A Case Report on Soojeom-san plus Jeungmiyijin-tang for Treatment of Gastroesophageal Reflux Disease with Hiatal Hernia (식도 열공 탈장을 동반한 위식도역류질환 환자의 수점산합증미이진탕 1증례 보고)

  • Objective: This study reports on a treatment case of Soojeom-san plus Jeungmiyijin-tang (SJJI) on gastroesophageal reflux disease with hiatal hernia. Method: We considered a male patient suffering from gastroesophageal reflux disease with hiatal hernia because of blood stasis and damp-heat of the spleen and stomach pattern and prescribed SJJI. The progress was evaluated using the frequency scale for the symptoms of gastroesophageal reflux disease (FSSG) score, and the visual analogue scale for abdominal pain, sore stomach, and other symptoms. Results: The patient's symptoms, which included abdominal pain, sore stomach, dyspepsia, anorexia, insomnia, etc., almost disappeared. Conclusion: SJJI can be used for patients who have a chronic condition of gastroesophageal reflux disease with hiatal hernia because of blood stasis and a pattern of damp-heat of the spleen and stomach.

A Case Report of Stage 4 Pressure Ulcer Patient Treated with Acupuncture and the Herbal Medicine Gamisipjeon-tang (침구 치료와 가미십전탕을 병행하여 호전된 4도 욕창 환자 치험 1례)

  • Objectives: The purpose of this case study is to report the effect of oriental medicine on pressure ulcers. Methods: We treated a patient with a pressure ulcer with acupuncture, Jaungo, and Gamisipjeon-tang. We started with herbal medicine only, and then added the acupuncture treatment after one month. We treated the sore for a total of 4 months. We measured the size and depth of the ulcer and graded it with a National Pressure Ulcer Advisory Panel (NPUAP) score once every two weeks. Results & Conclusions: Even though the wound was not cured completely, the total size of the pressure ulcer was reduced and the NPUAP stage was improved from 4 to 3. Therefore, Korean medicine could be an effective treatment for improving pressure ulcers.

The Treatment of Pressure Sore Using Hydrogel and Hydrocolloid Dressing (하이드로겔과 하이드로콜로이드 드레싱을 이용한 압박성 궤양의 치험례)

  • Pressure sores are a common complication of hospitalized patients. However, It is often impossible to correct surgically because the general conditions of these patients are poor. It is known that the hydrogel has a powerful autolytic effect by providing moist environments and facilitates wound healing and hydrocolloid dressing is also known to promotes granulation tissue formation and epithelialization. The patients were treated with hydrogel(Purion $gel^{(R)}$, Coloplast A/S, Denmark) and hydrocolloid dressing (Comfeel Plus Transparent $Dressing^{(R)}$, Coloplast A/S, Denmark) after surgical debridement of pressure sores progressed to stage III(n=2) and IV(n=7). This combination treatment could facilitate to debride the nectrotic tissue and promote granulation tissue formation epithelialization simultaneously. We could achieve complete healing of pressure sores using the combination treatment without requiring surgical correction. In conclusion, hydrogel in combination with hydrocolloid dressing is effective in acheiving complete healing of progressed pressure sores.

  • The reconstruction of recurrent pressure sores is challenging due to a limited set of treatment options and a high risk of flap loss. Successful treatment requires scrupulous surgical planning and a multidisciplinary approach. Although the tensor fascia lata flap is regarded as the standard treatment of choice-it provides sufficient tissue bulk for a deep trochanteric sore defect-plastic surgeons must always consider the potential of recurrence and accordingly save the second-best tissues. With the various applications of anterolateral thigh (ALT) flaps in the reconstructive field, we report two cases wherein an alternative technique was applied, whereby pedicled ALT fasciocutaneous island flaps were used to cover recurrent trochanteric pressure sores. The postoperative course was uneventful without any complications. The flap provided a sound aesthetic result without causing a dog-ear formation or damaging the lower-leg contour. This flap was used as an alternative to myocutaneous flaps, as it can cover a large trochanteric defect, recurrence is minimized, and the local musculature and lower-leg contour are preserved.

