FCST 맞춤형 음양균형장치를 위주로 한 보조생식술 실패 후 난임 치료 후 출산 증례 (An Infertility Treatment Case Mainly Managed by Customized Intraoral Balancing Appliance of FCST, a TMJ Therapy for the Balance of Meridian and Neurological System)

  • 본 증례에서는 난임의 치료방법으로 FCST의 음양균형장치를 포함한 PBT, 평형 침 치료, 부항, 한약 등의 복합치료로 전신 증상의 개선 및 감소에 현저한 치료효과가 나타남을 관찰하였다. 또한 그 결과로 자연 임신에 성공하여 출산하였고 출산한 아기도 건강한 상태임을 확인하였다.

대학생의 스트레스와 구강건강, 삶의 질 관련성 분석 (Relationship between Stress, Oral Health, and Quality of Life in University Students)

  • 스트레스, 구강건강, 구강건강관련 삶의 질 연관성 분석을 위해 2016년 6월 2주 동안 D지역 대학생 452명을 대상으로 조사한 결과 다음과 같은 결론을 얻었다. 스트레스는 여학생이 남학생보다 높게 조사되었고(p<0.05), 스트레스 상위군이 하위군보다 구강건조증, 구취, TMJ 통증 인식이 높게 조사되었으며(p<0.05), 스트레스 상위군이 하위군보다 OIDP에 부정적 영향을 경험하는 것으로 조사되었다(p<0.05). 스트레스와 구강건강, OIDP 상관관계 분석결과 스트레스는 구강건조증, 구취, TMJ 통증과 통계적으로 유의한 양의 상관관계가 있었고, OIDP는 구강건조증, 구취, TMJ 통증, 스트레스와 통계적으로 유의한 양의 상관관계가 있었다. 이와 같이 스트레스는 구강건강, 구강건강관련 삶의 질에 부정적 영향을 미치는 것으로 조사되었다. 그러므로 대학생들의 스트레스 대처능력과 자가구강관리능력을 향상시켜 심신이 건강한 사회인으로 성장할 수 있도록 교육이 이루어져야 할 것이다.

Comparative analysis of craniofacial asymmetry in subjects with and without symptoms of temporomandibular joint disorders: a cross-sectional study

  • Objectives: The aim of the study was to quantify and compare craniofacial asymmetry in subjects with and without symptoms of temporomandibular joint disorders (TMDs). Materials and Methods: A total of 126 adult subjects were categorized into two groups (63 with a TMDs and 63 without a TMDs), based on detection of symptoms using the Temporomandibular Joint Disorder-Diagnostic Index (TMD-DI) questionnaire. Posteroanterior cephalograms of each subject were traced manually and 17 linear and angular measurements were analyzed. Craniofacial asymmetry was quantified by calculating the asymmetry index (AI) of bilateral parameters for both groups. Results: Intra- and intergroup comparisons were analyzed using independent t-test and Mann-Whitney U test, respectively, with a P<0.05 considered statistically significant. An AI for each linear and angular bilateral parameter was calculated; higher asymmetry was found in TMD-positive patients compared with TMD-negative patients. An intergroup comparison of AIs found highly significant differences for the parameters of antegonial notch to horizontal plane distance, jugular point to horizontal plane distance, antegonial notch to menton distance, antegonial notch to vertical plane distance, condylion to vertical plane distance, and angle formed by vertical plane, O point and antegonial notch. Significant deviation of the menton distance from the facial midline was also evident. Conclusion: Greater facial asymmetry was seen in the TMD-positive group compared with the TMD-negative group. The mandibular region was characterized by asymmetries of greater magnitude compared with the maxilla. Patients with facial asymmetry often require management of temporomandibular joint (TMJ) pathology to achieve a stable, functional, and esthetic result. Ignoring the TMJ during treatment or failing to provide proper management of the TMJ and performing only orthognathic surgery may result in worsening of TMJ-associated symptoms (jaw dysfunction and pain) and re-occurrence of asymmetry and malocclusion. Assessments of facial asymmetry should take into account TMJ disorders to improve diagnostic accuracy and treatment outcomes.

