요양시설 노인의 건강관련 삶의 질에 영향을 미치는 요인 (Factors related to the Health related Quality of Life among Institutionalized Elders)

  • Purpose: This purpose of this study was to identify the factors related to the health related quality of life among institutionalized elders. Methods: The subjects were 247 elders institutionalized among one of the four nursing homes in Busan. Data were collected through interview with four standardized questionnaires from August to October, 2008. Questionnaires were related to health related quality of life (SmithKline Beecham's Quality of Life scale, SBQoL), quality of sleep(Pittsburgh Sleep Quality index, PSQI), activity of daily living (Katz ADL Index), and depression (Geriatric Depression Scale Short Form Korea, GDSSF-K). Data were analyzed for descriptive analysis, t-test, one-way ANOVA, Pearson's correlation coefficient, and stepwise multiple regression by using SPSS/WIN 17.0 program. Results: The mean HRQoL in the subjects was $6.62{\pm}0.91$. Percentage of the respondents reporting sleep disorders were 72.5% and 27.1% reported being vulnerable to depression. The significant predictors of the HRQoL were sleep quality, activity of daily living (ADL), level of education, and depression. The factors accounted for 28.8% of variance in the health related quality of life of institutionalized elders. Conclusion: Sleep quality, activity of daily living, and depression need to be considered as the important control factors for improving the health related quality of life in the institutionalized elders.

  • Purpose: The purpose of this study was to identify the sleep patterns of nursing students using self-report questionnaire and physiologic measurement, to examine the factors influencing sleep patterns in based on Spielman's model. Methods: Participants were 119 nursing students who were in the clinical practice period. Self-report questionnaires and actigraphy were used to collect the data. Data were analyzed using descriptive statistics, correlation and regression by the SPSS/WIN 21.0 programs. Results: When sleep was measured by self-report questionnaire, 84 students (70.6%) showed poor sleep quality. The mean sleep efficiency was 82.6%, and 67 students(56.3%) showed low sleep efficiency (less than 85.0%). The factors affecting subjective sleep pattern measured by KMLSEQ were circadian sleep type (${\beta}=.28$, ${\rho}=.003$) and alcohol (${\beta}=.20$, ${\rho}=.031$). The factors affecting total sleep time were sedentary behavior (${\beta}=-.27$, ${\rho}=.003$) and daytime sleepiness (${\beta}=-.33$, ${\rho}$<.001). Conclusion: Many nursing students in their clinical practice period expressed sleep disturbance. Factors affecting the perceived sleep measured by the self-report questionnaire and objective sleep evaluated by physiologic measures were different. The evening type of students perceived poor sleep quality, however, sedentary life style and daytime sleepiness resulted in short sleep time. Therefore, more studies measuring the objective sleep characteristics are needed using subjective and objective characteristics.

  • Standark polysomnograms, and questionnaires were measured using 4 subjects during simulated 150m heliox saturation dives. These measurements were performed on 4 pre-dive nights, 8 bottom nights, 6 decompression nights and post decompression nights. Sleep disturbances caused by frequent awakening were found throughout the dive period, but the disturbances were more intense on the second and fourth nights at the bottom and on the third and fourth nights of decompression. Stage 4 of sleep decreased every night at the bottom and from the first through third nights of decompression. The subjective self-evaluation of sleep was in agreement with the physiological variables. Complaints of subjective feeling of fatigue slightly increased throughout the dive period. It could be surmised that sleep disturbance or fatigue was not as significant under the 150m deep hyperbalic environment of He-O/ sub 2/ mixture, and therefore should be able to be coped with partially, through there moght be individual differences in adaptation.

