Parents' and Health-Care Providers' Perspectives on Side-Effects of Childhood Cancer Treatment in Indonesia

  • Background: Efficacy of childhood cancer treatment in low-income countries may be impacted by parents' and health-care providers' perspectives on chemotherapy-related side-effects. This study explores prevalence and severity of side-effects in childhood cancer, and compares health beliefs about side-effects between parents and health-care providers, and between nurses and doctors in Indonesia. Materials and Methods: Semi-structured questionnaires were filled in by 40 parents and 207 health-care providers in an academic hospital. Results: Parents exporessed a desire to receive more information about side-effects (98%) and worried about this aspect of treatment (90%), although side-effects were less severe than expected (66%). The most frequent was behavior alteration (98%) and the most severe was hair loss. Only 26% of parents consulted doctors about side-effects. More parents, compared to health-care providers, believed that medicines work better when side-effects are more severe (p<0.001), and accepted severe side-effects (p=0.021). More health-care providers, compared to parents, believed that chemotherapy can be stopped or the dosage altered when there are side-effects (p=0.011). More nurses, compared to doctors, stated that side-effects were unbearable (p=0.004) and made them doubt efficacy of treatment (p<0.001). Conclusions: Behavior alteration is the most frequent and hair loss the most severe side-effect. Apparent discrepancies in health beliefs about side-effects exist between parents and health-care providers. A sustainable parental education program about side-effects is recommended. Health-care providers need to update and improve their knowledge and communication skills in order to give appropriate information. Suchmeasures may improve outcome of childhood cancer treatment in low-income countries, where adherence to therapy is a major issue.

  • The global aging phenomenon and new lifestyles have a worldwide attention to the new tourism form of health tourism. The purpose of this study is to analyze and suggest the competitive types of health tourism. Through the literature review, the theoretical base on the concept and type of health tourism and hierarchical spatial types such as medical space, semi-medical space, non-medical space, and natural healing space, based on medical intervention was established. A questionnaire survey was conducted for the professionals in Jeju area about tourism, health care, policy. As a result of analysis based on competitiveness advantage factors of Jeju health tourism, Jeju health tourism was confirmed to have higher importance in the natural healing space than the health service center space. Three kinds of resources such as natural resources, healing resources, and active resources of Jeju type health tourism were derived and applied to two hierarchical spaces, natural healing space and non-medical healing space, based on medical intervention.

Validity and Reliability of a Dish-based, Semi-quantitative Food Frequency Questionnaire for Korean Diet and Cancer Research

  • This study evaluated the validity and reliability of applying a newly developed dish-based, semi-quantitative food frequency questionnaire (FFQ) for Korean diet and cancer research. The subjects in the present study were 288 Korean adults over 30 years of age who had completed two FFQs and four 3-day diet records (DRs) from May 2008 to February 2009. Student's t-tests, Chi-square tests, and Spearman's rank correlation coefficients were used to estimate and compare intakes from different dietary assessment tools. Agreement in quintiles was calculated to validate agreement between the results of the second FFQ (FFQ-2) conducted in February 2009 and the DRs. Median Spearman's correlation coefficients between the intake of nutrients and foods assessed by the FFQ-1 and FFQ-2 were 0.59 and 0.57, respectively, and the coefficients between the intake of nutrients and foods assessed by the FFQ-2 and the DRs were 0.31 and 0.29, respectively. The quintile classifications of same or adjacent quintile for intake of nutrients and foods were 64% and 65%, respectively. Misclassification into opposite quintiles occurred in less than 5% for all dietary factors. Thus this newly-developed, Korean dish-based FFQ demonstrated moderate correspondence with the four 3-day DRs. Its reliability and validity are comparable to those reported in other studies.

  • Ginseng is renowned for both its medicinal and herbal uses and successful cultivation of Panax ginseng in Asia and Panax Vtiinvtiefolilim in North America has until recently taken place in the native geographical ranges of the plants. As a consequence of the potential high capital return and anticipated increases in consumer consumption, commercial cultivation of American ginseng now occurs well outside the native range of the plant in North America. In fact, the region of greatest expansion of cultivation is in the semi-arid interior region of British Columbia, Canada. Linked with this expansion is the potential domination of the ginseng industry by agricultural corporations. In the interior of British Columbia, the native decidous forest environment of eastern North America is simulated with elevated polypropylene shade and a sllrface covering of straw mulch. The architecture of these environments is designed to permit maximillm machinery useage and to minimize labour requirements. Further, with only a four-year growth cycle, plant densities in the gardens are high. In this hot, semiarid environment, producers believe they have a competitive advantage over other regions in North America because of the low precipitation rates. This helps to minimize atmospheric humidity such that the conditions for fungal disease development are reduced. If soil moisture levels become limited, supplemental water can be provided by irrigation. The nature of the radiation and energy balance regimes of the shade and much environment promotes high soil moistilre levels. Also, the modified environment reduces soil heating. This can result in an aerial environment for the plant that is stressful and a rooting zone environment that is sub-optimal. The challenge of further refining the man modified environment for enhanced plant growth and health still remains.

