Error Evaluation of Linearized Equation for a Servovalve in Hydraulic Control Systems (유압 제어계에서 서보밸브 선형화 방정식의 오차 평가)

  • This study evaluates the approximation errors of the existing linearized equation for a servovalve nonlinear flowrate characteristic. At first, the errors are evaluated on flowrate/pressure characteristics diagrams. Subsequently, they are investigated with time response simulation results for several hydraulic control systems. To enable systematic evaluation of computational error, the authors propose three kinds of equations with restructured forms of the existing linearized equation. As results of the evaluations, it is ascertained that comparatively good computational accuracy can be achieved with the existing linearized equation when both an operating point for the linearized equation and operating range of the hydraulic system stay near the flowrate axis of the flowrate/pressure characteristics diagram. In addition, the results show that comparatively big computational error may occur when operating range of a hydraulic system stay apart from the flowrate axis of the flowrate/pressure characteristics diagram.

Economic Length of Stay and Opportunity Income of Appendectomy and Pneumonia Using Activity-based Costing (활동기준원가를 이용한 충수절제술과 폐렴의 경제적 재원일과 재원일 단축에 따른 기회이익)

  • Background: This study aimed to measure the opportunity income by identifying the economic length of stay (ELOS) which is the intersection point of daily revenue and cost on appendectomy and pneumonia cases. Methods: The research subjects were 460 patients of appendectomy and 606 patients of pneumonia, discharged from a general hospital between July 1, 2009 and June 30, 2010. ELOS calculated with both of total revenue on diagnosis-related group (DRG) and fee-for service (FFS). The cost is calculated by activity-based costing system of the hospital. Results: Average length of stay (ALOS) of appendectomy was 4.48 days and its average revenue per case were 1,710,215 (1,989,105) won by DRG (FFS). The variable cost was 491,262 won which was 28.7% (24.7%) of DRG (FFS) total revenue. And 97.2% of the total variable cost was incurred within 2 days from admission. The ELOS was 4 (5) days in DRG (FFS). Shortening three days (two days) would increase opportunity income 52.0% (82.2%) in DRG (FFS). ALOS of pneumonia case was 4.86 days and its average revenue per case were 489,448 (761,426) won by DRG (FFS). The variable cost was 27,230 won which was 5.6% (3.6%) of DRG (FFS) total revenue. Thirty-eight point nine percent of the daily variable cost was incurred in discharge date. The ELOS was 2 (4) days in DRS (FFS). Shortening three days (one day) would increase opportunity income 27.6% (37.2%) in DRG (FFS). Conclusion: The ELOS would be used by strategic index for achieving minimum profit and developing the ways to get there. But we also should not pass over that the opportunity income obtained by the reducing ALOS may cause some problem of quality.

End-point control of a flexible arm under base fluctuation

  • A theoretical study is presented for the end-point holding control of a one-link flexible arm, whose base is subjected to a lateral fluctuation. The arm is clamped on a rigid hub mounted directly on the shaft of d.c. servomoter. The tip position is measured by a gap sensor fixed in space isolated from the system vibration. The arm is controlled so as to make the end point stay precisely at its initial position even if the base is fluctuated.

Changes in Photosynthetic Characteristics during Grain Filling of a Functional Stay-Green Rice SNUSG1 and its $F_1$ Hybrids

