우리나라 청소년의 수면시간이 자살생각에 미치는 영향 (Effect of Sleep Duration on Suicidal Ideation in Korean Adolescents)

  • Purpose: The purpose of this study is to examine the effect of sleep duration on suicidal ideation in Korean adolescents. Methods: This study used the data drawn from Korea Youth Risk Behavior Web-Based Survey in 2013. A total data of 63,301 adolescents were included in the analysis. Included variables were general characteristics, health status variables, hours of sleep, and suicidal ideation. For sample description weighted percentage and means were used. Rao-Scott ${\chi}^2$ test and simple and multiple logistic regression with dummy variables were conducted with use of SAS 9.2. Results: Among general characteristics and health status variables, gender, grade, school level, school achievement, family's economic status, living with parents, subjective health, happiness, stress, and feeling of despair were found to be significantly related to suicidal ideation. Rates of having suicidal ideation were 22.4% in the case of students with less than 5 hours of sleep and 12.9% in students with 8 or more hours of sleep. After controlling general characteristics and subjectively perceived variables, hours of sleep was still significantly related to suicidal ideation. That is, compared to adolescents with 8 hours or more of sleep, those with less than 5 hours of sleep showed significantly higher risk for having suicidal ideation (adjusted odds ratio: 1.20, p<.001). Conclusion: These findings suggest that short sleep has negative effects on psychological health in adolescents. Particularly, less than 5 hours of sleep has harmful effect on adolescents' increased risk for having suicidal ideation.

수면 무호흡과 수면이 기억기능에 미치는 영향 (The influence of sleep and sleep apnea on memory function)

  • Objectives : Disturbance of sleep with or without sleep apnea may impair the memory function. Sleep deficiency, sleepiness, sleep apnea and emotional problem in sleep disorders can induce an impairment of memory function. Methods : In this study, the polysomnographies were administered to 58 sleep apnea patients and 38 sleep disorder patients without sleep apnea. Their clinical symptoms were quantitatively evaluated. Short term and long term memory were evaluated before and after polysom no graphy with Digit symbol test and Rey-Osterrieth complex figure test. And correlations among various sleep, repiratory and clinical variables were statistically studied in order to explore which variables may influence on memory function. Results and Conclusions : Results are as follows. Depth of sleep cis positively correlated with memory function. As sleep apnea increases and average saturation of blood oxygen decreases, memory function is more impaired. Emotional depression, high blood pressure, obesity or alcohol impaired memory function. However, daytime sleepiness was not significantly correlated with memory function. The possible mechanisms how above factors influence on the memory function were discussed.

요양시설 입소 노인에서 인지, 수면양상, 타액 멜라토닌 농도 및 수면장애행동의 관련성 (Relationship among Cognition, Sleep Patterns, Salivary Melatonin Level and Sleep Disorder Inventory of Older Adults in Nursing Homes)

  • Purpose: The purpose of this study was to investigate sleep quality in older adults in nursing home with objective data collection. Methods: Participants included 74 older adults in nursing homes in Korea aged 65 years or above. Data were collected using a wearable device (Fitbit), salivary melatonin level and Sleep Disorder Inventory (SDI). The Pearson correlation coefficient was calculated to examine whether there was any correlation between sleep-related variables such as Total Sleep Time (TST), Rapid Eye Movement (REM) sleep, shallow sleep, deep sleep, salivary melatonin level and SDI. Results: There were distortion of sleep structure, as TST comprised short REM sleep (15.93±5.47%), long shallow sleep (74.18±8.08%) and short deep sleep (9.89±5.03%). Also, salivary melatonin levels were low (15.06±7.77 pg/mL). Moreover, we found than melatonin was significantly associated with TST (r = .251, p= .044), REM sleep (r= .294, p= .020) and deep sleep (r= .391, p= .002). But there was no correlation between SDI and other sleeprelated variables. Conclusion: These findings highlight that insufficient sleep structure is associated with the salivary melatonin level among older adults in nursing home. We suggest developing programs to promote sleep quality of older adults in nursing homes.

