간호·간병통합서비스병동 간호사의 직무스트레스가 소진에 미치는 영향: 감성지능의 조절 및 매개효과 (Factors Influencing Professional Quality of Life in Intensive Care Unit Nurses of University Hospitals)

  • Purpose : This study aims to identify job stress, emotional intelligence, and exhaustion of the nurses in comprehensive nursing service units and then verify the moderating and mediating effects of emotional intelligence on the relationship between job stress and exhaustion. Methods : Participants are 118 nurses working in two general hospitals. The collected data are analyzed using the SPSS WIN 27.0 and AMOS 18.0 programs by assessing frequency and percentage, mean and standard deviation, independent t-test and one-way ANOVA, Scheffé test, Pearson correlation coefficient, and hierarchical multiple regression analysis and bootstrapping. Results : Exhaustion has a statistically significant positive correlation with job stress (r=.40, p <.001) and a statistically significant negative correlation with emotional intelligence (r=-.26, p =.004). A partial mediating effect of emotional intelligence is found between job stress and exhaustion, however, there is no moderating effect. Conclusion : It is expected that to reduce the exhaustion of nurses in comprehensive nursing service units, it is necessary to assess job stress and emotional intelligence, and strengthen emotional intelligence along with job stress intervention.

신생아집중치료실 입원 미숙아 어머니를 위한 임파워먼트 프로그램이 스트레스, 불안, 우울 및 양육자신감에 미치는 효과 (Effects of Maternal Empowerment Program on Stress, Anxiety, Depression and Parenting Confidence in Mothers of Preterm Infants in the Neonatal Intensive Care Unit)

  • Purpose: This study aimed to evaluate the effects of an empowerment program on maternal stress, anxiety, depression and parenting confidence. Methods: A total of 44 mothers of preterm infants were assigned into an experimental or a control group (n=22 each). The experimental group received the usual nursing care and 7 sessions of an empowerment program. The control group only received the usual care. The program was implemented from June to December, 2016 in the neonatal intensive care unit of K university-affiliated hospital in Daegu, Korea. The outcome variables measured were parental stress (PSS: NICU), anxiety (STAI), depression (CES-D) and parenting confidence. Data were analyzed using t-test or repeated measures ANOVA. Results: Scores for both parental stress (t=3.07 p=.004) and depression (F=3.76, p=.26) were significantly lower in the experimental group than in the control group. However, there were no significant differences in anxiety between the groups (F=0.79, p=.505). Parenting confidence scores (F=9.05, p=.001) were significantly higher in the experimental group than in the control group. Conclusion: A maternal empowerment program can be an effective means of reducing parental stress and depression as well as enhancing parenting confidence, for mothers of preterm infants.

보육담당 공무원의 직무만족도에 영향을 미치는 경로 (Pathways to Job Satisfaction of Local Officials in the Child Care Sector)

  • The purpose of this study was to examine pathways between job characteristics and job satisfaction among local officials in the child care sector. Participants of this study comprised of 128 local officials who worked in the child care sectors. Structural equation modeling was used to test the direct and indirect effects of job characteristics on job satisfaction through job stress and job burnout. Results confirmed the initial model that assumed there would be an indirect effect of job characteristics based on job satisfaction. The path involving three variables of job characteristics, however, were not statistically significant. The initial model was modified by removing the job characteristics variables. The modified model that confirmed the initial model showed that job stress of local officials in the child care sector was likely to increase job burnout; in turn, the job burnout tended to decrease job satisfaction.

항암치료환자에게 시행한 자기간호교육과 자원동원성 수준이 적극적 대처행위에 미치는 효과 (An Effect of Self-Care Education and Level of Resourcefulness on Active Coping in Patients with Chemotherapy)

  • The purpose of this study was to identify how internal and external coping resources influenced active coping in the process of stress-coping. The model was established theoretically by comparing and integrating the following theories : Stress-Coping, Self-Care, and Resourcefulness. The subjects consisted of sixty eight patients undergoing chemotherapy(experimental group 34, control group 34) at two general hospitals from January to July, 1995. The results were as follows : After self-care education, the active coping score of the experimental group was significantly higher than that of the control group. The active coping score of the high resourcefulness group was significantly higher than that of the low resourcefulness group. The interaction effect between self-care education and resourcfulness was not significant statistically. Specifically as to such scores of seeking social support, problem-oriented strategy and self care behavior, there were significantly higher in the experimental group and high resourcefulness group than in each of the other groups. Considering them both, self-care education and resourcefulness are effective nursing strategies to promote active coping including self-care. Consequently, the synthesis and testing of theories of stress-coping, self-care, and resourcefulness in this study are mostly proven to enhance the explanation and prediction of the change of active coping including self-care. Therefore the result of this study will contribute in the development of practice theory of nursing. A further study is necessary to reevaluate the interaction effect between self-care education and resourcefulness and to identify the difference between resourcefulness and self-efficacy.