진폐 환자의 심폐증상과 수면이 피로에 미치는 영향 (The Effect of Cardiopulmonary Symptoms and Sleep on Fatigue in Pneumoconiosis Patients)

  • Purpose: The purpose of this study was to investigate the effect of cardiopulmonary symptoms and sleep on fatigue in pneumoconiosis patients. Methods: Self-administered questionnaires were given to 211 neumoconiosis patients during the period from May 12 to May 20, 2010. Results: The mean score of fatigue was as high as 6.7. The level of fatigue depended on the subjects' leisure activity and complication. Fatigue was significantly correlated with cardiopulmonary symptoms and sleep. The influential factors affecting fatigue were cardiopulmonary symptoms, complication, sleep and leisure activity, which explained about 46.5% of the variance. Conclusion: The results of this study indicate that nursing interventions are needed to reduce fatigue and to improve cardiopulmonary symptoms and insomnia in pneumoconiosis patients.

  • Snoring is a very common sleep disorder. Approximate 20~30 percent of population and 40~60 percent of middle and old age population are reported to have symptoms of snoring. Snoring patients do not frequently recognize snoring until spouse or colleague has noticed. Hence, for most people snoring has been not a serious health issue but one of bad sleeping habits. However, it recently draws social attention because most patients with diagnosed "Obstructive Sleep Apnea Syndrome" cased by obstruction of the upper airway are reported snoring. In this point, the author attempts to review the clinical aspects and treatments about snoring and "Obstructive Sleep Apnea Syndrome" in the medical and dental perspective.

전립선 조직검사 사전정보교육이 전립선비대증 환자의 불안, 우울 및 수면의 질에 미치는 효과 (Effects of providing procedural information to patients undergoing prostate biopsy on anxiety, depression and sleep quality)

  • Purpose: This study aimed to investigate the effects of providing prostatic hypertrophy patients with information about an upcoming prostate biopsy procedure on their anxiety, depression, and sleep quality. Methods: Sixty-two participants were divided equally into an experimental and control group. Experiments were conducted from July 31, 2015, to March 30, 2016. After providing information, we evaluated anxiety, depression, and sleep quality using structured questionnaires. Data were analyzed using chi-square tests, Fisher's exact tests, t-tests, and ANCOVA using SPSS. Results: The experimental group demonstrated significantly lower levels of anxiety and depression than the control group. The experimental group also demonstrated significantly higher sleep quality. Conclusion: Information on an upcoming prostate biopsy improved psychological outcomes in patients with prostatic hypertrophy. This education should be incorporated into nursing practice.

율동적 동작프로그램이 노인의 일상생활 수행능력, 우울 및 수면에 미치는 효과 (The Effect of Rhythmic Activity Program on IADL, Depression and Sleep of the Elderly)

  • Purpose: This study was performed to verify the effect of rhythmic activity program on the elderly particularly their level of instrumental activities of daily living(IADL), depression and sleep. Method: The design of this study is one-group pretest-post test design. Three self-reported questionnaires were used as follows: first, the IADL Scale developed by Lawton and Brody(1969) and revised by Suh(1996) for measuring IADL, second, the Geriatric Depression Scale developed by Sheikh and Yesavage(1986) and revised by Song(1991) for measuring depression, third, Korean Sleep Scale developed by Oh, Song, & Kim(1998) for measuring sleep aspects; fourth, the applied version of Facial Pain Scale developed by Wong & Baker(1988) and revised by Song(2004) for measuring sleep quality. Twenty-seven elderlies carried out 50 minutes of rhythmic activity program for 4 weeks. Before and after the experiment, they were tested for IADL, depression and sleep. Collected data were processed using the SPSS/WIN 12.0 program analyzed by the frequency, percentage, mean and paired t-test. Results: The improvement in IADL of the elderly was not enhanced significantly but depression and sleep disorders significantly decreased. Conclusion: Rhythmic activity program is an effective nursing intervention for the elderly.

