Design the Expert Systems for the Stroke Early Diagnosis based in Web Environment (Web환경기반의 뇌졸중 초기진단 전문가시스템 설계)

  • In this study, we designed the expert system for the diagnosis of stroke. The causes of stroke in central nervous systems are very diverse, so a doctor who treats the patients with stroke must have the expert knowledge for the quick and correct diagnosis and for the adequate medical management. But the primary physician who engaged in the primary care of the patient with stroke does not have the export knowledge for the stroke. So, we need to develop the expert system for assisting the diagnosis of stroke. Also the diagnosis system can be used as simulator for the medical students who study the neurology. In this study, we developed the diagnosis expert system that offer a pathological name provided by artificial neural networks. And we designed the inference engine and interfaces. The artificial neural network is a system that provide a possible diagnosis of stroke. We implemented the system using Windows2000 Server, IIS5.0 and ASP.

  • An expert system for stroke diagnosis was designed in this study. The causes of stroke in the central nervous systems are very diverse, symptoms may not appear in the early stage, so diagnosis ran be difficult. Also, doctors who treats patients with stroke must have expert knowledge for the quick and correct impending diagnosis. Therefore, an expert system for assisting the impending diagnosis of stroke has needed to be developed. In addition, the diagnosis system can be used as an simulator for medical students who study neurology. In this study, and diagnosis expert system was developed. It serves a pathological data bus provided by an interface. An inference engine makes an impending diagnosis of stroke possible. We implemented the system using Windows2000 Server, IIS5.0 and ASP.

  • Objectives: The aim of this review was to compare the findings of 14 studies of tongue diagnosis in Korean stroke patients. Methods: The purpose, subjects, observation methods, statistical methods, classification of tongue diagnosis, significance, and limitations were analyzed and compared. Results: In Korean stroke patients, tongue diagnosis is significant in pattern identifications, prognosis evaluation, degree of neurological deficit, past history, blood tests, and pulse diagnosis. Interobserver reliability of tongue diagnosis is relatively high.

  • Objectives : Hypertension(HTN), diabetes mellitus(DM) and hyperlipidemia(HL) are the most common risk factors of cerebro-vascular attack(CVA). To prevent CVA, early diagnosis (awareness before CVA) of HTN, DM and HL is essential, and for the effective early diagnosis we need to analyze the data of CVA patients concerned with HTN, DM and HL. In this study, we studied characteristics of patients early diagnosed before CVA. Methods : We analyzed the data of 363 patients with acute ischemic stroke from Hanbang Stroke Registry supported by the Ministry of Science and Technology of Korea. Subjects' enrollment was done in the oriental medical hospitals of 3 universities located in the metropolitan region from October 2005 to October 2006. We analyzed all registered data about HTN. DM and HL, for example, how many patients were aware of their HTN, DM or HL before CVA, differences in characteristics of early diagnosed patients, effects of early diagnosis on CAV, etc. Results : Patients aware of their HTN. DM and HL before CVA have more past history and family history about HTN, DM or HL. Early diagnosis rate of HL is lower than HTN and DM, and early diagnosed HL patients were younger and deucated longer than patients who weren't aware of their HL before CVA. These results mean that publicity is the key to promoting early diagnosis rates of HTN, DM and HL. In DM and HL, early diagnosed patients had more SVO type cerebral infarction than patients who weren't aware of risk factors. We can thus see that early diagnosis even affects CVA types. Conclusion : This work helps us to understand the realities and importance of early diagnosis of HTN, DM and HL before CVA. There should be further research, which can contribute to much more improved early diagnosis rates of HTN, DM and HL.

  • Stroke is a leading cause of disability and death. The condition requires prompt diagnosis and treatment. The quality of care provided to patients with stroke can vary depending on the availability of medical resources, which in turn, can affect prognosis. Recently, there has been growing interest in using machine learning (ML) to support stroke diagnosis and treatment decisions based on large medical data sets. Current ML applications in stroke care can be divided into two categories: analysis of neuroimaging data and clinical information-based predictive models. Using ML to analyze neuroimaging data can increase the efficiency and accuracy of diagnoses. Commercial software that uses ML algorithms is already being used in the medical field. Additionally, the accuracy of predictive ML models is improving with the integration of radiomics and clinical data. is expected to be important for improving the quality of care for patients with stroke.

  • Pediatric stroke is relatively rare but may lead to significant morbidity and mortality. Along with the advance of brain imaging technology and clinical awareness, diagnosis of pediatric stroke is increasing wordwide. Pediatric stroke differs from adults in variable risk factor/etiologies, diverse and nonspecific clinical presentation depending on ages. This review will be discussed pediatric stroke focusing on their clinical presentations, diagnosis and etiologies/risk factors.

