The Effect of Defecation on Blood Pressure in Stroke Patients (배편이 중풍 환자의 혈압에 미치는 영향)

  • Background : Patient, family, society and country are badly damaged by stroke (CVA : cerebrovascular accident). To reduce sequelae and return rapidly to society, treatment in the acute stage is very important. In many studies on purgation therapy (下法) used in acute stages of stroke, the changes of blood pressure, pulse and so on before and after purgation therapy were reported. However, the changes in the human body according to the day elapsed after defecation were not reported, so study was needed to confirm such changes. Methods : Seventy-six patients were studied. We observed forty-three patients at acute stroke and thirty-three patients at convalescent stroke. To confirm the importance of regular defecation in stroke patients, we analyzed blood pressure according to the day elapsed after defecation. Results : The following result were obtained. 1. In stroke group, compared with the day after defecation, the mean blood pressure of the third day after defecation increased. It increased the most in the acute stroke group. 2. Especially in stroke and acute stroke groups, compared with the second day after defecation, the mean systolic blood pressure of the third day after defecation increased significantly. 3. In the infarction group, compared with the day after defecation, the mean blood pressure of the third day after defecation increased. It increased the most in the acute infarction group. 4. Especially, in the infarction and acute infarction groups, compared with the second day after defecation, the mean systolic blood pressure of the third day after defecation increased significantly. Conclusion : In stroke patients, regular defecation is important, so we think that purgation therapy (下法) is needed in stroke patients over the third day elapsed after defecation. We think that the bowel movement control in acute stroke patients is more important than in convalescent stroke patients.

Effects of One-time bamboo stepping on the Elderly's Blood Pressure and the Autonomic Neural System (일회성 대나무 밟기가 노인의 혈압과 자율신경계에 미치는 영향)

  • Background: The purpose of this study is to investigate the change of blood pressure and autonomic neural system through one-time bamboo stepping in hypertensive elderly people. Design: Randomized Controlled Trial. Methods: We recruited 26 elderly people aged 65 or older for study. They sat for 10 minutes in a chair, rested, and measured pre-exercise blood pressure and autonomic neural system. The blood pressure was measured three times and the mean value was used and the autonomic neural system was measured once. Bamboo stepping exercise was performed for 20 minutes. Blood pressure and autonomic neural system were measured 5 times immediately after exercise, and every 15 minutes at 60 minutes after bamboo stepping exercise. Results: There was a significant difference in the decrease of systolic blood pressure after one-time bamboo stepping exercise, the change of parasympathetic activity (HF), heart rate variability (HRV) and BPM measured immediately after exercise at 15 minutes after exercise, There was no significant difference in autonomic neural system change. Conclusion: One-time Bamboo stepping exercise is effective in decreasing systolic blood pressure of the hypertensive elderly.

Comparisons of the Prognostic Predictors of Traumatic Brain Injury According to Admission Glasgow Coma Scale Scores Based on 1- and 6-month Assessments

  • Purpose. The purpose of this study was to identify the clinical variables that predict functional and cognitive recovery at 1- and 6-month in both severe and moderate/mild traumatic brain injury patients. Methods. The subjects of this study were 82 traumatically brain-injured patients who were admitted to a Neurological Intensive Care Unit at a university hospital. Potential prognostic factors included were age, motor and pupillary response, systolic blood pressure, heart rate, and the presence of intracranial hematoma at admission. Results. The significant predictors of functional disability in severe traumatic brain injury subjects were, age, systolic blood pressure, the presence of intracranial hematoma, motor response, and heart rate at admission. In moderate/mild traumatic brain injury patients, motor response, abnormal pupil reflex, and heart rate at admission were identified as significant predictors of functional disability. On the other hand, the significant predictors of cognitive ability for severe traumatic brain injury patients were motor response and the presence of intracranial hematoma at admission, whereas those for moderate/mild patients were motor response, pupil reflex, systolic blood pressure at admission, and age. Conclusions. The results of the present study indicate that the significant predictors of TBI differ according to TBI severity on admission, outcome type, and outcome measurement time. This can be meaningful to critical care nurses for a better understanding on the prediction of brain injury patients. On the other hand, the model used in the present study appeared to produce relatively low explicabilities for functional and cognitive recovery although a direct comparison of our results with those of others is difficult due to differences in outcome definition and validation methods. This implies that other clinical variables should be added to the model used in the present study to increase its predicting power for determining functional and cognitive outcomes.

