Investigation of Espoused Theories in Nursing Practice (간호실무에서 신봉이론(Espoused theories)에 대한 탐색적 연구)

  • As a nursing practice involves nurses'actions in a specific context of health care, this study has focused on exploring the espoused theories in nursing practice within the action science perspectives. Espoused theories are the belief, principles, and rationale expressed by the practitioner as guiding her/his actions in a situation of practice. The data were analysed qualitatively and 25 elements of espoused theories of nursing action were identified and clustered into 6 categories. The 25 elements of espoused theories are as follows: The clinical nurse worked in wholistic and individual nursing, focussed on the patient's needed, comfort and supportive nursing (5 theories of nursing goal); excellent skills, knowledge based, assessment and data collection, explaining, educating or a scientific basis(6 theories of nursing intervention): advocacy, value oriented, treatment, accountability and commitment(4 theories of nursing ethics); human respect, partnership, trust(3 theories of patient-nurse relationship) : knowledgable, accumulated clinical experiences and personally lived experiences, positive perspectives(4 theories of nurse), role of intervention, rewarding, peer relationship(3 theories of situations). The above mentioned espoused theories are similar to that of nursing textbooks which students learned through basic nursing education and almost the same as the Acts ofa Nurse in Korean. However, we are doubtful whether nurses actually do as they think. Therefore, it is recommended to review the theories-in-use in order to find any discrepancies between the espoused theories and the reality of nursing actions.

Psychosocial Responses and Quality of Life among Amyotrophic Lateral Sclerosis Patients and Their Caregivers

  • Purpose: The purpose of this study was to explore the psychosocial responses and quality of life (QOL) among Amyotrophic Lateral Sclerosis (ALS) patients and their caregivers in South Korea. Methods: A cross-sectional design was used. Purposive sample of 15 ALS patients and their 14 caregivers were recruited via Korean Amyotrophic Lateral Sclerosis Association (KALSA) website. Demographic characteristics, hopelessness, quality of life, physical function, and caregiver burden were measured. Results: The mean period after being diagnosed with ALS was 57.73 months. The mean score of amyotrophic lateral sclerosis functional rating scale and quality of life was 21.33 (SD=11.97) and 5.70 (SD=1.23) respectively. The mean score of hopelessness was 11.87 (SD=4.72). The caregivers' mean score of McGill quality of life was 4.29 (SD=1.46), and the mean score of McGill quality of life-single item scale was 4.29 (SD=2.02). Conclusion: Since the cause of ALS has not been identified and cure is yet to be discovered, supportive care should be provided for not only quality of life but hope of patients. The findings may be used to develop knowledge based nursing intervention for patients diagnosed with ALS and their caregivers.

Experiences of Family of Patient with Newly Diagnosed Advanced Terminal Stage Hepatocellular Cancer

  • Background: Hepatocellular carcinoma (HCC) is the most common primary liver cancer and the third leading cause of cancer-related death worldwide due to its generally poor prognosis. Caregiver burden for liver cancer cases is higher than with other cancer and needs especial attention. Methods: To explore the experiences of families of patients with newly diagnosed advanced terminal stage hepatocellular cancer by interview. Results: Nine participants were recruited in this study. Content analysis of the interviews revealed four themes: blaming oneself, disrupting the pace of life, searching all possible regimens, and not letting go. Conclusions: This study provides new insight into the needs and support of family members especially when they are facing loved ones with newly diagnosed advanced terminal stage HCC. These results will inform future supportive care service development and intervention research aimed at providing assistance in reducing unmet supportive care needs and psychological distress of these family members.

Health Risk Behaviors Affecting the Process of Pregnancy (임신경과에 영향을 미치는 건강위험행위)

