남성 간경변 환자의 성기능, 수면의 질, 우울에 관한 연구 (A study of Sexual Function, Sleep Quality and Depression in Male Patients with Liver Cirrhosis)

  • 본 연구는 남성 간경변 환자의 성기능, 수면의 질, 우울 간의 관계를 확인하기 위한 서술적 연구이다. 대상자는 간경변을 진단 받고 외래 추적 관찰 중인 환자 144명으로, 자료 수집은 구조화된 설문지를 이용하여 2017년 8월 16일부터 2018년 1월 30일까지 실시하였다. 성기능은 Rosen 등이 개발한 International Index of Erectile Function의 한국어 번안한 것을 이용하여 측정하였고, 수면의 질은 Buysse 등이 피츠버그 수면센터에서 개발한 Pittsburgh Sleep Quality Index의 한국어 버전을 사용하여 측정하였다. 우울은 Beck Depression Inventory-II를 한국어로 번안한 한국어판 BDI-II 도구를 사용하였다. 분석은 t검정, 분산분석, 상관관계분석을 이용하였다. 연구 결과 남성 간경변 환자들의 성기능이 낮을수록 수면의 질이 낮고(r=-.232, p=.005) 우울 점수가 높았으며(r=-.381, p<.001), 수면의 질이 낮을수록 우울 점수가 높게(r=.561, p<.001) 나타났다. 대상자들의 성기능, 수면의 질, 우울은 통계적으로 유의한 상관관계가 있는 것으로 나타났다. 본 연구 결과는 남성 간경변 환자를 위한 간호 중재 개발의 기초 자료로 활용 될 수 있을 것이다.

성폭력 외상이 소아청소년의 정신병리와 삶의 질에 미치는 영향 (Impact of Trauma due to Sexual Violence on Psychopathology and Quality of Life in Children and Adolescents)

  • Objectives : This study was conducted in order to investigate the influence of sexual trauma on the psychopathology and quality of life of children and adolescents in Korea. Methods : Twenty-seven children and adolescents and their caretakers who visited the Kangwon Sunflower Center participated in a cross-sectional study. Participants completed the Korean version of Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version, Child Behavior Checklist (CBCL), and Child Health Questionnaire-Parent Form-50 (CHQ-PF-50). Their scores were compared with those of a age and sex-matched control group of 27 healthy children and adolescents. Results : Victims of sexual violence showed higher t score in Withdrawn, Social problems, Delinquent behavior, Externalizing problems, and Total problems in CBCL, compared with control group. In CHQ-PF-50, there were lower scores on Bodily pain/discomfort, Behavior, Mental health, Time impact in parents, and Family activities subscales in victims of sexual violence. While Behavior and Family activities subscale showed negative correlations with Delinquent behavior, Externalizing problems, and Total problems in CBCL. Mental health subscale showed negative correlations with Social problems, Delinquent behavior, Externalizing problems, and Total problems. In addition, Time impact on parent subscale showed a negative association with Delinquent behavior in CBCL. Conclusion : The current study provided evidence suggesting that victims of sexual violence had a higher level of psychopathology and lower level of quality of life.

요실금 여성의 성기능에 영향을 미치는 요인 (Factors Influencing the Sexual Function of Women with Urinary Incontinence)

  • Purpose: Sexual function involves a complex interaction of emotions, body image, and intact physical responses. The purpose of this study was to determine the sexual functioning of women who are incontinent and to identify associated factors. Methods: For this descriptive correlation study, data were collected from 147 women with urinary incontinence. Data were analyzed using t-test, ANOVA, and stepwise multiple regression. Results: Mean scores were 22.39 (sexual dysfunction ${\leq}26.55$) for sexual function, 13.38 (of 63) for depression, and 55.47 (range of score 17~85) for body image. Urinary symptoms and daily life symptoms averaged 36.04 (range of score 20~100) and 16.03 (range of score 8~40). Sexual function had a positive correlation with body image and negative correlation with daily life symptoms. Sexual satisfaction, daily life symptoms, marital satisfaction, and frequency of sexual intercourse were factors affecting sexual function. Conclusion: Study results indicate that urinary incontinence has a negative impact on various aspects of sexual function. Nurses should be aware of the wider consideration that needs to be made in relation to general and sexual quality of life when caring for clients suffering from urological diseases.