The Study on the Diameter Ratio of the Artery-PTFE Anastomosis for the Optimized Deformed Shape (변형후 형상의 최적화를 위한 동맥과 PTFE 문합의 직경비 연구)

  • In this paper we introduced optimized deformed shape to prevent the blood vessel disease caused by the discord of deformed shape in the end-to-end anastomosis. This study considered the preliminary deformed shape induced by suture in the anastomosis of artery and PTFE, artificial blood vessel, with different diameters. Then we analyzed the final deformed shape of the anastomotic part under the systolic blood pressure. 120mmHg(16.0kPa). The final deformed shape of the anstomotic part was analyzed with respect to the change of initial diameter ratio(R$_{I}$) and the PTFE thickness. Equivalent and circumferential stresses induced by the systolic blood pressure in the anastomosis were also analyzed with respect to the initial diameter ratio(R$_{I}$). The results obtained were as follows : 1. Considering the preliminary deformed shape induced by suture and the systolic pressure in the anastomosis, not intimal hyperplasia, the optimal initial diameter ratio(R$_{I}$) was 1.073. 2. As the initial diameter ratio(R$_{I}$) became larger, higher equivalent and circumferential stresses were induced. And all the maximum stresses occurred on the side of PTFE 0.4mm apart from the anastomosis.

Effects of Intaking of Red Ginseng Products on Correlationship between Obesity and Blood Lipids (홍삼류의 섭취가 비만과 혈중 지질의 상호관계에 미치는 영향)

  • Obesity is the risk factor of atherosclerosis and not only increases triglyceride concentration in blood but also decreases relatively the ratio of TG to HDL-Cholesterol in blood. In case of obesity, systolic blood pressure is also increased in responding the increase of TG in blood. Index of obesity in red ginseng-taking group (ginseng group) was lower as compared with non-red ginseng-taking group (control group). The TG concentration, the ratio of triglyceride to HDL-cholesterol in blood and systolic blood pressure were decreased in the subjects of ginseng group compared with that in control group. It is inferred that long-term intake of ginseng products may help to prevent the risk of atherosclerosis and obesity.

The Effects of Visual and Auditory Feedback on Pain Reduce (시각과 청각되먹임이 통증감소에 미치는 영향)

  • This study set out to investigate what kind of effects the consistent visual stimuli and verbal and non verbal auditory stimuli have on pain alleviation, as well as to see the influence of joint application of visual and auditory stimuli at the same time on pain alleviation, according to lightness of 50lux and 200lux, ultimately providing basic data in setting up an environment in case of treating pain. The subject were comprised of 30 male and female adults with pain in the neck and back area. The subject were treated in their pain area with Transcutaneous Electrical Nerve Stimulator(TENS) 100HZ for 20 minutes in the research set where each visual, auditory, and joint visual and auditory stimuli was given. For analysis methods, Visual Analogue Scale(VAS) and McGill Pain Questionnaire were adopted to see the changes before and after treatment, and the electrocardiogram, systolic and diastolic pressure, number of heart rate and breathing frequence and endorphin were compared and analyzed using the Wilcoxon singed-rank test. And The Kreskal-walllis test was used to compare the two subgroups from each group. Wilcoxon singed-rank test and the Kreskal-walllis test was used to compare the two subgroups from each group. The results were as follows: 1. The group of 50lux and 200lux were compared given varying degrees of visual stimuli. The group of 200lux showed more reduction in pain points, average systolic and diastolic pressure and average endorphin. 2. The group of verbal and non verbal were compared given varying degrees of auditory stimuli. The group of non-verbal showed more reduction in average systolic and diastolic pressure. 3. The group of 200lux+verbal and 200lux+non verbal were compared given varying degrees of joint visual and auditory stimuli. There was found a statistical significance(p<0.05) in endorphin between the two groups, with more endorphin reduction for 200lux+non verbal group. And there was a statistically significant reduction in VAS and McGill before and after the treatment between the two groups.