  • Functional stay-green is a beneficial trait that may increase grain yield through the sustained photosynthetic competence during monocarpic senescence in cereal crops. The temporal changes of photosynthesis and related characteristics throughout the grain filling period of a stay-green japonica rice "SNU-SG1" was compared in growth chamber conditions with three high-yielding cultivars(HYVs) and their $F_1$ hybrids with SNU-SG1. SNU-SG1 exhibited a typical characteristic of functional stay-green in terms of chlorophyll degradation and photosynthetic competence during grain filling. According to the photosynthesis-light response curve measured at 10 and 35 d after heading for the flag leaf, SNU-SG1 exhibited higher initial light conversion efficiency and thus higher gross photosynthetic rate at light saturation compared to HYVs. Light saturation point was not different among genotypes, ranging from 1000 to 1500 ${\mu}mol$ photon $m^{-2}s^{-1}$. Net photosynthetic rate at light saturation($P_{max}$) of the upper four leaves in SNU-SG1 was much higher and sustained longer throughout grain-filling than HYVs and $F_1$ hybrids. The sustained high photosynthetic competence of SNU-SG1 during grain filling was ascribed to the longer maintenance of high mesophyll conductance that resulted from not only high chlorophyll content and its delayed degradation but also the slow degeneration of photosystem II(PS II) as judged by chlorophyll fluorescence($F_v/F_m$) of flag leaves. $F_1$ hybrids showed slow degeneration of photosystem II similar to the male parent SNU-SG1 while chlorophyll degradation pattern close to female parents, thus exhibiting a little higher $P_{max}$ than female parents. These results suggest that SNU-SG1 has a typical functional stay-green trait that can be utilized for increasing rice yield potential through the improved dry matter production during grain filling.

  • A cable element happens to vibration easily rather than other elements because a cable element has few rotational stiffness. Dynamic motion of stay cable is distinguished from vibration by wind and/or rain and excitation by support movement. Mostly a stay cable is vibrated by wind and/or rain except that when natural periods coincide between stiffening girder and stay cable. It happens to deterioration of serviceability and durability by vortex shedding, rainy-wind induced vibration, and galloping. Additional damping generated by installation of cable damper is well known good scheme against above phenomena. Researchers have lack of effort to develop the recommendations even if cable stayed bridges are designed and constructed in Korea. Therefore, development of the domestic recommendations should be achieved as soon as possible. This study suggests the consistent and systematic recommendations on vibration controlling design of stay cable by installation of damper. It gives readers two important methodologies that one evaluates required damping ratio, the other determines installing point considering efficiency.

Factors Affecting Length of Stay and Death in Tuberculosis Patients(2008-2017): Focus on the Korean National Hospital Discharge In-depth Injury Survey (결핵 환자의 재원기간과 사망에 영향을 미치는 요인(2008-2017): 퇴원손상자료를 중심으로)

  • The purpose of this study is to identify factors affecting length of stay(LOS) and death in tuberculosis(TB) patients by disease type, patient characteristic, admission and disease characteristic, and hospital characteristic from 2008 to 2017. Survey data was using Korean national hospital discharge in-depth survey data produced by Korea Disease Control and Prevention Agency. Study subjects were 10,634 inpatients with TB(A15, A16, A17, A18, A19, U88.0, U88.1, U84.30, U84.31) and analyzed frequency, chi-square test, Fisher's exact test, and logistic regression by using STATA 13.0. As a study result, the type of TB(extrapulmonary TB, multidrug-resistant TB, extensively drug-resistant TB), sex(woman), age(35-49, 50-64, 65-74, 75 years old or older), admission type(outpatient department), CCI(1-2 point, 3 point over), hospital location(metropolitan city) and bed size(300-499, 500-999, over 1000) were significantly influence LOS. Also, the type of TB(extrapulmonary TB, extensively drug-resistant TB), sex(woman), age(50-64, 65-74, 75 years old or older), residence(small town/rural), admission type(outpatient department), CCI(1-2 point, 3 point over), hospital location(provincial) were significantly influence death. In conclusion, the existing tuberculosis management has been patient management with rapid diagnosis and treatment following early detection. But other studies should be carried out for the system that identifies and supports high-risk groups of the long-term length of stay in hospital or high mortality rates as a result of treatment.

