여자 중학생의 월경불편감과 수면장애와의 관계 (Relationship between Menstrual Distress and Sleep Disturbance in Middle-school Girls)

  • Purpose: To identify factors associated with menstrual distress and characterize the relationship between menstrual distress and sleep disturbance in middle-school girls. Methods: Participants in this correlational study were 117 middle-school girls who were recruited through convenience sampling. Data were collected from March 2018 to April 2018 using self-reported structured questionnaires and analyzed using IBM SPSS Statistics 23.0. Factors associated with menstrual distress included physiological, psychological, and situational factors based on the theory of unpleasant symptoms. Results: Age of menarche (${\beta}=-.28$, p<.001), amount of menstruation (${\beta}=.23$, p=.004), lifestyle-related exposure to endocrine-disrupting chemicals (${\beta}=.21$, p=.027), and academic and peer-relationship stress (${\beta}=.19$, p=.025) influenced menstrual distress, explaining 47.4% of the variance in this regression model. The relationship between menstrual distress and sleep disturbance was statistically significant. Sleep disturbance was increased 1.26 folds when dysmenorrhea score increased by one unit (OR=1.26, 95% CI: 1.01~1.58). However, parental support was not a significant moderating factor of sleep disturbance. Conclusion: This study provides basis to develop an intervention strategy to alleviate menstrual discomfort in adolescents and improve their quality of sleep.

Treatment of GABA from Fermented Rice Germ Ameliorates Caffeine-Induced Sleep Disturbance in Mice

  • ${\gamma}$-Aminobutyric acid (GABA), a major inhibitory neurotransmitter in the mammalian central nervous system, is involved in sleep physiology. Caffeine is widely used psychoactive substance known to induce wakefulness and insomnia to its consumers. This study was performed to examine whether GABA extracts from fermented rice germ ameliorates caffeine-induced sleep disturbance in mice, without affecting spontaneous locomotor activity and motor coordination. Indeed, caffeine (10 mg/kg, i.p.) delayed sleep onset and reduced sleep duration of mice. Conversely, rice germ ferment extracts-GABA treatment (10, 30, or 100 mg/kg, p.o.), especially at 100 mg/kg, normalized the sleep disturbance induced by caffeine. In locomotor tests, rice germ ferment extracts-GABA slightly but not significantly reduced the caffeine-induced increase in locomotor activity without affecting motor coordination. Additionally, rice germ ferment extracts-GABA per se did not affect the spontaneous locomotor activity and motor coordination of mice. In conclusion, rice germ ferment extracts-GABA supplementation can counter the sleep disturbance induced by caffeine, without affecting the general locomotor activities of mice.

상한문헌에 나타난 수면장애 치료법에 대한 연구 (A Study on Treatment of Sleep Disturbance in the Books on Cold Damage)

  • Purpose : There are two types of sleep disturbance: inability to sleep (不眠) and somnolence (多眠). This study is to examine treatments of the two types of sleep disturbance in the Books of Cold Damage and how those theories were established and formed. Methods : 1. Verses including the words related to inability to sleep and somnolence were extracted from "Treatise on Cold Damage Diseases" (傷寒論). 2. Among the Books on Cold Damages that are classified according to the symptoms, 17 books with contents related to inability to sleep and somnolence were selected to collect and classify data in three perspectives: mechanism of disease (病機), method of treatment (治法) and disease pattern (病證). 3. Data collected through the above methods were compared and diagramed. Results & Conclusions : 1. On Cold Damage, inability to sleep is mostly rooted when human body lacks yin energy while having excessive yang energy (陽盛陰虛) due to fire and heat (火熱). The reason could be misuse of perspiration inducing method (汗法) or purgation (下法) on the doctors' part. 2. On Cold Damage, somnolence is rooted when pathogen (邪氣) is spread to yin meridians (陰經) and the human body lacks yang while having excessive yin energy (陰盛陽虛) or when heat (熱邪) is invaded into interior parts (裏部). 3. Many scholars of Cold Damage in later periods had copied the "Treatise on Cold Damage Diseases" for treatments of sleep disturbance but many others have applied the verses from "Treatise on Cold Damage Diseases" or added new treatments. Do jeol-am (陶節庵) and Wang Geung-dang (王肯堂) particularly had deep understanding on "Treatise on Cold Damage Diseases" and utilized the content freely or suggested new remedies because they had thorough knowledge on relating formula as well.