  • Background The pedicled anterolateral thigh (ALT) flap has become more popular for the reconstruction of soft-tissue defects in neighboring areas. Nonetheless, few studies in the literature have explored the use of this flap for trochanteric ulcer reconstruction. The aim of our study is to present the author's experience of utilizing the ALT flap, with a focus on technical elements regarding the flap design and the tunneling method to maximize the reach of the flap. Methods The medical records of patients who received pedicled ALT flaps for the reconstruction of trochanteric pressure sores were retrospectively reviewed. The patients' demographic data, operative details, and postoperative complications were evaluated. Results Between October 2018 and December 2019, 10 consecutive patients (age range, 13-45 years) underwent 11 pedicled ALT myocutaneous flaps for trochanteric pressure sore reconstruction. Each flap was designed around the most distal cutaneous perforator that was included in the proximal third of the skin paddle. The flaps ranged in size from 11×6 to 14×8 cm. The ALT flap was transposed through a lateral subcutaneous tunnel in five patients, while the open tunnel technique was used in six patients. All flaps survived, and no vascular compromise was observed. Conclusions The pedicled ALT flap is a safe and reliable option for reconstructing trochanteric pressure sores. An appropriate flap design and a good choice of the tunneling method are crucial for successful flap transposition.

Capstone Design Trail in Nursing Education and Its Outcome (간호학에 적용한 캡스톤 디자인의 적용사례 및 결과)

  • This study is a methodological study analyzing the results of applying capstone design to nursing education. Among the nursing college students who completed the fundamental nursing subject, the students who agreed to participate in the research were composed of the project team and proceeded as team activities. It was conducted through knowledge - team composition - topic selection / analysis - design / development - simulation - evaluation / feedback process starting from knowledge learned through fundamental nursing class. The research period was from January 1, 2015 to October 30, 2015 for 6 months. BBS (Bed Sore Socks) was developed for the prevention of pressure ulcers, and BBS was applied for seven days to randomly selected patients in the long term care facility. The incidence and pressure ulcer risk scores were assessed. The results of the pilot study showed that the intervention group had an effect on the incidence of pressure ulcer ($x^2=.40$, p = .500) and the pressure ulcer risk score (z = -.45, p = .690), but it was not statistically significant. Capstone design was trailed in the field of nursing education and produced the bed sore socks. In the nursing education, not many capstone design trails appeared yet, this study might be the first trail in the field of nursing, and it could be challenging for expanding of nursing education.

A Literature Study on the Korean Acupuncture for Eye diseases (국내침구서적의 안질환(眼疾患)치료에 관한 문헌 연구 - "치종지남(治腫指南)" "동의보감(東醫寶鑑)" "침구경험방(鍼灸經驗方)" "교감(校勘) 사암도인침법(舍岩道人鍼法)"의 비교연구 -)

  • Background : Eye diseases refer to a wide range of disconveniences from conjunctivitis, pterygium, glaucoma to even blindness. Acupuncture has been widely used in Korea throughout the history and provides an efficient method in the treatment of them. Objectives : Establish a distinctive and efficient acupuncture method for the treatment of eye diseases based in literature research. Method : We reviewed four Korean medical literature, "Guide to Swollen Sore Treatment[治腫指南]", "Treasured Mirror of Eastern Medicine[東醫寶鑑]", "Experiential Prescriptions of Acupuncture and Moxibustion[鍼灸經驗方]", and "Essential Rhymes on Acupuncture and Moxibustion by Master Saam[舍岩鍼法]", and analyzed the therapeutic characteristics in the treatment of eye diseases. Result : 1. According to "Guide to Swollen Sore Treatment[治腫指南]", various methods were applied in the treatment of eye diseases. We can cite salt water washing method after needling, pricking bloodletting method using three-edged needle, surgery method using bent needle and lance needle, or sore treatment using sliced bean-curd and ground Aristolochiae Fructus among others. Acupuncture points like GV20[百會], BL1[睛明], EX-HN5[太陽], GB20[風池], GV24[神庭], GB1[瞳子髎], and GB15[臨泣] were mostly needled. 2. In "Treasured Mirror of Eastern Medicine[東醫寶鑑]", pricking bloodletting method were most frequently used in comparison to single acupuncture or moxibustion methods. Applied points were GV20[百會], BL1[睛明], LI4[合谷], EX-HN5[太陽], GB37[光明], BL18[肝兪], GB20[風池], BL2[攢竹], GB1[瞳子髎], and ST36[三里]. Also selections of adjacent points were considered important. 3. In respect to treatment methods "Experiential Prescriptions of Acupuncture and Moxibustion [鍼灸經驗方]" has some similarity to "Treasured Mirror of Eastern Medicine[東醫寶鑑]" as pricking bloodletting method were mostly used. Also focused on normal Qi flow through meridian. Points like BL18[肝兪], BL1[睛明], LU5[尺澤], EX-HN5[太陽], LI4[合谷] were used. 4. "Essential Rhymes on Acupuncture and Moxibustion by Master Saam[校勘舍岩道人鍼法]" considered visceral pattern identification method fundamental in the diagnosis and treatment of eye diseases. Specifically, Liver, Heart, Stomach, Lung, Kidney identification methods are presented. Combined both corresponding and connecting meridians supplementation and draining methods according to mother-child relation. Also Saam master's own experiential prescriptions are noted. Conclusions : After previous study on stroke, we could also find various efficient methods according to eye diseases, through literature research of korean medical classics. This study will concurrently result in establishing distinctive therapeutic method characteristic of Korea.

