Background: The recent change in pharmaceutical education system following the paradigm shift to patient-oriented pharmacy service requires an in-depth discussion to reorganize a future direction and establish a basis for maximizing social values of community pharmacy service. Objective: This study was conducted to review the current status of community pharmacy service provision in Korea based on published literatures. Methods: The electronic databases of National Digital Science Library and Electronic National Assembly Library were used to search the journal articles and dissertation papers. A search term "community pharmacy" was used and the published period was limited to papers published after year 2001, when the legal separation of prescribing and dispensing was implemented. Relevant study reports were also searched manually. Information about pharmacy service provision and study outcomes were retrieved from the selected papers, and classified by predefined individual service scope. Results: A total 33 papers reporting services provided by community pharmacies were selected (journal article 11, dissertation paper 17, and study report 5). Pharmacy services identified in these papers could be classified into prescription dispensing service, pharmaceutical care service, self medication service, other products service, and health promotion service. Twenty papers reported prescription dispensing services, three papers reported pharmaceutical care service, and only two papers reported health promotion service. Current community pharmacy services are highly dependent on prescription drugs while expanded services such as pharmaceutical care and health promotion are peripheral. Most prevalent research topic was medication counseling service (18 papers), reflecting that community pharmacists generally consider it to be the most important and fundamental service. Overall, current pharmacy services are very limited and focus on prescription dispensing service. Conclusion: At this point of time requiring expansion and quality improvement of community pharmacy services, we suggest further lively discussion to strengthen pharmacist's functional identity and set conditions for providing socially expected services.
Background : Asthma is one of the most prevalent diseases in Korea. Although the guidelines of asthma management were reported in Korea, the present pattern of asthma management by primary physicians has not been studied. The purpose of this study is to elucidate the pattern of asthma management by primary physicians. Methods : In November 2002, 710 primary physicians specializing in internal medicine in Seoul, Korea were provided with two scenarios of asthmatic patients, one mild and the other severe. By mail or interview, the physicians were asked several questions about their present pattern of asthma management for the patients in each scenario. Results : Among 710 primary physicians, we obtained the answers from 325 physicians (response rate 46%). The most preferred prescription was oral theophylline. 71% and 81% of the physicians answered that they would prescribe oral theophylline for the mild and severe asthmatics, respectively. The next prescription preferred were mucolytics and oral ${\beta}_2$-agonist, in that order. However, 36% and 56% of the physicians answered that they would prescribe inhaled steroids for the mild and severe asthmatics, respectively. Among diagnostic tests, physicians preferred pulmonary function test to the rank next to chest radiography. Conclusion : The primary physicians in Seoul prefer oral bronchodilators to inhaled steroids in asthma management. More efforts should be made to reduce the difference between the present pattern of asthma management by primary physicians and the asthma guidelines.
Background: Medication policy development in Thailand is continually promoting rational drug use. Letrozole, an endocrine therapy drug, is usually prescribed for post-menopausal status early and advanced stage breast cancer. After Ministry of Public Health announced Letrozole as compulsory licensed drug in 2009, more breast cancer patients can access to this drug at low cost especially those within universal coverage schemes. To ensure that Letrozole is rationally prescribed, the drug utilization study was conducted. Objectives: The aim of this study was to describe the appropriate use of Letrozole in breast cancer and the relationship between appropriate use and health benefit schemes. Materials and Methods: A retrospective study to evaluate use of Letrozole in breast cancer patients was performed for six months between January - June 2010 in seven regional cancer hospitals, Thailand. All prescriptions of Letrozole were identified from pharmacy dispensing databases and prescription papers. A medical record review was also performed to evaluate appropriate use referring to the drug use evaluation criteria. The approved criterion of this study was referred from the guideline of Thai National Formulary version 2010. Results: There were 681 prescriptions of Letrozole for 254 breast cancer patients with an average age of $58.6{\pm}10.0years$. The patients in universal coverage scheme (UCS), civil servant medication benefit scheme (CSMBS) and social security scheme (SSS) were 77.7%, 18.5% and 8.7% respectively. 10.6% were prescribed Letrozole for the first time. Letrozole were prescribed by oncologists (82.8%). The average number of tablets per prescription was $58{\pm}10$. Calcium supplements were prescribed concomitant with Letrozole for 19.4%. To assess drug use evaluation criteria, 45 prescriptions were excluded because of uncompleted clinical data, 636 prescriptions were evaluated. The study showed 86 prescriptions (13.5%) with inappropriate use including 6 (0.9%) not prescribed for estrogen receptor (ER) and/or progesterone receptor (PR) positive, 31 (4.9%) not prescribed for post-menopausal and 49 (7.7%) not prescribed for an appropriate duration. Appropriate use percentages in different health benefit schemes were similar, 85.7% of CSMBS, 86.4% of SSS and 86.7% of UCS. The relationship between health benefit scheme and appropriate use of Letrozole was not significantly different, ${\chi}^2$ (2, N = 636) = 0.081, p > 0.05. Conclusions: The study showed inappropriate use in breast cancer patients because of non-compliance with duration, menopausal status and hormone receptor requirements. To prescribe appropriate indication did not referred to the appropriate practice along the treatment. Drug use evaluation proved very useful for detecting the sign of inappropriate use and allows immediate feedback to the stakeholder for developing medication policy in the future. Importantly, there was no significantly difference in appropriate use of Letrozole across health benefit schemes.

