Development of the implant surgical technique and assessment rating system

  • Purpose: There has been no attempt to establish an objective implant surgical evaluation protocol to assess residents' surgical competence and improve their surgical outcomes. The present study presents a newly developed assessment and rating system and simulation model that can assist the teaching staffs to evaluate the surgical events and surgical skills of residents objectively. Methods: Articles published in peer-reviewed English journals were selected using several scientific databases and subsequently reviewed regarding surgical competence and assessment tools. Particularly, medical journals reporting rating and evaluation protocols for various types of medical surgeries were thoroughly analyzed. Based on these studies, an implant surgical technique assessment and rating system (iSTAR) has been developed. Also, a specialized dental typodont was developed for the valid and reliable assessment of surgery. Results: The iSTAR consists of two parts including surgical information and task-specific checklists. Specialized simulation model was subsequently produced and can be used in combination with iSTAR. Conclusions: The assessment and rating system provided may serve as a reference guide for teaching staffs to evaluate the residents' implant surgical techniques.

저비용항공사 체계적 안전관리 활동을 통한 경영활성화 방안에 대한 연구 (A Study on the Revitalization Method of National Carrier(Low cost carrier) Safety Management System)

  • Purpose - In this study, the safety related tasks for low cost carriers' were examined and measures to improve them were suggested. As the air demand increases rapidly, number of passenger has mushroomed in Korea and in order to keep with the demand low cost carriers have played a role. However, low cost carriers' safety related tasks are not reliable level so far. Thus, in this study, the low cost carriers' current situations in terms of safety are investigated, especially regarding how well the ICAO's new safety policies are applied. Research design, data, and methodology - The results of the Ministry of Land, Infrastructure and Transportation' investigation are introduced and field study results are analysed. Current status of retaining experts, staffs, administrators in terms of aviation safety tasks of low cost carriers is compared to those of major airliner and low cost carriers' aviation safety culture is also analyzed by examining the systems and questioning staffs. Especially the culture regarding aviation safety is very important because the culture surrounding the safety tasks plays a major role in every respect of conducting the tasks. Results - Overall the current status of low cost carriers' aviation safety tasks is below standard. Especially retaining experts and staffs is basically below the required level. Also, system and organization to conduct safety tasks are not satisfactory. In particular, aviation safety culture is not settled to operate appropriately. Conclusions - The International Civil Aviation Organization (ICAO) and member states are continuing to launch new safety policies in response to the surging demand for air travel worldwide. The most urgent and important issue among the various safety policies is to reduce air accidents. In order to reduce the number of accidents, ICAO decided to reduce the number of accidents by using the Reactive Safety Management. ICAO has to ensure that each member country can implement proactive safety management for aviation safety. A safety management system (Safety Management System) is a system in which each member state implements a fulfillment standard. The current situation and problems of the safety management system for each airline are suggested and proposed for improvements.

요양시설 규모에 따른 직원의 감정노동, 직무만족이 조직몰입에 미치는 영향 (The Effects of Emotional Labor and Job Satisfaction on Organizational Commitment in Staffs by the Size of Facility)

  • Purpose: The purpose of this study was to identify the factors influencing organizational commitment of staffs according to the size of long-term care facility. Methods: A cross-sectional descriptive study was designed. Data collection was conducted for a total of 315 employees in long-term care facilities located in Seoul, Gyeonggi, Gangwon, Gyeongbuk, and Chungnam. Data were collected from July 2018 to October 2018 using questionnaires which included emotional labor, job satisfaction, organizational commitment, and general characteristics. In order to confirm the differences in the size of the facility, the facilities with less than 30 beds, those with 30-99 beds, and those with more than 100 beds were analyzed. Data were analyzed using descriptive statistics, t-test, ANOVA, Mann-Whitney U test, Kruskal-Wallis H test, Pearson's correlation analysis, and multiple regression. Results: The job satisfaction and organizational commitment were significantly different according to the size of long-term care facility. Organizational commitment was influenced by 'external job satisfaction' in less than 30 beds, was influenced by 'external job satisfaction, and attentiveness to required display rules of emotional labor' in 30~99 beds, and then was influenced by 'type of job, and internal job satisfaction' in more than 100 beds. The predict variables accounted for 23.0%, 41.0%, and 34.0% of organizational commitment respectively. Conclusion: These findings show that tailored interventions should be provided depending on the size of facility in order to increase organizational commitment. In addition, organizational commitment programs should be developed by considering strategies to reduce the emotional labor and to increase job satisfaction.

