작업치료사의 임상 경력에 따른 작업치료직무 중요도에 관한 조사 (A Survey on the Job Importance of Occupational Therapists According to Length of Clinical Career)

  • Purpose : This study examined the differences in occupational therapists' jobs depending on their past clinical careers using a survey of the importance of occupational therapy tasks in South Korea. This survey sought to provide fundamental data for strengthening occupational therapists' capacities and developing clinical training programs for therapists' future careers. Methods : An online survey was used to collect data from 93 South Korean occupational therapists. The subjects were divided into three groups for the analysis of the importance of occupational therapy tasks, depending on the length of their clinical careers: less than 3 years, from 4 to 6 years, and over 7 years. Results : A within-groups comparison of task importance found that "occupational therapy evaluation" was most important in all three groups. In between-groups comparisons, "patient information check", "assessment planning and establishment", "sensory function assessment", "social skills assessment", "developmental function improvement", "basic daily activities training", "social skills improvement", and "patient education" demonstrated statistically significant differences (p<.05). Each group showed different priorities. In the less-than-3-years group, "understanding of patient's needs" was most important, followed by "arm function improvement", "arm function assessment", "nerve, musculoskeletal, and motor-related functional improvement", and "basic daily activities training". In the 4-to-6-years group, "arm function assessment" was most important, followed by "basic daily activities assessment", "patient's needs", "arm function improvement", and "patient information check". In the over-7-years group, "understanding of patient's needs" was most important, followed by "patient information check", "basic daily activities assessment", "basic daily activities training", and "sensory function assessment". Conclusion : This study found that what occupational therapists consider important in their jobs varies according to their past clinical career and that occupational therapists with more than 7 years' experience are more aware of the importance of diverse fields of occupational therapy. Differentiated education that considers length of career and a range of training programs for more experienced occupational therapists will be needed in the future.

감각통합치료를 하고 있는 작업치료사의 놀이에 대한 인식 (Play Awareness of Occupational Therapists With Using the Sensory Integration Approach)

  • Objective : This study is to know about the play awareness of occupational therapists with using the sensory integration approach, and is to suggest educational strategies of play as occupation and research. Methods : 124 occupational therapists completed the questionnaires including the general use of play in occupational practice and the use of play assessments with preschoolers, and factors influence on the use of play in occupational therapy for children. Results : Respondents used the play as therapy modality and performed much with based on sensory integration and development approach in individual occupational therapy room with variable play tools. It was restricted to use as objective assessment tools and methods, and had important factors that influence on the use of play in occupational therapy for children; role of occupational therapy in work setting, personal frame of reference and availability of play assessments. Conclusion : The results suggest a need for increasing the emphasis on play in entry-level curricula and continuing education, improving clinician access to valid and reliable play assessments, and completing studies designed to examine the use and efficacy of play in sensory integration therapy intervention.

감각통합치료를 통한 적응반응의 변화 : 사례보고 (A Study on the Change of Adaptive Response Through the Sensory Integration Intervention : Case Study)

  • 서론: 본 연구는 감각통합장애(감각조절장애와 실행장애의 혼합) 아동의 사례보고를 통해 아동의 내적동기를 유발하기 위한 감각통합 치료의 방향설정 과정을 살펴보고 적응반응의 변화에 대해 제시하고자 하였다. 본론: 연구대상은 2008년 감각통합치료과정의 단기집중 프로그램에 참여한 만 5세 10개월 된 남아이다. 아동의 평가결과에 기초하여 연구목표, 치료계획 및 내용을 제시하였고 4 세션 치료기간 동안의 적응반응 변화를 분석하였다. 결론: 본 연구결과, 감각통합치료를 통한 적응반응의 향상을 볼 수 있었고 아동의 내적동기 유발이 중요하다는 것을 알 수 있었다. 그러나 향후에는 감각통합치료를 통한 치료효과를 보다 객관적으로 제시하고 일상에서의 변화를 함께 고려해야 할 것이다.

