Objectives: The aim of this study is to investigate chiropractic spinal manipulation and internal diseases in articles. Methods: It was investigated how many articles had been searched for spinal manipulation applied to internal disease in Pubmed Database Results: 1. There are 19 articles of chiropractic spinal manipulation and internal diseases were investigated. 2. It was reported that there are improvements of spinal manipulative therapy on asthma, injury of the optic nerve, hypertension, vertigo, tinnitus, hearing loss, etc. Conclusions: There are several reports on effectiveness of spinal manipulative therapy on asthma, injury of the optic nerve, hypertension, vertigo, tinnitus, hearing loss, etc. And It is considered that spinal manipulative therapy shows improvement on the internal diseases.

Objectives: The purpose of this case study was to report the effect of Chuna therapy include Space Spinal Conduction Exercise and Manipulation Treatment for Functional dyspepsia patients. Methods: A 32-years-old male patients with functional dyspepsia visited the clinic ten times from March 17th 2023 to April 22th 2023. To assess the treatment outcomes, we used the Standard tool for pattern identification of Functional dyspepsia and Nepean Dyspepsia Index-Korean version (NDI-K) symptom score. Result: After treatment, Standard tool for pattern identification of Functional dyspepsia and Nepean Dyspepsia Index-Korean version (NDI-K) score decreased. Conclusion: Chuna therapy include Space Spinal Conduction Exercise and Manipulation Treatment was effective in the treatment of patient with Functional dyspepsia.

Objectives : The aims of this study were to introduce the theory of traditional Chinese orthopedic theory(整脊理論) and to analyze the guideline of diagnosis and treatment of common spinal disorders by treating Chinese spinal manipulation technique(常見病中醫整脊指南). Methods : We have searched the web sites of traditional Chinese orthopedics association (整脊學會), a branch of the China academy of traditional Chinese medicine (TCM), and related articles. Additionally, we analyzed the guideline of diagnosis and treatment of common spinal disorders by treating Chinese spinal manipulation technique which was introduced by that association at 7th academic conference in China in 2011. Results : The guideline was prepared by 2 year project leaded by professor Yi-zhong Wei who was a person in charge of the spinal manipulation guideline of national standardization committee of TCM in China. It was a summary of the whole theory of China spinal manipulation therapy in TCM and contained 22 common spinal disorders by the treatment of traditional Chinese Tuina methods. Spinal manipulation theory was summarized by one doctrine, two theories, and two important view points. The treatment method was abstracted as 8 major Tuina techniques. Conclusions : The guideline will be a way of advance of Chinese spinal manipulation technique. This will be a good reference to Korea Chuna manipulation for improving their techniques.

Objectives : The aim of this systemic review was to summarize and compare adverse effect of spinal manipulation at domestic and abroad. Methods : We searched articles in Korean databases and Pubmed. We selected and analysed the articles met inclusion criteria. Results : 51 cases were included. 32 cases in Pubmed show that adverse effect related to cervical rotation and high velocity low amplitude(HVLA) thrust manipulation. It leads to severe adverse effect and sequela about vascular. While, 19 cases in Korean articles shows that domestic spinal manipulation is more safe becase of lower use of high velocity low amplitude(HVLA) thrust. It leads to low incidence of vascular adverse effect and no sequela. Conclusions : We concluded that Chuna is difficult manipulation because of possibility of adverse effect. Chuna is recommended to be taken by licensed medical practitioners who are skilled and complete the regular curriculum. Further studies like meta analysis about spinal manipulation are needed to estimate incidence of adverse effect. And adverse effects are going to be collected continually through randomized controlled trials(RCT) related to spinal manipulation.

Objective: This study aimed to investigate the short-term effects of flexion-distraction spinal manipulation on intervertebral height, pain, spine mobility in patients with lumbar degenerative disc disease. Design: Randomized controlled trial with a pretest-posttest control group design Methods: A total of 96 participants with degenerative disc disease participated in the study and were randomly divided into two groups. Both groups received intervention for 3-5 minutes a day. The experimental group (n=48) underwent flexion-distraction spinal manipulation for 3-5 minutes, and the control group (n=48) was maintained in the same position as the experimental group for 5 minutes without any intervention. The intervertebral height was measured by computed tomography, pain was assessed using visual analog scale, and the spine in flexion mobility was measured using the finger-to-floor distance test and passive straight leg raise test. Pre-test and post-test measurements were obtained. Results: The experimental group showed significant improvement in intervertebral height, degree of pain, and spinal mobility (p<0.05). The intervertebral height increased from 6.32±1.90 to 6.93±1.85 mm (p<0.05), lower back pain decreased from 69.17±13.35 mm to 48.48±12.20 mm (p<0.05), lumbar spine mobility changed from 17.37±4.49 to 12.69±4.34 cm (p<0.05), and passive straight leg raise test range increased from 46.94±13.05° to 56.01±12.20° (p<0.05). Conclusions: This study suggests that flexion-distraction spinal manipulation could be an effective treatment for decreasing pain and improving function in patients with degenerative disc disease.