산후 6개월 이내 산모의 수면의 질, 산후 피로도 및 산후 우울 (Sleep Quality, Fatigue, and Postpartum Depression of Mother at Six Months after Delivery)

  • Purpose: This study was correlation study to identify the factors influencing sleep quality, fatigue, and postnatal depression in mothers who have given birth during the past 6 months. Methods: The study was conducted using a survey with questionnaires to 329 mothers who visited E University Medical Center, or three local clinics located in D city, between August and October 2013. Collected data were analyzed using the SPSS/WIN 20.0 program. Results: Out of 329 subjects, 18.2% showed that they had mild postnatal depression whereas 24.3% had severe postnatal depression. Accordingly, 42.5% reported having postnatal depression. Postnatal depression had a significant correlation with sleep hours after childbirth (r=-.16, p=.003), spousal support (r=-.28, p<.001), sleep quality (r=-.35, p<.001), physical fatigue (r=.66, p<.001), psychological fatigue (r=.69, p<.001), and neurosensory fatigue (r=.56, p< .001). Factors influencing postnatal depression include psychological fatigue, sleep quality, number of child births, and neurosensory fatigue, and these accounted for 53% of postnatal depression. Conclusion: Results indicate that factors influencing postnatal depression involve psychological fatigue, sleep quality, number of child births, and neurosensory fatigue. Therefore for nursing intervention for postpartum mothers, it is necessary to assess the level of depression, fatigue, and sleep quality, and to provide interventions to relieve depression.

방사선치료를 받는 유방암 환자의 피로, 수면장애, 삶의 질에 대한 연구 (Fatigue, Sleep Disturbance, and Quality of Life among Breast Cancer Patients Receiving Radiotherapy)

  • Purpose: The purpose of this study was to examine fatigue, sleep disturbances, and quality of life (QOL) among patients with breast cancer receiving radiotherapy. Methods: A cross-sectional, descriptive design was used. Data were collected through questionnaires distributed to 201 breast cancer patients in a hospital. The data were analyzed using SPSS 21.0. Results: The fatigue scores showed significant differences depending on exercise and duration since diagnosis. The sleep disturbance scores showed significant differences depending on duration since diagnosis. QOL scores showed significant differences depending on exercise, duration since diagnosis, and treatment site. Fatigue and sleep disturbances (r=.40, p<.001) showed statistically significant positive correlations, while fatigue and QOL (r=-.55, p<.001), and sleep disturbances and QOL (r=-.45, p<.001) showed statistically significant negative correlations. The multiple regression analysis, which was used to determine the variables influencing on QOL after radiotherapy, resulted in a significant regression model (F=23.88, p<.001), which accounted for approximately 45% of the explanatory power. Fatigue (${\beta}=-.39$, p<.001) and sleep disturbances (${\beta}=-.27$, p<.001) were revealed to adversely affect quality of life. Conclusion: The nursing intervention is necessary to reduce fatigue and sleep disturbance and to promote exercise in order to enhance QOL of patients with breast neoplasm while receiving radiotherapy.

노년기 요실금 여성의 수면의 질, 일상생활 수행능력과 우울 (The Relationship of Quality of Sleep, Depression, Late-life Function and Disability (LLFDI) in Community-Dwelling Older Women with Urinary Incontinence)

  • Purpose: The purpose of this study was to investigate the relationships among quality of sleep, depression, late-life function and disability in community-dwelling older women with urinary incontinence. Methods: A stratified random sampling method was conducted to recruit participants from May 10 to August 17, 2007. Data were collected by questionnaires, which were constructed to include lower urinary tract symptoms, quality of sleep, depression, and late-life function and disability in 128 community-dwelling older women. Results: The major findings of this study were as follow: 1) 56.3% of participants belonged to urinary incontinence group. 2) There were significant relationships between depression and sleep latency, sleep duration, daytime dysfunction, quality of sleep, function component, frequency dimension, and limitation dimension. 3) Depression was significantly associated with frequency dimension, limitation dimension in capability which explained 44% of variance in depression. Conclusion: These results may contribute to a better understanding of sleep quality, depression, late-life function and disability in the community-dwelling older women with urinary incontinence. Therefore, health programs for prompting older women's health should be planned based on results of the study.