지역사회 거주 성인의 수면문제 발생률과 위험요인 (Prevalence and Risk Factors of Sleep Disturbance in Community Dwelling Adults in Korea)

  • Purpose: The purpose of this study was to identify the prevalence and risk factors of sleep disturbance among community dwelling adults in Korea. Methods: Data were collected from nine hundred fifty nine adults dwelling in a metropolitan city in Korea. Subjects were older than 45 years, did not take medication for sleep problems on a regular basis, and did not work at night. The Pittsburgh Sleep Quality Index was used to collect data about sleep disturbance. Sociodemographic, life style, and health related factors were included as risk factors. Chi square test and logistic stepwise regression were used to identify the risk factors. Results: The prevalence of sleep disturbance was 13.1%. The significant risk factors were being female, age, number of toilet use at night, perceived health, and pain. Risk factors were somewhat different by gender. Emotional and job status were the risk factors only for men and menopausal status were for women. Conclusion: Prevalence of sleep disturbance in community dwelling adults in Korea was not greater than those dwelling in other countries. Females, older adults, the number of toilet use at night, perceived health, and pain were the most important risk factors for sleep disturbances. Gender was a factor in reported sleep disturbances. It might be useful to investigate the factors that contribute to the number of toilet use at night as this might have some underlying factors that could be corrected.

Associations between Brain Perfusion and Sleep Disturbance in Patients with Alzheimer's Disease

  • Background and Purpose Although sleep disturbances are common and considered a major burden for patients with Alzheimer's disease (AD), the fundamental mechanisms underlying the development and maintenance of sleep disturbance in AD patients have yet to be elucidated. The aim of this study was to examine the correlation between regional cerebral blood flow (rCBF) and sleep disturbance in AD patients using technetium-99m hexamethylpropylene amine oxime single-photon emission computed tomography (SPECT). Methods A total of 140 AD patients were included in this cross-sectional study. Seventy patients were assigned to the AD with sleep loss (SL) group and the rest were assigned to the AD without SL group. SL was measured using the sleep subscale of the Neuropsychiatric Inventory. A whole-brain voxel-wise analysis of brain SPECT data was conducted to compare the rCBF between the two groups. Results The two groups did not differ in demographic characteristics, severity of dementia, general cognitive function, and neuropsychiatric symptoms, with the exception of sleep disturbances. The SPECT imaging analysis displayed decreased perfusion in the bilateral inferior frontal gyrus, bilateral temporal pole, and right precentral gyrus in the AD patients with SL group compared with the AD patients without SL group. It also revealed increased perfusion in the right precuneus, right occipital pole, and left middle occipital gyrus in the AD with SL group compared with the AD without SL group. Conclusions The AD patients who experienced sleep disturbance had notably decreased perfusion in the frontal and temporal lobes and increased rCBF in the parietal and occipital regions. The findings of this study suggest that functional alterations in these brain areas may be the underlying neural correlates of sleep disturbance in AD patients.

  • Purpose: The purpose of this study was to determine the effects of a 6- week Tai-Chi program on improving pain, sleep disturbance, mood, and fatigue in rheumatoid arthritis patients. Method: Sixteen subjects were assigned for experimental group and fifteen were assigned for control group. The subjects of the experimental group participated in a 6-week program of Tai-Chi. Each Tai-Chi class was conducted by a certified Tai-Chi instructor. In order to evaluate the effects of Tai-Chi program, pain, sleep disturbance, mood and fatigue were measured before and after the 6th session Result: After 6-week Tai-Chi program, there were significant improvement in mood & sleep disturbance in the experimental group when compared to the control group However, there were no significant differences in pain intensity and fatigue between Tai-Chi group and control group. Conclusion: These results suggest that a 6-week Tai-Chi program can be utilized as an safe and effective nursing program to improve mood and sleep disturbance in rheumatoid arthritis patients. Long term studies of the effects of Tai-Chi on the pain intensity and fatigue may be import.

