한국판 우울과 신체 증상 평가 척도(Depression and Somatic Symptom Scale, DSSS)의 신뢰도와 타당도 (Reliability and Validity of Korean Version of Depression and Somatic Symptom Scale(DSSS))

  • Objectives : We examined the reliability and validity of the Korean version of the Depression and Somatic Symptom Scale (DSSS) in Korean patients with depressive symptoms. Methods : Participants were 55 outpatients diagnosed with major depressive disorder, a depressive episode of bipolar I disorder, somatoform disorder, panic disorder, generalized anxiety disorder, or post-traumatic stress disorder according to the DSM-IV criteria. We assessed them using the Korean versions of the DSSS, Hamilton Depression Rating Scale (HDRS), and Quick Inventory of Depressive Symptomatology Self Report (QIDS-SR). Results : The Korean version DSSS had a Cronbach's alpha of 0.90. Moreover, each item's correlation with the total score was statistically significant (r=0.24-0.71, p<0.01). The test-retest correlation coefficient (r=0.83, p<0.01) was relatively high, and the DSSS correlations with the HDRS and QIDS-SR were 0.77 and 0.74, respectively. Conclusion : These results demonstrate that the Korean version of the DSSS could be a reliable and valid tool for screening and assessing depressive patients. The Korean version of the DSSS will be a useful tool for screening both depressive and somatic symptoms in Korea.

신체증상을 주증상으로 호소하는 우울증 환자에 대한 침치료와 명상의 결합을 응용한 치험 2례 (Report of 2 Depression Patients Accompanying Somatic-symptom Treated with Meditation of the Retaining Needle)

  • Depressive Disorders are widespread and increasing recently. Somatic-symptom Group in Major Depressive Disorders are treated with current medical treatment but the results are not satisfactory. We tried to apply more effective and satisfactory oriental medical treatment. We treated 2 depression patients accompanying somatic-symptom treated with herbal medication and Meditation of the retaining needle. The Meditation of the retaining needle(鐵膜想), which combines Meditation and Acupuncture is a kind of medical meditation originating from Oriental Medicine. After our treatments, various somatic-symptoms were improved and Beck's depression Inventory(BDI) decreased from 28 and 27 to 14 and 12. This result suggests that meditation of the retaining needle(鐵膜想) is effective on somatic symptom and able to be developed as a useful way of oriental medical psychological care.

Causal Attributions, Social Support and Psychological Morbidity in Patients Undergoing Thyroid Surgery

  • Purpose: The primary objective of the present study was to investigate the course of anxiety and depression in thyroidectomy. The second objective was to investigate the association between somatic symptoms and psychological symptoms, and the final objective was to identify the sociodemographic, clinical, and psychosocial factors that affect psychiatric morbidity. Methods: This prospective study, 101 patients who were admitted to the Endocrine Surgery Clinic of the Department of General Surgery, Istanbul Faculty of Medicine, University of Istanbul, with thyroid pathology were included in the study. Data were collected before surgery, and in the early, and late periods after surgery. Data were collected using a semi-structured interview form, a visual analogue scale, and the Hospital Anxiety and Depression Scale. Results: The average scores of anxiety and depression before surgery were statistically significantly higher compared with the early and late periods after surgery (P<0.001). Psychological morbidity was identified about 10% of patients before surgery and remained after thyroidectomy. A poor positive correlation founded between somatic symptoms and psychological condition in period surgery (P<0.05). Various sociodemographic features and psychosocial parameters affected anxiety and depression level (P<0.05). Conclusion: Anxiety and depression level in thyroid diseases decreased after thyroidectomy. These patients may express psychologic distress through somatic symptoms. Screening of psychological morbidity is suggested in patients planned thyroid surgery, especially in women, low-educated, inadequate social support, attributed to psychological causes.

음악치료를 적용하여 호전된 신체형 장애 여환 치험 1례 (Application of Music-Listening Intervention for Somatoform Disorder : A Case Report)

  • Objectives : This study was performed to report the effect of music-listening intervention and relaxation for somatoform disorder. Methods : A 51 year-old female patient suffered from several somatic symptoms without identifiable physical cause. We saw her symptoms were related to her psychological factors. We applied music-listening intervention and relaxation besides Korean traditional medical treatment to make her be aware of her psychological condition such as inner conflict and know symptoms are related to her psychological condition. Results : After the music-listening intervention and relaxation, the patient revealed her unrecognized hidden feelings and emotions and got some insights about herself. Then somatic symptoms such as dysesthesia and urinary frequency and anxiety were decreased considerably. Conclusions : These results suggest that the music-listening intervention and relaxation can be effective on improvement of soma to form disorder.

