Economic Burden of Colorectal Cancer in Korea

  • Objectives: The incidence and survival rate of colorectal cancer in Korea are increasing because of improved screening, treatment technologies, and lifestyle changes. In this aging population, increases in economic cost result. This study was conducted to estimate the economic burden of colorectal cancer utilizing claims data from the Health Insurance Review and Assessment Service. Methods: Economic burdens of colorectal cancer were estimated using prevalence data and patients were defined as those who received ambulatory treatment from medical institutions or who had been hospitalized due to colorectal cancer under the International Classification of Disease 10th revision codes from C18-C21. The economic burdens of colorectal cancer were calculated as direct costs and indirect costs. Results: The prevalence rate (per 100 000 people) of those who were treated for colorectal cancer during 2010 was 165.48. The economic burdens of colorectal cancer in 2010 were 3 trillion and 100 billion Korean won (KRW), respectively. Direct costs included 1 trillion and 960 billion KRW (62.85%), respectively and indirect costs were 1 trillion and 160 billion (37.15%), respectively. Conclusions: Colorectal cancer has a large economic burden. Efforts should be made to reduce the economic burden of the disease through primary and secondary prevention.

Identifying Adverse Events Using International Classification of Diseases, Tenth Revision Y Codes in Korea: A Cross-sectional Study

  • Objectives: The use of administrative data is an affordable alternative to conducting a difficult large-scale medical-record review to estimate the scale of adverse events. We identified adverse events from 2002 to 2013 on the national level in Korea, using International Classification of Diseases, tenth revision (ICD-10) Y codes. Methods: We used data from the National Health Insurance Service-National Sample Cohort (NHIS-NSC). We relied on medical treatment databases to extract information on ICD-10 Y codes from each participant in the NHIS-NSC. We classified adverse events in the ICD-10 Y codes into 6 types: those related to drugs, transfusions, and fluids; those related to vaccines and immunoglobulin; those related to surgery and procedures; those related to infections; those related to devices; and others. Results: Over 12 years, a total of 20 817 adverse events were identified using ICD-10 Y codes, and the estimated total adverse event rate was 0.20%. Between 2002 and 2013, the total number of such events increased by 131.3%, from 1366 in 2002 to 3159 in 2013. The total rate increased by 103.9%, from 0.17% in 2002 to 0.35% in 2013. Events related to drugs, transfusions, and fluids were the most common (19 446, 93.4%), followed by those related to surgery and procedures (1209, 5.8%) and those related to vaccines and immunoglobulin (72, 0.3%). Conclusions: Based on a comparison with the results of other studies, the total adverse event rate in this study was significantly underestimated. Improving coding practices for ICD-10 Y codes is necessary to precisely monitor the scale of adverse events in Korea.

Soft-tissue Sarcomas in the Asia-Pacific Region: A Systematic Review

  • Background: Soft-tissue sarcomas require tailored and multidisciplinary treatment and management. However, little is known about how sarcomas are treated and managed throughout the Asia-Pacific region. Materials and Methods: MEDLINE was systematically searched using prespecified criteria. Publications (previous 10 years) that reported tumour characteristics, treatment patterns, survival outcomes, and/or safety outcomes of patients with soft-tissue sarcoma were selected. Exclusion criteria were studies of patients <18 years of age; ${\leq}10$ patients; countries other than Australia, Hong Kong, Indonesia, Korea, Malaysia, New Zealand, Philippines, Singapore, Taiwan, or Thailand; >20% benign tumours; sarcomas located in bones or joints; gastrointestinal stromal tumour; Kaposi's sarcoma; or not reporting relevant outcomes. Results: Of the 1,822 publications retrieved, 35 (32 studies) were included. Nearly all patients (98%, 1,992/2,024; 31 studies) were treated with surgery, and more studies used adjuvant radiotherapy than chemotherapy (24 vs 17 studies). Survival outcomes and recurrence rates varied among the studies because of the different histotypes, sites, and disease stages assessed. Only 5 studies reported safety findings. Conclusions: These findings highlight the lack of specific data available about soft-tissue sarcomas in the Asia-Pacific region. Better efforts to understand how the sarcoma is managed and treated will help improve patient outcomes in the region.