  • The purpose of this study was to analyze the claim data of Health Insurance Review & Assessment Service to determine what factors affected the length of stay in patients aged 65 and older and undergoing total knee arthroplasty due to the principal diagnosis of gonarthrosis by the type of medical institutions. As a result of making an analysis, gender, age, medical security type, severity, residential area and the number of sickbeds were identified as the factors that influenced the length of stay in each type of medical institutions. At this point in time when an increase in the elderly population triggered by population aging and another subsequent increase in medical expenses put a heavy strain on household and national economy, it's necessary to consider how to shorten the length of stay and how to ensure the efficient management of sickbeds based on the findings of this study. In addition, this study is of significance in that it could be used as basic data on quality life-care for elderly patients by the introduction of a systematic management system geared toward lessening patient burden for medical expenses.

Comparative Study on Dietary Life and Nutrient Intakes of Elderly Persons at Nursing Home or Their Home in Chungnam (충남지역 일부 시설노인과 재가노인의 식생활 및 영양섭취상태 비교)

  • The purpose of this study was to compare the dietary life and nutrient intakes among elderly persons residing at nursing homes and their homes in order to provide fundamental data for improved foodservice and dietary management. We recruited 150 subjects at a nursing home stay and 150 home stay elderly persons for a survey. The results were as follows. There was no significant difference in frequency of napping between the two groups. The frequency of going out was higher in the home stay subjects, but regular exercise was higher in nursing home stay subjects. Subjects possessed the following diseases: dementia and digestive disorders for nursing home stay elderly men, blood vessel disorder and diabetes for home stay elderly men, digestive and blood vessel disorders for nursing home stay elderly women, and blood vessel and heart disease for home stay elderly women. In a health-related assessment, smoking, drinking, and teeth condition of nursing home stay subjects were not higher than those of home stay subjects. The ratio of subjects who ate meals everyday was higher in nursing home stay elderly women than in home stay women. While most nursing home subjects ate adequate amount of meals, home stay subjects ate until they were full. The daily energy intakes of men and women were 2,229.9 kcal and 2,302.1 kcal for nursing home subjects and 1,894.0 kcal and 1,885.9 kcal for home subjects, respectively. Nutrient intake was also higher in nursing home than home stay subjects. In summary, the dietary life of elderly persons residing in a nursing home was higher than that of elderly persons residing at their home from the view point of meal frequency, meal intake, and nutrient intakes. Therefore, systematic assessment and management of nutrition for elderly staying at a nursing home or at home alone should be conducted continually.

Optimal Opportunistic Spectrum Access with Unknown and Heterogeneous Channel Dynamics in Cognitive Radio Networks

  • We study the problem of optimal opportunistic spectrum access with unknown and heterogeneous channel dynamics in cognitive radio networks. There is neither statistic information about the licensed channels nor information exchange among secondary users in the respective systems. We formulate the problem of maximizing network throughput. To achieve the desired optimization, we propose a win-shift lose-stay algorithm based only on rewards. The key point of the algorithm is to make secondary users tend to shift to another channel after receiving rewards from the current channel. The optimality and the convergence of the proposed algorithm are proved. The simulation results show that for both heterogeneous and homogenous systems the proposed win-shift lose-stay algorithm has better performance in terms of throughput and fairness than an existing algorithm.

The Prediction of Health care Outcome of Total Hip Replacement Arthroplasty Patients using Charlson Comorbidity Index (Charlson Comorbidity Index를 활용한 고관절치환술 환자의 건강결과 예측)

  • The objectives of the present study is to examine the validity of Charlson Comorbidity Index(CCI) based on medical record data; to utilize the index to determine outcome indexes such as mortality, length of stay and cost for the domestic patients whose have received total hip arthroplasty. Based on medical record date, 1-year Mortality was analyzed to be 0.664 of C statistic. The $R^2$ for the predictability for length of stay and the cost was about 0.0181, 0.1842. Fee of national health insurance and total cost including the cost not covered by insurance, also had statistically significance above 3 points of Charlson point score(p=0.0290, 0.0472; $p.{\le}0.05$). The 1-year mortality index, length of stay and cost of the total hip arthroplasty patients which was obtained utilizing CCI have a limitative prediction power and therefore should be carefully analyzed for use.
