Evaluation of Combine IGRT using ExacTrac and CBCT In SBRT (정위적체부방사선치료시 ExacTrac과 CBCT를 이용한 Combine IGRT의 유용성 평가)

  • Purpose : The purpose of this study is to compare and analyze the set-up errors using the Combine IGRT with ExacTrac and CBCT phased in the treatment of Stereotatic Body Radiotherapy. Methods and materials : Patient who were treated Stereotatic Body Radiotherapy in the ulsan university hospital from May 2014 to november 2017 were classified as treatment area three brain, nine spine, three pelvis. First using ExacTrac Set-up error calibrated direction of Lateral(Lat), Longitudinal(Lng), Vertical(Vrt), Roll, Pitch, Yaw, after applied ExacTrac moving data in addition to use CBCT and set-up error calibrated direction of Lat, Lng, Vrt, Rotation(Rtn). Results : When using ExacTrac, the error in the brain region is Lat $0.18{\pm}0.25cm$, Lng $0.23{\pm}0.04cm$, Vrt $0.30{\pm}0.36cm$, Roll $0.36{\pm}0.21^{\circ}$, Pitch $1.72{\pm}0.62^{\circ}$, Yaw $1.80{\pm}1.21^{\circ}$, spine Lat $0.21{\pm}0.24cm$, Lng $0.27{\pm}0.36cm$, Vrt $0.26{\pm}0.42cm$, Roll $1.01{\pm}1.17^{\circ}$, Pitch $0.66{\pm}0.45^{\circ}$, Yaw $0.71{\pm}0.58^{\circ}$, pelvis Lat $0.20{\pm}0.16cm$, Lng $0.24{\pm}0.29cm$, Vrt $0.28{\pm}0.29cm$, Roll $0.83{\pm}0.21^{\circ}$, Pitch $0.57{\pm}0.45^{\circ}$, Yaw $0.52{\pm}0.27^{\circ}$ When CBCT is performed after the couch movement, the error in brain region is Lat $0.06{\pm}0.05cm$, Lng $0.07{\pm}0.06cm$, Vrt $0.00{\pm}0.00cm$, Rtn $0.0{\pm}0.0^{\circ}$, spine Lat $0.06{\pm}0.04cm$, Lng $0.16{\pm}0.30cm$, Vrt $0.08{\pm}0.08cm$, Rtn $0.00{\pm}0.00^{\circ}$, pelvis Lat $0.06{\pm}0.07cm$, Lng $0.04{\pm}0.05cm$, Vrt $0.06{\pm}0.04cm$, Rtn $0.0{\pm}0.0^{\circ}$. Conclusion : Combine IGRT with ExacTrac in addition to CBCT during Stereotatic Body Radiotherapy showed that it was possible to reduce the set-up error of patients compared to single ExacTrac. However, the application of Combine IGRT increases patient set-up verification time and absorption dose in the body for image acquisition. Therefore, depending on the patient's situation that using Combine IGRT to reduce the patient's set-up error can increase the radiation treatment effectiveness.

Evaluation of accuracy in the ExacTrac 6D image induced radiotherapy using CBCT (CBCT을 이용한 ExacTrac 6D 영상유도방사선치료법의 정확도 평가)

  • To verify the accuracy of the image guided radiotherapy using ExacTrac 6D couch, the error values in six directions are randomly assigned and corrected and then the corrected values were compared with CBCT image to check the accurateness of ExacTrac. The therapy coordination values in the Rando head Phantom were moved in the directions of X, Y and Z as the translation group and they were moved in the directions of pitch, roll and yaw as the rotation group. The corrected values were moved in 6 directions with the combined and mutual reactions. The Z corrected value ranges from 1mm to 23mm. In the analysis of errors between CBCT image of the phantom which is corrected with therapy coordinate and 3D/3D matching error value, the rotation group showed higher error value than the translation group. In the distribution of dose for the error value of the therapy coordinate corrected with CBCT, the restricted value of dosage for the normal organs in two groups meet the prescription dose. In terms of PHI and PCI values which are the dose homogeneity of the cancerous tissue, the rotation group showed a little higher in the low dose distribution range. This study is designed to verify the accuracy of ExacTrac 6D couch using CBCT. It showed that in terms of the error value in the simple movement, it showed the comparatively accurate correction capability but in the movement when the angle is put in the couch, it showed the inaccurate correction values. So, if the body of the patient is likely to have a lot of changes in the direction of rotation or there is a lot of errors in the pitch, roll and yaw in ExacTrac correction, it is better to conduct the CBCT guided image to correct the therapy coordinate in order to minimize any side effects.

