뇌종양 환자의 양성자 치료 시 Range Compensator의 Smooth Thickness 적용에 따른 선량비교 (Dose comparison according to Smooth Thickness application of Range compensator during proton therapy for brain tumor patient)

  • 목 적 : 양성자 치료 시 사용되는 Range Compensator는 Target의 Distal Margin의 선량에 대해 정상조직에 전달되는 양성자 빔 선량을 보정하는 역할을 한다. 이에 뇌종양 치료에 사용되는 Range Compensator의 Smooth Thickness를 다르게 적용함에 따른 PTV와 OAR의 선량을 비교하여 대상 부위의 선량이 개선되는 것을 확인해 보고자 한다. 대상 및 방법 : 본원에서 양성자 치료를 받은 뇌종양 환자 10명을 대상으로 Eclipse Proton Planning System(Version 10.0, Varian, USA)의 Compensator Editor를 사용하여 Range Compensator에 적용되는 Smooth Thickness를 각각 1회에서 5회까지 순차적으로 적용하였다. 치료계획의 알고리즘은 Proton Convolution Superposition(version 8.1.20 or 10.0.28)을 사용하였고, Smooth Thickness를 단계적으로 적용함에 따른 PTV의 Dmax, Dmin, Homogeneity Index, Conformity Index 그리고 종양주위의 OAR 선량을 비교하였다. 결 과 : Smooth Thickness를 1회에서 5회까지 적용하였을 때 PTV의 최대선량(Dmax)은 최대 4.3%, 최소 0.8%, 평균 1.81% 감소하였으며, 최소선량(Dmin)은 최대 1.8%, 최소 0.2%, 평균 0.82% 증가하였고, 최대선량과 최소선량의 차이는 최대 5.9%, 최소 1.4%, 평균 2.63% 감소하였다. Homogeneity Index는 평균 0.018 감소하였고 Conformity Index는 거의 변화가 없었다. OAR 선량은 Brain Stem에서 최대 1.6%, 최소 0.1%, 평균 0.59% 감소하였으며, Optic Chiasm에서 최대 1.3%, 최소 0.3%, 평균 0.45% 감소하였으나, C와 E환자가 각각 0.3%, 0.6% 증가하였다. 그리고 Rt. Optic Nerve에서 최대 1.5%, 최소 0.3%, 평균 0.8% 감소하였으나, B환자가 0.1% 증가하였다. Lt. Optic Nerve에서는 최대 1.8%, 최소 0.3%, 평균 0.67% 감소하였으나, H환자가 0.4% 증가하였다. 결 론 : 뇌종양 환자의 양성자 치료에 사용되는 Range Compensator의 Smooth Thickness가 단계적으로 적용될수록 Compensator의 해상도가 증가하여 가장 최적화된 양성자 빔 선량을 전달할 수 있다. 이는 PTV에 좀 더 균일한 선량을 조사할 수 있고 또한 OAR에 작용하는 불필요한 선량을 감소시켜 부작용을 줄일 수 있을 것으로 사료된다.

Preparation of harvested skin using the Versajet Hydrosurgery System in full-thickness skin grafts

  • During a full-thickness skin graft procedure, the thickness of the harvested donor skin is adjusted based on the location of the recipient site and the judgment of the surgeon. Conventionally, the thickness of the harvested skin is roughly adjusted using surgical instruments such as scalpels and scissors. However, this method is not only time-consuming, but also requires effort to obtain both the desired thickness of the harvested skin and a smooth surface of that skin. Moreover, there is a possibility of skin perforation. Hence, the authors devised a method of adjusting harvested skin thickness using the Versajet Hydrosurgery System. The Versajet device is a handheld hydrosurgical tool that delivers a high-speed jet stream of saline solution, which enables the precise debridement of tissue. This method makes it easier and faster for the surgeon to obtain the desired thickness of harvested skin. In addition, by obtaining a smooth surface and an even thickness of harvested donor skin, this technique may lead to improved graft viability.

