Clinical Application of the Adenosine Triphosphate-based Response Assay in Intravesical Chemotherapy for Superficial Bladder Cancer

  • Objective: To investigate correlations between adenosine triphosphate chemotherapy response assay (ATP-CRA) and clinical outcomes after ATP-CRA-based chemotherapy for drug selection in patients receiving intravesical chemotherapy to prevent recurrence of superficial bladder cancer after surgery. Methods: The chemosensitivities of 12 anticancer drugs were evaluated, including 5-Fu ADM, and EPI, using ATP-CRA and primary tumor cell culture in 54 patients. In addition, a further 58 patients were treated according to clinical experience. Differences in post-chemotherapeutical effects between drug sensitivity assay and experience groups were compared. Results: The evaluable rate of the test was 96.3%, the clinical effective rate was 80.8%, the sensitivity rate was 97.6% (41/42), the specificity was 20%, the total predicting accuracy was 74.3%, the positive predictive value was 83.7% (41/49), the negative predictive value was 66.7% (2/3); in the drug sensitivity test group, the clinical effective rate was 80.8%, the experience group response rate was 63.8%, with a significant difference in clinical effects between the ATP-based sensitivity and experience groups (${\chi}^2$=7.0153, P<0.01). Conclusion: ATP-CRA is a stable, accurate and potentially practical chemosensitivity test providing a predictor of chemotherapeutic response in patients with superficial bladder cancer.

Research Progress in Potential Urinary Markers for the Early Detection, Diagnosis and Follow-up of Human Bladder Cancer

  • Objective: To summarize and evaluate various urinary markers for early detection, diagnosis and follow-up of human bladder cancer. Methods: A MEDLINE and PUBMED search of the latest literature on urinary markers for bladder cancer was performed. We reviewed these published reports and made a critical analysis. Results: Most urinary markers tend to be less specific than cytology, yielding more false-positive results, but demonstrating an advantage in terms of sensitivity, especially for detecting low grade, superficial tumors. Some tumor markers appear to be good candidates for early detection, diagnosis, and follow-up of human bladder cancer. Conclusion: A number of urinary markers are currently available that appear to be a applicable for clinical detection, diagnosis, and follow-up of bladder cancer. However, further studies are required to determine their accuracy and widespread applicability.

방광암 환자의 요세포 검사에서 p53 단백 발현의 의의 (Immunohistochemical Detection of p53 Gene Mutation in Urine Samples in the Patients with Bladder Cancer)

  • Although bladder cancers are very common, little is known about their molecular pathogenesis. It is known that p53 alteration is found in about 60% of muscle-invasive bladder cancer, necessiating aggressive therapy and poor outcome. We examined the nuclear expression of p53 protein, using D07 monoclonal antibody in the urine samples from 31 patients with transitional cell carcinoma of the bladder to investigate the correlation of p53 overexpression with histologic grades and depth of invasion. The positive rate of p53 protein was 27% in superficial bladder tumor, but increased up to 71% in the invasive bladder carcinomas. The overexpression of p53 protein increased according to Mostofi grading system from 18% in grade I, 45% in grade II, and up to 100% in grade III. The p53 expression tended to be higher in the invasive and high grade bladder cancers than in the superficial and low grade ones(p<0.05). These results suggest that immunohistochemical analysis of the urine specimen in the bladder cancer patients could be a useful method of screening for the presence of p53 mutant protein. The mutant p53 protein expression may be an indicator of bladder cancer with more proliferative potential and/or aggressive biologic behavior.

  • 표재성 방광암의 치료는 경요도적 절제술 후 방광내 BCG 투여가 표준적인 치료로 알려져 있다. 방광내 BCG 투여 후 가장 흔한 부작용은 방광자극, 빈뇨감, 배뇨 곤란 등이며 항결핵제를 사용해야 할 정도의 전신적인 부작용은 5% 이하로 알려져 있다. 저자들은 표재성 방광암 환자에서 경요도적 절제술 후 방광내 BCG 주입치료 후 폐합병증이 발생한 2예를 경험하여 문헌고찰과 함께 보고하는 바이다.