소방공무원의 지각한 현장출동 충격경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향 (The Influence of Field-Shock Experience and Post Traumatic Stress Perceived by Fire Officials upon Somatic Symptoms and Coping Methods)

  • 본 연구는 소방공무원들의 지각한 현장출동 충격 경험과 외상 후 스트레스가 신체증상 및 대처방식에 미치는 영향을 규명하기 위하여 강원도 소재 소방안전센터에 근무하고 있는 소방공무원 남녀 362명을 대상으로 조사하였다. 수집된 자료는 Windows용 SPSS 19.0과 AMOS 19.0을 사용하여 빈도분석, 타당도와 신뢰도 분석을 위한 요인분석과 Cronbach's ${\alpha}$ 분석, 기술통계와 상관관계를 실시하였고, 가설 검정을 위해 구조방정식 모형을 이용한 분석을 실시하였다. 모든 통계분석은 유의수준은 p<.05로 설정하였다. 결과는 다음과 같다. 첫째, 지각한 현장출동 충격 경험은 외상 후 스트레스에 영향을 미치는 것으로 나타났다. 둘째, 지각한 현장출동 충격 경험이 신체증상에 영향을 미치는 것으로 나타났다. 셋째, 지각한 현장출동 충격 경험은 대처방식에 영향을 미치는 것으로 나타났다. 넷째, 외상 후 스트레스는 신체증상에 영향을 미치는 것으로 나타났다. 다섯째, 외상 후 스트레스는 대처방식에 영향을 미치는 것으로 나타났다. 여섯째, 신체증상은 대처방식에 영향을 미치는 것으로 나타났다. 이러한 결론으로 볼 때 충격적 스트레스를 해소하기 위한 제도적 방안으로 전 세계적으로 검증이 되어 실제 운영되고 있는 위기상항 스트레스 해소법(Critical Incident Stress Debriefing : CISD)을 소방서의 실정에 맞게 도입하여 운영하는 것이 필요할 것이다.

다기관 모집 외상 후 스트레스장애 환자에서 외상의 특성과 신체적 증상과의 관계 (The Relationship between Somatic Symptoms and the Trauma-Related Characteristics of Patients with PTSD : A Korean Multicenter Study)

  • Objective : The aim of this study is to examine the characteristics of somatic symptoms in patients with PTSD according to trauma type. Methods : The subjects of this study were 84 patients diagnosed with PTSD according to the DSM-IVTR and CAPS criteria. The subjects ranged in age from 18 to 76 years, and they were recruited from 18 hospitals across the nation. All participants were asked about their trauma history, and they all completed the Davidson Trauma Scale (DTS), Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), Impact of Event Scale-Revised (IES-R) and Korean-Physical Somatic Symptom Inventory (K-PSI). The items of the K-PSI were categorized into 5 groups according to organ system, and the participants were classified into 6 groups according to trauma type. We compared the results of the social demographical scale, DTS, BDI, STAI, IES-R and each recategorized subscale of the K-PSI among the six groups. Results : There were significant differences between the 6 groups in terms of the Korean-Physical Somatic Symptom Inventory scores for each organ system, except for the cardiopulmonary system, as well as the BDI scores. Post hoc analysis revealed differences between the combat-related trauma group and all other groups but not among any of the other groups. Conclusion : Our result showed that there were significant differences in the somatic symptom scores among the 6 trauma groups. However, patient age and the time elapsed since the traumatic event may have hada crucial influence on the result of this study.

  • 신체화를 이해하는데 신체화를 형성하게 된 행동이면의 정신적인 갈등배경을 이해하는 것이 중요하다. 그러나 신체화의 생물학적 과정을 파악하고 증상의 유지요인과 악화요소를 알아보아 변화되어 가는 형성과정을 이해하는 점이 더욱 중요하다. 그리고 문화-사회적 특성에 따라 신체증상호소의 방법이나 내용이 다르다는 점을 염두에 두어야 한다. 신체화를 이해하는 데에는 심리적, 생물학적, 사회, 인종, 역사적인 요소가 모두 함께 영향을 미치므로 통합적인 관점에서 파악해 나가지 않으면 안된다.

