A Case of Torticollis with Korean Medicine (사경증 환자의 한방 치험 1례)

  • ■ Objectives This clinical report is to report the effect of Korean medicine on a patient with torticollis. ■ Methods We used herbal medicine, acupuncture, moxibustion, pharmaco-acupuncture and bee venom acupuncture to treat a torticollis patient during hospitalization and out patient department (OPD) treatment for 3months. We observed the changes of symptoms using Toronto Western Spasmodic torticollis Scale (TWSTRS). ■ Results After treatment, the patient's symptoms including torticollis, pain and the general condition were improved. The TWSTRS scores were also improved after treatment. ■ Conclusion This clinical case study suggests that Korean medicine treatment could be effective in the treatment of torticollis.

A Case of Nonconvulsive Status Epilepticus Patient Complaining Delirium Treated with Combined Korean Medical Treatment (섬망을 호소하는 비경련성 간질 중첩증 환자에 대한 복합 한의치험 1례)

  • Nonconvulsive Status Epilepticus(NCSE) refers to continuous epileptic condition with altered mental status and behavioral symptoms, but without convulsive movement at least 10 minutes. In Salzburg criteria, well known as the diagnostic criteria of NCSE, it is diagnosed with clinical symptoms, electroencephalogram (EEG), and effects before and after the use of antiepileptic drugs (AEDs). Commonly being used to treat NCSE, AEDs are likely to have adverse effects. In the present case, a 85-year-old female NCSE patient complaining delirium underwent combined Korean medical treatment by acupuncture and herbal medicine for 13 days. The effect of treatment was assessed with delirium scales including Delirium Rating Scale-Revised-98(DRS-R-98) and Assessment Test for Delirium & Cognitive Impairment(4AT) per 2 days. After the treatment, both DRS-R-98 and 4AT scores decreased, and orientation and mental status of patient improved. This case report suggests that Korean medical treatment might be an effective option without side for those NCSE patients complaining delirium.

  • A case of a 73-year-old male patient who fell down, presented with unusual traumatic subdural hematoma localized on the lesion of tentorium cerebelli. He was treated with acupuncture, electroacupuncture and herbal medicine - 五苓散(Oreong-san). To evaluate the progress, we followed up computed tomography imaging and checked up changing of symptoms. After 17days of treatment, there were notable improvement in computed tomography imaging and symptoms. Oreong-san might be effective in treating tentorium cerebelli subdural hematoma.

  • ■ Objectives This case is to report the effect of Oryeong-san and acupuncture therapy on serum creatinine level of a cerebral infarction patient with diabetic nephropathy. ■ Methods A female Korean patient was treated with Oryeong-san, acupuncture for total 32 days. We observed renal function test, input/output balance, body weight, hand and foot circumferences, and other symptoms like edema, urination, and any adverse event. ■ Results After treatment, serum creatinine level was improved to 2.48mg/dL from 3.45mg/dL at admission, and foot circumference was decreased to 18.5~18.9cm from 22.0~22.5cm without any adverse event. However, we couldn't find any significant differences on input/output balance, body weight, or symptoms of urination. ■ Conclusion This case suggests that Oryeong-san and acupuncture therapy could be effective in improving serum creatinine clearance of cerebral infarction patient with diabetic nephropathy.

A Review of Korean Studies on Amyotrophic Lateral Sclerosis Treated with Traditional Korean Medicine (근위축성 측삭경화증 환자의 한방치료에 대한 국내 연구 문헌 고찰)

  • ■ Objectives This study aimed to investigate the domestic research trend about Amyotrophic Lateral Sclerosis(ALS) treated with Traditional Korean Medicine(TKM). ■ Methods Searches of two major TKM databases were conducted for articles published up to July 2015. Searching keyword was "Lou Gehrig's disease" OR "amyotrophic lateral sclerosis". Studies dealt TKM for ALS were included. ■ Results 18 studies were met our inclusion criteria(two prospective studies, one retrospective study, one follow-up study, 14 case reports). Complex TKM intervention of herbal medication, acupuncture and pharmacopuncture was the most frequently used treatment. Individually, herbal medication was the most frequent followed by acupuncture. Short-term assessment showed improvement in ALS symptoms but long-term assessment showed progression of disease. Accessory symptoms including depression, anxiety and insomnia were improved in four studies. ■ Conclusion For more definite evidence, larger studies with long-term follow-up period are needed. Moreover, benefits that TKM can provide for ALS patients as a part of multi-disciplinary approach should be studied in depth.