Toxicity of doxycycline, a broad-spectrum semi-synthetic antibiotic, in Pacific whiteleg shrimp (Litopenaeus vannamei) (흰다리새우(Litopenaeus vannamei)에서 광범위 반합성 항생제 doxycycline의 독성연구)

  • Doxycycline is a semi-synthetic broad-spectrum antibiotic, and it has been used to get rid of bacteria in animals and humans. The use of antibiotics has greatly contributed to the aquaculture production although its misuse sometimes presents public health problems. This study was performed to investigate the toxic effects of doxycycline on whiteleg shrimp (Litopenaeus vannamei) administered for possible infection treatments. The shrimp were allocated into four groups and doxycycline was fed three times a day for 7 days at 0, 20, 50 and 100 mg/kg to each group. After 24 hr following the 7-day treatment, hemolymph and hepatopancreas were used for blood and biochemical analysis: Total hemocyte counts, Total protein, Total cholesterol, Gluscose, Glutamic pyruvic transaminase, Glutamic oxaloacetic transaminase, Glutathione peroxidase, Superoxide dismutase, Glutathione-s-transferases, Total antioxidant capacity colorimetric and Acid phosphatase. In addition, histopathological examination was performed on the hepatopancreas and muscle. It was observed that body weight gain was significantly retarded in 100 mg/kg doxycycline group. Doxycycline was found to induce biochemical or functional disorders at 100 mg/kg as observed many of the blood and biochemical parameters were significantly reduced. In conclusion, it was judged that there will be no major toxicity problems with doxycycline when used for shrimp aquaculture at regular doses.

Clinicopathological Correlation of Cervical Carcinoma: A Tertiary Hospital Based Study

  • Background: To study the clinical presentation of cervical carcinoma correlating with histopathological findings in a tertiary hospital situated in the southern part of India catering to rural and semi-urban populations Materials and Methods: 199 cases histopathologically diagnosed as cervical cancer over a period of one year were considered for the study. Clinical details of the patients were noted with the help of semi-structured proforma. The data was analysed by descriptive analysis using SPSS software. Results: Out of 199 patients, 109 had moderately differentiated squamous cell carcinoma, 51 poorly differentiated and 35 well differentiated. Adenocarcinomas numbered only four. 121 cases were in the age group of 40-59 years, 59 in 60-80 years and 19 in 20-39 years. All four cases of adenocarcinoma were seen between 40-59 years. 95 (47.7%) cases were in women who had 4 or more children, 120 presented with white discharge, 89 with bleeding per vagina and 68 had constitutional symptoms. Most of the patients with adenocarcinoma presented with bleeding per vagina. 151 was in stage IIIB, 29 in stage IIB, 14 in stage IVA and 5 in stage IB. Conclusions: Screening of cervical cancer should be emphasised in women with white discharge especially in rural areas for early detection of dysplastic cells and reduce mortality and morbidity in productive age. In addition health education has to be given to women regarding the awareness of hygiene, risk factors and symptoms of cervical cancer.

Visibility Evaluation for Agricultural Tractor Operators According to ISO 5006 and 5721-1 Standards

  • Purpose: A system to measure the visibility of agricultural tractor operators was designed and evaluated according to ISO standards, and a blind area diagram around the tested tractor was created based on the manual method recommended by the National Institute for Occupational Safety and Health (NIOSH). Methods: A visibility measurement system was designed and evaluated based on the ISO 5006 and ISO 5721-1 standards. Two bulbs used to simulate the operator's eyes were mounted on a bar with a supporting frame. A wooden frame was used to determine the seat index point position. The 12-m visibility test circle was divided into six sectors of vision, and the test tractor was placed at the center of the circle. Artificial light was supplied in the darkened environment, and shadow or masking effects were measured manually around the 12-m circle. Results: When the bulbs were placed at the operator's eye level, front visibility was good; no masking was found in the "A" vision sector, but larger masking widths were found in the "B" and "C" vision sectors. Since the masking width exceeded 700 mm, additional tests, such as movement of the light sources to both sides of the operator's eye level, were performed. Less than six masking effects were found in the semi-circle of vision to the front, and more than one masking was found in the "B" and "C" visual fields. The minimum distance between the centers of two masking effects exceeded 2500 mm when measured as a chord on the semi-circle of vision. A blind area diagram was created to define the exact nature of the blind spots and mirror visibility. Conclusions: Visibility evaluation is an effective way to enable proper and safe operation for agricultural tractor operators. Inclusion of this visibility evaluation test in the general testing process might aid tractor manufacturers.

