Analysis of Pathogenic Factors in the Menopausal Symptoms of Middle-aged Women in Relation to Sasang Constitutional Type

  • Background: Pattern identification is a unique diagnostic method of traditional Oriental medicine that has recently been the target of questionnaire-based research. Sasang (four-types) constitutional medicine (SCM) is a practice in traditional Korean medicine that seeks to promote objectivity in diagnostics. This paper attempts to illuminate the relationship between constitutions and the pathogenic factors of pattern identification through questionnaires completed by menopausal women about their symptoms. Methods: From March to October 2012, we examined 291 women from the general population, with ages ranging from 40 to 60 years, applying the Kupperman index, the Menopause-Specific Quality of Life Questionnaire (MENQOL), pattern identification based on the Diagnosis System of Oriental Medicine (DSOM), and SCM. We then analyzed the relationship between constitutional type and pathogenic factors. Results: No significant differences were found in the scores of either the Kupperman index or MENQOL questionnaire in relation to constitutional type. However, in a statistical analysis correlating the DSOM pathogenic factor scores (PFS) with the scores of the Kupperman index and MENQOL vasomotor subscale, heat showed a significant positive correlation with SoYang type (SY) and TaeEum type (TE), but not SoEum type (SE), while insufficiency of yin and insufficiency of yang, as well as blood deficiency, showed a significant positive correlation with the TE and SE types. Conclusion: The pathogenic factors in the menopausal symptoms of middle-aged women, specifically the prominent menopausal symptom of facial flushing, differed significantly according to constitutional type.

  • Objective : This study investigated the relationship between warning signs and Sasang constitution (SC) in acute stroke patients. Methods : From October in 2005 to March in 2007, 629 acute stroke patients were studied. Patients were hospitalized within 14 days after the onset ofstroke at one of 3 oriental medical hospitals in Seoul and Kyong-gi province. We assessed the type of SC of acute stroke patients by Questionnaire for Sasang Constitution Classification II (QSCC II). We investigated warning signs (motor weakness, blindness, dysarthria, sensory disturbance, finger dullness, facial & eyelid spasm, neck stiffness), general characteristics, etc. Results : This study showed a higher proportion of Tae-eum & So-eum experience upper extremity sensory disturbance but So-yangexperience lower extremity sensory disturbance. Smoker So-eumexperience 1st & 2nd finger dullness more than non-smoker So-eum. So-eum women experience motor weakness more than So-eum men. Conclusion : Besides these results, we could observe almost no relationship between warning signs and Sasang constitution (SC) in acute stroke patients. More data from prospective cohort studies will help people better understand the relationship between warning signs and Sasang constitution (SC) in acute stroke patients.

소현세자의 체질과 사망원인분석 (The Crown Prince Sohyen's Constitution on the Basis of Sasang Constitituional Medicine and Cause of Death)

  • 1. Objectives : The Crown Prince Sohyen(1612-1645) has been believed ro have been poisoned to death because of the political conflict with the King Injo. However, the Crown Prince Sohyen who was So-yangin(SY type) didn't be treated properly at that time. This paper is supposed to explain the reason why he died by reference. 2. Methods : The prescription which was made out to the Crown Prince Sohyen is analyzed, based on the Sasang Constitutional Medicine(SCM) through Shimyang-ilgy and Ulyudongkung-ilgy. 3. Results : 1) In the case of the prescription where So-yang medical stuffs were contained a lot or Gunyak(the main madical stuff) was So-yang one, the Crown Prince Sohyen was better, while that of Tae-eum and So-eum medical stuffs didn't work. Therefore, the Crown Prince Sohyen can be considered So-yangin. 2) The So-yang in's symptom was misunderstood as Hakjil and treated, which could lead him to be put to death 4. Conclusions : The Crown Prince Sohyen is supposed to be Soyang-in(SY type), and accordingly the So-yang in's symptom was misunderstood as Hakjil and treated, which could cause him to pass away.

  • Korean constitutional type(Sasang Chejil) was considered with anxiety level and personality type in order variability frequently encountered in EEG research. Sasang Chejil type seems a very important factor to be controled to minimize the variabilty since Sasang Cheji subgroups showed distinct differences in EEG relative power ao more recording sites and frequency bands than the subgroups of anxiety level and personality type, well-reconized variability-contribution factors. It is likely that Tae-Eum Chejil type is predispositioned to be more relaxed and feel happier and less unhappy than So-Yang type since the former showed greater alpha relative powers and left-hemisphere activations at both of the resting and emotional states.