Nursing Students' Experience of Sexual Harassment During Clinical Practicum: A Phenomenological Approach

  • Purpose: To describe nursing students' experience of sexual harassment during clinical practicum. Methods: An interpretive phenomenological qualitative approach was used to understand contextual experiences of participants. Individual in-depth interviews were conducted to collect data from thirteen nursing students who experienced sexual harassment during clinical practice in general hospitals at D metropolitan city. All interviews were recorded and transcribed into Korean and English. Transcripts were analyzed using the data analysis method described by Diekelmann, Allen, and Tanner. Results: The following 12 themes emerged from the data: 'unprepared to respond', 'lack of education', 'unsure about when behavior crosses the line', 'power differential for nursing students', 'balancing self-preservation with obligations to patients', 'shame', 'feeling responsible for not being able to prevent the harassment', 'impact on patient care', 'fear of what might have happened', 'fear of repercussions', 'long term impact', and 'peer support'. Conclusion: Participants in this study described feeling an obligation to care for their patients. However, they seemed to be unable to balance this while feeling vulnerable to sexual harassment with strong negative feelings. Helping students recognize and effectively deal with sexual harassment is a critical element to assure quality learning for participants and maintain quality of care during clinical practice.

Determination of Sexual Problems of Turkish Patients Receiving Gynecologic Cancer Treatment: a Cross-sectional Study

  • Background: The present study aimed to determine the prevalence and types of sexual problems of Turkish patients receiving gynecologic cancer treatment. Materials and Methods: A cross-sectional convenience sample of 168 women completed the Index of Female Sexual Function (IFSF) and a Patient Identification Form in a hospital in Ankara, Turkey. Mean IFSF scores of the patients were low ($15.7{\pm}5.72$, out of a possible 45), indicating high rates of sexual problems. Results: Women frequently reported problems with dyspareunia (97.1%), vaginal dryness (97.6%), decreased sexual desire (91.1%), and difficulties of sexual arousal (92.9%) related with the cancer treatment process. They reported increased sexual problems following the period of treatment as compared to before treatment (p<0.05). Sexual dysfunction was associated with low educational and income levels, advanced age, TAH-BSO-LND surgery (total abdominal hysterectomy-bilateral salphingoopherectomylymph node dissection), experiencing side effects of chemotherapy, receiving chemotherapy in addition to surgery and radiotherapy (CT+RT+Surgery), and having a large number of chemotherapy cycles (p<0.05). Conclusions: Patients hoped for and expected counseling from healthcare professionals about their sexual functioning in relation to cancer treatments. Nurses and physicians can help to improve the overall quality of life for gynecologic cancer patients through sexual counseling.

중년여성의 성만족에 영향을 미치는 요인 (Factors Influencing the Sexual Satisfaction of Middle-aged Women)

  • 본 연구는 중년 여성의 성기능, 성 의사소통 및 성만족 정도를 파악하고 성만족에 영향을 미치는 요인을 파악함으로써 중년 여성의 성건강 증진을 위한 중재프로그램의 기초자료를 제공하고자 시도되었으며, 2012년 10월부터 2013년 1월에 걸쳐 국내 4개 도시에 거주하는 40세~60세의 중년여성 126명의 설문지를 분석하였다. 본 연구 결과, 성기능, 성 의사소통 및 교육 수준이 중년 여성의 성만족에 영향을 미치는 요인으로 나타나 성기능 정도와 성 의사소통 정도가 높은 경우와 높은 교육 수준을 가진 경우에 성만족 정도가 높아지는 경향이 있음을 알 수 있었다. 또한 본 연구 대상자의 73%가 성기능 장애를 가지고 있는 것으로 나타나 중년 여성이 발달적, 상황적 변화에 더불어 사회경제적 변화로 인해 성건강을 위협받고 있음을 알 수 있었다. 중년 여성의 성문제를 해결하기 위한 개인의 인식이 부족함과 더불어 사회적 차원의 자원이 충분하지 않은 상황에서 의료인은 여성 성기능 장애에 대한 문제의 심각성과 적극적인 해결방안의 중요성을 인식하고, 성 관련 문제 해결 역량을 높이기 위한 적극적인 노력이 필요할 것이다.