내수용 감각과 촉각처리 및 심박변이도 간의 관계 (Relationship Between Interoceptive Awareness, Tactile Processing, and Heart Rate Variability)

  • 목적 : 본 연구의 목적은 내수용 감각(interoception)과 감각처리, 그리고 자율신경계의 균형을 반영하는 심박변이도와의 관계를 알아보는 것이다. 연구방법 : 젊은 성인을 대상으로 청소년/성인 감각프로파일(Adolescent/Adult Sensory Profile; AASP)과 다차원 내수용 자각 척도(Multidimensional Assessment of Interoceptive Awareness; MAIA)를 사용하여 감각처리와 내수용 감각을 평가하였다. 심박은 안정상태 심전도로 기록되었고, 심전도에서 심박표준편차(Standard Deviation of Normal to Normal interval; SDNN)를 추출하였다. AASP 감각영역과 MAIA 하위척도 간 상관을 분석하였고, 높은 SDNN과 낮은 SDNN 집단 간의 감각처리 및 내수용 감각의 차이를 비교하였다. 결과 : AASP 촉각 영역이 MAIA 하위척도와 상대적으로 강한 상관을 나타내었다. AASP 촉각 영역의 점수가 높을수록 MAIA 걱정하지 않음과 주의 조절 점수가 낮은 반면, 정서자각 점수는 높았다. 높은 SDNN 집단은 낮은 SDNN 집단과 비교하였을 때 내수용 감각이 높은 경향성을 나타냈다. 결론 : 본 연구는 촉각 처리와 내수용 감각의 관련성을 확인하였으며, 심박변이도가 높은 그룹은 내수용 자각이 높은 경향성이 있음을 제시하였다. 본 연구는 최근 작업치료와 감각통합에서 강조되는 내수용 감각에 대한 정보를 제공하고, 감각통합의 신경과학적 기전에 대한 이해를 심화시킨다.

Risk and protective factors affecting sensory recovery after breast reconstruction

  • Background Although loss of sensation in patients with breast cancer after mastectomy followed by breast reconstruction is an important factor affecting patients' quality of life, the mechanism of sensory recovery is still unclear. Our study aimed to identify variables that affect sensory recovery, especially pain, in reconstructed breasts. Methods All patients with breast cancer who underwent mastectomy followed by immediate breast reconstruction, including nipple reconstruction or areolar tattooing, were included in this study. Sensation was evaluated in the nipple as an endpoint of sensation recovery of the whole breast. Patients rated pain severity using a 3-point verbal rating scale (VRS): grade 0, no pain; grade 1, mild to moderate pain; and grade 2, severe pain. The VRS was assessed by a single experienced plastic surgeon. Results In the univariate analysis, the odds ratio (OR) for sensation recovery was 0.951 for age (P=0.014), 0.803 for body mass index (P=0.001), 0.996 for breast volume before surgery (P=0.001), 0.998 for specimen weight after mastectomy (P=0.040), and 1.066 for the period between mastectomy and sensory assessment (P=0.003). In the multivariate analysis, the variables that showed a significant effect were age (OR, 0.953; P=0.034), the period between mastectomy and sensory assessment (OR, 1.071; P=0.006), and reconstruction using abdominal tissue instead of prosthetic reconstruction (OR, 0.270; P=0.004). Conclusions Based on our results, it can be inferred that aging has a negative impact on the recovery of sensation, breast sensation improves with time after surgery, and the recovery of sensation is better in prosthetic reconstruction.

환경에 따른 여성외의용 신합섬 폴리에스테르 직물의 접촉감성 (Tactile Sensibility of New Generation High Touch Polyester Fabrics for Women's Outerwear at Different Environmental Conditions)

  • Purposes of this study were firstly to refine the measurement technique of tactile sensation and sensibility induced by touching the fabrics and secondly to investigate the differences of tactile sensibility depending on environmental conditions and the type of polyester fabrics. The environmental conditions were controlled at 20$\pm$1$^{\circ}C$, 65$\pm$3%RH(standard) and at 29$\pm$1$^{\circ}C$, 75$\pm$3%RH(warm & humid). As results, three factors were extracted from factor analysis of sensory assessment scale. The first factor was thickness and weight, the second one was surface-related property and the third one was stiffness in both environments. Depending on the environmental conditions, heat and moisture-related sensations such as [cold], [damp] and [dry & drapable] were affected significantly. Regression equations with the sensation descriptors were developed for the prediction of tactile sensibility(R2>.89). While the most efficient descriptor was [thin & flexible] in the standard environmental conditions, that was [dry & drapable] in the warm & humid environmental condition.