Development of a Semi-Quantitative Food Frequency Questionnaire for Assessing the Usual Dietary Intake of Korean Adolescents (우리나라 청소년의 일상식이섭취량 평가를 위한 반정량적 식품섭취빈도조사지의 개발)

  • This study was performed to develop the semi-quantitative food frequency questionnaire (SQFFQ) for assessing the usual dietary intake of Korean adolescents. For that, we used 24 hour recall data from the 2005 NHANES(the Third Korean National Health and Nutrition Examination Survey). The cumulative percent contribution and cumulative multiple regression coefficients of 17 nutrients(energy, protein, fat, carbohydrate, fiber, calcium, phosphorus, iron, sodium, potassium, vitamin A, retinol, ${\beta}$-carotene, thiamin, riboflavin, niacin, vitamin C) of each food were computed. Among 687 food items, 265 food items were selected and grouped depending on similarities in ingredients, nutrient profiles, and/or culinary usage and re-added food items which were excluded for seasonal effect. Finally, total 19 food groups, 87 food items, were included in SQFFQ. Food intake frequency was quantified using nine categories. The portion size was classified depending on the average size of each selected food item. Each portion size was then categorized as one of three amounts: small (0.5 times), medium (1 times), and large (1.5 times). The SQFFQ covered 91.9% of the intake of 17 nutrients in 2005 NHANES and 86.6% in 2001 NHANES. Therefore, by testing the validity of developed SQFFQ using nutrient intakes, this list was valid to evaluate the usual daily intake in Korean adolescents.

The Characteristics of Coastal Currents to the Northwest of the Taean Peninsula in the Yellow Sea (서해 태안반도 북서 연안해역에서의 연안류 특성)

  • To investigate the characteristics of tidal currents and water circulation in the coastal waters off the Taean Peninsula, tidal currents and sea levels were measured at the study area from 1998 to 2004. In the central waterway to the south of Changan Sand Ridge, mean speed of tidal currents and residual currents were 74.0cm/s, 17.8cm/s respectively; the dominant residual currents flowed northeastward, and the amplitudes of semi-diurnal components $(M_2,\;S_2)$ were larger than diurnal components $(O_1,\;K_1)$. The flood and ebb tidal currents were northeastward and southwestward, respectively, and each period was about 6 hours for them, which was consistent with the period of sea levels at the study area. In the coastal region near Hakampo, Taean, mean velocities of tidal currents and residual currents were 46.1cm/s, 30.8cm/s respectively, and the dominant residual currents flowed southwestward. The amplitudes of shallow water constituents $(M_4,\;MS_4)$ were relatively laige, which were weaker to the northeastern coastal region off Mineodo. The northeastward flow continued for about $2{\sim}3$ hours, while the southwestward flow continued for about $9{\sim}10$ hours near Hakampo during the tidal period. Tidal currents flowed northeastward in the central area of the waterway during the period from the Low Water Level (LWL) to the High Water Level (HWL). While the currents in the coastal region flowed northeastward for the first 3 hours after the LWL, southwestward counter-currents flowed between 3 and 6 hours after the LWL. During the period from the HWL to the LWL, the dominant currents flowed southwestward in the study area except to the northeastern coastal region off Mineodo. Along the shorelines, the counter-currents flowed northward between 4 and 6 hours after the HWL. It seems that the counter-currents near the coastal region are caused by the topography and the geography of the shorelines at the study area.

Experimental and numerical study of Persian brick masonry barrel vaults under probable structural hazards

  • Understanding and analysing the behaviour and response of historical structures in the face of climate changes and environmental conditions is of utmost significance for their preservation. There are several structural hazards associated with climate and hydrology changes in the region, including the settlement of piers, the rotation of piers, and temperature changes. The present study investigates the experimental and numerical structural behaviour of skewed and non-skewed Persian brick masonry barrel vaults under various conditions. The external loading conditions included pier rotation in five modes, settlement, and temperature variations in four states. Initially, the experiments extracted the mechanical properties of the scaled materials. Then, three semi-circular brick barrel vaults were tested with gravitational loads. The outcomes were used to develop and validate the finite element model. Following the development of the finite element model, numerical and parametric studies were conducted on the effect of the aforementioned structural hazards on the response of brick masonry barrel vaults with various Persian geometries (semi-circular, drop pointed, and four-centred), angles of skew (0, 15, 30, and 45 degrees), and dimensional ratios. According to the findings, the fragility of masonry materials makes historical structures susceptible to failure under different loading. A brick barrel vault fails in the presence of minor rotation and settlement of the piers. The four-centred geometric shape has the lowest performance among the seven Persian geometries; therefore, its health monitoring and retrofitting should be prioritised. In Isfahan, Iran, temperature variations, particularly during the warm seasons, cause critical conditions in such structures.
