Guillain-barré Syndrome after Multiple Bee Stings (다발성 벌 자상에 의한 길랑 바레 증후군 1례)

  • Severe systemic responses including neurologic complications such as myasthenia gravis, myeloradiculopathy, optic neuropathy, parkinsonism, stroke and Guillain-$barr{\acute{e}}$ syndrome can occur after bee stings. This case describes a 78-year-old female who presented with symptoms of acute progressive bilateral symmetrical weakness in both lower legs after multiple bee stings. Nerve conduction study findings were consistent with acute sensorimotor axonal neuropathy and recovered by treatment with intravenous immunoglobulin. This case highlights that bee stings can result in acute onset Guillain-$barr{\acute{e}}$ syndrome, although the pathophysiologies of bee venoms need to be investigated accurately.

Predictors of a Favorable Outcome after Emergent Carotid Artery Stenting in Acute Anterior Circulation Stroke Patients (급성 전방순환 뇌경색 환자에서 응급 경동맥 스텐트 삽입술 후 양호한 임상 결과의 예측인자)

  • Purpose This study aimed to identify independent predictors of favorable outcomes associated with emergent carotid artery stenting (CAS) in patients with acute anterior circulation stroke. Materials and Methods This study included 93 patients with acute stroke who underwent emergent CAS to treat stenoocclusive lesions in the cervical internal carotid artery (ICA) within 6 hours of the onset of the associated symptoms. Data were compared between patients with and without favorable outcomes. The independent predictors of a favorable outcome were determined via logistic regression analysis (modified Rankin Scale 0-2 at 90 days). Results Intracranial tandem occlusion was noted in 81.7% of patients (76/93) among which (76/93), 55 of whom underwent intracranial recanalization therapy. Intracranial reperfusion was successful in 74.2% (69/93) and favorable outcomes were noted in 51.6% of patients (48/93). The mortality rate was 6.5% (6/93). In logistic regression analysis, diffusion-weighted imaging-Alberta Stroke Program Early CT Score [odds ratio (OR), 1.487; 95% confidence interval (CI), 1.018-2.173, p = 0.04], successful reperfusion (OR, 5.199; 95% CI, 1.566-17.265, p = 0.007), and parenchymal hemorrhage (OR, 0.042; 95% CI, 0.003-0.522, p = 0.014) were independently associated with a favorable outcome. Conclusion Baseline infarct size, reperfusion status, and parenchymal hemorrhage were independent predictors of favorable outcomes after emergent CAS to treat stenoocclusive lesions in the cervical ICA in patients with acute anterior circulation stroke.

A Case Report of Korean Medical Treatment on Atypical Parkinsonism Patient Complaining of Gait Disturbance Using 3-Demensional Gait Analysis System (3차원 보행분석기를 통해 보행장애의 호전이 확인된 비정형 파킨슨증후군 환자 한방치험 1례)

  • ■Objectives This case study is to report the effects of Korean medicine on parkinsonism patient's Gait Disturbance. ■Methods During 12 days of hospitalization, the patient was treated by acupuncture, pharmaco-acupuncture, moxibustion, herbal medicine, especially Cheongsimyeonjatang-gamibang. In order to assess the change of symptoms, we used a 3-Dimensional(3D) gait analysis system, Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait video and self-evaluation of discomfort. ■Results After treatment, The improvements of walking pattern were observed in both objective analysis results of gait analysis system and subjective video analysis. And the UPDRS score decreased, especially Part III score decreased more than minimal clinically important difference(MCID). In addition, There was improvement in self assessment of the patient. ■Conclusion This study suggests that Korean medical treatment might be effective in motor disorder of parkinsonism patient.

Endovascular Approach in Patients with Acute Complete Occlusion Due to Middle Cerebral Artery Dissection

  • Objective : Dissection of the middle cerebral artery (MCA) is less common than dissection of vessels in the vertebrobasilar system or carotid artery. Acute complete occlusion related to MCA dissection is extremely rare. We report an endovascular approach in patients with acute complete occlusion due to MCA dissection. Methods : We reviewed retrospectively the endovascular procedure and clinical results for acute-stroke patients who underwent recanalization from October 2014 through December 2018. Initial imaging findings and the endovascular procedure were analyzed for patients with acute complete occlusion due to MCA dissection. Results : We undertook first-line aspiration thrombectomy using a Penumbra catheter in 294 patients with acute occlusion of the M1 segment. Of these patients, seven were confirmed to have acute complete occlusion due to MCA dissection. All patients had angiographic findings of an intimal flap at the proximal occlusion site of the MCA. One patient complained of severe headache during microcatheter passage through the occluded lesion and died due to massive bleeding caused by rupture of the false lumen. The remaining patients underwent initial contact aspiration thrombectomy without microcatheter passage. After aspiration thrombectomy, six patients had delayed flow through the MCA. One patient underwent stenting of the MCA because of progressive symptoms. Conclusion : An intimal flap at the proximal portion of an occluded MCA can suggest the possibility of MCA dissection. Contrast aspiration thrombectomy without microcatheter passage can reduce the risk of false lumen rupture in cases of MCA dissection.