Presentation of central motion techniques: limpness motion function and limpness sensory unit function

  • Central motion techniques are to mention the central-motion by the limpness motion function and limpness sensory unit function on the body. Central body motion is consisted of the limpness central function by the central body system. To evaluate the signal of central body motion, we are investigated a limpness value of the central function by the central body function on the static state. The concept of limpness motion function was checked the reference of limpness motion signal and limpness sensory signal by the central motion body. For assessment on the limpness sensory variation of the maximum and average in terms of central motion from the static function, and limpness value that was a limpness value of the vision condition of the Vi-${\lambda}_{MAX-AVG}$ with $8.71{\pm}-3.2units$, that was a limpness value of the vestibular condition of the Ve-${\lambda}_{MAX-AVG}$ with $3.05{\pm}-6.52units$, that was a limpness value of the somatosensory condition of the So-${\lambda}_{MAX-AVG}$ with $2.4{\pm}1.9units$. The static sensory motion was made mention of check out at the condition of the limpness sensory unit motion for the comparable values of limpness central motion that was expressed the analysis capacity by the limpness nerve system. Limpness sensory system will be to propose of the minute motion by static central motion situation and was to imply a limpness motion data of static body sensory function.

가토의 하치조 신경 손상 형태에 따른 전기생리학적 및 조직학적 변화에 관한 실험적 연구 (AN EXPERIMENTAL STUDY OF ELECTROPHYSIOLOGICAL AND HISTOLOGICAL ASSESSMENT ON THE INJURY TYPES IN RABBIT INFERIOR ALVEOLAR NERVE)

  • Inferior alveolar nerve dysfunction may be the result of trauma, disease, or iatrogenic injury. Inferior alveolar nerve injury is inherent risk in endodontic therapy, orthognathic surgery of the mandible, and extraction of mandibular teeth, particularly the third molars. The sensory disturbances of inferior alveolar nerve associated with such injury have been well documented clinical problem that is commonly evaluated by several clinical sensory test including Tinels sign, Von Frey test(static light touch detection), directional discrimination, two-point discrimination, pin pressure nociceptive discrimination, and thermal test. These methods used to detect and assess inferior alveolar nerve injury have been subjective in nature, relying on the cooperation of the patients. In addition, many of these techniques are sensitive to differences in the examiners experience and skill with the particular technique. Data obtained at different times or by different examiners are therefore difficult to compare. Prior experimental studies have used electro diagnostic methods(sensory evoked potential) to objectively evaluate inferior alveolar nerve after nerve injury. This study was designed with inferior alveolar nerve of rabbit. Several types of injury including mind, moderate, severe compression and perforation with 19 gauze, 21 gauze needle and 6mm, 10mm traction were applied for taking the sesory evoked ppterntial. Latency and amplitude of injury rabbit inferior alveolar nerve were investigated with sensory evoked potential using unpaired t-test. The results were as follows : 1. Intensity of threshold (T1) was $128{\pm}16{\mu}A$ : latency, $0.87{\pm}0.07$ microsecond : amplitude, $0.4{\pm}0.1{\mu}V$ : conduction velocity, 23.3 m/s in sensory evoked potential of uninjured rabbit inferior alveolar nerve. 2. Rabbit inferior alveolar nerve consists of type II and III sensory nerve fiber. 3. Latency was increased and amplitude was decreased in compression injury. The more injured, the more changed in latency and amplitude. 4. Findings in perforation injury was similar to compression injury. Waveform for sensory evoked potential improved by increasing postinjured time. 5. Increasing latency was prominent in traction injury rabbit inferior alveolar nerve. 6. In microscopic histopathological findings, significant degeneration and disorganization of the internal architecture were seen in nerve facicle of severe compression and 10mm traction group. From the above findings, electrophysiological assessment(sensory evoked potential) of rabbit injured inferior alveolar nerve is reliable technique in diagnosis and prognosis of nerve injury.