Case Study on Dynamics of RDA PLA Model with Agri-SMEs (농업인 참여식 실천학습모델 개발과 성과분석 -농촌진흥청 강소농 사업을 중심으로-)

  • This case study aims to explore how RDA PLA model affects the agri-SMEs' empowerment. As an agri-business management renovation program from main workshop it was conducted on March to December 2011 with agri-SMEs and extension officials nationwide by RDA. Especially, as a packaged action learning process in the model used participatory action research. This study collected data with participants observation, interviews, situational analysis and systematic review of discourse in qualitative method. For the validity and identifying empirical results, this study used statistic analysis as a mixed method. Further including various pedagogic methods and business coaching skills, this model was conducted from workshop in RDA, in turn, on-farm business coaching as follow-up, CoPs' activities, and local ATCs extension services by each actors. The dynamic process and effects of each process led some change for farmers' innovative knowledge, skills, attitude, practice and aspiration on their farm business. RDA PLA model development based on the previous practices and research, which provided a configurated picture in the holistic action learning process. In statistic research, this study focused on 279 farmers as respondents who had participated in the program. It shows that their income and benefits increased from their renovative practices on farm business. Following the sampling group, it was surveyed by four indicators - products, customer, quality and cost. The level of contribution of education on economic impact 15% is quoted from previous paper. Even in some limitations of public sector, RDA PLA model actively suggests the paradigm shift of agricultural HRD and development of alternative extension-service system.

Pre- and Post-Tax Audit Differences of The Firm Value (세무조사전후의 기업가치의 차이)

  • This study addresses and examines differences in firm value after tax audits by the Korean Internal Revenue Service. Tax audits can potentially depreciate a firm's value due to the mass cash outflow that often results from the additional tax charges involved. However, tax audits that reveal negative aspects of a business, such as excessive entertainment expenses, fraudulent accounting, or inappropriate business practices, may have positive effects on a firm's value, as the monitoring involved can improve accounting transparency and reduce agency costs. This study shows that there is typically an increase in a firm's value in the year after a tax audit has been conducted, in comparison with the previous year. This result suggests that firm value can increase after a tax audit is conducted, despite the possible value depreciation resulting from a mass cash outflow.

  • In this highly competitive century, selling products and service through Internet and smart phones offers both opportunities and challenges. Online commerce is expanding it's wings to the offline market. The connection between online market and offline market is called O2O(Offline to Online) market. In this study we examine the best practice case study of an Internet company's successful efforts to connect users and offline merchants. Based on Dianping.com success story in China, a successful framework for building online to offline ecosystem is examined. Dianping.com successful experience may provide suggestions for other online companies operate in the convergent field.

Collaborating for Science and Technology Under "One China, Two Systems"

  • Since Deng Xiaoping's implementation of the "One China, Two Systems" policy, mainland China and the other Chinese regions of Hong Kong and Macau have cooperated in various ways to work towards successfully developing China's overall economy and industries. Particularly, cooperation between Guangdong Province and adjoining Hong Kong have been contributing to China's development, and this study explores their industry conditions including their current two governments policies designed to promote collaboration. The two partners were in a cooperative relationship even before the handover of Hong Kong, beginning with a "front shop, back factory" model built on their respective comparative advantages in labor-intensive industries in the 1980s. This cooperation effectively propelled the Pearl River Delta Region's industrialization process and enabled Hong Kong to transform from a manufacturing industry-based economy to a service industry-based economy. From the early 2000s, Guangdong and Hong Kong diversified their collaboration project from culture to high-tech. Also, both authorities produced several types of policies not only to promote both industries but also to harmonize their two different economic levels and models. As a result, the Guangdong and Hong Kong economies have developed remarkably well during the past two decades and continue to form future plans that carry plenty of optimism. Nonetheless, this study showed discrepancies between engineers and scientists from the two areas in their perception of their technology and science cooperation. Hong Kong experts were more negative in their responses but noted some successes of the collaboration, while Guangdong's group showed overall positive responses. This difference results from an unbalanced role in cooperation. Hong Kong's side responds to cooperation plans and takes on leading roles with more frequency than Guangdong's side in actual cooperation project processes.