A Study of Dietary Intake, Physical Status and Biochemical Status of Children in Taejon (대전지역 학령기 아동의 체위, 혈액성상 및 영양실태에 관한 연구)

  • This study was conducted to investigate the relationship among Rohrer index, and nutritional intake and biochemical status. We examined physical status, dietary intakes, serum cholesterol, blood pressure and other biochemical of children in Taejon. Thin survey was carried out in October, 1995. The subjects were 362 children, aged from 6 to 11 years old. The results were summarized as follows. By Rohrer index, 4.5% of the subjects were lean, 70.2% were normal, 17.0% were overweight and 8.6% were obese. Systolic and diastolic blood pressures of the subjects were 111.73 and 69.88mmHg, respectively. The systolic blood pressure of obese subjects(119.93mmHg) was higher than any other groups. Total cholesterol level was 160.87mg/dl, cholesterol levels in each group were not significantly different. Hemoglobin level of total subjects was 13.10mg/dl, especially hemoglobin levels of 11 year-old girls was lower than that of the same aged boys suggesting that the girls should be supplied with more protein, iron and other nutrients. Energy and protein intakes wee 86.6% and 94.5% of RDA, respectively. The nutrients above the RDA were only two, phosphorus and-vitamin C. The intakes of iron, zinc and vitamin A were lower than 50% of the RDA's. The levels of zinc and niacin were highest in obese group than any other groups. The subjects in overweight and obese groups had eaten more calories from snack and less calories from breakfast than other groups. Rohrer index was correlated with cholesterol, systolic blood pressure and creatinine levels.

The utility of ambulatory blood pressure monitoring in obese children (소아 비만아에서 ambulatory blood pressure monitoring의 유용성)

  • Purpose : Obesity in children and adolescence is highly correlated with adult obesity, which can provoke hypertension. Therefore, it is important to evaluate the blood pressure of obese children regularly. In this study, the results of ambulatory blood pressure monitoring (ABPM) in obese children and adolescents were evaluated. Methods : ABPM was conducted for selected patients who visited Handong University Sunlin Hospital from Feb. 1, 2006 to Dec. 1, 2007. The patients were classified into 3 groups: group 1 was normal, group 2 had normal casual blood pressure and a body mass index over the 95th percentile, and group 3 had high casual blood pressure over 120/80 mm Hg and a body mass index over the 95th percentile. Systolic and diastolic 24-hour blood pressure was measured, including both day and night. Results : There were 49 patients in the study. The results showed a significant difference for average systolic blood pressure between the three groups ($105.1{\pm}4.7$, $111.0{\pm}7.1$, $117.8{\pm}6.6mmHg$, P<0.001), but for average diastolic blood pressure only between groups 1 and 3 ($69.1{\pm}5.3$, $77.9{\pm}6.3mmHg$, P=0.001). In the daytime, only groups 1 and 3 showed a statistically significant difference for systolic and diastolic blood pressure. During the night the systolic pressure of group 3 was significantly higher than the other groups, but the diastolic pressure of group 3 was only higher than that group 1. No statistical difference was found in night dips among the groups. Conclusion : Ambulatory blood pressure monitoring in children and adolescents showed statistically higher blood pressure in obese patients with high casual blood pressure.