아이돌보미 서비스 이용유무에 따른 사회적 지지와 양육스트레스의 차이 (The Difference of Child Care Support and Parenting Stress Depending on Using In-Home Care Service)

  • 본 연구에서는 아이돌보미 서비스의 이용유무에 따른 사회적 지지와 양육스트레스의 차이를 분석하여, 아이돌보미 지원사업의 발전방향을 모색하고자 한다. 본 연구대상은 K구 아이돌보미 서비스 등록 가정(293가정)의 어머니이다. 2009년 9월 1일부터 10월 25일까지 총 193명의 어머니에게 전자우편을 송신하여 총 69명의 응답을 최종적으로 분석하였다. 연구결과는 다음과 같다. 첫째, 아이돌보미 서비스 이용 유무와 관계없이 수단적 원조는 남편과 친족의 지원을 이용하였으며, 정보적 원조와 정서적 원조는 남편과 친구/이웃/직장동료, 인터넷 카페를 이용하고 있는 것으로 나타났다. 둘째, 아이돌보미 서비스 이용희망자가 이용자보다 남편, 친정부모, 시댁부모의 육아지원이 더 많은 것으로 나타났다. 셋째, 아이돌보미 서비스 이용 유무에 따른 양육스트레스는 통계적으로 유의한 차이는 나타나지 않았다.

장기요양기관 노인의 일상스트레스, 자아탄력성이 삶의 질에 미치는 영향 (The Impacts of Daily Stress, Ego Resilience, and Quality of Life of the Elderly in Long-term Care Institutions)

  • 이 연구는 장기요양기관 노인의 일상스트레스, 자아탄력성, 삶의 질 변인 간 인과관계 모델을 설정하고 경로분석을 실시하였다. 수도권에 소재한 민간 노인장기요양기관의 이용자를 대상으로 최종 380명을 SPSS 22.0과 AMOS 22.0을 이용하여 일상스트레스, 자아탄력성, 삶의 질 변인 간 관계를 분석하였다. 연구결과, 첫째, 노인의 일상스트레스는 삶의 질에 부(-)적인 직접 효과가 있는 것으로 나타났다. 둘째, 노인의 자아탄력성은 삶의 질에 정(+)적인 직접 효과가 있었다. 셋째, 노인의 일상스트레스는 자아탄력성에 정(+)적인 직접 효과가 나타났다. 넷째, 노인의 자아탄력성을 매개로 일상스트레스는 삶의 질에 간접 효과 및 부분 매개 효과가 있는 것으로 나타났다. 따라서, 노인의 삶의 질을 향상하기 위해서는 일상스트레스를 감소하고 자아탄력성을 높이기 위한 프로그램 개발 및 제도적 지원 방안이 필요하다.

요양보호사의 직무스트레스와 정신건강의 관계에서 정서조절곤란의 매개효과 (The Mediating Effects of Emotional Dysregulation on the Relationship between Care Worker's Job Stress and Mental Health)

  • 본 연구는 요양보호사의 직무스트레스와 정신건강과의 관계에서 정서조절곤란의 매개효과를 검증하였다. 노인요양기관에서 근무하는 요양보호사 154명을 대상으로 요양보호사 직무스트레스척도, 한국판 정서조절곤란 질문지, 간이정신진단검사를 시행하였다. 위계적 회귀분석을 통해 직무스트레스, 정서조절곤란, 정신건강 간의 직접 효과를 검증하고, 부트스트랩 검증을 통해 직무스트레스와 정신건강에 대한 정서조절곤란의 매개효과 유의성을 검증하였다. 분석결과, 직무스트레스와 정신건강과의 관계에서 정서조절곤란의 하위요인 중 전략적 접근 제한과 명료성의 부족은 완전매개역할을 하는 것으로 나타났다. 이는 요양보호사의 정신건강을 위해서는 직무스트레스를 경감시키는 것만으로는 부족하고, 요양보호사들이 자신의 부정적인 정서를 다룰 수 있는 조절능력을 기를 수 있는 방안이 필요함을 시사한다.