방광종물에 대한 방광초음파검사의 진단적 유용성 (The role of bladder sonography in patients with gross hematuria or microscopic hematuria and follow-up of patients with superficial bladder cancer)

  • Ultrasonography is used to examine gross or microscopic hematuria without side effects. It is one of the methods of diagnosing bladder lesions, but in some cases, the lesions are not found. We attempted to identify the problems during the ultrasonic examination by analyzing the symptoms, location of lesion, and medical history of urothelial cancer for cases of undetected bladder lesions. Thirty-three patients who underwent transurethral resection of a bladder tumor from January 1 to May 4, 2018 in one hospital were enrolled in this study. Patients who underwent preoperative ultrasonography and cystoscopy were treated. Ultrasonography did not detect bladder lesions in five patients. The size of the lesion was 0.5~2.5 cm in various locations, such as the side, front, and so on. Ultrasonic examination requires more attention if there is gross hematuria or a history of urothelial cancer, and it is necessary to detect recurrence by conducting cystoscopy at the same time, especially when there are lesions on the anterior wall of the bladder.

3.0 테슬러 자기공명영상에서 Stalk 및 Inchworm Sign이 있는 방광암의 T1 병기 진단 (T1-Staging for Urinary Bladder Cancer with the Stalk and Inchworm Signs with 3.0 Tesla MRI)

  • 목적 본 연구의 목적은 수술 전 표재성 방광암(T1 또는 그 이하 병기)을 확인하기 위한 자기공명영상의 stalk sign 및 inchworm sign의 진단적 능력을 평가하는 것이다. 또 다른 목적은 두 sign 간의 진단적 능력의 차이를 비교하는 것이다. 대상과 방법 방광전절제술을 시행 받은 총 240명의 환자들(505개의 종양들)을 후향적으로 검토하였다. 모든 종양은 3.0 테슬러 자기공명영상에서 T2 강조영상의 stalk sign 또는 확산강조영상의 inchworm sign을 발견함으로써 표재성 종양 및 침윤성 종양으로 분류하였다. 이를 수술 후 병리학적 T 병기와 비교함으로써 통계학적으로 진단적 의의를 평가하였고 두 sign 간의 진단적 능력 차이를 비교하였다. 결과 Stalk sign 및 inchworm sign은 높은 특이도(93%, 91%), 양성예측도(89%, 90%), 정확도(70%, 74%)를 보였으나 낮은 민감도(54%, 61%)와 음성예측도(60%, 63%)를 보였다. 두 sign 간의 진단적 능력에서 통계학적 유의미한 차이는 없었다(p > 0.05). 결론 수술 전 T2 강조영상의 stalk sign과 확산가중영상의 inchworm sign을 발견함으로써 표재성 방광암을 쉽게 진단할 수 있다.

Oncogenic Activation of Fibroblast Growth Factor Receptor-3 and RAS Genes as Non-Overlapping Mutual Exclusive Events in Urinary Bladder Cancer

  • Background: Urinary bladder cancer is a common malignancy in the West and ranks as the $7^{th}$ most common cancer in our region of Kashmir, India. FGFR3 mutations are frequent in superficial urothelial carcinoma (UC) differing from the RAS gene mutational pattern. The aim of this study was to analyze the frequency and association of FGFR3 and RAS gene mutations in UC cases. Materials and Methods: Paired tumor and adjacent normal tissue specimens of 65 consecutive UC patients were examined. DNA preparations were evaluated for the occurrence of FGFR3 and RAS gene mutations by PCR-SCCP and DNA sequencing. Results: Somatic point mutations of FGFR3 were identified in 32.3% (21 of 65). The pattern and distribution were significantly associated with low grade/stage (p<0.05). The overall mutations in exon 1 and 2 in all the forms of RAS genes aggregated to 21.5% and showed no association with any clinic-pathological parameters. In total, 53.8% (35 of 65) of the tumors studied had mutations in either a RAS or FGFR3 gene, but these were totally mutually exclusive in and none of the samples showed both the mutational events in mutually exclusive RAS and FGFR3. Conclusions: We conclude that RAS and FGFR3 mutations in UC are mutually exclusive and non-overlapping events which reflect activation of oncogenic pathways through different elements.