Purpose : Neck pain is commonly experienced by both adolescents and adults. The purpose of this study was to measure neck disability index(NDI) and the cervical range of motion(CROM) following spinal manipulation and mobilization techniques. Methods : Thirty participants(manipulation=15, mobilization=15) with mild neck disability volunteered for this study. It was used to measure NDI with NDI questionnaire and six motions(neck flexion and extension, left and right lateral flexion, left and right rotation) with the CROM instrument. Independent t-test and paired t-test were used to estimate NDI and CROM, and compared left with right motions. Results : A significant decrease in NDI were found after spinal manipulation and mobilization treatment(p<.05). A trend toward increase in all cervical motions(flexion, extension, left and right lateral flexion, left and right rotation), after interventions, were revealed(p<.05). The significant differences were not found on NDI and CROM between the group(p<.05). It was presented asymmetric motions in cervical lateral flexion(p<.05) before and after treatment. Conclusions : Our results suggest that manipulation and mobilization probably provide at least short-term benefits for patients with mild neck pain. It is needed to be studied the factors and preventive methods influencing the asymmetric cervical motion.

Objective: This study aimed to analyze the effects and characteristics of the height of the treatment table on the force and time of ground reaction (GR) and contact hand (CH) generated from the therapist's feet to generate thrust during spinal manipulation (SM). Design: A cross-sectional survey study Methods: Thirty-six healthy subjects were recruited. SM was performed on the ilium using a knee-high table, where the therapist felt it was easy to control the subject's posture and body shape and comfortable to generate force, as well as a relatively high thigh-high table. The force and time generated by the therapist's GR and CH were simultaneously measured through a force plate. Results: As a result, there was a significant difference in peak force and rundown force at the therapist's GR according to the table height (p < 0.05). In the therapist's CH, there was a significant difference between PreMin (preload minimum) force and peak force (p < 0.05), and there was a significant difference between the time from PreMin to peak and the time of the entire section (p < 0.05). Conclusions: As a result, the generation of increased CH force and faster thrust duration were confirmed by mobilizing the reduced GR force of the therapist to generate thrust than the relatively high table on the knee-high table.

Background: The purpose of this study was to investigate the effect of the combined application of manipulation and stabilization exercises on pain and spinal curvature in patients with chronic back pain. Design: Randomized controlled trial Methods: The research subjects included 24 women in their 40s and 50s who have chronic back pain. The sample was evenly divided into an experimental group, which received the combined application of manipulation and stabilization exercises, and a control group, which received stabilization exercises only. The 30-minute intervention was applied five times a week for eight weeks. A bivariate repeated measures analysis of variances was conducted to identify the differences between the two groups before the experiment, after the fourth week, and at the end of the eight-week experiment. The level of statistical significance was set at.05. This analysis examined the within-group changes and the between-group changes using a paired t-test and an independent t-test, respectively. Results: Changes in pain differed significantly depending on the time of the measurement, the interaction between the time of the measurement and each group, and between the two groups (p<.05). Changes in the curvature of the bones of the neck, the bones of the back, and the lumbar vertebrae differed significantly depending on the time of the measurement and the interaction between the time of the measurement and each group (p<.05). Conclusion: The combined application of manipulation and stabilization exercises demonstrated a positive effect on changes in pain and spinal curvature, and the method is expected to be a useful intervention for reducing pain and improving spinal curvature in patients with back pain.

Examination at the spine is dependent on assessment of function. At all spine levels a number of general anatomical considerations hold sway which together dictate the format of the evaluation. Because disc lesions are common and generally responsive to treatment, the examination sets out to differentiate between disc lesions and the other sources of pain as well as establishing the particular treatment that will benefit any given displacement. As elsewhere in the body, displacements give rise to certain characteristic symtoms and signs. First, the history is indicative. Second, any loose fragment in the joint restricts spinal movement in some but not all directions, producing the non-capsular pattern characteristic of internal derangement. Third, a displacement protruding posteriorly interferes with the dura mater ; apart from pain, this adversely affects the dura's normal painless mobility. Fourth, a displacement protruding laterally connects with the appropriate nerve root emerging from the dura mater. Finally, compression of the spinal cord strongly contraindicates manipulation, the primary treatment for cartilaginous displacements.

Objectives This study aims to comprehensively review research utilizing ultrasonography for assessing manipulative treatment on spinal diseases, with the goal of promoting the wider integration of ultrasound imaging into clinical practice. Methods A systematic search was conducted on three international databases (Embase, PubMed, Cochrane) up to July 23, 2023. The search included key terms such as ultrasonography, manipulation, and skeletal muscle. The inclusion criteria narrowed down the selection to studies specifically related to lumbar and cervical vertebrae. Results Eleven studies were included in the review, with 10 focusing on lumbar vertebrae and one on cervical vertebrae, all employing spinal manipulation treatment. Among the 11 selected studies, nine primarily focused on ultrasound imaging to measure muscle thickness, while two utilized shear wave elastography to assess muscle stiffness. Also, rigorous measures were taken to ensure the reliability of the ultrasonography data. Conclusions This scoping review highlights the limited but growing evidence supporting the use of ultrasonography to assess manipulative treatment for spinal diseases. Despite a scarcity of studies in South Korea, it is crucial to recognize the potential of ultrasonography in becoming a widely used and practical tool for evaluating the effectiveness of manipulative treatments in the near future.
