방사선촬영에서 면적선량 및 새로운 실질면적선량 개념의 비교 평가 (Comparative Evaluation of Kerma Area Product and New Fundamental of Kerma Area Product on Radiography)

  • Kerma Area Product (KAP) is best indicator of radiation monitoring on radiographic examinations. KAP can be measured differently depending on the X-ray irradiation area, air kerma, souce-skin distance, type of equipment, etc. The major factors are exposure area and the air krema. The KAP currently used only considers the exposure area with X-rays and has a problem that KAP is always excessively overestimated from the dose received by an actual subject. Therefore, in this study, in order to measure the accurate KAP, a new area dose calculation that can be calculated by dividing the area where the actual X-ray is irradiated is presented, and the KAP is the real area. We compared and analyzed how much it was overestimated compared to the dose. The Skull AP projection and seven other projection were compared and analyzed, and the KAP was overestimated in each test by 52% to 60%. In this way, the effective KAP (EKAP) calculation developed through this study should be utilized to prevent extra calculation of the existing KAP, and only the accurate patient subject area should be calculated to derive the accurate area dose value. EKAP is helpful for control the patient's exposure dose more finely, and it is useful for the quality control of medical radiation exposure.

Digital Radiography 환경하에서 일반촬영시 적정 노출조건에 따른 CR과 DDR의 Image Quality 비교 (The Comparison of Image Quality between Computed Radiography(CR) and Direct Digital Radiography(DDR) which Follows the Proper Exposure Conditions in General Photographing under the Digital Radiography(DR))

  • DR은 방사선과 뿐만 아니라 병원전체의 생산성 및 업무의 효율성에서 중요한 요소로 자리잡아가고 있다. DR환경은 CR보다 다양한 Parameter를 가지고 있어 양질의 의료서비스 제공이 가능하다. 현재 각 병원의 방사선과는 Film-Screen system에서 Full-PACS를 통한 DR 환경으로 변화하고 있다. Full-PACS를 사용하고 있는 본원에서는 DR환경에서의 일반촬영 System중 CR과 DDR의 최적노출조건과 그에 따른 Image quality가 어떻게 표현되는 지 알아보고자 본 연구에 임하게 되었다. 본 실험을 통하여 CR과 DDR의 Image quality는 같은 노출조건 하에서는 DDR이 더욱 우수하다는 것을 알 수 있었다. 그리고 DDR system에서도 AEC를 사용했을 때의 Image에 대한 점수가 사용하지 않았을 때보다 조금 더 높은 것을 알 수 있다. 특히 Skull, Chest부분에서는 DDR의 AEC기능이 Image quality를 향상시키는데 유용함을 간접적으로 알 수 있었다. (AEC기능이란 피사체를 통과한 X선을 Detector내에 있는 Ion chamber를 이용하여 전리전류를 검출하는 기구로서 적정농도 도달 시 X선의 조사를 제어하도록 되어있다.) 이 기능은 다양한 피사체의 두께에 따른 노출조건을 고려하지 않아도 적정농도를 나타낼 수 있기 때문에 보다 손쉬운 촬영이 이루어질 수 있다. 이 실험결과로 좋은 Image quality를 얻기 위해서는 적절한 노출조건의 선정이 중요함을 알 수 있었다. 앞으로도 무한한 발전 가능성이 있는 DDR system에 대한 연구가 좀 더 필요하겠다.

Case series of cleidocranial dysplasia: Radiographic follow-up study of delayed eruption of impacted permanent teeth

  • This report describes 3 cases of cleidocranial dysplasia (CCD) and presents relevant findings on long-term follow-up radiographic images of impacted permanent teeth with delayed eruption. Radiographic images of 3 CCD patients were reviewed retrospectively. These images were mainly composed of panoramic and skull radiographs, and the follow-up periods were 3, 13, and 13 years, respectively. The distinct features revealed by the images were described, and the eruption state of impacted permanent teeth was evaluated. The features common to the 3 cases were multiple supernumerary teeth, the presence of Wormian bone, underdevelopment of the maxilla and the maxillary sinus, and clavicular hypoplasia. The eruption of impacted permanent teeth was not observed without proper dental treatment in adult CCD cases, even after long time periods had elapsed. When proper orthodontic force was applied, tooth movement was observed in a manner not significantly different from the general population.