The Association between Blood Pressure and Obstructive Sleep Apnea-Hypopnea Syndrome

  • Obstructive sleep apnea-hypopnea (OSAH) is known to be related to nocturnal blood pressure (BP) and hypertension. The aim of this study was to evaluate the prevalence of hypertension according to the apnea-hypopnea grading. A total of 2,210 adults with snoring and obstructive sleep apnea were referred to our sleep center from July 2009 to May 2013. Clinical blood pressure (BP) was measured before sleeping (bedtime BP) and immediately after waking up in the next morning (morning BP). Subjects were classified into four groups based on the apnea-hypopnea index (AHI) from PSG as follows: control group (n=470) simple snoring and with AHI<5; mild group (n=577) with $AHI{\geq}5$ and <15; moderate group (n=508) $AHI{\geq}15$ and <30; and severe group (n=655) with $AHI{\geq}30$. The differences and correlations between BP and PSG parameters according to the AHI groups were analyzed. Patient's were classified as nomentensive (blood pressure <120/90 mmHg, n=700), prehypertensive (blood pressure < $140-120{\leq}mmHg$, n=1297) hypertensive (blood pressure ${\geq}140/90mmHg$, n=214) according to the office blood pressure measurements. The comparison of sleep parameters showed that OSA groups had a significantly higher stage N1 (control group vs. moderate OSA, severe OSA; $66.4{\pm}30.7$ vs. $85.5{\pm}36.6$, $128.4{\pm}57.3$, p<0.001) and total arousal number (control vs. moderate OSA, severe OSA; $110.7{\pm}47.7$ vs. $150.8{\pm}56.6$, $236.6{\pm}95.8$, p<0.001) compared to control group. The comparison of sleep parameters showed that OSA groups had a significantly lower stage N2 (control group vs. moderate OSA, severe OSA; $172.6{\pm}47.2$ vs. $150.7{\pm}50.5$, $120.3{\pm}57.4$, p<0.001), stage N3 (control group vs. moderate OSA, severe OSA; $38.4{\pm}33.4$ vs. $27.4{\pm}26.0$, $56.1{\pm}27.5$, p<0.001), REM (control group vs. moderate OSA, severe OSA; $64.3{\pm}25.5$ vs. $56.1{\pm}27.5$, $47.3{\pm}25.9$, p<0.001) and mean SaO2% (control group vs. moderate OSA, severe OSA; $90.0{\pm}3.5$ vs. $82.5{\pm}5.5$, $70.0{\pm}8.8$, p<0.001) compared to control group. The Apnea-hypopnea index was significantly higher in OSA groups, increased systolic and diastolic blood pressure than in the nomentensive group (bed time systolic pressure vs. AHI; <120 vs. 120-139, 140-159, >159; $17.5{\pm}18.6$ vs. $24.9{\pm}21.0$, $31.0{\pm}25.7$, $42.3{\pm}31.7$, p<0.001), (bed time diastolic pressure vs. AHI; 60-79 vs. 80-89, 90-99, >99; $19.3{\pm}19.7$ vs. $22.4{\pm}20.3$, $29.8{\pm}23.3$, $38.8{\pm}28.5$, p<0.001). AHI was positively correlated with morning systolic pressure, diastolic pressure, bed time systolic pressure and diastolic pressure (r=0.314, 0.279, 0.233 and 0.200, respectively, p<0.001). We conclude that BMI, Age, neck circumference and AHI increase with the blood pressure.

The Effect of Auricular Acupuncture Therapy on Blood Pressure (이침시술이 혈압에 미치는 영향)

  • Objectives : Hypertension is very prevalent disease, and causes serious cardiovascular complications. Nowadays optimal hypertension treatment is emphasized to reduce the incidence of cardiovascular complications. Auricular acupuncture therapy is economical, safe and effective. Its clinical application is various, and it can be used to control blood pressure. So The effect of auricular acupuncture therapy was investigated. Daily variation of blood pressure during needle-embedded period and variation of blood pressure by blood pressure classification were observed. Methods : The auricular acupuncture points we used were Gangapjeom (降壓點), Sinmun (神門), Gyogam (交感), Sim(心), and Icheom (耳尖). Auricular acupuncture needles were embedded for three days. Blood pressure was checked four times per day and the mean obtained. Results : The following results were obtained : 1. During needle-embedded period, systolic and diastolic blood pressure decreased. Blood pressure decreased most significantly 2 days after treatment. 2. The change of blood pressure in the higher blood pressure group was more remarkable than that of the lower blood pressure group. Conclusions : Through this research, auricular acupuncture therapy is considered as an effective and safe method to lower blood pressure.
