Comparison of $^{99m}Tc$-tetrofosmin and $^{99m}Tc$-sestamibi Myocardial Perfusion SPECT in Detecting Coronary Artery Fisease (부하-휴식 1일 영상법을 이용한 심근관류 SPECT에서 $^{99m}Tc$-tetrofosmin과 $^{99m}Tc$-MIBI의 관동맥질환 진단율 비교)

  • Purpose: $^{99m}Tc$-tetrofosmin(TF) is a recently proposed myocardial imaging agent which has similar biokinetic characteristics to $^{99m}Tc$-sestamibi(MIBI). The aim of this study was to compare TF and MIBI myocardial perfusion SPECTs in detecting coronary artery disease. Materials and Methods: One hundred and sixty patients(101 males, 59 females, mean age $57{\pm}9yr$) who had undergone both myocardial perfusion SPECT(M-SPECT) and coronary angiography within 1 month were studied. M-SPECT was performed using TF in 115 patients and MIBI in 45 patients. Stress-rest one day protocol was used in all patients. A coronary stenosis was considered significant when the luminal diameter ${\geq}$50% was compromised. The chi square test was used to compare differences in sensitivity and specificity between the two groups. Results: There was no difference in age and diseased coronary artery branches between the two groups. There were more male patients in TF group: Male-to-female ratios of TF and MIBI groups were 78:37 and 23:22, respectively(p<0.05). The overall sensitivity of TF study was 92% and specificity 36%. The overall sensitivity and specificity of MIBI study were 93% and 30%, respectively. There was no difference between TF and MIBI groups in identifying individual diseased coronary artery branches. Conclusion: One day stress-rest myocardial SPECT using either TF or MIBI was comparable and was a very sensitive test in detecting coronary artery disease.

  • Two cases of bleeding Meckel's diverticulum diagnosed by $^{99m}$Technetium-pertechnetate scanning are presented. $^{99m}TC$-pertechnetate was used in the diagnosis of Meckel's diverticulum by Jewette et al. in 1970 for the first time. The affinity of this isotope for the parietal cell of the gastric mucosa makes it ideal for delineating ectopic gastric tissue. Its noninvasiveness can be used in early screening test for occult gastrointestinal bleeding in pediatric age group.

Glomerular Filtration Rate Determined in Conjunction with $^{99m}TC-DTPA$ Routine Renal Scintigraphy (통상적 $^{99m}TC-DTPA$ 신장스캔을 이용한 GFR 측정)

  • Many previously described nuclear medicine procedures to assess glomerular filtration rate (GFR) required numerous blood samples obtained over a period of several hours to determine plasma concentrations of the injected radiopharmaceuticals. And other indirect methods of determining renal clearance have some problems due to individual variations in volume of distribution of the radionuclides used. Rescently reported Jackson's method have the great advantages that is a direct measurement method requiring less than 40 min of imaging time and single blood sampling. And it correctly accounts for individual variations in volume of distribution of the radiopharmaceuticals and can be done with routine renal scintrgraphy. We measured $^{99m}Tc-DTPA$ renal clearance with Jackson's method during the routine $^{99m}Tc-DTPA$ renal scintigraphy in 63 patients admitted to department of internal medicine in SNUH. In 23 cases among 63 patients creatinine clearence was accounted simultaneously. The range of $Cl_{DPDA}$ was from 19.9 ml/min to 170 ml/min and the correlation of $Cl_{DPDA}$ and creatinine clearance was discribed by Y=16.2570+0.7852 X($X=Cl_{DTPA}$ Y=creatinine clearance). And the correlation coefficient r was 0.88. We concluded that $^{99m}Tc-DTPA$ renal clearance measurement with Jackson's method was clinically useful to account GFR that can be done with routine $^{99m}Tc-DTPA$ renal scintigraphy simultaneously.

  • Cerebral rete mirabile는 잘 알려진 뇌혈관 질환으로 뇌혈관촬영상 내경동맥이 심한 협착 혹은 폐색, 뇌저부의 이상혈관망 형성 및 다수의 연수막동맥간문합과 경경수막 외내경동맥간문합의 특징적 소견을 나타낸다. 최근 방사성의약품의 발전과 함께, Tc-99m-HMPAO를 이용한 뇌SPECT는 뇌혈류역학의 평가, 병소의 조기발견 및 병소의 정확한 위치를 알기위한 상용진단 방법이 되었다. 저자들은 뇌 혈관촬영으로 확진된 10명 의 Cerebral rete mirabile 환자에서 뇌 전산화단층촬영과 Tc-99m-HMPAO를 이용한 국소뇌혈류 SPECT를 검토하였다. 대상환자중 남자는 3명, 여자는 7명이었으며, 연령분포는 6세에서 38세이었다. 1) Tc-99m-HMPAO를 이응한 국소 뇌혈류 SPECT는 뇌의 경색부위 뿐만 아니라 허혈부위까지 나타냈으며, cerebral rete mirabile에서의 뇌의 침습된 부위를 나타내는데 있어서 뇌전산화단층촬영보다 우월하였다. 또한 모든 예에서 뇌전산화반층촬영보다 더 넓은 허혈부위를 보여주었다. 2) 다른 허혈성 뇌혈관질환과 비교하여 cerebral rete mirabile는 비균질성 방사능을 나타냈고, 이는 측부순환로에 의한 것으로 추측된다. 3) Tc-99m-HMPAO를 이용한 국소뇌혈류 SPECT는 뇌의 허혈부위의 범위를 평가, 문합수술의 계획 및 cerebral rete mirabile의 경과를 추적하는데 유용한 검사이다.