  • Comprehensive review of the literature was conducted to determine 1) selected risk factors and its impact that affect pregnancy outcome such as smoking, alcohol consumption, and substance abuse 2) these factors can facilitate future strategies for health promotion and prevention for both pregnant women and fetus. Review of literature were extracted from searching MEDLINE(1966 - Oct. 2000). CINAHL (1982 - Oct. 2000) and the domestic literature. The following factors were identified: 1. The effects of risk behaviors on pregnancy. ${\cdot}$Maternal smoking was associated with the occurrence of premature or LBW delivery, fetal growth retardation, extremities defects, heart defects and sudden infant death syndrome. ${\cdot}$Maternal alcohol consumption was associated with spontaneous abortion, premature or LBW delivery, morphologic/neurologic problems, especially fetal alcohol syndrome. ${\cdot}$Heroin was associated with withdrawal after birth in which were born to heroine addicts for gestational age and lung maturation in animal studies. ${\cdot}$Cocaine was associated with spontaneous abortion, abruptio placenta and a poor response to environmental stimuli. ${\cdot}$So far, the effects of caffeine on pregnancy was controversial, but severe caffeine consumption was associated with premature or LBW delivery, spontaneous abortion, still birth and dystocia. 2. Intervention methods and its effects identified were as follows ${\cdot}$Conducted intervention for smoking, alcohol and drug consumption were single or combined. ${\cdot}$Intervention methods were counseling, phone contact, mailing, use of educational videotape, booklet, support person and alternatives such as nicotine patch. ${\cdot}$The interventions increased the rates of smoking cessation during pregnancy and awareness of the risk of drug consumption, and decreased amount of alcohol consumption. ${\cdot}$The intervention outcome found positive effect on birth weight and length. 3. Our recommendations were as follows ${\cdot}$The personal and social cognition should be enhanced through education and the mass media. ${\cdot}$It's necessary to educate and give information of preconceptional care, planned pregnancy and early prenatal care for optimal pregnancy outcome. ${\cdot}$It's necessary to develop comprehensive assessment tool which is reliable and valid on smoking, alcohol consumption and substance abuse to identify supportive or interventional program.

A Study on the Mediating and Moderating Effect of Work-Family Conflict in the Relationship Among Emotional Labor, Occupational Stress, and Turnover Intention (간호사의 감정노동, 직무 스트레스와 이직의도 관계에서 직장-가정 갈등의 매개 및 조절효과)

  • Purpose: The present study was aimed to examine the degree of nurses' emotional labor and occupational stress and demonstrate the moderating and mediating effects of work-family conflict in the relationship among emotional labor, occupational stress, turnover intention of nurses. Methods: The participants for this study were 307 nurses currently working in two general hospitals in G city in Korea. Data were collected using structured questionnaires and analyzed for descriptive statistics, t-test, ANOVA, Mann-Whitney U Test, Kruskal Wallis Test, Pearson correlation coefficients and hierarchical multiple regression with SPSS/WIN 21.0 program. Results: It turned out that work-family conflict plays an important role in the relationship among emotional labor, occupational stress, turnover intention of nurses. Conclusion: Therefore, to reduce nurses' turnover intention, it is necessary to have an intervention focusing on work-family conflict. Also, it is necessary for both the government and hospitals to establish a supportive system and a program to relieve nurses from their work-family conflict.

Comparison of Attitude Toward Elders, Cognitive Level, and Mood State According to Living Arrangement of the Elderly (노인의 동거유형에 따른 노인에 대한 태도, 인지기능, 기분상태 비교)

  • Purpose: The purpose of this study was to compare the degree of attitude toward elders, cognitive level, and mood state by living arrangements of the elderly. Method: The subjects consisted of 238 elderly who divided by living arrangements(living alone, living with spouse, living with children). The data were collected by structured questionnaire that included general characteristics, attitude toward elders, MMSE-K and mood state scale, from March to December, 2005. The collected data were analyzed by Windows SPSS program including descriptive statistics, ${\chi}^2-test$, Fisher's exact test, ANCOVA, $Scheff{\grave{e}}$ test and Pearson Correlation Coefficient. Results: In MMSE-K and mood state, there were statistically significant differences among the three groups. In depression and anxiety factor of mood state, the living alone group showed higher scores than the other two groups. There was a positive correlation between attitude toward elders and MMSE-K in the elderly living with a spouse group and a negative correlation between MMSE-K and mood state in all three groups. Conclusion: It is necessary to develop a supportive program for decreasing the risk of health in the elderly and perform a differential approach according to their living arrangementa. Especially, more concern and intervention are needed to be performed for the elderly living in solitude.

A Study on the Relationship between Social Support and the Infertility Stress of Infertile Women (불임 여성의 사회적 지지와 불임 스트레스)

  • The purpose of this study was determine the relationship between social support and the infertility stress of infertile women. The subjects for this were 64 infertile women living in S city, Kyung Book Province. The data was collected during $4^{th}$ July and $14^{th}$ August, 1988 using the social support scale developed by Park(1985) and the infertility stress scale developed by Kim et. al.(1995) The data analysis was done by descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression using SPSS PC program. The results were as follows. 1. The mean of social support of the infertile women was 3.80. 2. The negative correlation was revealed between social support and the infertility stress(r=-.56, p=.001). 3. Research has revealed that the support need of social support was a predictor of the infertility stress of the infertile women, explaining 30.9% of total variance. 4. the general characteristic variable significantly related to the level of the infertility stress of the infertile women was marriage satisfaction(t=-3.28, p=.004). The results of this study suggest that social support is related to the infertility stress of the infertile women. Therefore, The nurse is recommended to apply supportive intervention in caring for infertile women.

