인공호흡기 적용 환자의 수면양상과 수면장애 요인 (Sleep Patterns and Factors Related to Sleep Disturbance in Mechanically Ventilated Patients)

  • Purpose: The purpose of this study was to investigate the sleep patterns and the factors related to sleep disturbance in mechanically ventilated patients in intensive care units (ICUs). Sleep patterns in this study included both quality and quantity of sleep. Methods: Forty-four subjects were recruited in ICUs at a tertiary university hospital in D city. Data were collected using questionnaires, observation, and medical record from June, 2008 to May, 2009. Results: The total mean of sleep quality was $1.99{\pm}1.68$. The mean of total sleep time per 24-hour period was $3.75{\pm}1.94$hour (range 1-7.25) in mechanically ventilated ICU patients. The main sleep disturbance factors were listed as frequent nursing activities, attachment of medical appliances, and noise. All these factors were also significantly related to the sleep quality. Conclusion: These results indicated that mechanically ventilated patients were both qualitatively and quantitatively deprived of sleep far more than other ICU patients. In summary, a deeper understanding of sleep characteristics in mechanically ventilated ICU patients could help nurses to improve sleep derangements and patient outcome.

정상인과 비만인의 수면 일주기 리듬과 수면 질의 차이 (Difference in Sleep Circadian Rhythm and Sleep Quality between Normal-weight and Obese Group)

  • Purpose: The purpose of this study was to understand sleep circadian rhythm and sleep quality between normal-weight and obese group according to Body Mass Index to develop education and nursing intervention programs for the obese. Methods: This study involved 186 subjects who visited at S hospital obesity clinic, K province. They were divided into 2 groups: normal-weight group 91 and obese group 95. Data were collected from October 18th to November 12th in 2013. Data were analyzed with frequency, percentage, ${\chi}^2$-test, Mann-Whitney U test, ANCOVA, t-test and ANOVA with using SPSS version 20.0. Results: The results showed that morning type 1.1%, middle type 91.2% and evening type 7.7% of sleep circadian rhythm in normal-weight group and middle type 92.6% and evening type 7.4% of sleep circadian rhythm in obese group. There were statistically significant results on sleep quality with covariance sex and stress, sleep duration, habitual sleep efficiency and sleep disturbance between normal-weight and obese group. There were statistically significant results on sleep quality, sleep duration, habitual sleep efficiency and sleep disturbance in middle type of sleep circadian rhythm between normal-weight and obese group. Conclusion: Therefore, it is necessary to consider subject's sleep pattern to develop education and nursing intervention programs for the obese.

우리나라 청소년에서 수면시작시간과 우울감의 상관관계: 청소년 건강행태온라인조사를 바탕으로 (Relationship between Sleep Timing and Depressive Mood in Korean Adolescents: Based on the Korea Youth Risk Behavior Web-based Survey)

  • Purpose: The purpose of this study was to investigate the relationship between sleep timing and depressive mood in Korean adolescents. Methods: The study analyzed the data from the 2007~2015 Korea Youth Risk Behavior Web-based Survey. A total of 541,693 students in grades 7~12 were included in the final analysis. Multivariable logistic regression was used to examine their sleep timing and depressive mood, adjusted for sex, grade, region, socioeconomic status, academic performance, alcohol, smoking and physical activity. Sleep duration and sleep quality were also included in our model to identify whether or not the effect of sleep timing on depression is mediated by sleep duration or sleep quality. Results: The prevalence of depressive mood was 32.7% and the mean sleep timing was 12:13 AM. After adjustment for eligible covariates, the association between sleep timing and depressive mood showed a J-shaped curve. Adolescents who slept at 8 pm~10 pm were 39% more likely to be depressive (OR = 1.39, 95% CI 1.30~1.40) and at 3 am~ 4 am were 67% more likely to be depressive (OR=1.67, 95% CI 1.64~1.70) than adolescents who slept at 11 pm~12 am. These associations persisted after being adjusted for sleep duration and sleep quality. Conclusion: Sleep timing was related to depression in adolescents, independent of sleep duration and sleep quality. It appears that there is a certain sleep timing beneficial to mental health of adolescents.