청소년의 수면시간과 수면 후 피로 회복이 학교 내 손상에 미치는 영향 (The Effect of Sleep Duration and Relief of Fatigue after Sleep on the Risk of Injury at School among Korean Adolescents)

  • Purpose: To explore the association between sleep and the risk of accidental injury at school among Korean adolescents. Methods: From the database of the Ninth Korea Youth Risk Behavior Web-based Survey (KYRBS), the researcher selected 63,307 adolescents who responded to a survey on sleep hours. We conducted logistic regression with sleep duration and fatigue after sleep as independent variables, the risk of injury at school as a dependent variable, and gender, grade, school type, economic status, parents' education level, number of participations in physical education, and current smoking and drinking as control variables. Results: Using 9 hours of sleep as the reference, the adjusted injury risk (odds ratio) was 1.74 for those sleeping less than 5 hours a day, 1.61 for 5 hours, 1.45 for 6 hours, 1.31 for 7 hours, 1.13 for 8 hours, and 1.40 for 10 hours or longer. The difference between each pair of groups was statistically significant. In this study, injury risk increased as sleep duration decreased and fatigue after sleep increased. Conclusion: The findings suggest that a short nightly duration of sleep and fatigue after sleep can be considered potential risk factorsfor unintentional injuries at school among Korean adolescents.

불면증 환자의 수면인식과 객관적, 주관적 수면변수의 연관성에 대한 연구 (Association of Sleep Perception With Objective and Subjective Sleep Variables in Insomnia Patients)

  • 목 적 : 본 연구의 목적은 불면증 환자의 주관적 수면인식과 관련 있는 임상적, 객관적 요인들을 알아보고 임상에서 불면증 환자의 이해와 평가 및 치료에 도움을 주기 위함이다. 방 법 : 109명의 불면증 환자들을 대상으로, 수면다원검사 결과와 자가보고 설문지를 후향적으로 분석하였다. 주관적 수면인식은 아침자가보고 설문지의 주관적 총 수면시간(subjective total sleep time, subjective TST), 입면 잠복시간(subjective sleep onset time; subjective SOL), 각성횟수, 아침 개운함 정도로 측정하였으며, 주관적-객관적 수면차이를 계산하였다. 임상적 특징은 Pittsburgh Sleep Quality Index (PSQI), Beck Depression Inventory (BDI), Epworth Sleepiness Scale (ESS)를 측정하였다. 변수들 사이의 관련성을 알아보기 위하여 상관분석을 시행하였다. 결 과 : 109명의 불면증 대상자(남성 = 72명, 나이 43.49 ± 15.32세)를 분석한 결과, 주관적 총 수면시간(subjective TST)은 수면다원검사에서의 입면 후 각성시간(wake after sleep onset, WASO) (p < 0.001), N1 수면(p = 0.039), PSQI (p < 0.001), BDI (p = 0.014)와 유의한 음의 상관관계를 보였다. 수면차이는 PSQI (p = 0.018)와 유의한 음의 상관관계가 있었다. 아침 개운함 정도는 PSQI (p = 0.019), BDI (p < 0.001)와 유의한 음의 상관관계가 있었다. 결 론 : 불면증 환자에서 주관적 수면인식은 수면다원검사의 입면 후 각성시간(WASO), N1 수면과 관련성이 있었으며, 우울감과 평소 수면의 질과 관련성이 있었다. 불면증 환자에서 다양한 객관적 수면변수, 평소 수면의 질 및 우울감이 주관적 수면인식에 영향을 미치므로, 이를 고려하여 환자를 평가 및 치료해야 하겠다.

