The safety of food is occasionally questionable, as there have been some reports of products contaminated with illegal adulterants. In this study, the presence of 16 sedative-hypnotics and sleep inducers in dietary supplements was determined by ultra-high-performance liquid chromatography with UV detection (UPLC-UV) and quadruple Orbitrap mass spectrometry (Q-Orbitrap-MS). The UPLC method was validated, providing a linearity (R2) of more than 0.999, and LODs and LOQs that ranged from 0.2 to 0.5 and 0.6 to $1.5{\mu}g\;mL^{-1}$, respectively. The repeatabilities were 0.2-8.4 % (intra-day) and 0.3-4.5 % (inter-day), and the accuracies were 89.0-117.0 % (intra-day) and 87.8-111.9 % (inter-day). The mean recoveries of the spiked samples ranged from 98.7 to 107.3 %. The relative standard deviation (%RSD) of the stability was less than 2.4 %. Using the developed method, one sedative-hypnotic compound, phenobarbital, was detected in one of the nineteen samples tested. In addition, the major characteristic fragment ions of each target compound were confirmed using Q-Orbitrap-MS for higher accuracy. Monitoring the presence of these 16 sedative-hypnotics and sleep inducers in dietary supplements should be pursued in the interest of human health, and the results of this study confirmed that the developed method has value for this application.

Purpose: Drug overdose is easily found in the emergency department (ED). Sedative-hypnotics overdose causes the aspiration pneumonia in patients with decreased mental status. The purpose of this study was to investigate the risk factors of aspiration pneumonia in patients poisoned with sedative-hypnotics. Methods: One hundred seventy eight patients who were poisoned with sedative-hypnotics and who visited ED between 2009 and 2015 were included. This study was conducted retrospectively, with collection of data by review of medical records. We collected the data concerning the characteristics of patients and classified them into two groups based on the development of aspiration pneumonia. Logistic regression analysis was performed to investigate the factors for the development of aspiration pneumonia. Results: Thirty five patients had an aspiration pneumonia during their hospital stay in 178 patients. The age, amount of ingestion, Glasgow Coma Scale (GCS) score at admission, a history of hypertension and diabetes mellitus, and the hypotension at admission were significantly different between two groups in univariate analysis. The age, amount of ingestion and GCS score at admission were associated with the development of aspiration pneumonia in the multivariate logistic regression analysis (OR 1.028 (95%CI, 1.002-1.056, p=0.037), 1.026 (95%CI, 1.004-1.043, p=0.001), 0.737 (95%CI, 0.683-0.915, p=0.002)). All patients with aspiration pneumonia were discharged without a sequelae. Conclusion: The development of aspiration pneumonia in the patients of sedative-hypnotics overdose is associated with old age, amount of drug ingestion, and GCS score at admission.

Anemarrhenae Rhizoma has been clinically used as a sedative drug in oriental medicine. We observed the sedative action and effect on the movement of isolated-ileum with each water extract of Anemarrhenae Rhizoma (F-I), saline solution treated preparation (F-II) and 25% ethanol treated preparation (F-III). The results obtained were summerized as follows; 1) Sedative actions were recognized with F-I, F-II and F-III. 2) Prolonged effect of sleeping time was recognized significantly with F-I, F-II and F-III. 3) Each fraction of Anemarrhenae Rhizoma showed the released action on the isolated-ileum of the mouse, rat, rabbit and guinea pig, and antagonic action against contraction induced by acetylcholine, barium chloride and histamine. Thus the mechanism in the released action of the smooth muscle was partly considered as the direct dependence of the smooth muscle.

