  • We experienced rare cases of Snapping plicae on radiohumeral joint which arthoscopic plicae resection improved symptom of painful snapping elbow. We report that Arthroscopy can be used as a effective therapeutic and diagnostic method for snapping plicae on radiohumeral joint with a review of its literature.

  • Elastofibroma dorsi is a benign soft tissue mass, not well-known because of its low incidence, and usually located between the chest wall and the inferomedial aspect of the scapula. This lesion is not true neoplasm but rather reactive hyperplasia of elastic fibers. It is mostly nontender mass, but occasionally causes snapping symptom. This tumor should be considered as a differential diagnosis of snapping scapula. The clinical diagnosis is made by magnetic resonance imaging and confirmed by pathologic findings. We present a case report of a female with elastofibroma dorsi, who had that the chief complaint was snapping scapula and palpable mass. We emphasize that snapping lesions located deep beneath the inferior tip of the scapula on the chest wall should arouse suspicion of an elastofibroma dorsi.

  • Painful and disabling snapping scapula is an unusual condition and there have been several reports that have revealed good results after surgical treatment for unilateral snapping scapula. We experienced a case of bilateral painful snapping scapula in a young man, but with successful treatment by partial resection of the superomedial angle of both scapula. Preoperative 3-dimensional CT revealed bony prominence of the superomedial angle of both scapula and narrowing between superior scapular angle and chest wall.

  • The snapping scapula is a relatively uncommon phenomenon occuring as a consequence of some anomalous conditioin existing between the thoracic wall and the undersurface of the scapula. It may present in several ways like crepitus, pain during scapular movement and limitation of scapular motion. The causes of snapping scapula are changes in the intervening soft tissues, the muscles, or the bursae between the scapula and the chest wall; and changes in the congruence between the anterior scapular surface and the underlying chest wall. The congenital elbow fusion or humeroradioulnar synostosis is an extremely rare inheritable disorder that previously described just a few reports. Recently we experienced a case of snapping scapular with congenital bilateral elbow fusion treated by arthroscopic scapulothoracic bursectomy and consequent open superomedial scapulectomy. We would describe the clinical feature of a snapping scapula and result of treatment with literature review.

  • Purpose: Snapping triceps syndrome is dynamic condition in which medial head of triceps snaps (dislocates) over the medial epicondyle as the elbow is flexed. Materials and Methods: The symptoms are pain or snapping at the medial aspect of the elbow and/or symptoms from coexisting ulnar nerve irritation. The diagnosis can be made by dynamic ultrasonography. Results and Conclusion: And successful outcome can be archived by operative treatment, which are ulnar nerve anterior transposition and tenotomy of medial head of triceps.

Arthroscopic Resection of Synovial Plica in Elbow (관절경을 이용한 주관절 활막 추벽의 제거)

  • Purpose: To report our experience with nine cases of pathologic synovial plica on radiohumeral joint which symptom of painful snapping elbow was improved by arthroscopic resection. Materials and Methods: Between 1999 and 2004, 43 cases of elbow arthroscopy were performed by one surgeon. Eight patients with nine cases showed pathologic synovial plica in conjunction with snapping or posterolateral elbow pain. The mean age of eight patients (man: 7, woman: 1) was 29 years (range $16{\sim}56$ years). All patients had a trial of conservative treatment at least six months (range $6{\sim}16$ months). The diagnosis was confirmed before surgery in six cases and at the time of surgery in three cases. Pain, snapping, and subjective results were evaluated at least 12 months in the average(range $12{\sim}24$). Results: All patients showed a hypertropic lateral synovial plica with local synovitis. Seven of them had an associated lesion of chondromalacia on radial head. One of them was associated with radiocapitellar arthritis and had a wrapping over the radial head. Six patients experienced improved posterolateral pain at the end of study (VAS<1). However, the other two patients had occasional pain with activity. One of them persisted with mechanical symptoms, which was treated with additional arthroscopic procedure. Conclusion: Synovial plica in elbow should be considered as an important entity of common elbow disease, especially when pain in the lateral aspect of the elbow with a snapping sensation during motion is featured in patients. The arthroscopic resection seems to be safe and efficient in the short and long term treatment of plica in the elbow joint.

  • Osteochondromadevelop most commonly at distal femur, proximal humerus and proximal tibia, but the rib osteochondroma was reported less commonly. In this report, scapular snapping syndrome complicated by adventitious bursa and soft tissue pseudotumor surrounding the osteochondroma of the $6^{th}$ rib body was treated successfully by surgical excision of them. We report this rare case with reviewing the relevant literature.

  • Iatrogenic damage of the rotator cuff followed by postoperative shoulder function loss is a potential complication after antegrade intramedullary nailing (AIN) for a humeral fracture. The authors present a case of arthroscopic rotator cuff repair and subacromial decompression of a non-healed rotator cuff tendon (mainly supraspinatus) and secondary impingement syndrome caused either by the tear or a proud nail after AIN for a mid-shaft humeral fracture. At presentation, the patient complained of right shoulder pain and 'snapping', especially during forward elevation and abduction of the shoulder, of 4 years duration. Right shoulder pain started sometime after pain due to the humeral shaft fracture, operation had subsided, and persisted after nail removal. Arthroscopic findings showed a longitudinal rotator cuff tear at the nail entry point that had not healed and severe fibrous hypertrophy on the acromion underspace, which is a unique finding since most longitudinal splits of tendon fibers are more likely to heal than conventional rotator cuff tears detached from bone. The torn rotator cuff was repaired after debridement and placing side-to-side sutures. At his 34-month follow-up after rotator cuff repair, the patient showed complete recovery and had excellent clinical scores.

  • 발음성 삼두근 증후군은 주관절을 굴곡하거나 신전할 때 삼두근 말단부 일부와 척골 신경이 내상과 전방으로 탈구되는 드문 질환이다. 이학적 검사 만으로는 다른 질환으로 오진되는 경우가 많아 정확한 진단이 필수적이며 확진을 위하여는 척골 신경 및 삼두근의 비정상적인 움직임을 관찰할 수 있는 영상 검사가 필요하다. 초음파 검사는 자기 공명 영상 촬영 등 다른 검사에 비해 간편하고 효과적으로 연부 조직의 동적 검사를 시행할 수 있는 장점이 있다. 저자들은 발음성 삼두근 증후군 환자를 동적 초음파 검사를 사용하여 진단하고 척골 신경 전방 전위술 및 삼두근 내두의 이전술을 시행하여 치유하였기에 문헌 고찰과 함께 초음파 검사의 유용성을 보고하고자 한다.