Pantographic Reproducibility Index(PRI)를 이용한 하악운동의 재현성에 관한 연구 (A Study on Reproducibility of Mandibular Movements Using Pantographic Reproducibility Index (PRI))

  • In order to determine if a relationship exists between the clinical symptoms of TMJ dysfunction and the reproducibility of mandibular movements, twenty one subjects were chosen. The control group consisted of 5 subjects who were determined to be free from signs and symptoms of dysfunction . The sixteen experimental subjects were selected on the basis of their having dysfunctional symptoms. The author obtained two sets of pantographic tracings. Each set consisted of tracings from three both lateral and one protrusive movements. A second set of tracings were recorded immediately on the new recording papers using the same procedure as the first tracing. The tracings were scored by Pantographic reproducibility index (PRI). The obtained results were as follows. 1. Mean PRI scores of groups increased as the degree of dysfunction were increased. 2. For the groups of no or slight dysfunctional symptoms the PRI scores of the second tracing were smaller than the first one, wherea the scores of the second tracing from $D_2,\;D_3$ group were larger than the first tracing. 3. Differences between the mean PRI scores of control group and those of experimental group were statistically significant. 4. The second sets of tracings were more reliable statistically than those of first ones. 5. PRI can be used as a meaningful aid for the evaluation of the diagnostic and therapeutic results of treatment modalities for the TMJ dysfunction. 6. At 3east two sets of tracings should be recorded when the PRI is to be used to detect the incoordinated movements of TMJ dysfunction patient. 7. PRI scores of control group ($D_0$) was 137.7, thus, mandibular movement was reproducible, whereas PRI scores of experimental groups ($D_1,\;D_2,\;D_3$) were 22.5, 27.7, 30.45 respectively, thus were nonreproducible.

두개하악장애증상발현과 교합요인 사이의 상관성에 관한 연구 (A Study on the Correlation between Signs/Symptoms of the Craniomandibular Disorders and Possible Contribution Occlusal Factors)

  • To evaluate the correlationship between sign/symptoms of craniomandibular disorder(CMD) and possible contributing occlusal factors, the author analyzed craniomandibular index(CMI), clinical dysfunction index(CDI) and occlusal index(Oi) obtained from 88 subjects (32 males, 56 females, mean age 28.7) consisted of 49 CMD patients(15 males, 34 females, mean age 28.7) and 39 non-CMD patients (17 males, 22 females, mean age 24.5). The obtained results were as follows : 1. There was very high significant correlation bebween CMI and CDI in total subjects (r=0.83, p<0.01) 2. There was very low correlation between working-side interference and TMJ noise which was observed as a statistically significant value(p<0.05). And also there ws very low correlation between nonworking side interference and muscle tenderness, CDI, dysfunction index(DI), palpation index(PI) which were observed as statistically significant value (p<0.01) 3. There was very low correlation between as ymmetry of tetruded contact position(RCP)-intercuspal position(ICP) slide and muscle tenderness, TMJ tenderness, TMJ noise, CMI, DI, PI which were observed as statistically significant values(p<0.05) 4. In general there was low correlation between Oi and muscle tenderness, CDI, CMI, DI, PI which were observed as statistically significant values (p,0.05). But Oi had a relatively higher correlation with muscle tenderness, PI than whith CDI, DI.

The Effectiveness of Occlusal Splint for the Treatment of Temporomandibular Joint Dislocation

  • Temporomandibular joint (TMJ) dislocation is defined that the disc-condyle complex is positioned anterior to the articular eminence in the open mouth position, and is unable to return to a normal closed mouth position without a manipulative maneuver. TMJ dislocation can recur habitually and result several problems to patients such as discomfort, pain, fear, and anxiety. The only definitive treatment for TMJ dislocation is surgical alteration of the joint itself. In most cases, however, a surgical procedure is far too aggressive for the symptoms experienced by the patient. In addition, the effect of surgical treatment may be insufficient, and the recurrence have been reported. It is also possible to develop several complications after surgical treatment. Therefore much effort should be directed at supportive therapy in an attempt to eliminate the disorder or at least reduce the symptom to tolerable levels. Through this cases the authors present favorable treatment outcome using occlusal splint with the patient of TMJ dislocation. Occlusal splint therapy can be considered as easy, safe, and useful non-invasive modality to treatment of TMJ dislocation.

