Predisposing Factors Related to Shunt-Dependent Chronic Hydrocephalus after Aneurysmal Subarachnoid Hemorrhage

  • Objective : Hydrocephalus is a common sequelae of aneurysmal subarachnoid hemorrhage (SAH) and patients who develop hydrocephalus after SAH typically have a worse prognosis than those who do not. This study was designed to identify factors predictive of shunt-dependent chronic hydrocephalus among patients with aneurysmal SAH, and patients who require permanent cerebrospinal fluid diversion. Methods : Seven-hundred-and-thirty-four patients with aneurysmal SAH who were treated surgically between 1990 and 2006 were retrospectively studied. Three stages of hydrocephalus have been categorized in this paper, i.e., acute (0-3 days after SAH), subacute (4-13 days after SAH), chronic (${\geqq}14$ days after SAH). Criteria indicating the occurrence of hydrocephalus were the presence of significantly enlarged temporal horns or ratio of frontal horn to maximal biparietal diameter more than 30% in computerized tomography. Results : Overall, 66 of the 734 patients (8.9%) underwent shunting procedures for the treatment of chronic hydrocephalus. Statistically significant associations among the following factors and shunt-dependent chronic hydrocephalus were observed. (1) Increased age (p < 0.05), (2) poor Hunt and Hess grade at admission (p < 0.05), (3) intraventricular hemorrhage (p < 0.05), (4) Fisher grade III, IV at admission (p < 0.05), (5) radiological hydrocephalus at admission (p < 0.05), and (6) post surgery meningitis (p < 0.05) did affect development of chronic hydrocephalus. However the presence of intracerebral hemorrhage, multiple aneurysms, vasospasm, and gender did not influence the development of shunt-dependent chronic hydrocephalus. In addition, the location of the ruptured aneurysms in posterior cerebral circulation did not show significant correlation of development of shunt-dependent chronic hydrocephalus. Conclusion : Hydrocephalus after aneurysmal subarachnoid hemorrhage seems to have a multifactorial etiology. Understanding predisposing factors related to the shunt-dependent chronic hydrocephalus may help to guide neurosurgeons for better treatment outcomes.

Current Status and Future Strategies to Treat Spinal Cord Injury with Adult Stem Cells

  • Spinal cord injury (SCI) is one of the most devastating conditions and many SCI patients suffer neurological sequelae. Stem cell therapies are expected to be beneficial for many patients with central nervous system injuries, including SCI. Adult stem cells (ASCs) are not associated with the risks which embryonic stem cells have such as malignant transformation, or ethical problems, and can be obtained relatively easily. Consequently, many researchers are currently studying the effects of ASCs in clinical trials. The environment of transplanted cells applied in the injured spinal cord differs between the phases of SCI; therefore, many researchers have investigated these phases to determine the optimal time window for stem cell therapy in animals. In addition, the results of clinical trials should be evaluated according to the phase in which stem cells are transplanted. In general, the subacute phase is considered to be optimal for stem cell transplantation. Among various candidates of transplantable ASCs, mesenchymal stem cells (MSCs) are most widely studied due to their clinical safety. MSCs are also less immunogenic than neural stem/progenitor cells and consequently immunosuppressants are rarely required. Attempts have been made to enhance the effects of stem cells using scaffolds, trophic factors, cytokines, and other drugs in animal and/or human clinical studies. Over the past decade, several clinical trials have suggested that transplantation of MSCs into the injured spinal cord elicits therapeutic effects on SCI and is safe; however, the clinical effects are limited at present. Therefore, new therapeutic agents, such as genetically enhanced stem cells which effectively secrete neurotrophic factors or cytokines, must be developed based on the safety of pure MSCs.

Effects of Mancozeb on the Activities of Murine Peritoneal Macrophages In Vitro and Ex Vivo

  • Mancozeb (MCZ) is known to have detrimental effects on the reproductive system, but the toxicity of MCZ on immune responses has not been systematically investigated. We investigated the effects of MCZ exposure on the activities of murine peritoneal macrophages through evaluation of MCZ-induced alteration of nitric oxide (NO) production and tumor necrosis $factor-{\alpha}(TNF-\alpha)$ synthesis. Macrophages were examined ex vivo from mice orally treated with various doses of MCZ for 5 consecutive days per week for 4 weeks (subacute exposure, 250, 1000, 1500 mg/kg/day) followed by culture for 2 $(TNF-{\alpha})$ or 3 days (NO) in the presence of LPS plus $IFN-{\gamma}$. Macrophages from naive mice were also cultured with various concentrations of MCZ (0.05, 0.25, 0.5, 1 and 2 ${\mu}g//mIL$ in the presence of LPS plus $IFN-{\gamma}$ for 2 $(TNF-{\alpha})$ or 3 days (NO) in vitro. NO production was decreased with the in vitro exposure to all concentrations of MCZ. However, the amount of NO production by peritoneal macrophages from MCZ-subacutely exposed mice was increased in comparision with that of control group. In vitro, MCZ suppressed $(TNF-\alpha)$ secretion with significant reduction at 2 ${\mu}g/mL$ MCZ. Conversely, $(TNF-{\alpha})$ release was enhanced ex vivo. This study provides the substantial evidence on MCZ-induced alternation in macrophage activity. In order to clearly understand the contrasting effect of MCZ on peritoneal macrophage activity, it is necessary to further investigate the influence of major metabolite of MCZ (ETU) exposure on the NO production and $(TNF-{\alpha})$ synthesis.