아동감각처리척도(Sensory Processing Scale for Children; SPS-C)의 판별타당도 연구 (The Study of Discriminant Validity for the Sensory Processing Scale for Children (SPS-C) of Children with and without Sensory Processing Disorder)

  • 목적 : 본 연구의 목적은 국내에서 개발된 아동감각처리척도(Sensory Processing Scale for Children)를 사용하여 일반발달 아동과 감각처리문제 아동의 감각처리 능력을 비교하여 판별타당도를 알아보고자 하였다. 연구방법 : 연구대상자는 3-5세의 일반발달 아동 68명과 감각처리문제 아동 83명의 보호자로 총 151명을 대상으로 하였다. 연구 참여에 동의한 아동의 보호자에게 연구의 목적과 방법을 설명한 후 연구목적, 동의서, 아동감각처리척도(Sensory Processing Scale for Children), 단축감각프로파일(Short Sensory Profile), 일반적 특성 문항을 포함한 설문지를 발송 또는 전달하여 자료를 수집하였다. 분석방법은 아동감각처리척도(Sensory Processing Scale for Children)의 판별타당도를 알아보기 위해 성별을 공변량으로 하여 공분산분석을 실시하였다. 결과 : 연구결과 일반발달 아동과 감각처리문제 아동의 감각처리능력의 차이는 공변량인 성별의 영향(F=6.663, p=.807, partial =η2.000)을 받지 않은 것으로 나타났으며, 두 군에서 감각처리능력에 차이가 있는 것으로 나타났다(F=37.629, p=.000). 아동감각처리척도(Sensory Processing Scale for Children)의 감각영역에서 총점과 모든 감각 영역에서 감각처리문제 아동이 일반발달 아동보다 평균점수가 낮았으며, 고유감각(p=.097) 영역을 제외하고 통계적으로 유의한 차이가 있었다. 아동감각처리척도(Sensory Processing Scale for Children)의 감각 요소에서 감각과민반응, 감각저반응, 그리고 감각구별 요소에서 감각처리문제 아동과 일반발달 아동은 통계적으로 유의미한 차이(p=.000)가 있었으나 감각찾기 요소는 통계적으로 유의한 차이가 없었다(p=.112). 결론 : 아동감각처리척도(Sensory Processing Scale for Children)는 감각처리문제 아동을 선별할 수 있는 판별타당도가 있는 도구로 제시할 수 있으며, 앞으로 임상 환경에서 감각처리의 문제를 선별하거나 분석, 해석, 그리고 중재계획 시 사용할 수 있으리라 생각한다.

전정감각, 고유감각 중심의 감각통합중재가 발달장애 아동의 공간 지각력과 시-운동 협응력에 미치는 영향 (The Effect of Vestibular-Proprioceptive Based Sensory Integration Intervention on Spatial Relations and Visual-Motor Coordination in Children with Developmental Disabilities)

  • 목적 : 본 연구를 통하여 발달장애 아동에게 적용한 전정감각, 고유감각 중심의 감각통합 중재가 공간 지각력, 시-운동 협응력에 어떠한 영향을 미치는지 알아보고자 한다. 연구방법 : 본 연구는 발달 장애 아동 만 5~6세 아동 3명을 대상으로 개별실험연구방법(single subject research design) 중 ABA' 설계를 사용하였다. 매주 3회기씩 6주간 30분씩 총 18회기를 진행하였다. 기초선 3회기와 재기초선 3회기는 중재 없이 공간 지각력과 시-운동 협응을 평가하였고 중재기 12회기는 전정감각, 고유감각 위주 감각통합 중재를 실시하였다. 중재의 효과를 알아보기 위한 매회기 측정으로는 전산화 인지 프로그램(CoTras-C)을 이용하였으며, 공간지각력 평가를 위한 퍼즐하기, 시-운동 평가를 위한 미로 찾기를 실시하였다. 연구의 사전, 및 사후 평가는 카우프만 아동용 지능 검사(Kaufman Assessment Battery for Children; K-ABC II)와 한국판 시지각 발달 검사(Korean-Developmental Test of Visual Perception-2: K-DTVP-2)의 하위평가를 이용하였다. 결과 : 발달장애 아동에게 전정감각, 고유감각 중심의 감각통합중재를 적용하기 전보다 후에 공간지각력과 시-운동 협응력 점수에 유의한 차이가 있었고, 세 아동 모두 통계적으로 유의한 차이가 있었다. 결론 : 전정, 고유감각 중심의 감각통합 치료가 발달장애 아동의 공간지각력과 시-운동 협응력에 유의미한 영향을 미쳤다.