This study analyzed the awareness and demand of participants in a physical activity therapy program(PATP) of a social welfare-type cultural regeneration space in urban and rural areas. As a result of analyzing the demographic characteristics, the female(68.4%) were more than the male, and 30-39 years old(53.3%) were the most generation in the PATP participants. As a result of frequency analysis by all subjects, gender and age, 78.4% of the respondents had never participated in PATP before. The most duration of the PATP was 30 to 60 minutes(24.6%). 73.5% of the respondents answered that it was not feeling difficult at all to the intensity of PATP, and 94.8% of the respondents were satisfied with the intensity of PATP. The most respondents preferred to participate again with if the new PATP starts(97.2%) and to join the new PATP with his or her family members(85.7%). In addition, the participants under the age of 39(24.3%) perceived that the effect of improving physical fitness by PATP was higher than over 40 years of age(11.9%). As a result of logistic regression analysis, it was found that only the age had a significant effect(p<.01) on intensity of PATP. It means that the perceived intensity of PATP was different between the older and young generation. However, the PATP was not designed to meet the participants age and fitness levels, and even the general exercise prescription guidelines based on professional scientific data such as exercise frequency, exercise intensity, exercise time, exercise type, and characteristics of the participants in the spaces. In conclusion, to encourage and promote the participants motivation and health-related fitness level in the spaces for the future. The various type of PATP that include a few different intensities for all genders and age groups, and a customized program based on systematic and scientific exercise prescription guidelines.

Through analyzing the frequencies of medicinal herb combinations in the prescriptions of "wenrejingwei(溫熱經緯)", we could understand the characteristics of prescription composition and author's medical thoughts. So we conducted this study that analyze combination of 187 medicinal herbs in 113 prescriptions, and found meaningful combinations of 273. Among these, the most meaningful combinations are the one of 7 herbs, Glycyrrhiza uralensis Fischer, Zingiber Rhizoma, Ziziphus jujube Mille, Pinellia ternata Breitenbach, Panax ginseng C.A.Meyer, Cyprinus carpio, Coptis chineis Franch. These are the same medicinal herbs which consist of Banhasasim-tang(banxiaxiexin-tang). It means these combinations are effective in stuffiness caused by stomach deficiency. In conclusion, prescriptions of "wenrejingwei(溫熱經緯)" involve herbs mostly used to treat stuffiness caused by stomach deficiency, and this is the author's important one of medical view points.