Computational Study for the Performance of Fludic Device during LBLOCA using TRAC-M (최적계산코드를 이용한 대형 냉각재상실사고시 유량조절기 성능평가에 관한 연구)

  • The APR1400 is an Advanced Pressurized Water Reactor with 3983 MWt power, 2×4 loops, and direct vessel injection system. The Fluidic Device (FD) is adopted to regulate the safety injection flow rate in a Safety Injection Tank (SIT) of APR1400. The performance of a newly designed fluidic Device is evaluated by analyzing a Large Break Loss-of-Coolant Accident (LBLOCA) using TRAC-M/F90, version 3.782. The analysis results show that the TRAC-M code reasonably predicts the important phenomena of blowdown, refill and reflood phases of LBLOCA. The sensitivity studies about gas/water volume changes in a SIT and K factor changes in a SI system were also done to understand the important phenomena with a Fluidic Device in APR1400.

A Study on the Additional Absorbed Dose of Normal Tissues by Image Guided Radiation Therapy(IGRT) (영상유도 방사선 치료(IGRT)에 따른 정상 조직의 추가 피폭에 대한 연구)

  • The recent radiation therapy field can provide treatment which guarantees a high degree of accuracy, due to patient set-up using various image guided radiation therapy(IGRT) instruments. But the additional absorbed dose to patient's normal tissues is increasing. Therefore, this study measured the absorbed dose to surrounding normal tissues which is caused by patient set-up using OBI, CBCT, ExacTrac, among various IGRT instruments. The absorbed dose to the head, the chest, the abdomen, and the pelvis from CBCT was 12.57 mGy, 20.82 mGy, 82.93 mGy, and 52.70 mGy, respectively. Also, the absorbed dose from OBI and ExacTrac ranged from 0.76 to 8.58 mGy and from 0.14 to 0.63 mGy, respectively. As a result, CBCT's absorbed dose was far higher than other instruments. CBCT's surface dose was far higher than others, too, but OBI's entrance skin dose was almost the same as CBCT's.

  • 최근 웨스팅하우스사가 SECY-83-472에 근거하여 개발한 최적평가방법론인 WCOBRA/TRAC UPI EM을 이용하여 Appendix K, Superbounded, Nominal 계산을 고리 1호기에 대하여 수행하였다. 15%증기발생기 관막음율, 출력 첨두 계수 2.35, 최대 선형 열출력 15.588kw/ft을 사용하여 계산한 결과 Appendix K 최대 피복재 온도는 1941℉로서 KAERI/Siemens가 RTSR 작성시 수행한 해석에 비하여 운전여유도가 증가한 것으로 나타났으며, 그 결과가 SECY 방법론에 합당함을 확인하였다.

Application of the ExacTrac System in Respiratory Gated Radiotherapy for Lung Cancer Patients (폐암 환자의 호흡연동방사선치료를 위한 ExacTrac 시스템 적용)

  • This study analyzed the movement of tumors using 4DCT. Appropriate uniform IM were identified using TC, II and CI depending on ITV margins. DVH and NTCP were also compared in each case. Dose analysis on tumors with uniform IM showed that the optimal treatment plan for satisfying all TC, CI, II was evaluated as 2 mm in phase 20 and 3 mm in 40%. That was compared to the dose from the normal tissues of $PTV_{20}$, $PTV_{40}$. In the 20% radiation field, V5, V10, and V20 for the lungs increased 1.49, 1.26, and 0.65%, while 40% increased by 1.9, 2.41 and 1.23%. NTCP had a dose increase of 0.57 to 0.029% from 20% and 40%. There was a dose increase in the spinal cord and heart at uniform IM, but there was no significant difference. These data suggest that the ITV setting of 20%, phase for Respiratory Gated Radiotherapy using Novalis ExacTrac system can be applied with a uniform IM 2 mm and 40% with 3 mm for optimal treatment plan.