소아청소년 신체증상 관련장애의 한의학적 치료에 대한 고찰 : 국내 증례 보고 논문을 중심으로 (A Review of Korean Medicine Treatment for Children and Adolescents' Somatic Symptom and Related Disorder : Focusing on Case Study Reports in Korea)

  • Objectives The goal for this study is to find out the recent trends of the Korean medicine treatment on children and adolescents' somatic symptom and related disorder by reviewing case studies. Methods We found case study reports relevant to 'Korean medicine treatment on somatic symptom and related disorder' through Oriental Medicine Advanced Searching Integrated System (OASIS), National Discovery for Science Leader (NDSL) and Research Information Sharing Service (RISS). The age of the patients was limited to under 20. Results All reported articles showed that the treatments were selected based on both emotional problems and physical symptoms. Herbal medicine was the most commonly used method, and the other methods including acupuncture and psychotherapy were also used. Conclusions This study shows that the common Korean medicine treatment can be widely used for children and adolescents' somatic symptom and related disorder. Further research is needed to evaluate the effectiveness of the treatments, and to develop guidelines for the treatment.

우울 및 불안장애 환자에서 신체 통증과 관련된 인지정서조절전략 (The Relationship Between Somatic Pain and Cognitive Emotion Regulation Strategies in Patients with Depression and Anxiety Disorder)

  • Objective : Emotional state and emotion regulation strategies are considered to be important factors influencing the pattern and severity of somatic pain. The aim of this study is to investigate the relationship between cognitive emotional regulation strategies and somatic pain in patients with depression and/or anxiety disorders. Methods : A total of 140 outpatients, diagnosed with depression and/or anxiety disorders according to DSM-IV-TR, were evaluated using the Beck Depression Inventory (BDI), the State-Trait Anxiety Inventory (STAI), the Cognitive Emotion Regulation Questionnaire (CERQ), and the Numerical Rating Scale (NRS) for somatic pain. Pearson correlations and independent t-tests were performed to analyze the relationship between somatic pain, the severity of depression and/or anxiety and cognitive emotion regulation strategies. Results : The severity of pain was significantly correlated with depressive symptoms, but not with anxiety. Patients with somatic pain tend to use maladaptive cognitive emotion regulating strategies more frequently, especially rumination and catastrophizing. Conclusion : These findings suggest that somatic pain correlates with maladaptive cognitive emotional regulating strategies. Interventions which modulate these non-productive strategies, especially rumination and catastrophizing, would be a new approach for managing patients with depressive and/or anxiety disorders who are suffering from somatic pain.

화병환자를 대상으로 한 음악청취 중재의 효과에 대한 연구 (Effectiveness of Music-listening intervention on Hwa-byung Patients)

  • Objective: The objectives of this preliminary study was to develop cost-effective and accessible self-intervention program and to assess the effects of music listening self intervention program for Hwa-byung patients, who had undergone conflicts with family and economical problems, in quantity and quality. Subjects: Seven middle age women who bad been diagnosed with Hwa-byung by Hwa-Byung Diagnotic interview Schedule participated in this program. Method: Participants learned music listening techniques and visited hospital three times a week during 2 weeks. Music-listening self intervention for Hwa-byung patients was consisted of three sad musics(in minor) and three sedative musics(in major) and perlO1med at home for 2 weeks. Patient's somatic symptoms and self awareness (of Hwa-byung scale), anxiety(STAD, anger(STAXD, depressive mood(BDD were evaluated before and after intervention and 6 weeks later after study completed. And various and subjective responses of Music-listening self intervention for Hwa-byung patients were collected by semi- structured interview made for this study. Results: Their Somatic symptoms(especially insomnia), self-awareness of Hwa-byung and depressive mood were improved except anxiety, anger. This improvement had maintained over a 6-week follow-up period. Patients had used constantly the music-listening program for Hwa-byung patients after the study completed. Conclusion: The music-listening program will be an effective for Hwa-byung patient's somatic symptoms and depressive mood and had been used constantly by them. But, much methodological supplementation will be needed to assess clinical effects on this program.