Newborn English Bulldogs with Multiple Malformation Syndrome and Lethal Characters

  • A female English bulldog was gave birth two neonates by cesarean section on the sixty one days after mating, but both neonates were died soon after birth. The bodies of neonates were diagnosed using radiography, ultrasonography, computed tomography and necropsy immediately after death. Both neonates had caudal regression syndrome, butterfly vertebra, hydrocephalus, umbilical hernia, cleft palate and bow-legged hind-limb. At necropsy, neonates had mild fetal anasarca, cleft lip and the skull was remained non-union. At thoracic cavity, only three ribs and thoracic spines were existed and patent ductus arteriosus was found. At abnormal cavity, the renal ectopia was found with abnormal morphology. In the present case, those English bulldog neonates with multiple congenital malformation syndromes seriously suffered vertebral column anomalies and that may induced by neural tube defects in during embryonic period. To prevent congenital malformation occurring in English bulldog, further in depth studies are needed for the breed specific genetic diversity and for the reason of behind genetic abnormality in these breed.

촬영술식에 따른 악관절 방사선 사진상의 비교연구 (COMPARATIVE STUDY OF TEMPOROMANDIBULAR JOINT RADIOGRAMS USING SOME RADIOGRAPHIC PROJECTIONS)

  • For the enhancement of a comprehension in temporomandibular joint radiographs, the author has compared and analysed the roentgenographic images of the temporomandibular joint of human dry skull which was taken by submentovertex projection, panoramic radiography, oblique lateral transcranial projection, corrected anterio-posterior tomogram and corrected lateral tomogram. The obtained results were as follows. 1. The submentovertex projection represented in detail the both poles and the posterior surface of the condylar head of the mandible. 2. The oblique lateral transcranial projection represented the articular space, the outer contour of the condylar head and the position of the condylar head within the mandibular fossa, but the relationship of the temporomandibular joint was not revealed accurate, because of the oblique direction of a central ray in taking radiographs. 3. The corrected antero-posterior tomogram was superior method in representation of roent- genographic images of the superior surface and the both poles of the condylar head and the corrected lateral tomogram was considered as the most accurate method among some radiographic techniques for the interpretation of articular space and condyle-fossa relationship. 4. It was possible to observe three-dimensionally the head of condyle with the combinated use of submentovertex projection, corrected antero-posterior tomogram and corrected lateral tomogram.

A Case of Nasal Diffuse Large B-Cell Lymphoma in a Cat

  • A 6-year-old spayed female Turkish Angora cat presented with sneezing, nasal discharge, and decreased appetite lasting for 21 days. Skull radiography revealed slightly increased density of soft tissue in the left nasal cavity. Computed tomography (CT) scan revealed an extensive mass with nasal septum destruction and moderate contrast enhancement in the left nasal cavity. After surgical biopsy, histopathological examination confirmed that the mass was an infiltrative round cell neoplasm, composed of sheets of large neoplastic cells. Immunohistochemical analysis revealed that most of the neoplastic cells were strongly positive for CD79a and weakly positive for PAX5. Additionally, numerous mature lymphocytes were found to be positive for CD3. This is the first reported case of nasal diffuse large B-cell lymphoma (DLBCL) in a Turkish Angora cat in Korea.

  • A 9-month-old, intact female mixed dog was referred to Veterinary Medical Teaching Hospital of Gyeongsang National University with symmetrically enlarged and protruded upper jaw. The patient was diagnosed as acute renal failure due to ethylene glycol poisoning and was treated for 1 month in a local animal hospital. In spite of treatment, the patient proceeded to chronic renal failure. Also, the patient's upper jaw begun to enlarge continuously. To evaluate this upper jaw, radiographic examination was performed. Skull radiographs revealed thickening of maxilla, decreased bone opacity, cortical thinning, loss of lamina dura and periodontal space in the maxilla. Diagnosis of rubber jaw syndrome is based on clinicial signs, radiographic findings and laboratory evidence of chronic renal failure due to ethylene glycol poisoning.