The Analysis of the Outcome of Standardized Hospital Pharmacist Training and Repeated Asthma Patient Education (천식환자에 대한 표준화된 병원약사대상 교육과 반복적인 환자대상 복약지도를 통한 성과 분석)

  • Training for asthma patients by professionals has been highly emphasized for patients' self-management. In the present study, three standardized training sessions targeting hospital pharmacists were performed. On the basis of the number of training sessions taken, a non-educated group (CG), an once-educated group (NG), and a twice more-educated group (IG) were allocated. The most frequent errors of using an inhalant were shaking before the use in MDI while taking breathe out before inhaling in the case of DPI, and the total average number of errors were the biggest for CG and the smallest in IG. On comparison in the number of the four symptoms of asthma according to the level of seriousness, it was revealed that the total average number was the biggest in CG and the smallest in IG. In the level of awareness regarding the contents of training for patients, patients over 50 tended to answer that they were aware of education contents, and particularly in mouth rinsing and the use before/after a meal, significant similarity was examined. Regular asthma patient training performed by hospital pharmacists appeared to reduce errors in the inhaler usage, increase the quality of life of an patient, and help a patient remember the contents of the training. Especially among patients over 50, it was found to be more effective and retraining of more than two sessions was requisite.

Literature Review of Herbal Medicines on Treatment of Obesity Since 2000;Mainly about Ephedra Herba (2000년 이후 비만치료에 사용되는 처방(處方) 및 본초(本草)에 대한 문헌연구;마황(麻黃)을 중심으로)

  • Objectives : This study has been done to know which herbs and herbal formulae are frequently used for the treatment of obesity in Korea and overseas since 2000. Methods : We conducted a computerized literature search in KISS (Korean studies Information Service System), Pubmed, Journal of Oriental Association for Study of Obesity ,and Journal of Oriental Rehabilitation Medicine(2000 -Apr. 2006) using search key words obesity, obese, weight loss, weight control combined with herb, herbal formula. Result and conclusion : Seventy-two studies have been identified(50 Korean papers and 22 pubmed papers) and 81 herbs have been found. In USA it was mainly used as single herb for food, but in Korea it was mainly used as herbal formula. For single herb, ephedra(麻黃) was the most studied single herb both in Korea and the USA. For herbal formulae, Sasang Constitutional Medicine(SCM, 19times) was the most frequently used herbal formula, in which the herbs for Tae-eum type person like Taeyeumjowui-tang(太陰調胃湯, 7times) and Choweseungcheng-tang(調胃升淸湯, 5times) were mainly prescribed. The next was Chegameuiin-tang(體減薏苡仁湯, 11times) and Bangpungtongsungsan( 防風通聖散, 3times). Calculating each herb consisted in each herbal formula, Coicis Semen(薏苡仁), Angelicae gigantis Radix(當歸), and Glycyrrhizae Radix(甘草), Raspberry Ketone, Cnidii Rhizoma(川芎), and Ephedra Herba(麻黃) turned out to be the most frequently used herb.

Regional Difference of Chronic Periodontal Care Services in Korea (의료기관 종별 만성 치주염 진료의 지역 간 차이)

  • Objectives: This study aimed to investigate the regional difference of chronic periodontal care services in Korea by the analysis of 2010 raw data from Health Insurance Review and Assessment Service. Methods: The subjects were the chronic periodontitis patients over 35 years old from dental care facilities in Korea. The study population was 278,319 including 264,994 claims made by dental clinics, 8,084 by dental hospitals, 3,509 by general hospitals, and 1,732 by tertiary hospitals. Results: There was a significant difference in medical care cost benefit between the provinces(p<0.0001). The age groups showed a clear difference in the patient charge, cost of insurance, and medical care cost benefit(p<0.0001). In consideration of the first visit or revisit, there were differences in the rate of prescription, dental examination, and surgical procedures of the chronic periodontal patients from dental facilities. The radiographic use rate in the tertiary hospitals was 2.6 times higher than that of the dental clinic in the treatment of the new chronic periodontal patients. Conclusions: The use of dental services in the periodontitis is influenced by the types of medical services facility, cost of medical insurance, and patient charge. In consideration of cost benefit analysis, prevention is the most important care for the periodontitis. Regional difference in peridontitis is cause by the use of medical services and quality of treatment.
