Clinical Effects of Preoperative treated-Methylprednisolone in Pediatric Cardiac Surgery with Cardiopulmonary Bypass

  • Cardiopulmonary bypass (CPB) for cardiac surgery induces the production and release of numerous chemotactic substances and cytokines, ensuing systemic inflammatory response that causes postoperative major organ dysfunctions. We performed a randomized, prospective study to investigate clinical effects of preoperative treated-methylprednisolone for preventing inflammation in pediatric cardiac surgery with CPB. Thirty pediatric patients scheduled for elective cardiac surgery were randomized to either control(n=15) or steroid group (n=15, 10 mg/kg of methylprednisolone). Arterial blood samples were taken before and after the operations for measuring total leukocyte (T-WBC) and differential counts, platelet counts, interleukin-6 (IL-6), myeloperoxidase (MPO), neuron specific enolase (NSE), troponin-I (TNI), aspartate aminotransferase (AST), alanine aminotransferase (ALT), blood urea nitrogen (BUN), and creatinine levels. Postoperative parameters such as pulmonary index (PI, $PaO_2/FiO_2$), 24 hrs and total bleeding volumes, mechanical ventilating (MVP) and intensive care unit (ICU)-staying periods, and hospitalization were assessed. T-WBC, neutrophil fraction, IL-6, MPO, NSE, TNI, AST and creatinine levels, bleeding volumes, PI, and MVP at the postoperative periods were lower or shorter in steroid group than in control group (P<0.05). These findings indicated that preoperative administration of methylprednisolone attenuated CPB-induced inflammatory reactions, contributing to postoperative recovery of patients underwent cardiac surgery.

  • Cisplatin-based concurrent chemoradiation plays an undisputed key role as definitive treatment in unresectable patients with locally advanced squamous cell carcinoma head and neck or as an organ preservation strategy. Treatment with 100 mg/m2 3-weekly cisplatin is considered the standard of care but is often associated with several adverse events. The optimum drug schedule of administration remains to be defined and presently, there is insufficient data limiting conclusions about the relative tolerability of one regimen over the other. This review addresses regarding the optimal dose schedule of cisplatin focusing mainly on three-weekly and weekly dose of cisplatin based concurrent chemoradiotherapy in locally advanced head and neck cancer with an emphasis on mucositis, dermatitis, systemic toxicity, compliance, and treatment interruptions. To derive a definitive conclusion, large prospective randomized trials are needed directly comparing standard 3-weekly cisplatin ($100mg/m^2$) with weekly schedule ($30-40mg/m^2$) of concurrent cisplatin based chemoradiotherapy in locally advanced squamous cell carcinoma head and neck.

The Effect of Supplementary Selenium on Leukocytes and HSP70 Expression after Half-Body Immersion (반신욕 중 셀레늄 섭취가 백혈구와 HSP70 발현에 미치는 영향)

  • This study examined the effect of supplementary selenium on leukocytes and heat shock protein (HSP) 70 expression in serum during half-body immersion. The subjects were male college tennis athletes. All subjects participated in two repeated experiments with a 1 week interval. During the 30 min intermittent half-body immersion, subjects were given 500 mL of water with or without selenium (100 ${\mu}g$). Blood samples were taken from the antecubital vein, and differential counts were made. Serum HSP70 protein was analyzed using a commercial ELISA kit. After half-body immersion, leukocytes and lymphocytes increased significantly but neutrophils decreased significantly in both trials (with or without selenium). Selenium supplementation, compared with placebo, decreased levels of leukocytes, neutrophils, and monocytes, but not lymphocytes, to the resting level or below 60 min after immersion. Only lymphocytes continued to increase in both trials during the recovery period. Serum HSP70 protein level did not change after immersion, but it decreased 60 min after immersion with the administration of selenium. In conclusion, supplementary selenium reduced the systemic immune response and serum HSP70 protein accumulation after half-body immersion.

