Hyperperfusion in DWI Abnormality in a Patient with Acute Symptomatic Hypoglycemic Encephalopathy

  • The perfusion change in acute symptomatic hypoglycemic encephalopathy (ASHE) is not well known. We present the perfusion-weighted imaging of a patient with ASHE. The area of diffusion-weighted imaging abnormalities and adjacent normal-appearing white matter showed increased cerebral blood volume and flow, and shortening of time-to-peak.

  • 임피던스 혈량 측정법(impedance plethysmography)은 인체의 어느 특정부위에서의 혈량의 변화를 환자에게 전혀 고통을 주지않고(noninvasive) 안전하고 간편하게 측정하는 방법으로서, 특히 심장의 빅동량(stroke volume)를 측정하는 분야를 임피던스 카디오그라피(impedance cardiography)라고 한다. 혈량 측정법은 인체의 어느 부위에서 박동하는 혈량은 그 부위로 유입되는 혈량과 유출하는 혈량의 차이 만큼 유발한다는데 근거를 두고 있다. 한편 심전도는 주로 심장의 전기적인 면을 측정하는 반면 임피던스 카디오그라피는 박동량과 심장 근육의 수축능력(contractility)등 심장의 기계적인 면을 측정할 수 있다.

Optimization of Measuring Cardiac Output by Both Hands Electrode Impedance Method (양손 전극의 임피던스법을 이용한 심박출량 측정의 최적화)

  • In this study, a new method that can estimate ICG data from a subject's both hands to measure Cardiac Output under convenient sensor environment. With this aim, a grip-type electrode was implemented to measure ICG. To improve the accuracy of measurement, the regression equation was extracted using multiple bio-parameters and our result was compared with the thoracic ICG equipment(Physio Flow$^{(R)}$, PF104D, Manatec Biomedical, France), which is being used in clinics. The subjects consist of 26 men and 4 women(age in $22.0{\pm}3.32$). They are no cardiac disease. Parameters available for regression model were used gender, BMI, MBP, LVET, dZ/dt(max), distance between the measured electrodes, body impedance, and PTT. As a result of analyzing the ICG measurement value on thorax and both hands, the correlation with stroke volume, heart rate, and cardiac output was $R^2$=0.853, $R^2$=0.958 and $R^2$=0.899, respectively.

  • Background and Purpose : Cerebrovascular accident is a serious neurologic event. It can have temporary or permanent effects on survivors, including memory, cognition and volitional control of voiding. The purpose of this study is to evaluate the clinical effect of moxibustion in patients with voiding dysfunction after a cerebrovascular accident. Methods : Twenty patients with post-stroke voiding dysfunction were studied. All patients had computerized tomography(CT) and magnetic resonance imaging(MRI) to localize the lesion in the central nervous system. They were randomly divided into two groups : the control and moxibustion group. Ten of twenty patients underwent moxibustion treatment by randomization. The moxibustion group receieved moxibustion at three points : Chung-guk (Conception Vessel CV3), Kuanwon (CV4) Kihae (CV6). Residual urine volume evaluation was undertaken in all patients. Results : 1. IIn the balanced bladder time, the moxibustion group had the shorter time than non moxibustion group. But there was no significiant difference between the moxibustion group and non-moxibustion group. 2. In the residual urine volume, the moxibustion group and non-moxibustion group showed a tendency to decrease. But there was no significiant difference between the moxibustion group and non-moxibustion group. Conclusions : Though further study is needed, our findings suggested that the time of achieving balanced voiding was shorter with moxibustion than in the control group.

Recovery of an Injured Corticospinal Tract in a Patient With Infarction on the Corona Radiata

  • Purpose: Many studies have attempted to elucidate the motor recovery mechanisms of stroke. In the current study, we report on a patient with infarction who showed recovery of an injured corticospinal tract (CST), using diffusion tensor imaging (DTI). Methods: A 20-year-old male presented with severe paralysis of the left extremities following an infarction in the right corona radiate and basal ganglia. The patient showed good motor recovery as much as being able to manipulate object using his left hand and could walk independently on stairs at 9 months after onset. Results: At 1 month after onset, the right CST showed decreased fiber volume due to the partial injury at the level of the corona radiate, and no (motor evoked potential) MEP was evoked from the right hemisphere. By contrast, the results at 9 months showed increased fiber volume of the right CST and the amplitude of MEP had improved to normal range on TMS. Conclusion: These results suggest that the severely injured right CST and MEP amplitude had been recovered during 9 months. Therefore, this result has important implications for brain plasticity and brain rehabilitation in patients with infarction.

  • This paper reports on a flexible flap valve actuated by electromagnetic force under a constant pressure source. The flexible flap valve consists of the three main components: a flexible flap with a steel disk embedded in PDMS, an electromagnetic actuator and two glass plates with inlet and outlet. Sealing lip structures for improving the valve characteristics are added on the outlet of the bottom glass substrate. The flap valve is fabricated by the spin-coating process, the EDM process, SU-8 mold process and oxygen plasma treatment. The dimension of an assembled flap valve is $12mm{\times}20mm{\times}28mm$. The stroke volume of the flap valve is measured for various pressures and open times. When the input voltage of 30 V is applied for 0.33 s, the minimum stroke volume of the flap valve is 70 ${\mu}L$ at 50 kPa.