호스피스 완화의료 전문기관 간호사의 임종간호수행 영향요인 (Factors Affecting End-of-life Care Performance of Nurses in Hospice and Palliative Nursing Institutions)

  • 본 연구는 호스피스 완화의료 전문기관 간호사의 임종간호스트레스, 죽음인식, 사전의사결정 태도가 임종간호수행에 미치는 영향 정도를 규명하기 위한 서술적 조사 연구이다. 본 연구의 대상자는 보건복지부로부터 호스피스 완화의료 전문기관으로 지정된 의료기관에 근무하는 호스피스 간호사 200명이다. 본 연구의 자료수집은 서면 설문지와 인터넷 설문조사 두가지 방법을 사용하여 2021년 8월 9일부터 9월 30일까지 이루어졌다. 자료분석방법은 대상자의 임종간호스트레스, 죽음인식, 사전의사결정 태도, 임종간호수행 간의 관계를 분석하기 위해 Pearson's correlation coefficient를 사용하였고 대상자의 임종간호수행에 영향을 미치는 요인을 파악하기 위해 Hierarchical Regression을 사용하였다. 본 연구결과 임종간호수행과 죽음인식(r=.22, p=.002), 임종간호수행과 사전의사결정 태도(r=.20, p=.004)는 유의한 상관관계를 나타냈다. 그리고 독립변수를 추가한 MODEL2에서 연령, 사전의사결정 태도, 죽음인식이 유의한 임종간호수행 영향요인이었으며 이들의 설명력은 10.4%이었다(F=3.90, p<.001). 호스피스 간호사의 임종간호스행 능력을 향상시키기 위해서는 그 영향요인을 고려한 중재가 요구된다.

COVID-19 환자를 간호한 간호사의 외상 후 스트레스에 영향을 미치는 요인 (Factors Affecting Post-Traumatic Stress of Nurses in Direct Care for COVID-19 Patients)

  • Purpose: The purpose of this study was to investigate post-traumatic stress, job stress, fatigue, and social support of nurses in direct care for COVID-19 patients and to identify the factors affecting post-traumatic stress among the nurses. Methods: The participants were 150 nurses from three hospitals. Data were collected from September 11, 2020 to September 21, 2020. The data were analyzed with SPSS/WIN/25.0 program. Results: The mean score of post-traumatic stress was 22.27±15.49 (range 0~88). The percentage of high risk group of post-traumatic stress was 35.3%, risk group was 20.0%, normal group was 44.7%. Post-traumatic stress showed statistically significant differences according to quarantined experience (t=2.15, p=.033), and provision of COVID-19 manual (t=-2.40, p=.026). Post-traumatic stress was positively correlated with job stress (r=.48, p<.001), and fatigue (r=.58, p<.001), and it was negatively correlated with social support (r=-.22, p=.005). Job stress was positively correlated with fatigue (r=.74, p<.001), and it was negatively correlated with social support (r=-.17, p=.030). Fatigue and social support (r=-.17, p=.029) had a negative correlation. The results of regression analysis showed that the factors affecting post-traumatic stress included fatigue (β=.56, p<.001), provision of COVID-19 manual (β=-.24, p<.001), and quarantined experience (β=.18, p=.006) and that the total explanatory power was 42.0%. Conclusion: In this study, fatigue, provision of COVID-19 manual, and quarantined experience were found as influential factors of post-traumatic stress among nurses in direct care for COVID-19 patients. Therefore, development of the intervention for reducing the fatigue should consider to prevent post-traumatic stress in nurses. Also, provision of COVID-19 manual for nurses and psychological intervention program for nurses experienced quarantine are necessary.

  • Purpose: This study aimed to identify stress levels due to end-of-life care, coping strategies, and psychological well-being among nurses in neonatal intensive care unit, and to investigate the effect of stress levels and coping strategies on their well-being. Methods: A total of 128 nurses in the neonatal intensive care units of general hospitals in B city participated. The data were collected using a self-report questionnaire. The collected data were analyzed using descriptive statistics, the t-test, ANOVA, the Pearson correlation coefficient, and hierarchical regression with SPSS version 22.0. Results: The coping strategy that nurses most often used was seeking social support. The factors affecting the well-being of the participants were wishful thinking, problem-focused coping and seeking social support, in order. Those 3 variables explained 21 % of the total variance in psychological well-being. Problem-focused coping and seeking social support were positively associated with psychological well-being, while wishful thinking showed a negative association. Conclusion: In order to improve the psychological well-being of nurses in neonatal intensive care units, it is necessary to provide nurses with a program to build a social support system and to improve their problem-based coping skills.