설문조사를 통한 건설업 작업환경측정제도 개선방안에 관한 연구 (Improvement of Work Environment Measurement in Construction Industry by Survey)

  • Objectives: The Purpose of this study is to improve work environment measurement in construction industry by survey. Especially, this study focused on the defferences between construction staffs(safety manger, health manager) and work environment measurement institution workers. Methods: Survey targets are construction staffs(safety manger, health manager) and work environment measurement institution workers. Respondent was selected by respective councils and conferences. A questionnaire consisting of items on general characteristics and the improvement of work environment measurement in construction industry. Results: Total of 134 people(39 safety managers, 27 health managers, 68 work environment measurement institution workers) responded to survey. Overall, current work environment measurement was deemed unsuitable for construction industry. There was a significant difference between construction staff and work environment institution workers in some question. On the other hand, there was no noticeable significant difference in general characteristics. Conclusions: As a result of survey, current work environment measurement was deemed unsuitable for construction industry in terms of measurement method, measurement timing, and actual site improvement. In addition, there is a statistically significant differences in response between construction staff and work environment measurement institution workers, so it is necessary to reflect both opinions in order to improve work environment measurement in construction industry.

  • Chemistry laboratory class was produced for senior high school freshmen with the cooperation of university staffs, high school teachers and the university students. Although the students who will take the lab class are senior high school freshmen, we decided to prepare four experiments that are simple and have highly visible reactions or transformation. That is, 1) Water purification, 2) Surface modification, 3) Briggs-Rauscher reaction, and 4) Polymer synthesis and characterization. After the safety guideline and experiment instructions by the faculty staffs, two teaching assistants (TAs) supervised each experiment. Since taking a direct part in it will provide stronger impact than only being one who is just watching the experiment, all experiments contained some process that the high school students must handle the reagents, tools or the equipment, by themselves. Although, the operation performed by the students was limited to a series of simple actions, the various unique phenomena presented by the experiments impressed the students. The lab class was fruitful not only for high school students, but also good for university students. The lab class provided good opportunity for them to improve the abilities to teach and guide someone.

국내수목원 조성 및 운영경향에 관한 연구 (A Study on the Composition and Operating Trend of Arboreta in Korea)

  • Research and conservation of plant species for the nation arboretum, collection and securing of the plant species for both public and private arboreta and education for the arboreta which is affiliated to the university are overall management goal of the arboreta in Korea. Most of the arboreta in Korea run the programs and events for guide and interpretation of forest, but the programs and events are not well specified by the arboreta. Regarding the reasonable level of entrance fee should be discussed further as the lack of information and understanding. But the specified entrance fee between public and private arboreta should be considered. An eighty three percent of arboreta in Korea has been properly managed the species information. The reasonable management should be considered in case of unusual management of collected plant genetic resources. Although the overall impending difficulties for the management of arboreta in Korea is the lack of trained or qualified staffs, securing tasks independently for the public arboreta and relatively low salary for the private arboreta are main difficulties. Most of the difficulties are derived from the lack of its mission of the arboreta. The management of arboreta by the mission statement is one of the core targets including the characters such as climate or region should be considered. The securing qualified or trained staffs should be considered in the development of arboreta in Korea including the modification of relevant regulations or law.

서울지역 일반 한의원과 네트워크 한의원의 환자만족도 및 추천의사에 미치는 영향요인 비교 (Comparison of Patients' Satisfactions with General Korean Medicine Clinics and Networked Korean Medicine Clinics in Seoul, Korea)

  • Objective : This study aimed to compare the patients' satisfactions with local Korean Medicine(KM) clinics and networked KM clinics, and to find out which factors had an effect on it. Method : Six local KM clinics and six networked KM clinics were selected, and patients' questionnaires on the satisfaction with KM clinic use were administered using exit poll method. The questionnaires were developed to measure satisfactions with facility, doctor and staffs, and services. 240 patients completed the questionnaires, and the data were statistically analyzed. Results : There was more diversity in patients' diseases among networked KM clinics users, however, treatments provided were not differentiated clinically between two types of KM clinics. Patients' satisfaction scores for local KM clinics were significantly higher than those for networked KM clinics in the aspects of doctors/staffs (9.35 vs 8.92, p<.01) and services (8.76 vs 8.42, p<.05). 90.0% of local KM clinic users expressed their willingness to recommend the clinics they used to others, while 75.0% of networked KM clinics users did. For both the local and networked clinics, 'KM doctors' kindness', and 'process and waiting time' were statistical factors affecting patients' satisfaction. Conclusion : The patients' satisfactions with general KM clinics were higher than those with networked KM clinics. Networked KM clinics need to develop services to meet the diverse medical demands which were created by them selves.

