when their complaints are sustained.

A Study on Nozzle Flow and Spray Characteristics of Piezo Injector for Next Generation High Response Injection (차세대 고응답 분사용 피에조 인젝터의 노즐유동 및 분무특성에 관한 연구)

  • Most diesel injector, which is currently used in high-pressure common rail fuel injection system of diesel engine, is driven by the solenoid coil energy for its needle movement. The main disadvantage of this solenoid-driven injector is a high power consumption, high power loss through solenoid coil and relatively fixed needle response's problem. In this study, a prototype piezo-driven injector, as a new injector mechanism driven by piezoelectric energy based on the concept of inverse piezo-electric effect, has been designed and fabricated to know the effect of piezo-driven injection processes on the diesel spray structure and internal nozzle flow. Firstly we investigated the spray characteristics in a constant volume chamber pressurized by nitrogen gas using the back diffusion light illumination method for high-speed temporal photography and also analyzed the inside nozzle flow by a fully transient simulation with cavitation model using VOF(volume of fraction) method. The numerical calculation has been performed to simulate the cavitating flow of 3-dimensional real size single hole nozzle along the injection duration. Results were compared between a conventional solenoid-driven injector and piezo-driven injector, both equipped with the same micro-sac multi-hole injection nozzle. The experimental results show that the piezo-driven injector has short injection delay and a faster spray development and produces higher injection velocity than the solenoid-driven injector. And the predicted simulation results with the degree of cavitation's generation inside nozzle for faster needle response In a piezo-driven injector were reflected to spray development in agreement with the experimental spray images.

Development and Application of Drought Index Based on Accumulative Pattern of Daily Rainfall (일 단위 강수량의 누적 패턴을 이용한 가뭄지수 개발 및 적용)

  • This study proposed a new drought index considering the accumulative pattern of daily rainfall, i.e., Rainfall Accumulation Drought Index (RADI). The RADI can be easily calculated at daily scale by comparing the long-term averaged cumulative rainfall to the observed cumulative rainfall for a specific duration. This study evaluated the availability of the RADI in the field of monitoring short-term and long-term droughts by investigating the spatial and temporal variability and the recurrence cycle of drought in South Korea. To present the short-term and long-term droughts, the various SPIs with different durations should be used in practice. However, the RADI can present and monitor both short-term and long-term droughts as a single index. By investigating the national average of the RADI, specific drought patterns of 20-year cycle were identified in this study. This study also proposed a five-level drought classification considering occurrence probability that would be a suitable alternative as a drought criterion for drought forecast/response.