Sleep Assessment During Shift Work in Korean Firefighters: A Cross-Sectional Study

  • Background: This cross-sectional study assessed the sleep quality using the ActiGraph and investigated the relationship between the parameters of sleep assessment and the type of shift work in Korean firefighters. Methods: The participants were 359 firefighters: 65 day workers (control group) and 294 shift workers (shift work group: 77 firefighters with 3-day shift, 72 firefighters with 6-day shift, 65 firefighters with 9-day shift, and 80 firefighters with 21-day shift). Sleep assessments were performed using the ActiGraph (wGT3X-BT) for 24 hours during day shift (control and shift work group) and night shift and rest day (shift work group). The participants recorded bed time and sleep hours during the measurement period. Results: Sleep efficiency, total sleep time, and percentage of wake after sleep onset during night work were lower in the shift work group than control group (p < 0.05). Sleep efficiency decreased in night shift and increased in rest day, whereas wake after sleep onset increased in night shift and decreased in rest day (p < 0.05). Among shift work groups, sleep efficiency of 6-day shift was higher in day shift, and sleep efficiency of 21-day shift was lower in night shift than other shift groups (p < 0.05). Conclusion: We found that the sleep quality in night shift of the shift work group was poorer than the control group. As to the type of shift work, sleep quality was good in 6-day shift and poor in 21-day shift. Thus, fast rotating shift such as 6-day shift may be recommended to improve the sleep quality of the firefighters.

간호대 재학생의 수면의 질, 우울, 학과만족도 간의 상관관계 연구 (Correlations among Sleep Quality, Depression and Satisfaction with Enrolled in the Nursing College Students)

  • Purpose : The purpose of this research is to measure the correlations among sleep quality, depression and department satisfaction in the students enrolled in the nursing college located in the B city. Method : 268 students enrolled in the nursing college located in the B city participated in this research from the 1st May 2016 to the 31st May 2016. SPSS 18.0 was applied to the collected data to calculate the frequency, percentage, average and standard deviation. In addition, t-test, ANOVA, Scheffe test and Pearson correlation coefficient were used for the analysis. Result : The result of the average sleep quality, depression and department satisfaction in the nursing college students were measured to be $2.41{\pm}0.57$, $3.59{\pm}0.50$ and $3.56{\pm}0.57$ respectively. The sleep quality based on the general characteristics indicated a significant difference when applied to the health status(F=4.46, p=.012). The depression indicated a significant difference when applied to the living with parents(t=-2.18, p=.030) and relationship with friends(F=8.62, p=.000). The department satisfaction indicated a significant difference when applied to the grade(F=3.45, p=.017), personality(F=3.46, p=.033), and reason for applying for their department(F=2.49, p=.044). As a result of analyzing the correlations among sleep quality, depression and department satisfaction in the nursing college students, the sleep quality indicated a negative correlation with the depression(r=-.528, p=.000) and a positive correlation with the department satisfaction(r=.144, p=.018). The depression indicated a negative correlation with the department satisfaction(r=.119, p=.051). Conclusion : It is necessary to conduct a replication research by increasing the number of the research subjects. In addition, it is necessary to conduct a systematic research on the factors influential on sleep quality, depression and department satisfaction in order to develop a program capable of enhancing the sleep quality and decreasing the depression.

대학생들의 수면의 질과 우울증상과의 관계 : 긍정심리의 조절효과 (The Relationship between Sleep Quality and Depressive Symptoms among University Students : Moderating Effect of the Positive Psychology)

  • Objectives : The aim of study was to estimate the prevalence of poor sleep quality and depressive symptoms as well as to examine the moderating effect of positive psychology on the relationship between poor sleep quality and depressive symptoms among Korean university students. Methods : A total of 2,338(1,071 male and 1,267 female) university students at seven universities of four cities in Korea were randomly selected to participate in a survey. Data were collected by self-report questionnaires, including Pittsburgh Sleep Quality Index(PSQI), Beck Depression Inventory(BDI), Satisfaction with Life Scale(SWLS), Positive Psychotherapy Inventory(PPTI), and sociodemographic characteristics. Results : The prevalence was 44.8% for poor sleep quality and 14.4% for depressive symptoms among Korean university students. By moderated regression analysis, the results showed that poor sleep quality(${\beta}=0.453$, p<0.001) aggravated depressive symptoms of university students, and life satisfaction(${\beta}=-0.087$, p<0.001) and happiness(${\beta}=-0.145$, p<0.001) respectively buffered the negative effect of the poor sleep quality on depressive symptoms. Conclusion : This study showed very high prevalence of poor sleep quality and depressive symptoms among Korean university students. Overall, these findings demonstrates that positive psychology can be effective in the enhancement of subjective and psychological well-being and may help to reduce depressive symptom levels. In the context of public health, positive psychology can be used as preventive, easily accessible and non-stigmatizing tools.