  • Restless leg syndrome(RLS) is a sensorimotor disorder characterized by a distressing urge to move the legs and triggered by rest or inactivity. It is usually accompanied with insomnia. We experienced two cases of RLS among ischemic stroke patients. They complained uncomfortable sensation in their affected leg and urge to move the leg at night. We treated them with electro-acupuncture, moxibustion and herbal medicine until retiring. As a result, the symptoms including unpleasant sensation or pain, urge to move the leg and insomnia had withdrawn. These cases suggest the possibility that we can treat RLS with oriental medicine, especially in the case which is refractory to conventional medication or shows periodic leg movements during sleep.

Comparison for Risk Estimate of Aspiration between the Revised Dysphagia Assessment Tool and Videofluoroscopy in Post-Stroke Patients (수정된 연하곤란사정도구와 비디오 연하영상 조영술의 흡인 위험 예측비교)

  • Purpose: The purpose of this study was to determine the significant factors for risk estimate of aspiration and to evaluate the efficiency of the dysphagia assessment tool. Methods: A consecutive series of 210 stroke patients with aspiration symptoms such as cough and dysphagia who had soft or regular diet without tube feeding were examined. The dysphagia assessment tool for aspiration was compared with videofluoroscopy using Classification and Regression Tree (CART) analysis. Results: In CART analysis, of 34 factors, the significant factors for estimating risk of aspiration were cough during swallowing, oral stasis, facial symmetry, salivary drooling, and cough after swallowing. The risk estimate error of the revised dysphagia assessment tool was 25.2%, equal to that of videofluoroscopy. Conclusion: The results indicate that the dysphagia assessment tool developed and examined in this study was potentially useful in the clinical field and the primary risk estimating factor was cough during swallowing. Oral stasis, facial symmetry, salivary drooling, cough after swallowing were other significant factors, and based on these results, the dysphagia assessment tool for aspiration was revised and complemented.

One Case of Neck pain Managed by Yinyang balance appliance of FCST for the Meridian and Neurologic Balance (FCST의 음양균형장치를 활용한 경항통 증례보고)

  • Objective : Therapeutic effect of Balance Appliance of functional cerebrospinal technique (FCST) for meridian and neurologic yinyang balance was observed in one neck pain case. Method : A neck pain one case with 3 months of duration was managed by the Balance Appliance on temporomandibular joint (TMJ), combined with acupuncture and manual medicine. Results : Assessment was made by self assessment of subjective symptoms and clinical observation. The patient reported over-90% remission and returned to ordinary daily life after 1 months of therapy. Conclusions : An impressive effect was observed and further clinical and biological research on FCST is expected.

  • ■ Objectives The purpose of this case is to report the effectiveness of Korean medicine in the treatment of early stage multiple system atrophy-cerebellar ataxia(MSA-C) ■ Methods The patient was treated with acupuncture, bee venom, and herbal medicine. The clinical symptoms was assessed using unified multiple system atrophy rating scale(UMSARS) and 20M gait time and steps. ■ Results After treatment, UMSARS score and 20M gait time and steps showed improvement. ■ Conclusion This study suggested that Korean medical treatment could be an effective option for treating early stage MSA-C.

  • ■ Objectives In this case report, we address a case of a 83-year-old female patient diagnosed with suspicious autoimmune hepatitis. ■ Methods A patient was treated with herbal medication, Injinoryung-san gagam. We evaluated the improvements of symptoms by AST, ALT, GGT, ALP and total score of Fatigue Severity Scale(FSS). ■ Results After 4 weeks of taking Injinoryung-san gagam, there were decrease in abnormal liver enzyme levels with improvement of the patient's fatigue. ■ Conclusion This study suggested that Injinoryung-san gagam might be effective in autoimmune hepatitis patient's liver enzyme levels and fatigue recovery.

  • Psychogenic Non Epileptic Seizure (PNES) refers to a case in which symptoms are similar to epileptic seizures (ES), but ES do not appear in EEG and temporary abnormalities caused by physical diseases are also excluded. Psychotherapy and pharmacotherapy are most commonly used, but standard treatment for PNES has not been established and there is no alternative treatment available in the case of patients those treatments are not applied. We herein report a case of 46 year old male diagnosed with PNES who suffered from generalized tonic-clonic seizures. The patient underwent korean medical treatment by acupuncture and herbal medicine. The clinical symptom was evaluated by assessing occurrence frequency, number of seizures per 28 days. Compared to the period without korean medical treatment, the number of seizures decreased during the treatment period. The present case report suggested that korean medical treatment could be an effective option for those PNES patients complaining generalized tonic-clonic seizures.