Suggestion of a Model for Filling Coefficient of Hydraulic Cylinder in Concrete Pump (콘크리트펌프 유압실린더의 충진율 모델 제안)

  • In general, piston pumps are frequently used for concrete pumping. Filling coefficient signifies the ratio volume of a hydraulic cylinder to volume of concrete inside the cylinder. Therefore, it may be considered as a parameter directly affecting the flow rate and efficiency for concrete pumping. However, accurate analyses on this aspect have not yet been performed. In this paper, the data measured from horizontal pipeline pumping tests for 350m and 548m in length was analyzed to identify the relationships of rheological properties of concrete and stroke time with the filling coefficient. In addition, an equation allowing prediction of the filling coefficient from rheological properties of concrete and stroke time has been suggested.

Research Trends of Oriental Medicine in Korea and Meta-analysis of Articles related to Stroke in the Journal of Korean Oriental Medicine (한의학회지의 뇌졸중 관련논문을 통한 한의학 연구동향과 질적 메타분석)

  • Objective : There have not been studies on the oriental medicine research trends and qualitative analysis of Korean oriental medicine articles in Korea, particularly in the oriental medicine area. This study's purpose is to appreciate Korean oriental medicine's research trends and assess the quality of the Korean oriental medical journals' level. This study will help upgrade Korean oriental medical journals. Method : We found 93 stroke-related articles in the Journal of Korean Oriental Medicine, published between 1980(Volume 1) and 2001(Volume 45). We surveyed the institutes involved in each study, and the number of people in each study. We seperated by epidemiological study design, the studies into categories. Campbell and Stanley's study design into 6 categories, statistical methods into 13 categories. Next, we assessed the 41 articles by a checklist, which had 10 categories. The quality score was analyzed by survey variables over the period, which were divided by the date of publication: the 1980s, the first half of the 1990s, second half of the 1990s and 2000s. Results : 46.3% od the studies didn't use statistical methods. 56.1%, were case series studies by classification of epidemiological study design, and 53.6% were one-shot case studies designed by Campbell and Stanley's study design. The quality score was 58.33 point 60 is average. In quality level by Browman GP etc.'s, 83.0% of the studies were included in V levels. But, the quality scores were significant by presence of reviewer system, statistical method's usage, and over the period. Conclusion : The Journal of Korean Oriental Medicine needs to improve the quality its articles. To improve the quality of oriental medicine related articles, the following are necessary: working together, joint work of inter-educational system, strengthening of reviewer system, use of statistical methods, and consulting to the expert of research at the stage of study design.

Hypertensive Retinopathy and the Risk of Hemorrhagic Stroke

  • Objective : Hemorrhagic stroke (HS) and hypertensive retinopathy are known end organ damage of the brain and eye respectively, with HS having deleterious consequence to the patients. This study is to correlate between hypertensive retinopathy and HS in hypertensive disease. Methods : A control group of hypertensive patients only, and an investigated group of hypertensive HS patients. Fundoscopic examination to determine the grade of retinopathy was performed and then divided into low or high severity hypertensive retinopathy. Clinical and radiological parameter included are demography, vital signs, Glasgow coma scale (GCS) on admission, clot volume, site of clot, Intracerebral hemorrhage (ICH) score and Glasgow outcome scale (GOS). Data were correlated with the severity of hypertensive retinopathy. Results : Fifty patient in the control group and 51 patients in the investigated group were recruited. In the hypertensive HS group, 21 had low severity retinopathy (no or mild retinopathy) accounting for 41.2% and 30 patients had high severity (moderate or severe retinopathy). In the hypertensive patients 49 had low severity and one had high severity (p-value of 0.001). In HS group low severity showed better GCS score of 9-15 on admission (p-value of 0.003), clot volume less than 30 mL (p-value 0.001), and also a better 30 days mortality rate by using the ICH score (p-value 0.006), GOS score of 4 and 5 the low severity retinopathy fair better than the high severity retinopathy (p-value of 0.001), and the relative risk to develop HS in low severity and high severity retinopathy was 0.42 and 29.4, respectively. Conclusion : Hypertensive retinopathy screening could be used as an indicator in hypertensive patient, to evaluate the risk of developing hypertensive HS in the future.

A Case Report of Korean Medical Treatment on Parkinsonism Patient Complaining of Motor Disorder and Aphonia (한방치료로 운동 기능장애와 실성증이 호전된 파킨슨증후군 환자 치험 1례)

  • ■Objectives This case study is to report the effectiveness of Korean medicine in Parkinsonism patient's treatment. ■Methods We used the acupuncture, electro-acupuncture, moxibustion, cupping therapy, herbal medicine, especially Palmulgunja-tang to the Parkinsonism patient with motor disorder such as Postural Instability and Gait Difficulty(PIGD) and aphonia. Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait pattern, voice dB and self-evaluation of speed and volume were used to assess the change of symptoms. ■Results ‌After treatment, the UPDRS score decreased in overall category and the walking pattern has improved. In addition, the improvement was observed in voice volume and in self assessment of the patient. ■Conclusion This case suggests the effect of Korean medical treatment on motor disorder and aphonia in Parkinsonism.