  • Background: Alzheimer's disease (AD) is a chronic neurodegenerative disease that causes disorientation, mood swings, problems with language, and difficulty remembering recent events. Acetylcholinesterase inhibitors (AchEIs) and memantine have been used to slow the course of the disease, but they can neither modify its progression nor prevent disease onset. Previous studies have suggested that Kami-guibi-tang (KGT) could be beneficial for supporting cognitive function in AD patients, but few clinical trials have been published. This pilot study aimed to evaluate the effect of KGT in improving cognitive function in AD patients. Methods: The study will be a randomized, placebo-controlled, double-blind, single-center trial conducted using subjects diagnosed with mild AD by neurologists. Study subjects will be randomly assigned to either a treatment or control group. The treatment group will receive KGT granules for 24 weeks, while the control group will receive placebo granules. AchEI administration will be maintained in both groups during the entirety of the study. Subjects will be assessed using the following exams: the Seoul Neuropsychologic Screening Battery (SNSB) for cognitive function; brain magnetic resonance imaging (MRI) for brain metabolite, neurotransmitter, and cerebral blood flow (CBF) measurements; the Korean version of Quality of Life-Alzheimer's Disease (KQol-AD) for quality of life; the Caregiver-Administered Neuropsychiatric Inventory (CGA-NPI) for neurobehavioral symptoms; blood tests for amyloid and tau proteins and general blood parameters; and electrocardiography (ECG) before and after taking the medication. Discussion: Our findings will provide insight into the feasibility of large-scale trials to consolidate evidence for the efficacy of KGT for dementia treatment.   • "Byun Jeung Rok(辨證錄)" written by Jinsatak(陳士鐸) is composed of 14 volumes. In relation to the contents, it is organized into 126 gates(門) and 770 remaining syndromes(餘證) where internal medicine, external medicine, pediatrics, gynecology(內科 外科 小兒 婦人), etc. are divided into sub-sections of cold damage, cold stroke, wind stroke(傷寒 中寒 中風), etc. He explained the symptom, cause of disease, method of treatment, prescription, construction of prescription, instruction of medicine and prognosis(症狀 病因 治法 處方 處方構成 服用法 預後) thoroughly. This study, as an inquiry of the sec 14 volume pediatrics(小兒), deals with febrile fit cacotrophy vomiting diarrhea(驚疳吐瀉), coprozoic parasite(便蟲), smallpox(痘瘡), eruption(疹症), eating mud(喫泥), fetal toxin(胎毒). It was written very logically so it is easy to understand. The analysis of the symptoms are brief and appropriate. Therefore, it is considered to have significant clinical value for future generations and is applied by them. Finally, this topic was studied in hopes of helping later pediatrics clinical treatment.

  • A 'pusher syndrome' encompassing postural imbalance and hemi-neglect is believed to aggravate the prognosis of stroke patients. The patients with pusher syndrome show a particular posture that pushing away from the unaffected side of the body. The objective of this study was to investigate associated proprioception, associated neuro-psychological symptoms and characteristics of functional outcome with and without pusher syndrome. The subjects of this study were 58 acute stroke patients who been rehabilitated at two university hospitals in Seoul and Buchun. Data were collected using proprioception test and line bisection test. The ability of ADL was assessed by the Modified Barthel Index, transfer by the Functional Independence Measure, and balance by the Modified Motor Assessment Scale. The results were that significant difference was found in the presence of proprioception, in the incidence of hemineglect and anosognosia, and in the score of ADL, transfer and balance between patients with and without pusher syndrome. Patients without pusher syndrome gained more motor score than patients with pusher syndrome. From improvement of view, patients with pusher syndrome gained the lowest score in ability of transfer. The finding suggest that the patients with pusher syndrome is a poorer functional outcome, be related to proprioception, hemineglect and anosognosia.

  • The purpose of this report is to investigate the clinical application of Korean Traditional treatment to abdominal pain patient after subdural hemorrhage occurred. The main herb medicine used in this study is Ondam-tang (wenda-tang) which is frequently used in psychiatric disorders. Also, acupuncture, moxibustion and physical rehabilitation treatment were practiced. After taking an Ondam-tang (wendan-tang), the clinical symptoms of the patient were improved.

  • A 44-year-old male complained of chest pain. Two years ago he was diagnosed with angina pectoris for stenosis of coronary artery in coronary angiography. Despite of medication, his chest pain aggravated to cardiovascular society classification class III. His electrocardiogram, cardiac enzymes were normal. Accordingly we diagnosed him with unstable angina. And we pattern differentiated him with heart heat syndrome considering his other symptoms. He wanted conservative care instead of coronary artery intervention generally recommended for treating unstable angina. We treated him for 13-days with tongxinluo, modified daochi-san, acupuncture, smoking prohibition which were thought to be effective for treating unstable angina based on experimental, clinical studies. Within the therapeutic period, frequency of chest pain and frequency of taking nitroglycerin were on the decrease.