감마선 조사 타락죽의 3개월 반복투여 독성평가 (Toxicity Evaluation of Irradiated Tarakjuk for Three Months)

  • 본 연구는 타락죽의 안전성을 확보하기 위해서 30 kGy 감마선 조사된 타락죽을 ICR 마우스에 3개월간 섭취시킨 다음 독성평가를 수행하였다. 비조사 타락죽 및 30 kGy 조사 타락죽을 암수 마우스에 투여를 한 결과 시험기간 동안 시험물질에 의한 임상증상이나 폐사 동물을 나타나지 않았으며, 체중변화, 사료섭취량 및 주요 장기 무게도 대조군에 비해 차이를 보이지 않았다. 혈액학적 검사 및 혈청학적 검사 모두 정상범위 내의 수치를 나타내었으며, 병리조직학적 검사 역시 간 및 신장 모두 염증, 괴사 등의 유의할 만한 병적 변화 없는 정상적인 구조를 유지하고 있었다. 따라서 30 kGy로 감마선 조사된 타락죽을 암수 마우스에 3개월간 섭취시켜도 본 시험조건에서는 독성이 없는 것으로 나타났다.

뇌졸중(腦卒中) 환자(患者)의 성별(性別)과 부위(部位)에 대한 Heart Rate Variability의 변화(變化) (Differenting Effects of Cerebral Hemispheric Lateralization and Gender by Power Spectral Analysis of Heart Rate Variability in Cerebrovascular disease)

  • Background and Purpose The increasing evidence for neurally mediated cardiac damage and sudden death has focused attention on the central autonomic control of cardiac function. Power spectral analysis of heart rate variability(HRV) can detect autonomic consequences of stroke. We performed power spectral analysis of heart rate variability from 24-hour holter recording to identify cerebral hemispheric lateralization and gender effect in cardiac autonomic control. Methods Data were obtained from 24-hour holter recordings in 30 consecutive patients with hemispheric brain infarction in the subacute phase. We analysed the time domain and frequency domain measures of HRV and ST segment by hemispheric lateralization and Gender. Results ST segment was depressed in women compared with man. There was no statistically significant differences between right and left hemisphere stroke patients in any standard index of HRV and ST segment changes. Conclusions These data are partial consistent with evidence from the recent literature that two cerebral hemispheres and gender have a differential influence on the nature and severity of cardiac dysfunction. We think the prospective and definite study was necessary.

교도소에서 의뢰된 급성 하악 지치 주위염의 보존적 감염관리: 증례보고 (Conservative infection control on acute pericoronitis in mandibular third molar patients referred from the prison)

  • In the presence of acute pericoronitis of mandilbular third molar, antibiotic therapy and early incision and drainage are the method of choice, followed by definitive surgical extraction of the tooth as soon as it becomes subacute. If excision of the overlying tissues is decided on, it should be done adequately. All overlying tissues must be throughly excised, and the crown portion of the unerupted tooth should be completely exposed. After excision has been completed, the wound should be managed with a surgical dressing. This should be allowed to remain approximately 7 days. And then, surgical extraction of the impacted mandibular third molar can be done usually. In this operation, there are many complications, such as, postoperative bleeding, infection, trismus, dysphasia and paresthesia. The surgeon are discredited and medicolegal problem may be occurred in the presence of many distressed complications. Therefore, the relatively nonsurgical treatment is the method of choice. So, authors selected the conservative treatment methods of incision and drainage, primary endodontic drainage, operculectomy without surgical extraction of the mandibular third molars. The results were more favorable without the postoperative complication in Wonju old offender prison.

  • 저자들은 객담, 기침, 발열과 호흡곤란을 보이고 단순 흉부촬영상 다발성 공동과 경변을 보이며 흉부 전산화 촬영과 폐동맥 혈관조영술상 폐 동맥류가 확인된 환자에서 폐 절제술후 만성 괴사성 침입성 폐 국균증으로 진단된 1예를 경험하였기에 이에 보고하는 바이다.