  • It was analyzed and experimented on the change of the material thickness according to the size of the "R" of the punch and die corners using the material of SCP-1 0.25mm As a result, the following conclusions were obtained. Tensile strength analysis and safety analysis of materials are very important process for each process in strip layout, and Through this, the Influx of material and the deformation of the material were found. As a result of safety analysis and tensile thickness analysis, when the corner R of the punch was 0.3 mm and the edge R of the die was 1.0 mm The inflow of the material was not smooth, and the thickness of the corner part became 0.2 mm, causing cracks. when the corner R of the punch was 0.5 mm and the edge R of the die was 1.5 mm The inflow of the material was smooth, The thickness of the corners of the product is 0.21mm and It was considered that cracks do not occur when the thickness of the bottom surface and the body part becomes thin. The results obtained by applying the results obtained from the analysis, In Experimental Condition 1, a crack occurred in the same part of the analysis In Experimental Condition 2, the flow of the material was smooth and the drawing processing could be performed without generating cracks.

Wet adhesion and rubber friction in adhesive pads of insects

  • Many animals possess on their legs adhesive pads, which have undergone evolutionary optimization to be able to attach to variable substrates and to control adhesive forces during locomotion. Insect adhesive pads are either relatively smooth or densely covered with specialized adhesive hairs. Theoretical models predict that adhesion can be increased by splitting the contact zone into many microscopic, elastic subunits, which provides a functional explanation for the widespread 'hairy' design. In many hairy and all smooth attachment systems, the adhesive contact is mediated by a thin film of liquid secretion between the cuticle and the substrate. By using interference reflection microscopy (IRM), the thickness and viscosity of the secretion film was estimated in Weaver ants (Oecophylla smaragdina). 'Footprint' droplets deposited on glass are hydrophobic and form low contact angles. IRM of insect pads in contact showed that the adhesive liquid is an emulsion consisting of hydrophilic, volatile droplets dispersed in a persistent, hydrophobic phase. I tested predictions derived from film thickness and viscosity by measuring friction forces of Weaver ants on a smooth substrate. The measured friction forces were much greater than expected assuming a homogenous film between the pad and the surface. The findings indicate that the rubbery pad cuticle directly interacts with the substrate. To achieve intimate contact between the cuticle and the surface, secretion must drain away, which may be facilitated by microfolds on the surface of smooth insect pads. I propose a combined wet adhesion/rubber friction model of insect surface attachment that explains both the presence of a significant static friction component and the velocity-dependence of sliding friction.

Precise ultrasonic coating and controlled release of sirolimus with biodegradable polymers for drug-eluting stent

  • In the current study, a drug-eluting stent coated with biodegradable polymers and sirolimus was developed by using an ultrasonic nanocoater and characterized in aspects of surface smoothness and coating thickness. In addition, in vitro release profiles of sirolimus by changing top coating layer with different biodegradable polymers were investigated. Smooth surfaces with variable thickness could be fabricated by optimizing polymer concentration, flow rate, nozzle-tip distance, gas pressure, various solvents and ultrasonic power. Smooth surface could be generated by using volatile solvents (acetone, chloroform, and methylene chloride) or post-treating with solvent vapor. Coating thickness could be controlled by varying injection volume or polymer concentration, and higher concentration could reduce the coating time while obtaining the same thickness. The thickness measurement was the most effectively performed by a conventional cutting method among three different methods that were investigated in this study. Release profiles of sirolimus were effectively controlled by changing polymers for top layer. PLGA made the release rate 3 times faster than PDLLA and PLLA and all top layers prevented burst release at the initial phase of profiles. Our results will provide useful and informative knowledge for developing drug-eluting stents, especially coated with biodegradable polymers.