국내 수면 질 관련 양적 연구동향 및 측정도구 분석 (Research Trends and Instrument for Assessment in Sleep Quality)

  • Purpose: The purpose of this study was to identify the research trends and instruments for assessment of sleep quality in Korea. Methods: 66 quantitative research reports published from 2000 to March 2009 in Korea were selected from 5 databases: RISS4U, KISS, KISTI, DBpia, and the National Assembly Library and were analyzed according to criteria such as publication years, type of journal, participants, study fields, research design, participants, key words and instruments for assessment. Results: Research in sleep quality has been rapidly increasing in recent years. More than half of the research analyzed was conducted in nursing. The majority of research methods were survey and correlational research. Patients with chronic illness and the elderly were the major subjects in those researches. Most research was performed based on hospitals and communities. The types of variables tested for their relation and influence on sleep quality were quite limited. With respect to the instrument used for assessing sleep quality, most studies did not consider whether or not the validity of their instrument had been established. Conclusion: It is recommended to conduct clinically applicable interventional research with an experimental design and to develop a reliability and validity established sleep quality instrument for Koreans.

교대근무 간호사의 수면, 식생활 행위, 신체활동이 삶의 질에 미치는 영향 (Association of Sleep, Dietary Behaviors and Physical Activity with Quality of Life among Shift-work Nurses)

  • Purpose: This study aimed to investigate the associations of sleep, dietary behaviors and physical activity with quality of life among shift-work nurses. Methods: For this cross-sectional descriptive study, data were collected from 191 shift-work nurses in a tertiary hospital. Multiple linear regression analysis was used to identify the relationships among variables. Results: The levels of sleep quality, dietary behaviors and physical activity were low. Factors associated with quality of life among shift-work nurses included sleep disturbance (B=-0.16, ${\beta}=-0.26$, p<.001), dietary behaviors (B=0.28, ${\beta}=0.24$, p<.001), and physical activity (B=0.05, ${\beta}=0.19$, p<.001). The strongest factor was sleep disturbance. Conclusion: In order to improve quality of life among shift-work nurses, it is significant to encourage and maintain health behaviors such as sleep, dietary behaviors and physical activity. Under the circumstances where shift-work cannot be avoided, personal efforts made by nurses to establish their own living standards for positive health behaviors will benefit their quality of life.

대학생의 SNS 중독경향성이 특성불안과 수면의 질에 미치는 영향 (The Effect of SNS addiction tendency on Trait-Anxiety and Quality of Sleep in University students')

  • Purpose: This study investigates university students' SNS addiction tendency, trait-anxiety and quality of sleep. This study is going to offer basic resource that required to make the way that is able to reduce the anxiety with figuring out the relation between variable. Method: 213 people participated in this study is 20 to 30 age of university students who use SNS. Collecting resource period was April 4 to May 11, 2018. Analyses consisted of descriptive statistics, ANOVA, Pearson's correlation analysis, and stepwise multiple regression analysis. Results: The findings showed SNS addiction tendency and quality of sleep(r=.45, p<.000). Major Influence factor is predicted addiction tendency of SNS using time (F=21.21, p=.000) and trait-anxiety (F=23.55, p=.000). Of the highest trait-anxiety affects quality of sleep (F=24.82 p=.000). Conclusion: These findings indicate that it is necessary to develop and implement information programs and design appropriate SNS addiction prevention programs that can induce trait-anxiety and improve quality of sleep. Thus follow-up study should have diverse participants to investigate SNS addiction tendency, trait-anxiety and quality of sleep that influenced by overusing SNS.

요양보호사의 직무스트레스, 우울과 피로가 수면의 질에 미치는 영향 (Effects of Job Stress, Depression and Fatigue on Sleep Quality of Care Workers in Long-term Care Facilities)

  • Purpose: The purpose of this study was to identify the effects of job stress, depression and fatigue on sleep quality of care workers employed in long-term care facilities. Methods: The participants were 213 care workers who worked in 11 long-term care facilities. The collected data was analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and multiple linear regression with using the SPSS 23.0 program. Results: We found that patient-related job stress was the greatest stressor among care workers' job stresses. A significant difference in sleep quality was observed according to the self-perceived health status of the care workers. The care workers' sleep quality was negatively correlated with job stress (r=-.16, p=.018), depression (r=-.31, p<.001) and fatigue (r=-.29, p<.001). The influencing factors of sleep quality were depression (${\beta}=-0.22$, p=.001) and fatigue (${\beta}=-0.15$, p=.047). Conclusion: It is necessary to improve the health status and sleep quality of care workers by developing and applying health promotion programs to reduce the depression and fatigue of the care worker. This will ultimately improve the quality of service care for the patients.