Investigation of Genetic Evidence for Sasang Constitution Types in South Korea

  • In Sasang constitutional medicine, both disease susceptibility and drug response are considered to be related to the characteristics of an individual's physiology and psychology: a theory which is central to traditional Korean medicine. Based on such observable characteristics, Sasang constitutional medicine classifies people into four constitutional types. Genetic studies of Sasang constitution would help reveal the inheritance patterns and models of the typological traits and, moreover, help with traditional medical diagnosis and treatment. To investigate the heritable aspect of Sasang constitution, we collected various pedigrees from South Korea. The study population has 101 pedigrees composed of 593 individuals. The determination of the Sasang constitution type of each individual was performed by doctors who diagnose the Sasang constitutional type of individuals as part of their professional practice. We calculated estimates of familial correlation and heritability. Parent-Offspring pairs showed the strongest familial correlation of Sasang constitutional type, with the correlation values of 0.21 and 0.28, followed by sibling pairs with the value ranging between 0.14 and 0.25. From the heritability analysis conducted with the Variance-Component method, the heritability of TE (Tae-Eum) type, SY (So-Yang) type, and SE (So-Eum) type were 55%, 41%, and 47%, respectively. This pattern of heritability was consistent with different set of analyses, which suggest the robustness of our result. Our result clearly shows that the Sasang constitution type is heritable, and further genetic analysis based on our result will shed light on the biological mechanism of Sasang constitution.

신경망을 사용한 사상체질 진단검사 개발 연구 (Development of Sasang Type Diagnostic Test with Neural Network)

  • The medical informatics for clustering Sasang types with collected clinical data is important for the personalized medicine, but it has not been thoroughly studied yet. The purpose of this study was to examine the usefulness of neural network data mining algorithm for traditional Korean medicine. We used Kohonen neural network, the Self-Organizing Map (SOM), for the analysis of biomedical information following data pre-processing and calculated the validity index as percentage correctly predicted and type-specific sensitivity. We can extract 12 data fields from 30 after data pre-processing with correlation analysis and latent functional relationship analysis. The profile of Myers-Briggs Type Inidcator and Bio-Impedance Analysis data which are clustered with SOM was similar to that of original measurements. The percentage correctly predicted was 56%, and sensitivity for So-Yang, Tae-Eum and So-Eum type were 56%, 48%, and 61%, respectively. This study showed that the neural network algorithm for clustering Sasang types based on clinical data is useful for the sasang type diagnostic test itself. We discussed the importance of data pre-processing and clustering algorithm for the validity of medical devices in traditional Korean medicine.

사상인(四象人)의 생리적 특성 연구 (Psychological, Physical and Genetic Traits of Sasang Typology)

  • The major concept of Sasang typology is that the disease susceptibility and drug response as well as physiological characteristics are presumed to be different depending on their Sasang types. Although characterizing fundamental basis of their traits are crucial in this research field, only pathological susceptibility and physical appearances were thoroughly studied. We evaluated their physiological characteristics by tapping psychological, physical and genetic traits of each Sasang types. After determining the Sasang type of one hundred three college students based on the Questionnaire for the Sasang Constitution Classification, the psychological, physical and genetic traits of each type were analyzed with the Myers-Briggs Type Indicator (MBTI), Bioelectrical Impedance Analysis and genetic polymorphism test, respectively. Each of the Sasang types showed significantly different profiles (Generalized estimation equation, coef=11.88, z=2.13, p=0.033), and could be distinctively classified based on their MBTI scores (discriminant analysis Wilks Lambda=0.611, df=8, chi-square=36.7, p<0.001). Subjects with the So-Eum type (Introversion and Judging) and the So-Yang type (Extroversion and Perceiving) showed contrasting psychological features, however they had similar anthropometric characteristics. Subjects with the Tae-Eum type showed bigger Body Mass Index ($R^2$=0.22, df=4, 74, F=5.07, p=0.001) and body shape compared to others. Although there were no significant differences in G-protein beta-3 subunit polymorphism, angiotensin-converting enzyme polymorphism and Methylenetetrahydrofolate reductase polymprhisms among groups with Sasang types, it was shown that the dopamine system could be one for genetic marker for Sasang typology. These results demonstrated distinctive and essential traits of Sasang typology using reproducible psychometric, anthropometric and genetic evaluations. We also found that the Sasang typology was a bio-psychological typology which could show trait-specific guideline for individualized medicine.