Relationship networks among nurses in acute nursing care units (종합병원 간호단위의 간호사 관계 네트워크 연구)

  • Purpose: The purpose of this study was to explore the characteristics of social networks among registered nurses in acute nursing care units. Methods: This study used a survey design. Four nursing units from two acute hospitals were selected using a convenience method, and 83 nurses from those nursing units participated in the study in July 2022. The positive influences among nurses included friendship, collaboration, advice, and referent networks, and the negative influences included avoidance and bullying networks. Using the NetMiner program, the k-means clustering technique was applied to create groups of nodes with similar characteristics. The general characteristics of the participants were analyzed by mean, standard deviation, frequency, and ANOVA or chi-squared test. Results: As a result of dividing the 83 nurse participants into four clusters, positive influencers, silent peers, unwelcome peers, and active bullies were identified. Positive influence group nurses were frequently mentioned in the friendship, collaboration, advice, and referent networks. On the other hand, nurses in the unwelcome group and the active bullying group were frequently mentioned in the avoidance and bullying networks. Conclusion: Social networks that have a positive or negative impact on nursing performance are created through different relationships between nurses. Nurse managers can use the findings to create a more supportive and collaborative environment. Further research is needed to develop intervention programs to improve interactions and relationships between fellow nurses.

The Effect of Home stayed Stroke Patients' gait, Valance, Activities of Daily Living, Depression in the Aerobic Walking Exercise Program. (유산소 걷기운동 프로그램이 재가 뇌졸중 환자의 보행, 균형, 일상활동 수행능력, 우울에 미치는 효과)

  • This study was a quasi-experimental study of nonequivalent control group pretest- posttest design to investigate the effect of aerobic walking exercise program on the physical & psychological functions of home stayed stroke patients. The data were collected during the period of May 20th to August 15th, 2001. The subjects for this study were 40 hemiplegic stroke patients with the experimental group consisting of 19 patients and the control group being composed of 21 patients. The patients selected for this study were: (a)living in J city who had been diagnosed with stroke and at home after being discharged from the hospital, (b)suffering from stroke for 6 months to 5 years, (c)without recognition disorder with the MMSE-K score above 25, (d)below 2 on the modified Ashworth scale, (e)free from heart and pulmonary disease (f)able to walk beyond 15 minutes for themselves. The aerobic walking exercise program for the experimental group was aerobic exercise and education and supportive care. The aerobic exercise was 8 weeks' period, three times a week, 35 to 50 minutes a day. And the education and supportive care was consisted of one home visiting and 2 times telephoning a week. The data were analysed by $X^2$-test, paired t-test and unpaired t-test and ANCOVA through SAS/PC program. The results of the study were as follows: 1. There was insignificant difference in the gait length experimental and control group. There was significant difference in the gait speed between the two groups. 2. There was significant difference in the dynamic valance between the two groups. 3. There was significant difference in ADL score between the two groups. 4. There was no significant difference in the depression between the two groups. As shown above, the results of 8 weeks' the aerobic walking exercise program for home stayed stroke patients produced positive effects on gait speed, dynamic valance, ADL score. And this program was expected that it was more effective in different intervention period, verified program. Also it was needed follow study.

Reliability and Validity of the Korean Version of the Helicopter Parenting Scale (한국어판 헬리콥터 양육 측정도구의 신뢰도 및 타당도 검증)

  • Purpose: Helicopter parenting is emerging in parenting as a way of rearing adolescents and adult children. The aims of this study were to develop a Korean version of the Helicopter Parenting and Autonomy Supportive Behaviors Scale (HPASB). Methods: The HPASB questionnaire items were translated into Korean and reviewed by experts and Content Validity Index (CVI) in a preliminary study with 10 university students. During September and October, 2014 data were collected from 229 nursing students from five different universities in different locations. For data analysis, SPSS 21.0 statistics which included exploratory factor analysis, t-test, one-way ANOVA were utilized. Results: Content validity was over CVI .8. The 6 factors of K-HPASB were extracted and accounted for 59.30% of variance. The Cronbach's alpha coefficient was .71 indicating high reliability. Conclusion: The Korean version of the HPASB was identified as a scale with a high degree of validity and reliability. The results of this study provide a valuable scale which can be useful in the study of parenting as a way of rearing adolescents and adult children in Korea. To enhance the positive aspect of helicopter parenting, we suggest the development of intervention programs on parenting.