Analysis of the Impact on Community Health after Accidental Leak of Hydrofluoric Acid (일개 응급의료센터에서 분석한 불화수소산 대량 누출이 지역사회 건강에 미친 영향)

  • Purpose: The purpose of this study is to verify the influence of a massive hydrofluoric acid spill on community health through patients who claimed to have been exposed. Methods: We analyzed 2459 patients who visited our emergency department with the claim of exposure to hydrofluoric acid, and retrospective analyses were performed. We analyzed changes in numbers of visitors per day from the day of the accidental hydrofluoric acid spill, symptoms presented by the 1924 patients, and general characteristics. Comparisons of symptoms and hematologic characteristics were made between the initially set evacuation zone(1.3 km radius parameters from the spill) and the outer zone. Results: A total of 2,459 patients who claimed exposure visited our ED from 27 September 2012 to 23 October 2012, and there was a significant increase in the number of visiting patients from day 8 of the hydrofluoric acid spill. The most common complaints were a sore throat, 729(37.9%) and no specific symptom with health concern, 547 (28.4%). Statistically significant findings were pulmonary symptoms (p=0.001), nasal symptoms (p=0.001), diarrhea (p=0.023), and skin symptoms (p=0.007). In hematologic study, a statistically significant difference was observed in white blood cell count (p=0.018), creatine phosphokinase (p<0.001), erythrocyte sediment rate (p=0.013), and phosphorus (p<0.001). Conclusion: A significant increase in the number of patients was observed one week after the accidental spill of hydrofluoric acid. The most frequent symptoms were sore throat, headache, cough, and sputum. Statistically significant increase in creatine phosphokinase level and decrease in phosphorus level were noted in patients within the evacuation zone.

Case Report of Pressure Injury in Intracerebral Hemorrhage Patients Improved by Combining Radix Astragali Pharmacoacupuncture Solution during Antibiotic Treatment (항생제 치료 중 드레싱과 황기 약침액 도포를 병행하여 호전된 뇌출혈 환자 욕창 1례)

  • Background: Pressure injuries are localized areas of damage to the skin and/or underlying tissue, usually over a bony prominence due to pressure. Cerebrovascular disease increases the risk of pressure injuries due to the immobility caused by physical paralysis. The general approach to managing a patient with pressure injuries should include pain relief, the treatment of the infection, optimizing nutritional intake, proper positioning, and contamination prevention. Nonetheless, the duration of treatment for pressure injuries varies from person to person. Case report: An 80-year-old female intracerebral hemorrhage patient developed a pressure injury. To improve the injury faster, a Radix Astragali pharmacoacupuncture solution was applied to the pressure sore. The pressure injury's width, length, and depth was assessed using a ruler, and the exudate amount and tissue types were assessed. The treatment was performed for 35 days. The rate at which the size of the pressure sore lessened increased since the Radix Astragali pharmacoacupuncture solution was applied to the pressure sore. In addition, the tissue type of the pressure injury improved, and the exudates decreased. There was no significant difference in the Pressure Ulcer Scale for Healing Tool 3.0, since the Radix Astragali pharmacoacupuncture solution was applied to the pressure injury. Conclusion: This clinical case study suggests that the Radix Astragali pharmacoacupuncture solution might be effective in speeding up the healing of pressure injuries.

Especially to eliminate the differences in severity among many raters the randomization procedure of raters sample was very effective in enhancing the reliability of ratings with comparatively small groups of examinees and raters. And we also introduced the new rating methods, i.e. the 2-step diagnostic procedures to check the sizes of the reliability stability of raters and the sore adjustment method to enumerate the optimal mean values in rating the examinees.
