Review on the Justifiable Grounds for Withdrawal of Meaningless Life-sustaining Treatment -Based on a case of Supreme Court's Sentence No. 2009DA17417 (May 21, 2009)- (무의미한 연명치료 중단 등의 기준에 관한 재고 - 대법원 2009.5.21 선고 2009다17417사건 판결을 중심으로 -)

  • According to a case of Supreme Court's Sentence No. 2009DA17417 (May 21, 2009), the Supreme Court judges that 'the right to life is the ultimate one of basic human rights stipulated in the Constitution, so it is required to very limitedly and conservatively determine whether to discontinue any medical practice on which patient's life depends directly.' In addition, the Supreme Court admits that 'only if a patient who comes to a fatal phase before death due to attack of any irreversible disease may execute his or her right of self-determination based on human respect and values and human right to pursue happiness, it is permissible to discontinue life-sustaining treatment for him or her, unless there is any special circumstance.' Furthermore, the Supreme Court finds that 'if a patient who is attacked by any irreversible disease informs medical personnel of his or her intention to agree on the refusal or discontinuance of life-sustaining treatment in advance of his or her potential irreversible loss of consciousness, it is justifiable that he or she already executes the right of self-determination according to prior medical instructions, unless there is any special circumstance where it is reasonably concluded that his or her physician is changed after prior medical instructions for him or her.' The Supreme Court also finds that 'if a patient remains at irreversible loss of consciousness without any prior medical instruction, he or she cannot express his or her intentions at all, so it is rational and complying with social norms to admit possibility of estimating his or her own intentions on withdrawal of life-sustaining treatment, provided that such a withdrawal of life-sustaining treatment meets his or her interests in view of his or her usual sense of values or beliefs and it is reasonably concluded that he or she could likely choose to discontinue life-sustaining treatment, even if he or she were given any chance to execute his or her right of self-determination.' This judgment is very significant in a sense that it suggests the reasonable orientation of solutions for issues posed concerning withdrawal of meaningless life-sustaining medical efforts. The issues concerning removal of medical instruments for meaningless life-sustaining treatment and discontinuance of such treatment in regard to medical treatment for terminal cases don't seem to be so much big deal when a patient has clear consciousness enough to express his or her intentions, but it counts that there is any issue regarding a patient who comes to irreversible loss of consciousness and cannot express his or her intentions. Therefore, it is required to develop an institutional instrument that allows relevant authority to estimate the scope of physician's medical duties for terminal patients as well as a patient's intentions to withdraw any meaningless treatment during his or her terminal phase involving loss of consciousness. However, Korean judicial authority has yet to clarify detailed cases where it is permissible to discontinue any life-sustaining treatment for a patient in accordance with his or her right of self-determination. In this context, it is inevitable and challenging to make better legislation to improve relevant systems concerning withdrawal of life-sustaining treatment. The State must assure the human basic rights for its citizens and needs to prepare a system to assure such basic rights through legislative efforts. In this sense, simply entrusting physician, patient or his or her family with any critical issue like the withdrawal of meaningless life-sustaining treatment, even without any reasonable standard established for such entrustment, means the neglect of official duties by the State. Nevertheless, this issue is not a matter that can be resolved simply by legislative efforts. In order for our society to accept judicial system for withdrawal of life-sustaining treatment, it is important to form a social consensus about this issue and also make proactive discussions on it from a variety of standpoints.

Synthesis of Ultrafine LaAlO$_3$ Powders with Good Sinterability by Self-Sustaining Combustion Method Using (Glycine+Urea) Fuel ((Glycine+Urea) 혼합연료를 이요한 자발착화 연소반응법에 의한 우수한 소결성의 초미분체 LaAlO$_3$ 분말 합성)

  • LaAlO3d single phase used as the butter layer on Si wafer for YBa2Cu3O7-$\delta$ superconductor application were prepared by solid state reaction method and by self-sustaining combustion process. The microstructure and crystallity of synthesiszed LaAlO3 powder studied using scanning electron microscope (SEM) and X-ray diffractometer(XRD), specific surface area and sintering characteristics fo powder were investigated by Brunauer-Emmett-Teller (BET) method and dilatometer respectively. In solid state reaction method, it is difficult to obtain LaAlO3 single phase up to 150$0^{\circ}C$ period. However, in self-sustaining combustion process, it is to easy to do it only $650^{\circ}C$. Based on the results of analysis of dilatometer it is easier to obtain high sintering density (98.87%) in self-sustaining combustion process than in the solid state reaction method. This reason is that the average particle size prepared by self-sustaining combustion process is nano crystal size and has high specific surface are value(56.54 $m^2$/g) compared with that by solid state reaction method. Also, LaAlO3 layer on the Si wafer has been achieved by screen printing and sintering method. Even though the sintering temperature is 130$0^{\circ}C$, the phenomena of silicon out diffusion in LaAlO3/Si interphase are not observed.

  • This paper describes the development of the dual-thrust rocket motor, which gets a significant change in thrust level by varying the burning area of the propellant grain. Reduced smoke propellant of low burning rate was formulated and the finocyl type grain was designed to get the boosting- and sustaining-phase of the thrust level. And the motor firing data were analyzed in detail. Developed motor was applied to the missile system to implement the successful flight test and this development helped to upgrade the performance of the missile system. The results will be usefully applied to the development of the similar rocket motors.