혈액투석환자의 피로와 수면장애, 스트레스, 자기효능감 및 우울 관계 (Relationships between Fatigue, Sleep Disturbance, Stress, Self-efficacy and Depression in Hemodialysis Patients)

  • Purpose: This study was conducted to investigate the differences in sleep disturbance, stress, self-efficacy and depression by fatigue level and to identify the factors influencing fatigue in hemodialysis patients. Methods: This study used a cross-sectional design and the subjects were 195 patients undergoing hemodialysis in a local unit. Data were collected using a structured questionnaire and medical records from June to August 2012. The data were analyzed using descriptive statistics, independent t-test, one way ANOVA, $Scheff{\grave{e}}$ test, and Pearson's correlation coefficients. Also a multiple regression analysis was used to assess the independent association between fatigue status and indices of sleep disturbance, stress, self-efficacy and depression. Results: A total of 99 (50.8%) subjects experienced a high level of fatigue and 96 (49.2%) experienced a low level of fatigue. There were significant differences in the levels of sleep disturbance, stress, self-efficacy and depression between the two groups. Fatigue was significantly correlated to sleep disturbance, stress, self-efficacy and depression. The most important factor related to fatigue was stress, followed by sleep disturbance. These factors explained about 37% of fatigue of the respondents. Conclusion: The results of this study showed that sleep disturbance and stress were significant predictors of fatigue in hemodialysis patients. Possible interventions for minimizing fatigue in patients undergoing long-term hemodialysis should be aimed to manage the known contributing factors.

An update on the cause and treatment of sleep disturbance in children and adolescents with autism spectrum disorder

  • Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by abnormalities in social communication/interaction and restrictive, repetitive patterns of behavior. ASD is a relatively common psychiatric disorder, with a prevalence of approximately 1.7% in children. Although many children and adolescents with ASD visit the hospital for medical help for emotional and behavioral problems such as mood instability and self-harming behavior, there are also many visits for sleep disturbances such as insomnia and sleep resistance. Sleep disturbances are likely to increase fatigue and daytime sleepiness, impaired concentration, negatively impact on daytime functioning, and pose challenges in controlling anger and aggressive behavior. Sleep disturbance in children and adolescents with ASD negatively affects the quality of life, nothing to say the quality of life of their families and school members. In this review, sleep disturbances that are common in children and adolescents with ASD and adolescents are presented. The developmental and behavioral impacts of sleep disturbances in ASD were also considered. Finally, non-pharmacological and pharmacological treatments for sleep disturbances in children and adolescents with ASD and adolescents are reviewed.

교통소음에 의한 수면방해에 관한 연구 (Disturbance of Sleep by Road Traffic Noise)

  • 수면의 방해는 대부분의 소음 연구에서 고려되는 여러 요소 중의 하나이다. 수면동안 소음에 노출 되었을 때 수면 단계에 따른 일반적 인간의 응답이 조사되었다. 그리고 깊은 수면율과 잠을 깨는 비율에 대한 연속적인 교통 소음의 효과가 연구되었다.

발반사마사지가 노인의 수면, 우울 및 생리적 지수에 미치는 영향 (The Effects of Foot Reflexion Massage on Sleep Disturbance, Depression Disorder, and the Physiological Index of the Elderly)

  • Purpose: This study was to examine the effects of foot reflexion massage on sleep disturbance, depression disorder, and the physiological index of the elderly in nursing homes. Method: The research design was a non-equivalent control group pretest-posttest quasi-experimental study. The subjects in this study were 50 elderly people who resided in two different nursing homes in the same region. An experimental group and a control group were organized with 25 subjects respectively, and foot reflexion massage was provided for 12 sessions, 30 minutes per session. The selected dependent variables were sleep disturbance, depression disorder, and physiological indices(blood plasma serotonin, serum cortisol), which were all measured before and after foot reflexion massage was offered. Data analysis included $x^{2}$-test, t-test, paired t-test, and ANCOVA, using the SPSS program package. Result: 1. The experimental group improved sleep quality more than the control group. 2. The experimental group had less depression disorder than the control group. 3. The experimental group had higher serotonin levels than the control group. Conclusion: It's necessary to give foot reflexion massage as a successful nursing intervention to elderly who undergo a change in sleep, and suffer from a depression disorder due to a deterioration in sleep.