일반성인과 노인의 수면양상과 수면방해요인의 비교연구 (Comparison of sleep and related variables between young and old adults)

  • Sleep is an essential component for health and the quality of life of individuals, and is affected by multiple factors. Thereby, sleep impairment is known to be frequent even in healthy subjects. The purpose of the study is to compare sleep patterns and related factors between healthy young and old adults and to identify aging effects upon sleep in a cross-sectional way. The survey questionnaire was developed by translation and modification of two well-known sleep questionnaires which were originally developed by Monroe(1967) and Ellis, et al. (1982). Discussion with experts and pilot study were completed to finalize the contents of the questionnaire that was used in this study. Results are as follows : 1. Sleep complaints were lower in older adults. 2. The most frequent variables that explain sleep satisfaction were feeling rested in the morning in the older adults and feeling rested and failing asleep within five minutes in the young adults. 3. Regarding sleep-disturbing factors, physical factors were more frequently reported in the older adults and environmental factors are reported more frequently in the young adults, and there was no difference related to the emotional factors between the two groups. 4. Young adults were practiced strategies for better sleep more often than the older adults, and they were mainly in-home activities just before sleep. 5. Sleep patterns which change with aging were as follows : going to bed and waking up earlier : not staying in the bed long after waking up ; getting harder to fall asleep ; frequent arousal after sleep onset ; getting harder to go back to sleep after arousal during night sleep. From the results of this study, it can be concluded that sleep and related factors of the young and the older adults are different. Also, sleep patterns change with aging and those changes seem to be negative for sleep in the elderly. Repeated studies are needed to establish more concrete information regarding sleep patterns. In addition, further research is needed to develop more reliable, valid, and feasible sleep measure tools, and to develop and evaluate nursing interventions.

체질별 성격요인이 수면에 미치는 영향 (Effects of Personality by Each Sasang Constitution on Sleep)

  • Objectives The purpose of this study was to identify the effects of personality by each Sasang constitution on sleep using Positive Affect and Negative Affect Schedule(PANAS) and NEO-personality inventory(NEO-PI). Methods The subjects of this study were 2,078 people who had information on Sasang constitutional type and personality(PANAS, NEO-PI) among the data established at the Korean Medicine Data Center. Sleep time and sleep quality were investigated through self-written questionnaires. Sleep time was collected by hand-writing, and sleep quality was checked on a 4 point Likert scale. Pearson correlation analysis was conducted to see the correlation between personality and sleep in each constitutional type. Logistic regression was performed using personality as independent variables to find out how much personality affects sleep time. In order to find out how much personality affects sleep quality, regression analysis was performed using personality as independent variables. Results & Conclusions Sleep time was hardly affected by personality. As a result of Pearson correlation analysis, sleep time in all subjects did not show a significant correlation with personality. In logistic regression on sleep time as the dependent variable, no statistically significant results were obtained except for the Negative Affect(NA) in Taeeumin. Sleep quality showed a statistically significant correlation with the negative affect(NA), neuroticism(N), extraversion(E), Physical Component Summary(PCS) and Mental Component Summary(MCS). As a result of regression analysis on sleep quality as the dependent variable, neuroticism(N), negative affect(NA), positive affect(PA), and extraversion(E) were found in the factors affecting sleep quality. Besides, how much personality affected sleep quality might differ in each constitution. In all constitutions, sleep quality was affected by N, but the rank of N was different in each constitution. The sleep quality of Soyangin was not affected by E, and the sleep quality of Taeeumin was specifically affected by O.

주기성 사지운동증에 따른 수면다원검사 상 수면 변수들의 변화 (The Changes in Polysomnographic Sleep Variables by Periodic Limb Movements During Sleep)