  • Objectives : The aim of this study was to assess the relationship between risk factors, warning signs, tongue diagnosis, pulse pattern, pattern identification and National Institute of Health Stroke Scale in acute stroke patients. Methods : We studied patients hospitalized within 4 wks after their ictus who were admitted at the Internal Medical Department at Kyunghee Oriental Medical Center, Kyunghee University East-West Neo Medical Center, Kyungwon University Songpa Oriental Medical Center, Kyungwon University Incheon Oriental Medical Center, Dongguk University Ilsan Oriental Medical Center from April 2007 to August 2009. We analyzed the relationship between risk factors, warning signs, tongue diagnosis, pulse pattern, pattern identification and National Institute of Health Stroke Scale in acute stroke patients. Results : 1506 subjects were included into the final analysis. 1. In the risk factors, the NIHSS mean score of atrial fibrillation was significantly higher than non-atrial fibrillation. 2. In the warning signs, the NIHSS mean scores of weakness, loss of eyesight, dysarthria, and sensory loss were significantly higher than in the non-warning signs. 3. There were no significant differences in lifestyle, tongue fur color, pattern identification between groups. 4. In the tongue color, the NIHSS mean score of red was significantly higher than pale or pale red. 5. There were significant differences statistically between forceful/weak, fine/not fine, slippery/not slippery pulse and NIHSS score. Conclusion : The above results show the relationship between risk factors, warning signs, tongue diagnosis, pulse pattern, pattern identification and National Institute of Health Stroke Scale in acute stroke patients. These results can be utilized in the future as a basis material.

  • Objectives : The aim of this study was to examine the Differences of symptom improvement depending on correspondance of Herb medicine with Oriental Medical Diagnosis in Acute Stroke Patient. Methods : We studied hospitalized patients within 10days after their ictus who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2011. We compared the improvement of MI and SSS score between Correspondance group(n=9) and Non-correspondance group(n=13). Results : The patients who had corresponding herb medicine with Oriental Medical Diagnosis show much improvement of MI score, but there is no significant difference. There is no significant difference on improvement of SSS score, between Correspondance and Non-correspondance. Conclusion : This study provides evidence that correspondance of herb medicine with Oriental Medical Diagnosis is important to improve the symptoms of acute stroke.

The Study on Relationship of Dampness-Phlegm Tongue Diagnosis to hyperlipidemia in Stroke Patients (중풍 환자의 습담 설진과 고지혈증의 연관성에 관한 연구)

  • Objectives : This study was aimed to clarify the relationship between the tongue diagnosis of dampness-phlegm and hyperlipidemia in acute stroke patients. Methods : We analyzed the data of 1405 patients with acute stroke in 10 oriental medical hospitals from November 2006 to December 2008. We classified patients into two groups, dampness-phlegm and non dampness-phlegm by tongue diagnosis such as the white coating of the tongue, thick coating of the tongue, swollen tongue and teeth printed tongue which is oriental medical diagnosis. And We analyzed their characteristics with type of stroke and lipid blood level. Results : 1. There was no significant difference of stroke type between the dampness-phlegm tongue diagnosis group and the non dampness-phlegm tongue diagnosis group. 2. The ratio of SVO was higher in dampness-phlegm tongue diagnosis group (76.06%) than the non dampness-phlegm tongue diagnosis group (61.26%). 3. According to the blood test, the dampness-phlegm tongue diagnosis group showed higher in total cholesterol, HDL cholesterol, LDL cholesterol and triglyceride than the control group. 4. Total cholesterol and HDL cholesterol were significant higher in dampness-phlegm tongue diagnosis than the control group. Conclusion : According to the analysis, the relationship between the tongue diagnosis of dampness-phlegm and hyperlipidemia in acute stroke patients were more clarified. Based on these results, more prospective studies are to be done with more clinical data.

  • Object : The aim of this study was to examine the differences of symptom improvement between different diagnosis classification groups in acute cerebral infarction patients. Methods : We studied inpatients within a month after the onset of cerebral infarction who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2012. We compared the improvement of Motricity Index and Scandinavian Stroke Scale score between Fire-heat group(n=20), Yin deficiency group(n=31), Dampness-phlegm group(n=30), and Qi deficiency group(n=13). Results : Yin deficiency group patients with cerebral infarction showed the most improvement in MI and SSS scores, and patients in Fire-heat group showed the poorest improvement in MI and SSS scores. There was a significant difference between the two groups, but there were no significant differences between all four diagnosis classification groups. Conclusions : This study provides evidence that diagnosis classification could be considered as an important factor in predicting the prognosis of acute cerebral infarction.