가와사끼병에서 정맥용 면역글로불린에 반응 후 아급성기에 발생한 관절염 (Arthritis in the Subacute Stage of Kawasaki Disease after Responding to Intravenous Immunoglobulin Treatment)

  • 저자들은 가와사끼병으로 IVIG 치료로 해열을 보인 후, 급격히 발병한 관절염을 보인 5명의 환아에 대해 임상적 경과, 실험실 소견 등을 조사하였다. IVIG 치료에 반응 후 평균 5.8일 후에 관절염 증상이 나타났으며, 임상적 분류상 소수 관절형 형이 3례, 다수관절형이 2례로 나타났다. 검사실 소견에서 류마토이드 인자는 1례에서 양성을 보였으나 이 후 음성으로 전환되었다. 3명에서 조사된 HLA B27은 모두 음성을 보였다. 치료로는 고용량 아스피린(2례), 비스테로이드성 소염제(이부프로펜, 3례), 스테로이드제(메틸프레드니솔론, 1례)를 사용하였다. 가와사끼병의 경과 중 정맥용 면역글로불린 치료 후 관절염이 드물게 관찰된다. 이러한 관절염은 스테로이드제를 비롯한 항염증제에 양호한 반응을 보였고 재발은 관찰되지 않았다.

요추추간판탈출증환자 49례의 탈출양상에 따른 한방치료에 대한 임상적 고찰 (The clinical study on difference of sex, age, medical history, part, or type in patients with lumbar spine herniation of nucleus pulpous)

  • Objective : In treatment of lumbar spine herniation of nucleus pulpous, the different result in accordance with difference of sex, age, medical history, part, or type. Methods : After 49 patients who were diagnosed as HNP of L-spine were grouped by sex, age, medical history, part, or type, they were compared and analyzed on the basis of difference between measuring VAS and ODI as they were hospitalized and as they were discharged. Results : 1. In terms of differences by gender, males' average degree of VAS improvement was 63.92 and females' average degree of VAS improvement was 59.8. Also, Males' average ODI improvement index was 12.36 and females' average ODI improvement index was 12.5. 2. In terms of differences by age, Teens recorded highest degree of VAS improvement and ODI improvement index. 3. In terms of differences by medical history, Most acute or acute patients showed higher degree of VAS improvement and ODI improvement index than subacute or chronic patients. 4. There were no significant distinguishing markscomparing between one diseased part and other. 5. As the research that focused on different type, Extrusion type had better improvement by medical treatment than Protrusion type. Conclusion : This research drew the result as above, but recognizable statistical relation could not be found in the result. Therefore there needs much sustainable research to deduce meaningful result.

The Toxicity and Anti-cancer Activity of the Hexane Layer of Melia azedarach L. var. japonica Makino's Bark Extract

  • In this study, the 4-week oral toxicity and anti-cancer activity of the hexane layer of Melia azedarach L. var. japonica Makino's bark extract were investigated. We carried out a hollow fiber (HF) assay and 28-day repeated toxicity study to confirm the anti-cancer effect and safety of the hexane layer. The HF assay was carried out using an A549 human adenocarcinoma cell via intraperitoneal (IP) site with or without cisplatin. In the result, the 200 mg/kg b.w of hexane layer with 4 mg/kg b.w of cisplatin treated group, showed the highest cytotoxicity aginst A549 carcinoma cells. For the 28-day repeated toxicity study, 6 groups of 10 male and female mice were given by gavage 200, 100, or 50 mg/kg b.w hexane layer with or without 4 mg/kg b.w of cisplatin against body weight, and were then sacrificed for blood and tissue sampling. The subacute oral toxicity study in mice with doses of 200, 100, and 50 mg/kg b.w hexane layer showed no significant changes in body weight gain and general behavior. The cisplatin-treated group significantly decreased in body weight compared to the control group but regained weight with 100 and 200 mg/kg b.w of hexane layer. The biochemical analysis showed significant increase in several parameters (ALT, total billirubin, AST, creatinine, and BUN) in cisplatin-treated groups. However, in the group given a co-treatment of hexane layer (200 mg/kg b.w), levels of these parameters decreased. In hematological analysis, cisplatin induced the reduction of WBCs and neutrophils but co-treatment with hexane layer (100 and 200 mg/kg b.w) improved these toxicities caused by cisplatin. The histological profile of the livers showed eosinophilic cell foci in central vein and portal triad in cisplatin treated mice. These results show that hexane layer might have an anti-cancer activity and could improve the toxicity of cisplatin.