턱관절에 발생한 감염에 대한 치험례 (Infection of the temporomandibular joint: a report of three cases)

  • An intracapsular and pericapsular infection of the temporomandibular joint (TMJ) is rare. The invasion of bacteria into the joint space can occur through several routes. Among them, hematogenous spread is most common. This report describes three cases of abscess formation in the TMJ (intracapsular and pericapsular infection). The patients were treated with supportive care and surgical intervention (incision and drainage) under hospitalization, and their symptoms had improved. Pain of the TMJ is a typical symptom of temporomandibular joint disorders (TMD). On the other hand, an infection of the TMJ can also cause pain on the affected side, and can be misdiagnosed as routine TMD. Therefore, the possibility of an infection of the TMJ cavity should be considered when treating TMD.

턱관절 균형의학 치료를 위주로 한 한방 난임 치료 출산 6례 (Korean Medical Treatments for Subfertility 6 Cases, Mainly Managed by FCST, TMJ Balancing Medicine)

  • 울산광역시 동구 보건소 협력 사업으로 한 2014년도 한방 난임 치료 1증례를 포함한 6증례중에서 난임의 치료방법으로 4증례는 FCST의 편차수정, 음양균형장치를 포함한 PBT, 침 치료, 부항, 뜸, 한약 등의 복합치료로 전신 증상의 개선 및 감소에 현저한 치료효과가 나타남을 관찰하였다. 2증례는 음양균형장치와 PBT는 제외되었다. 또한 그 결과로 6증례 모두 자연 임신에 성공하여 출산하였고 출산한 아기들도 건강한 상태임을 확인하였다.

정량적 교합력이 측두하악관절강 크기에 미치는 영향 (The Effects of Quantitative Occlusal Force on Size of Temporomandibular Joint Space)

  • The purpose of this study was to evaluation of temporomandibulr joint space according to increasing the occlusal force on working and non-working side during unilateral bite. For the study, 22 normal adults, age ranged from 23 to 25, who had no symptoms on TMJ area and masticatory muscles and had normal or class I molar relationship were selected. Transcranial TMJ radiograph was taken during unilateral biting with sensor of Bite force checker (Nihon Kohden Kygyo Co.Ltd., Japan) on each 1st molar teeth of right and left side which were forced each 0Kg, 10Kg, 20Kg and 30Kg by use of Accurad-200 (Denar Corperation's product). The radiographs were traced on the screen, which was magnified by 5. The size of temporomandibular joint space at anterior, superior and posterior compartment were measured with Dumas's method (reference line of between squamotympanic fissure and the most inferior point of articular eminence). On the basis of this study, the following results were obtained. 1. Size of anterior TMJ space was tend to decrease on the working side and increase on the non-working side according to increasing the occlusal force, but not significant statistically (p>0.05). 2. Size of superior TMJ space was tend to increase on the working side and decrease on the non-working side according to increasing the occlusal force (p<0.05). 3. Size of posterior TMJ space was tend to decrease on both working ad nonworking side, but non significant statistically.

Septic arthritis of the temporomandibular joint: a case report

  • Septic arthritis of the temporomandibular joint (TMJ) is a rare disease. The most common symptoms of this disease are acute malocclusion, limited mouth opening, swelling, and tenderness of affected TMJ. These symptoms are often confused with internal derangement of the articular disc, rheumatoid arthritis, retrodiscitis, or osteoarthritis. Therefore, differential diagnosis by image examination is required. Usually, antimicrobial treatment and surgical drainage by needle aspiration, arthroscopy, or arthrotomy are effective treatment approaches. In this study, a patient who was diagnosed with septic arthritis was treated with arthrocentesis and antibiotics without significant complications. We present a case report with a review of the literature.