Spontaneous Intraorbital Hemorrhage : A Case Report

  • Intraorbital hemorrhage is a rare clinical condition caused by orbital trauma, surgery around the orbit, intraorbital vascular abnormalities, and neoplasm. It was reported to occur spontaneously without any known causes and in association with orbital pseudotumor in a very few cases. A 59-year-old, female patient admitted with sudden onset of severe exophthalmos and pain on the left eye. Orbital CT and MR imaging suggested hemorrhage in the upper part of retrobulbar area of the left orbit. Cerebral angiography was taken to rule out any possible vascular abnormalities. On the left carotid cerebral angiography, the run-off of the distal ophthalmic artery was not seen and the engorgement of the supraophthalmic artery was noted. Systemic administration of corticosteroid did not improve the clinical status and craniectomy was done and retrobulbar hematoma was removed, and the clinical symptoms and signs were improved. Authors report a case of spontaneous intraorbital hemorrhage with the clinical features similar to those of orbital pseudotumor, requiring surgical decompression.

  • Carbamates have excellent insecticidal activities against a broad spectrum of insects. They possess knocking-down, fast-killing, and systemic effects, however, they are toxic to mammals. In this study, we have carried out in vitro genetic toxicity test of methylcarbamate and microarray analysis of differentially expressed genes in response to methylcarbamate. Methylcarbamate did not show mutations in base substitution strain TA1535 both with and without exogenous metabolic activation. Methylcarbamate did not show mutations in frame shift TA98 both with and without exogenous metabolic activation. Methylcarbamate showed DNA damage based on single cell gel/comet assay in L5178Y cells both with and without exogenous metabolic activation. Methylcarbamate did not increase micronuclei in CHO cells both with and without exogenous metabolic activation. Microarray analysis of gene expression profiles in L5178Y cells in response to methylcarbamate selected differentially expressed 132 genes that could be candidate biomarkers of genetic toxic action of methylcarbamate.

A survey on recognition about water fluoridation of the primary teachers at non-fluridated area in Jinhae (수돗물불소농도조정사업 비 혜택지역 진해시 초등학교 교사의 사업 인지도 조사연구)

  • The purpose of this study was to take inforamtion to increase support on water fluoridation primary school teachers in Jinhae. The survey was done by self-administration method to primary school teachers 88 at non-fluoridated area in Jinhae. The date was analysed using the SPSS 10.1 program. The results are as follows : The mean percentage of the respondents using the tap water and purified water for drinking was 51.1% and 33.3%, respectively. Recognition rate of water fluoridation was 64.8%, 33.3% of the respondents knew about water fluoridation from mass mesdia. 62.5% of the respondents agreed with water fluoridation. 76.1% of the respondents knew that the purpose of water fluoridation was the prevention of dental caries. 72.7% of the respondents agreed with continuous water fluoridation. 70.5% of the recognized about safety of water fluoridation. 72.7% of the respondents agreed with decision of professional group for the performance of water fluoridation. In order to secure residents' support for the continuation of the adjusted water fluoridation program and to reinforce the education for safety and economical efficiency for the residents through systemic education.

  • Recently there has been an increase in annual per capita consumption of alcohol beverage and the incidence of Alcoholic liver disease is steadily and significantly increasing. Alcoholic liver disease includes alcoholic fatty liver, alcoholic hepatitis, alcoholic cirrhosis and it may lead to systemic influence, in a case of CVA. This report is about one case's treatment for ICH with alcoholic liver disease. In this case, we administrated Taeumjowetang and did acupuncture treatment to a patient suffering from ICH with alcoholic liver disease and its withdrawal symptoms. After administration of Taeumjowetang medication, clinical symptoms and liver function were prominently improved. This report showed that Taeumjowetang might be useful for alcoholic liver disease.