간호대학생의 성행동에 영향을 미치는 요인 (Factors that Influencing Sexual Behavior in Nursing Students)

  • 본 연구는 성행동, 부모 및 친구와의 성 의사소통, 성태도 및 성지식 정도와 성행동에 영향을 미치는 요인을 파악함으로써 성건강 향상을 위한 성교육 프로그램의 기초자료를 제공하고자 시도되었으며, 2012년 3-4월 서울과 천안 소재 2개 간호대학에 재학 중인 간호대학생 218명의 설문지를 분석하였다. 연구결과 간호대학생의 성행동, 부모 및 친구와의 성 의사소통, 성태도 및 성지식 정도는 중간이상인 것으로 나타났다. 성행동은 부모와의 성 의사소통, 성태도 및 성지식과 부적상관관계를 나타내었고, 간호대학생의 성행동에 영향을 미치는 요인은 성 관련 정보의 출처, 성지식, 과거 이성교제 경험, 부모와의 성 의사소통 순으로 나타났다. 성문제는 삶의 다양한 측면에 영향을 미치므로 간호사는 성문제를 가지고 있는 대상자를 간호할 때 성문제에 대한 깊은 이해를 가질 필요가 있다. 따라서 간호대학생을 위한 성교육 프로그램은 부모와 친구를 포함한 개인간 접근 전략을 요구하며, 간호대학생 스스로 성건강 관리에 적극적으로 나설 수 있도록 유도해야 할 것이다.

노인 성생활 만족도와 영향요인 (Factors Influencing Elders' Satisfaction with Sexual Life)

  • Purpose: The purpose of this study was to explore the sexual life and sexual life satisfaction of elderly people. Method: The participants were 181 elders over 65 years of age. Data were collected from February 10 to December 15, 2007 by interviews and questionnaires. Results: Of the respondents 17.1% still had sex in their lives but sexual satisfaction was less than that of middle-aged adults. Further, 31.8% of the elders reported no experience of sex over the past 10-19 years. The reasons for not having sex were 'no sexual partner' or 'no sexual desire'. Finally, 27.2% wanted to have a date. The main reason for dating was to relieve loneliness and the reason for not dating was 'good as it is', 'having date is shameful'. The level of satisfaction with sexual life was 2.71. Satisfaction with sexual life was higher for elders who had a spouse and for elders who still had sex. Conclusion: This study provides bases on the usefulness of various nursing intervention programs to enhance the quality of life through better understanding of the sexual life and sexual life satisfaction of elders.

당뇨여성의 성기능 장애 조사연구 (A Study on the Sexual Dysfunction for Diabetic Women)

  • Purpose: The purpose of this study was to investigate the sexual dysfunction of diabetic women in order to provide basic data contributed in nursing intervention. Method: A convenience sample consisted of 67 subjects with diabetes who attended outpatient department of 2 university hospitals. After verbal consent was obtained, subjects were asked to complete a questionnaire including BISF-W developed by Taylor et al. Data were collected from April 27 to May 22, 2006, and were analyzed by descriptive statistics, t-test, oneway ANOVA using SPSS 12.0 program. Results: This study discovered that the degree of sexual dysfunction of women with diabetes was high, and in particular that most of the women with diabetes did not have satisfactory sex life because of lack of vaginal lubrication, lower sexual desire, lower orgasm, and sexual pain. The test of differences in sexual dysfunction according to demographic and disease-related characteristics revealed that significant differences existed only with regard to menopause and age. Conclusion: As the sexual function of women varies widely among individuals, we would like to emphasize the importance of sexual counseling and education programs to improve the quality of life of diabetic women to prevent or relieve their sexual dysfunction.

부인암 환자의 성기능 조사 (Sexual Functioning in Women with Gynecologic Cancer)

  • Purpose: This study was conducted to identify sexual functioning in women with gynecologic cancer. Method: Sexually active women with gynecologic cancer without evidence of distant metastasis were recruited in Seoul, Korea from a university medical center. Subjects were asked to complete an anonymous mail-back survey on their sexual functioning. Result: One hundred eighty four women completed questionnaires. Their mean age was 51.0 years and 96.2% lived with their husbands. Subjects were diagnosed with cervical cancer(53.8%), ovarian cancer (27.7%), or endometrial cancer(18.5%). Sexual functioning for women with gynecologic cancer was relatively low, 15.4, in comparison to Rosen's cutoff scores of 26.6. Univariate analyses indicated that age, employment status, and their monthly income were significantly associated with sexual functioning. Tumor staging, treatment modality, and hormone replacement therapy were also significantly associated factors with women's sexual functioning. Sexual arousal, orgasm, and pain were affected by time since last treatment. Conclusion: Sexual counselling or education for women with gynecologic cancer should be considered by medical professionals in order to improve their quality of life including sexual functioning.