  • 목 적 : 이 연구에서는 PLMS 지수의 변화에 따라 수면다원검사 상 나타나는 수면 변수들과 주간 졸림 증상의 변화가 있는지 살펴보고, PLMD의 바람직한 진단 기준으로서의 PLMS 지수에 대해 알아보고자 하였다. 방 법 : 총 4195명 환자들에 대해 수면다원검사를 시행한 기록을 조사하여, 17세 이상이고, 원발성 불면증, 수면무호흡증, 기면병, 렘수면 행동장애가 없는 666명의 대상 환자들을 선택하였다. 대상 환자들의 PLMS 지수에 따라 5 이하인 그룹(1 그룹)과, 5에서 15 사이인 그룹(2 그룹), 15를 초과하는 그룹(3그룹)의 세 그룹으로 나누어 비교하였다. 각 그룹별로 연령, 성별, Epworth 졸림 척도, 수면 효율, 입면 후 각성시간, 수면 잠복기, 1단계 수면분율, 2단계 수면분율, 3단계 수면분율, REM 수면분율 등의 변수를 비교하였다. 결 과 : 세 그룹은 연령과 성별에서 통계적으로 의미 있는 차이를 보였다. 수면 효율, WASO, 3단계 수면분율에서 1그룹과 2, 3그룹은 의미 있는 차이를 보였고, 2그룹과 3그룹 사이에는 차이가 없었다. 수면 잠복기와 Epworth 졸림척도에서 1그룹과 3그룹이 의미 있는 차이가 있었고, 2그룹과 3그룹 사이에는 차이가 없었다. 연령을 제어 변수로 설정하고 편상관분석을 실시한 결과 PLMS 지수는 수면 효율, WASO와 의미 있는 상관관계를 나타냈다. 결 론 : 이 연구 결과를 통해 PLMS 지수에 따라 수면다원검사 상 수면 변수의 변화가 있음을 알게 되었다. 또한 변화된 중요 수면 변수에서 2그룹과 3그룹이 차이가 없는 것으로 나타나서, PLMS 지수가 5 이상을 PLMD라고 진단했던 예전 진단 기준을 좀더 옹호하는 결과인 것으로 나타났다.

우리나라 성인에서 수면 시간과 비만과의 관련성 (The Association between Sleep and Obesity in Korean Adults)

  • Objectives : Obesity is currently an epidemic in Korea, and sleep duration is thought to be one of the risk factors for obesity. The objective of this study was to test the hypothesis that short sleep duration is associated with obesity in Korean adults. Methods : The data from the 2001 Korean National Health and Nutrition Survey were used, and 6,174 subjects aged $18{\sim}80$ years were included in the analysis. Sleep duration was measured using information obtained from self-reported questionnaires. Obesity, the main outcome variable, was measured according to body mass index. Multiple regression modeling was used to adjust for potential confounding variables. Results : The study results revealed a negative association between sleep duration and body mass index among Korean adults. These associations persisted after controlling for the potential confounding variables. Conclusions : These findings support the hypothesis that sleep duration is associated with obesity in Korean adults. In addition, these observations support earlier experimental sleep studies and provide a basis for future studies on weight control intervention by increasing the amount of sleep.

수면이득이 있는 파킨슨병 환자의 임상특징 및 수면다원검사에 대한 연구 (A Clinical and Polysomnographic Study of Parkinson's Disease Patients with Sleep Benefit)

  • 목 적 : 파킨슨병 환자의 상당수에서 아침에 일어났을 때 수면이득으로 알려진 운동성 기능의 일시적인 호전을 보고 한다. 파킨슨병 환자에서의 수면이득이 수면과 관련되는 현상임에도 불구하고 수면다원검사 등의 객관적인 방법을 이용한 연구는 드물다. 본 연구의 목적은 수면이득이 있는 환자 군과 없는 환자 군의 임상특징 및 수면다원검사 소견을 비교하는데 있다. 방 법 : 33명의 일차성 파킨슨병 환자(남 14, 여 19)를 대상으로 하였다. 모든 환자들은 수면이득의 여부를 확인하기 위하여 면담을 하였고, 의무 기록지를 통하여 임상적 자료를 얻었고, 수면검사실에서 하룻 밤의 수면다원검사를 실시하였다. 결 과 : 수면이득이 있는 환자가 16명(48.5%), 없는 환자가 17명(51.5%)이었다. 수면이득이 있는 환자들이 없는 환자들에 비하여 더 젊었고(p<0.02), 수면효율이 더 좋았고 (p<0.05), 그리고 수면잠복기가 더 짧았다(p<0.02). 그러나 수면이득은 유병기간, 레보도파 일일 복용 용량, 운동기능의 장애 등과 관련되지 않았고, 특정 수면단계와도 관련되지 않았다. 두 군 모두에서 렘수면행동장애 등의 수면장애의 빈도가 높았으나 일차성 코골이를 제외하고는 두 군간에 차이는 없었다. 결 론 : 본 연구의 결과는 수면이득이 젊은 나이, 좋은 수면효율, 짧은 수면잠복기와 관련됨을 시사한다.