혈관이식술 후 내막과다증식에 대한 Epigallocatechin-3-Gallate의 효과 (The Effect of Epigallocatechin-3-Gallate on Intimal Hyperplasia after Vascular Grafting)

  • 배경: 혈관내막과다증식은 혈관평활근세포가 내막에서 과다증식하여 나타나며, epigallocatechin-3-gallate(EGCG)는 혈관평활근세포의 증식을 억제하는 효과가 있는 것으로 알려져 있다. 따라서 EGCG는 혈관내막과다증식을 억제할 수 있을 것으로 생각된다. 대상 및 방법: 다른 농도의 EGCG를 포함한 배양 배지에서 인간제대정 맥내피세포(HUVEC)와 쥐대동맥평활근세포(RASMC)를 배양하고, 세포증식과 이동속도를 측정하였다. 20마리의 개의 양측 경동맥에 자가경정맥을 이식하였으며, 실험군(n=10)에 대해서는 정맥도관을 이식 전 30분간 EGCG용액에 보관하였으며, 이식 후 300mM의 EGCG를 혈관주위에 도포하였다. 6주 후에 경정맥이식편의 내막과 중간막의 두께를 측정하였다. 결과: 내피세포와 혈관평활근세포의 증식은 EGCG에 의해 억제되었다. 혈관평활근세포의 이동성은 EGCG에 의해 억제되었으나, 내피세포의 이동성은 영향을 받지 않았다. 동물실험 결과 대조군에 비해 EGCG군에서 내막의 두께가 얇았으며(p<0.05), 중간막의 두께는 두 군간에 차이가 얼었고, 내막/중간막의 두께비는 EGCG군에서 낮게 관찰되었다(p<0.05). 결론: EGCG는 혈관수술 후 혈관내막과다증식을 억제하는 효과가 있으며, 이는 혈관평활근세포의 증식 및 이동을 억제하여 효과를 나타낸다고 생각된다. 따라서 EGCG는 혈관내막과다증식을 예방하는 치료에 효과적으로 사용될 수 있을 것으로 생각된다.

열, 기계 하중을 고려한 지그재그 고차 복합재 쉘 이론 (Higher Order Zig-Zag Theory for Composite Shell under Thermo-mechanical load)

  • A higher order zig-zag shell theory is developed to refine the predictions of the mechanical and thermal behaviors partially coupled. The in-plane displacement fields are constructed by superimposing linear zig-zag field to the smooth globally cubic varying field through the thickness. Smooth parabolic distribution through the thickness is assumed in the out-of-plane displacement in order to consider transverse normal deformation and stress. The layer-dependent degrees of freedom of displacement fields are expressed in terms of reference primary degrees of freedom by applying interface continuity conditions as well as bounding surface conditions of transverse shear stresses. Thus the proposed theory has only seven primary unknowns and they do not depend upon the number of layers. In the description of geometry and deformation of shell surface, all rigorous exact expressions are used. Through the numerical examples of partially coupled analysis, the accuracy and efficiency of the present theory are demonstrated. The present theory is suitable in the predictions of deformation and stresses of thick composite shell under mechanical and thermal loads combined.

  • Rapid Prototyping(RP) systems, that fabricate objects with slicers, typically bear staircase artifacts when slices has a certain degree of thickness. A tangential cutting algorithm is developed in order to remove surface distortion as well as to generate smooth laser-cutting trajectory. For this, an energy function is defined with tangential line length and distance between tangential line and middle contour. Then, the energy is minimized to generate effective tangential line segments. The proposed algorithm is tested and verified on 3D object samples and the results show that the generated tangential lines effectively approximate layer surface and make laser trajectory smooth.

  • A higher order zig-zag shell theory is developed to refine accurately predict deformation and stress of smart shell structures under the mechanical, thermal, and electric loading. The displacement fields through the thickness are constructed by superimposing linear zig-zag field to the smooth globally cubic varying field. Smooth parabolic distribution through the thickness is assumed in the transverse deflection in order to consider transverse normal deformation. The mechanical, thermal, and electric loading is applied in the sinusoidal distribution function in the in-surface direction. Thermal and electric loading is given in the linear variation through the thickness. Especially, in electric loading case, voltage is only applied in piezo-layer. The layer-dependent degrees of freedom of displacement fields are expressed in terms of reference primary degrees of freedom by applying interface continuity conditions as well as bounding surface conditions of transverse shear stresses. In order to obtain accurate transverse shear and normal stresses, integration of equilibrium equation approach is used. The numerical examples of present theory demonstrate the accuracy and efficiency of the proposed theory. The present theory is suitable for the predictions of behaviors of thick smart composite shell under mechanical, thermal, and electric loadings combined.