CBCL을 통해 살펴본 아동 사상체질별 문제행동의 차이 분석 (Analysis of Child Behavior Checklist (CBCL) Problem Behaviors of Sasang Types in a Child Clinical Sample)

  • Objectives The goal of the present study was to elucidate the characteristics of behavior problems in Sasang typology with child patients. Methods Subjects consisted of 176 (95 boys, 81 girls) children from 36 months to 83 months were diagnosed by two clinical experts in pediatrics and Sasang medicine. There were 22 So-Yang types, 141 Tae-Eum types, and 13 So-Eum types. The behavior problems were measured with Child Behavior Checklist (CBCL) and ANOVA (analysis of variance) was used for the analysis. Results There were no significant differences between Sasang types among subject's age (F(2, 173)=.190, p=.827) and sex (${\chi}^2$(2, N=176)=1.639, p=.441) as well as their mother's age (F(2, 169)=.060, p=.942) and education level (${\chi}^2$(4, N=172)=.394, p=.983) and their father's age (F(2, 168)=1.184, p=.309) and education level (${\chi}^2$(4, N=172)=5.664, p=.226). So-Yang types ($50.14{\pm}22.35$; $17.27{\pm}8.60$) had higher scores than Tae-Eum types ($38.74{\pm}21.32$; $12.62{\pm}7.98$) in total problems and internalizing problems score of the CBCL, respectively. More specifically, So-Yang types ($5.90{\pm}2.81$; $3.77{\pm}1.90$) had significantly higher depression/anxiety and somatization subscale score than Tae-Eum types ($4.04{\pm}2.73$; $2.30{\pm}2.12$). Scores of So-Yang types were significantly higher than those of CBCL clinical group in depression/anxiety and somatization subscales. Conclusions There were significant differences between child outpatients with different Sasang types, which would be taken into consideration concerning development of Sasang type diagnosis in addition to parenting, treatment, and prevention for children.

KS-15 설문지를 이용한 사상체질 예측값의 변화와 관련요인 분석 (The change in Sasang constitution prediction value and the associated factors using KS-15 questionnaire)

  • Objectives The aim of this study was to investigate the change in Sasang constitution prediction value in 2 years and find the factors associated with it. Methods Cohort data from Korean medicine data center was used. Using Korean Sasang Constitutional Diagnostic Questionnaire (KS-15) which consist of questions related to body shape, temperament, and symptoms, participants were categorized into Tae-Yang (TY), Tae-Eum (TE), So-Yang (SY), and So-Eum (SE). Sasang constitution was assessed on the baseline and after two years. Result Total 5,784 participants were analyzed. (TE 3, 341; SE 911; SY 1,532). Among them, 1,402 participants (24.2%) showed different prediction value in KS-15 after two years. The proportion of participants showing different prediction value in two years was the highest in SY, and the lowest in TE group. The factors associated with the change in Sasang constitution prediction value were different by constitution type. The change in feeling after sweating was significantly associated with the change in prediction value in TE and SY groups, not in SE group. Although temperament was not significantly associated with the change in prediction value from TE to SE, it was significantly associated with that in the change from TE to SY. The change in BMI and appetite were associated with the change in constitution prediction value in all three constitution types. Conclusion Although the factors associated with the change in prediction value of Sasang constitution were different by each constitution type, BMI and appetite were significant in all three types. These factors could be useful for developing Sasang constitution questionnaire and deciding re-prediction needs of Sasang constitution. Further research about the factors related to Sasang constitution diagnosis need to be conducted.

다한증 환자의 사상의학적 특성에 관한 연구 (A Clinical Analysis of Hyperhidrosis Patients in Korea: A Single Center, Retrospective Study of 5 Years)

  • Objectives The purpose of this study was to analyze the clinical characteristics of patients with hyperhidrosis who visited a Korean medicine hospital, and provide information on treated hyperhidrosis, based on Sasang Constitutional medicine. Methods A retrospective review was made of the electronic medical record (EMR) charts for 157 Korean patients with hyperhidrosis (90 male, 67 female), who first visited hospital from January 1, 2013 to December 31, 2017. The following information was collected: sex, age, height, weight, body mass index (BMI) and clinical data (diagnosis, area of onset, duration of illness, aggravating factors, medication, healing process, et al). Results There were many patients with hyperhidrosis, in the other of So-Yang type (57.3%), Tae-Eum type (35.0%) and So-Eum type (7.6%). The proportion of male was higher than female. The most common area of hyperhidrosis was hands and feet (44.0%), followed by generalized whole body (27.4%) and head and face (25.5%). The cure rate of hyperhidrosis was 32.2%, and effectiveness rate of that was 72.7%. In individual patients, the better the usual health condition expressed by original symptoms, the higher the possibility of treatment success, with shortened duration of treatment and reduced total drug dose. The most frequent treatment modality in patients was that consistently improved, and this duration of treatment was statistically significantly shorter than that experienced worsening of excessive sweating during treatment period (p<0.001). Conclusions This study provides some original information on the Korean patients with hyperhidrosis, and suggests the suitability of Sasang Constitutional medicine as treatment methods for hyperhidrosis.