  • A new high efficiency phase shifted full bridge (PSFB) converter for the power sustaining module of a plasma display panel (PDP) is proposed in this paper. The proposed converter employs a voltage doubler rectifier without an output inductor. Since it has no output inductor, the voltage stresses of the secondary rectifier diodes can be clamped at the output voltage level. No dissipative resistor-capacitor (RC) snubber for rectifier diodes is needed. Therefore, high efficiency, as well as, a low noise output voltage can be realized. Due to the elimination of the large output inductor, it features a simple structure, lower cost, smaller mass and lighter weight. Furthermore, the proposed converter has wide zero voltage switching (ZVS) ranges with low current stresses of the primary switches. Also the resonance between the leakage inductor of the transformer and the capacitor of the voltage doubler cell reduces the current stresses of the rectifier diodes. In this paper, operational principles, an analysis of the proposed converter and experimental results are presented.

  • This paper describes the development of the dual-thrust flight motor for enhancing the hit probability of unguided rockets. We designed dual-thrust flight motor by shape modification of the double base propellant with high burning rate, and confirmed the dual-thrust performance by static firing tests. The test results showed the thrust ratio of about 1:7.6 between sustaining phase and boosting phase, and had a quietly normal dual-thrust characteristics. And the results showed that there was not the fire extinction phenomenon of propellant due to the pressure drop.

Preparation of NiO/YSZ Ultra-Fine Powder Composites Using Self-Sustaining Combustion Process (Self-Sustaining Combustion Process를 이용한 NiO/YSZ 초미세 복합분말 제조)

  • Ultrafine NiO/YSZ (Yttria Stabilized Zirconia) powders were made by using a glycine nitrate process which is used as anode material for solid oxide fuel cells. The specific surface areas of synthesized NiO/YSZ powders were examined with controlling pH of a precursor solution and the content of glycine. The binding of glycine with metal nitrates occurring in the precursor solution was analyzed by using FTIR. The characteristics of synthesized powders were examined with X-ray diffraction(XRD) Brunauer Emmett Teller with N2 absorption. scanning electron microscopy (SEM). and transmission electron microscopy (TEM). Ultrafine NiO/YSZ powders of 15-18 m2/g were obtained through GNP when the content of glycine was controlled to 1 or 2 times the stoichiometric ratio in the precursor solutions. Strongly acid precursor solution increased the specific surface area of the synthesized powders. This is suggested to be the increased binding of metal nitrates and glycine under a strong acid solution of pH=0.5 that lets glycine consist of mainly the amine group of {{{{ { NH}`_{3 } ^{+ } }}. After sintering and reducing treatment of NiO/YSZ powders synthesized by GNP the Ni/YSZ pellet showed ideal microstructure where very fine Ni particles of 3-5 ${\mu}{\textrm}{m}$ were distributed uniformly and fine pore around Ni metal particles was formed. leading to anincrease of the triple phase boundary among gas Ni and YSZ.

Phase-Separated Pixel Isolation Method for Roll-to-Roll Processing in Flexible Liquid Crystal Displays

  • We propose new fabrication methods of a pixel-isolated liquid crystal (LC) structure for flexible display applications. In the LC structure fabricated through the proposed method, the patterned interpixel walls for sustaining the cell thickness are supported by the solidified polymer layer through anisotropic phase separation of LC/polymer composite, causing the alignment of the LC molecules to have very good mechanical stability against external pressure. In addition, we show that such pixel-isolating walls can be made by the stamping method which can be applied to fabricate large size plastic LCDs by roll-to-roll processing.

  • Proposed Multi-level PDP sustain Driver make use of element which has a low voltage rating instead of suggested method by the existing Webber and it passes through a second phase both rising and falling at sustaining voltage waveform In accordance with the change of timing phase(Tr1, Ti1, Tr2) performance comparison of a electric discharge characteristics in a PDP module has been carried out. At the conclusion of experimentation, the feature is extended by alteration of Ti1 and it has little to do with alteration of Tr1.

The History of Hospice and Palliative Care in Korea (한국 호스피스 완화의료의 역사)

  • The first hospice care center in Korea dates back to the East West Infirmaries (Dongseodaebiwon in the Korean language) of the Goryeo period in the early 11th century. It has been 50 years since hospice care was introduced in Korea. Initially hospice care was provided in the private sector, including those with a religious background, and its development was slow. In the 1990s, related religious organizations and academic associations were established, and then, a full-swing growth phase was ushered in as the Korean government institutionalized hospice care in the early 2000s. As a result, enhanced quality of hospice care service could be provided, which meant better pain management and higher quality of life for late stage cancer patients and their families. Still, the nation lacked a realistic reimbursement system which was needed to for financial stability of the affected patients. However, the national health insurance scheme began to cover hospice palliative expenses in 2015. In 2016, the Act on Decisions on Life-Sustaining Treatment for Patients in Hospice and Palliative Care or at the End of Life was legislated, allowing terminally-ill patients to refuse meaningless life-sustaining treatments. As the range of diseases subject to hospice palliative care was expanded, more challenges and issues need to be addressed by the service providers.