Productivity of the Health Center and Efficient Inputs & Outputs in Kyungnam Province (보건소 보건사업의 효율성 평가와 정책적 의의 - DEA를 이용한 경상남도 사례분석 -)

  • The main purpose of this research is to measure and analyze how productive the health centers are and how much inputs(or outputs) in the inefficient health centers should be decreased(or increased) for efficiency. The evaluation of the performance of health centers gives a strong incentive to improve resource allocation in regional health planning. It has been, however, implemented very rarely until now. In this regard, this paper measures the performance of the health centers with a single indicator for multiple-outputs in terms of productivity(technical efficiency), based on Data Envelopment method. The 20 Kyungnam health centers which provide clinic services and specific primary health care services were analyzed. The results show that 50.0% of 20 health centers were productive with respective to overall technical efficiency, 65.0% were productive for pure technical efficiency, and 50.0% for scale efficiency. The inefficient group includes Geoje, Mahsan, Miryang, Sahchun, Tongyoung, Gosung, Nahmhae, Euryang, Hahmahn, Hahbchun health centers. The worst case was identified as Tongyoung health center which represented a 47.5% efficiency, compared with productive health centers. The empirical results for input-output analysis indicates that the low-productive health centers have excessive manpower in administration department, producing low outputs in clinical services and vaccination program. These findings imply that a systemic evaluation of the performance of the Korean health centers and the subsequent structural reform are strongly required.

Neurotoxic Desensitizing Effect of Capsaicin on Peripheral Sensory Nerve Endings in Guinea Pig Bronchi (기니픽 기관지 말초신경에 대한 캡사이신의 탈감작 효과)

  • In the present study, capsaicin-induced desensitization of peripheral sensory nerves were investigated by using guinea pig bronchi, in which these nerves are stimulated with cap saicin to produce a contractile response via the release of sensory neuropeptides such as substance P and neurokinin A. The contractile response to capsaicin was inhibited by the combination of CP96345 and SR 48968 suggesting that the excitatory effect of capsaicin is mediated via both the tachykinin NK-1 and NK-2 receptor. Capsaicin produced in vitro-desensitization in dose-dependent manner, but after this in vitro-desensitization the response to NK-1 and NK-2 receptor agonist did not change. Systemic administration (s.c.) of capsaicin also desensitized significantly bronchial tissues but could not produce any change in the contractile response to the selective agonists of NK-1 and NK-2 receptor. Therefore, the present results suggest that functional desensitization to capsaicin-induced contractile response in guinea pig bronchi does not involve NK-1 and NK-2 receptor, while excitatory effect of capsaicin is mediated via both NK-1 and NK-2 receptor. In conclusion, it is suggested that capsaicin- induced excitation and desensitization involves somewhat different pathways.

Antiasthmatic Effects on Scutellaria baicalensis Georgi Extracts Against Airway Inflammation and Hyperresponsiveness Induced by Diesel Exhaust Particles with Ovalbumin Sensitization (Ovalbumin과 디젤배기가스 입자로 유도된 기도염증과 기도 과민성에 대한 황금 추출물의 항천식 효과)

  • The feature of asthma are airway inflammation (AI), reversible airway obstruction, and an increased sensitivity to bronchoconstricting agents, elevated airway hyperresponsiveness (AHR), excess production of Th2 cytokines, and eosinophil accumulation in the lungs. This study was performed to investigate if oral administration of $Scutellaria$ $baicalensis$ Georgi water extracts (SBG) have the antiasthmatic potential for the treatment of asthma. Asthmatic HI and AHR were induced by systemic sensitization to ovalbumin (OVA) with intratracheal instillation with 0.1 mg/mL of diesel exhaust particles (DEP) suspension once a week for 10 weeks in BALB/c mice. SBG was orally administered with the concentraion of 200 mg/kg 5 days a week for 10 weeks. Long-term SBG treatment suppressed the eosinophil infiltration into airways from blood, the asthmatic AI and AHR by attenuating the production of cytokine IL-4, IL-5 and IL-13, histamine and OVA-specific IgE. Our data suggest that SBG has inhibitory effects on AI and AHR in a mouse model of asthma, may act as a potential Th2 cytokine antagonist, and may have a therapeutic effect on allergic asthma.