So we conducted this study that analyze combination of 324 medicinal herbs in 467 prescriptions, and found meaningful combinations of 5,045. Among these, the most meaningful combination is the one of 10 herbs, Glycyrrhiza uralensis Fischer, Angelica gigas Nakai, Ziziphus jujube Mille, Atractylodes japonica Koidzumi, Poria cocas Wolf, Zingiber officinale, Rehmanniae radix preparata, Cinnamomum loureirii, Panax ginseng C.A.Meyer, Astragalus membranaceus. This combination is Sibjeondaebo-tang(Shiquandabu-tang) without Cnidium officinale and Paeonia japonica which compose Samul-tang(Siwu-tang). It means this combination is kind of modified Sibjeondaebo-tang(Shiquandabu-tang), which is more effective in tonifying qi than tonifying blood. Prescriptions of "Bangyakhappyeon(方藥合編)" involve herbs mostly used to improve weak and frail health and the herbs which are more effective in tonifying qi than tonifying blood. Through this study, we know the author' s one of medical view points on tonifying qi and blood, attaching importance to tonifying qi relatively to tonifying blood.
As people get broader access to health information through the internet, there is a greater need for measure to maximize the social advantages of the internet and to minimize negative side-effects. With this concern, this paper classifies internet health information services sites into : on-line supply of health information, on-line consultation, on-line diagnosis, and on-line sales. As well this paper analyzes domestic laws supporting and/or regulating these services. The efficient provision of internet health services requires comprehensive laws on individual privacy protection, prevention and handling of medical accidents, an electronic prescription form for internet diagnosis, electronic signing, payment for medical expenses, qualifications for internet medical practitioners. Additional laws are required to establish internet pharmacies and internet health goods stores. These new laws can be prepared either separately or through revision of existing laws governing medical practice, pharmacies, and public health promotion. However, as the legal control by the government on cyber processes and entities has a fairly minimal effect, consumers should be encouraged to improve their own capacity for wisely using internet health services and health-service providers should be encouraged to promote voluntary supervision and control of their own services and practices.

To accept folk remedies in view of oriental medicine and to use as one of social health materials, 1 have looked for the phase institution of folk remedies in the oriental medicine and their adopting method to the oriental medicine, on the basis of the present social-communicating situation, the degree of interest in folk remedies among people and oriental medicine doctors and the content of folk remedies in Dongyibogam(東醫寶鑑). The results are following; The researches of relative importance in the folk remedy's meaning of Dongyibogam show that the proper use can help disease cured, not by unconditional choice but by symptoms. The use of folk remedies are different from the prescription in the oriental medicine, as showing the type and ratio of folk remedies and prescriptions in Dongyibogam. When considering used materials, forms and time in the meaningful content of folk remedies in Dongyibogam, they established two categories. One is radical treatment on acute disease by short-term use. The other is slow treatment on chronic disease by long-term use. For wide application to people, they also had the prudent attitude to accept symptom-oriented treatment and tried to connect with syndrome which needed diagnosis as precisely as possible.
Obesity is psychological and socioeconomic problems as well as health problems related to physical disease and disorder. The obesity epidemic, including a marked increase in the prevalence of obesity among pregnant women, represents a critical public health problem throughout the world. Gung-Gui-Tang (GGT), a prescription of traditional Korean medicine, has been used to treat dizziness due to loss of blood as well as static blood after childbirth. However, the therapeutic potential of GGT on postpartum obesity has not been fully elucidated in an experimental model. In our research, GGT inhibited the increases of body weight and adipose tissues in postpartum mice fed a high-fat diet. GGT also inhibited the elevations of plasma lipid profiles such as triglyceride, low-density lipoprotein cholesterol, total cholesterol, and glutamate pyruvate transaminase. Overall, these results provide evidence that GGT can help to inhibit postpartum obesity and open new perspective to recover the shape of mother into the moment of conception.
For giving a person to understand that the relation between being used medicine and psychiatric medicine in the Nae ui won(內醫院). The psychiatric medicine were studied in the Nae ui won's diary. The results are as follows ; 1. It is supposed that the symtoms of Geong Chung, Hyun hun (眩暈) and Bulmyn(不眠) mainly appeared in 1908. 2. It is supposed that the symptoms of Guibeong(氣病) and ulchung(蔚症)mainly appeared in 1920. 3. It is supposed that the symptoms of Guibeong(氣病) and Talyeong(脫營) mainly appeared in 1924. 4. There were Cheongshim(淸心), Anshin(安神) and chingan(鎭肝) which added to a regular medical prescription. Finally with the help of these conclusions of which we have refered, we found that the cause of disease were changed according to the social situation of being free or repressed, and it also coinside with what Nae ui won's record said and practical result.