Multidetector computed tomography in preoperative planning for temporomandibular joint ankylosis: A pictorial review and proposed structured reporting format

  • Ankylosis of the temporomandibular joint (TMJ) is a disabling disease resulting from fibrous or bony fusion of the mandibular condyle and the glenoid fossa. Early diagnosis and surgical treatment are essential to prevent facial deformity and other complications. Conventional radiography has limitations in demonstrating the true extent of ankylosis. It is important for surgeons to be aware of the size and degree of bony ankylosis in order to perform complete resection of the ankylotic mass. In addition, a detailed evaluation of the relationship with adjacent vital structures such as the internal maxillary artery, inferior alveolar nerve canal, external auditory canal, and skull base are crucial to avoid iatrogenic injury. Multidetector computed tomography (MDCT) is the current imaging modality of choice for preoperative assessments. Herein, the authors propose a structured CT reporting template for TMJ ankylosis to strengthen the value of the preoperative imaging report and to reduce the rates of intraoperative complications and recurrence.

개발한 진단용 다엽조리개 성능평가 및 X선영상과 적외선체열영상의 융합영상 구현 (Performance Evaluation of the Developed Diagnostic Multi-Leaf Collimator and Implementation of Fusion Image of X-ray Image and Infrared Thermography Image)

  • We have developed and applied a diagnostic Multi-Leaf Collimator (MLC) to optimized the X-ray field in medical imaging and the usefulness evaluated through the fusion of infrared image and X-ray image acquired by infrared camera. The hand and skull radiography with multi-leaf collimator(MLC) showed significant area dose reductions of 22.9% and 31.3% compared to ARC and leakage dose was compliant with KS A 4732. Also scattering doses of 50 cm and 100 cm showed a significant decrease to confirm the usefulness of MLC. It was confirmed that the fusion of infrared images with an adjustable degree of transparency was possible in the X-ray images. Therefore, fusion of anatomical information with physiological convergence is expected to contribute and improvement of diagnostic ability. In addition, the feasibility of convergence X-ray imaging and DITI devices and the possibility of driving MLC with infrared images were confirmed.

Digital Radiography(DR)에서 검출기의 X선 조건에 따른 흡수선량 및 영상화질 변화에 관한 연구 (Study on the Change of Absorbed Dose and Image Quality according to X-ray Condition of Detector in Digital Radiography(DR))

  • 본 연구는 진단용 검출기가 결함이 발생하는 경우 환자의 방사선 피폭과 영상의 화질에 영향을 줄 수 있다는 점에 착안하여 실험을 진행하였고 선량분석을 통해 검출기 성능을 신호 대 잡음비와 대조도 대 잡음비로 평가하였다. 선량계, 조직등가팬텀을 이용해 흡수선량과 신호 대 잡음비, 대조도 대 잡음비를 측정하였다. 실험방법은 팬텀 후면에 부착하여 나온 선량 값과 검출기 뒤에 부착한 선량 값이 일치하는지를 비교하였고 두개부 흉부 복부의 조건을 각각 75 kVp 25 mAs, 110 kVp에 8 mAs, 80 kVp에 20mAs로 설정하고 검사하였다. 그 결과 검출기의 뒤쪽에 부착한 선량값은 0.004 mGy, 0.006 mGy, 0.003 mGy로 나타났고, 팬텀 후면은 0.006 mGy, 0.016 mGy, 0.017 mGy로 차이가 있었다. 두 값을 일치시키기 위해서 조건이 증가하였고 신호 대 잡음비와 대조도 대 잡음비는 88.32, 88.10, 4.09, 1.63, 87.94, 79.97에서 93.87, 93.75, 4.91, 4.03, 92.02, 84.92로 증가하였다. 본 연구를 통해 검출기가 노후화 되는 경우 기기의 수명단축과 환자의 선량증가의 원인이 되었으며 또한 영상화질의 개선효과도 미비하다는 것을 알 수 있었다.