수도권 대학병원 행정 직원의 직무만족에 미치는 영향 (The Influence on Metropolitan University Hospital Administrative Staff Job Satisfaction)

  • 본 연구는 의료 환경이 공급자 중심에서 소비자 중심으로 변화함에 따라 행정직원에게 동기를 부여하여 직무만족도를 향상시킴으로써 경영의 효율성을 높이고자 하였다. 연구대상은 수도권 대학병원 행정직원 총 305명으로 설문된 데이터를 이용하였다. 분석방법은 AMOS 21 Ver.을 사용하여 구조모형방정식(SEM)으로 통계적 검증을 실시하였다. 첫째, 모형의 적합도 검증결과 $X^2=206.776$, df=77, Q=2.685, P<.001, GFI=.915, TLI=.927, CFI=.946, RMSEA=.074로 분석 되었으며, 본 연구를 검증하는데 타당한 것으로 나타났다. 둘째, 성과보상과 근무환경은 동기부여에 통계적으로 정(+)의 방향으로 영향을 미친다는 점이 나타났다. 셋째, 동기부여는 직무만족에 통계적으로 정(+)방향으로 유의하게 나타났다. 넷째, 의사소통이 직무만족에 정(+) 방향으로 영향을 미친다는 점이 통계적으로 유의하게 나타났다. 다섯째, 매개효과 검증 결과, 성과보상과 직무만족의 관계에 있어 동기부여는 완전매개로 나타났고, 근무환경과 직무만족의 관계에 있어 동기부여는 부분매개로 나타났다. 공정하고 합리적인 성과보상과 직장 안정성, 복리후생 등의 근무환경, 조직내외의 원활한 의사소통은 직무만족도를 높인다. 이를 위해서는 경영자의 관심과 의지가 필요하며, 다양한 교육훈련과 프로그램개발이 필요하다. 본 연구를 통하여 병원 행정 직원의 만족도 향상을 위한 제도적 개선 방안을 모색하는 기초적인 자료를 제공하고자 한다.

중재적 방사선시술에서 부가 차폐체 사용 시 종사자의 산란선 피폭 감소효과 (Effect of Reducing Scattering Radiation Exposure of Medical Staffs When Additional Shielding is Used in Interventional Radiology)

  • This article is designed to look into the radiation exposure dose to each body part and the shielding effect for workers using an additional shielding to reduce their radiation exposured by scattering radiation which is generated in a space between the operating table and lead curtain during interventional radiology(IR) procedures. After placing a human phantom on the table of SIEMENS' angiography machine, the following measurements were taken, depending on the presence of an additional shield of lead equivalent of 0.25 mmPb, manufactured for this purpose: dose to gonad, dose to an area where the personal dosimeter is placed, and dose to an area of eye lens is located. An ion chamber(chamber volume 1,800 cc) was utilized to measure scattering radiation. The two imaging tests were carried out as follows: fluoroscopy of the abdomen (66 kV, 100 mA, 60 seconds) and of the head (70 kV, 65 mA, 60 seconds); and digital subtraction angiography(DSA) of the abdomen (67 kV, 264 mA, 20 seconds) and of the head (79 kV, 300 mA, 20 seconds). In all the experiments, the shielding efficiency of the gonad position was the largest at 59.8%. In case an additional shielding was used as protection against scattering radiation that came through the operating table and the lead curtain during an IR, the radiation shielding efficiency was estimated to be up to 59.8%, leading to a conclusion that its presence may effectively reduce the radiation exposure dose of medical staffs.

의료기관 인증제에 대한 인식이 요양병원 간호인력의 환자안전관리 활동에 미치는 영향 (Influence of Recognition for Health Care Accreditation on Patient Safety Managing Activities of Nursing Staffs in Geriatric Hospital)

  • 본 연구는 2013년 요양병원의 의무 인증제 이후 간호인력의 의료기관 인증제에 대한 인식과 환자안전관리 활동 정도를 조사하고, 인증제 인식과 환자안전관리 활동 간의 상관관계 및 환자안전관리 활동에 미치는 영향요인을 확인하여, 요양병원 간호인력의 환자안전관리 활동을 증진하기 위한 기초자료를 제공하고자 시행되었다. 본 연구는 서술적 조사연구로 서울, 인천, 부천 소재의 의료기관 인증을 획득한 6개 요양병원 소속 간호사 및 간호조무사 대상으로 2016년 4월 18일부터 4월 30일까지 구조화된 설문지를 활용하여 총 182명의 자료를 수집하였다. 수집된 자료는 SPSS/WIN 15.0 Program을 이용하여 t-test, ANOVA, Pearson's correlation coefficient, 다중 회귀분석을 실시하였다. 연구결과, 의료기관 인증제의 인식정도는 문항평균 3.60점(최고 5점), 환자안전관리 활동은 문항평균 4.39점(최고 5점)이었다. 의료기관 인증제의 인식과 환자안전관리 활동 간에는 유의한 양적 상관관계(r=.339, p<.001)가 있었고, 환자안전관리 활동에 가장 큰 영향을 미치는 변수는 '내부서비스 품질향상'(${\beta}=.362$, p<.001)이었고, 이외 '최종학력'(${\beta}=.194$, p=.005)이 유의한 영향을 미쳤으며, 이들 변수의 설명력은 16.4% 이었다. 따라서 요양병원 간호인력의 환자안전관리 활동이 증진되기 위해서는 의료기관 인증제에 대한 인식, 특히 '내부서비스 품질향상' 인식을 높이기 위한 기관의 인증제 교육프로그램 운용의 정규화 및 활성화를 위한 제고방안이 포함되어야 한다.