Synthesis and In vitro Evaluation of 99mTc-diglucosediethylenetriamine (DGTA) as a Potential Tumor Imaging Agent

  • Using a single step chemical synthesis, we synthesized the potential tumor imaging agent $^{99m}Tc$-diglucose-diethylenetriamine (DGTA) from diethylenetriamine and natural D-glucose. 10 min Incubation of 10 mg of precursor with 50 ${\mu}g$ of $SnCl_2{\cdot}2H_2O$ at room temperature yielded over 95% of $^{99m}Tc$ labeling. The stability for 6 hours in saline or human plasma was over 90%. In vitro tumor cell uptake assays using the SNU-C5 and 9 L cell lines showed that, in 0-400 mg/dL glucose medium, cell uptake of $^{99m}Tc$-DGTA was 1.5-8 times higher than that of [$^{18}F$]FDG. Moreover, [$^{18}F$]FDG uptake was dependent on glucose concentration in the medium, whereas cellular uptake of $^{99m}Tc$-DGTA was not dependent on glucose concentration, suggesting that the two compounds have different uptake mechanisms by tumor cells.

  • Both interictal and ictal $^{99m}Tc-HMPAO$ brain SPECT were performed in 22 patients with medically intractable epilepsy. Localization of epileptic foci in our patients was made by combined results of EEG and other tests, including Wada test, magnetic resonance imaging, and neuropsychometric test. Among them, ictal $^{99m}Tc-HMPAO$ SPECT, localized epilptic foci in 20 of 22 patients and provided evidence of epileptic focus in 12 patients by demonstrating maximally increased regional cerebral perfusion (rCP) in epileptic foci during the ictal study with decreased rCP in interictal study. Ictal $^{99m}Tc-HMPAO$ SPECT was particularly useful for investigating epileptic foci, and when correlated with simultaneously recorded ictal EEG, provided further insight for localizing epileptic foci. Conclusively, $^{99m}Tc-HMPAO$ SPECT is. a useful, noninvasive method of localizing epileptic activity which may be particularly important for presurgical investigations, especially in those patients without large morphological lesions.

  • Pleural effusion due to hepatic cirrhosis with ascites is well known, although hepatic hydrothorax in the absence of ascites is a rare condition, the pathogenesis of which is still unknown. We report a case of hepatic hydrothorax without ascites confirmed by the intraperitoneal injection of Tc-99m mecroaggregated serum albumin (Tc-99m MAA) that demonstrated the passage of Tc-99m MAA into the right pleural cavity.

A Study on Effective Application of $Methylenediphosphonate-^{99m}Tc$ ($Methylenediphosphonate-^{99m}Tc$의 효과적 이용에 관한 연구)

  • Biodistribution studies has been carried out to elucidate the cause of poor bone imagings often encountered in using $methylenediphosphonate(MDP)-^{99m}Tc$ and to establish effective conditions in using the popular bone imaging agent. After 150 minutes from the I.V. injection of $MDP-^{99m}Tc$ to mice, the radioactivities accumulated at bone(B), liver(L), and stomach(S) were counted. The radiochemical purity (RCP), the volume, the radioactivity concentration and the amount of radioactivity of $MDP-^{99m}Tc$ were controlled. Data were expressed either in %cpm/g organ or % cpm/organ. The organ distribution ratios(B/L and B/S) were correlated with the RCP, the volume of injection, the radioactivity concentration etc. Results indicated that the RCP plays a major role in biodistributions. High radioactivity concentration and injection of a small amount is recommended. Negligible effect was observed with the amount of radioactivity. It has been confirmed that the up-to-date methods of RCP determinations cannot sensitively detect the sharply affecting trace impurities. A particular biodistribution pattern of crossed B/L and B/S lines was observed in case of using $MDP-^{99m}Tc$ of low RCP. In such a case, rather a higher dosage would be effective for improving the contrast between bone and liver.