Background: Dental phobia or anxiety of patients is the serious impediment to appropriate and effective dental treatment. Sedative technique helps to mitigate patients' fear and anxiety thus make them more cooperative and familiar to dental practices. With increasing attention to sedative dentistry in dentists, educational requirements and technical qualification also become stricter but actual survey on recent sedative dentistry has not been reported yet. Especially there is insufficient study reporting the survey of sedative dentistry subjected to Korean adults. In this paper, we conducted a survey study on the actual condition and practice related to sedation with a questionnaire to dentists in South Korea. Methods: The survey was done for members of The Korean Dental Society of Anesthesiology (KDSA), who had great interest in sedation and for whom survey-by-mail was convenient. 472 members of The KDSA having dental license and solid address and contact information were subjected to the survey by sending them survey questions about their sedative techniques and knowledge. In order to increase the response rate, small gifts were presented to those who accurately responded to the survey questions and text messages and phone calls were made to encourage their participation. We collected their responses over two months and examined the returned surveys. Statistical analysis was performed using IBM SPSS Statistics 21 for each question. Results: Out of 472 dentists, 181 responded (38.4% response rate). 63.0% (114 dentists; 77 male and 37 female) of respondents had experience on sedative technique and their average age was $39.8{\pm}7.6$ year. 74 of them were private practitioners, 17 of them were professors (14.9%), 11 of them were dentists-in-service (9.6%), 11 of them were residents (specialist training) (9.6%) and 1 of them was military doctors (0.9%). There were 89 dentists (78.1%) who were specialists or receiving trainings to be specialist, most of whom were pediatric dentists (55, 48.2%) and oral surgeon (31, 27.2%). The most popular route for drug medications was orderly oral, inhalational, intravenous medication. Combination of oral and inhalational medications or single use of intravenous medication was the most common. The most preferred sedative drug was pocral in oral sedation and midazolam in intravenous sedation. 48.2% of practitioners responded that they experienced side effects and emergency situations. Airway obstruction was the most frequent. Conclusions: Results from the survey show that the protocol and system for sedative dentistry have been improved compared to the past. Nevertheless, quality of emergency protocol, monitoring devices and preparation of sedative drugs was still insufficient to achieve safe sedative procedure. This study acquires novelty since actual survey on recent sedative dentistry for adult patients has not been reported yet.

In order to investigate the effect of Kamiondam-tang on the cardiovascular system and sedative action, pharmacological studies have been carried out with water extract. The results of this investigation were summarized as follows; The sedative action was significantly recognized in mice. Vasodilative action in rabbit peripheral blood vessel and hypotension in anesthetized rabbit were remarkably recognized. Inhibitory effect of cardiac systolic action was recognized by Straub method in frogs.

It is important to check the types of drugs when treating acute drug intoxication. This study researches the clinical characteristics and types of patients hospitalized in emergency medical center for drug addiction in 2009 and 2010. By applying a drug addiction selecting kit, it studied the flexibility and clinical efficiency during diagnosis and treatment. The study result shows, among the drugs causing addiction, Sedative accounts for 34.4%, Herdicide 23.5%, Analgesic 17.2%, Insectide 7.8% and else 17.2%. Sedative showed the highest proportion both in 2009 and 2010. Among the drug addicted patients, 39 cases did not know that the drugs are poisonous and among them, the drug addiction selecting kit was positive in 32 cases (82.0%). In 42 cases where addiction was suspicious, 25 cases (59.5%) were positive in the drug addiction selecting kit. In 57 cases of using drug addiction selecting kits, the cases in which benzodiazepine was positive, were 30 and the most frequently cases were as follows; Tricyclic Antidepressants (TCA) 13 cases, Amphetamines 3 cases, barbiturate 3 cases, and piate 3 cases. In the district using drug addiction kit meaningfully had Flumazenil injection much greater impact than the one not using the kit. This proves the efficiency of the kit (p<0.05). The uses of drug addiction selecting kits are for acute drug intoxicated patients and for providing objective and scientific information when emergency medical doctor are checking unchecked poisoning drugs. It is considered that drug addiction selecting kits would give help when treating the early stage of drug addicted patients in emergency medical treatment.

This paper includes a review of 555 drug-facilitated sexual assault (DFSA) cases analyzed at the National Forensic Service (NFS), South Korea, between 2006 and 2012. The results of toxicological analyses of blood and urine samples were also reported, and furthermore the results were interpreted with respect to the number of drugs detected. The number of DFSA cases was highest during warmer summer months and the mean age of the victims was 25 years, with 48% being between 20 and 29 years. Accommodations or entertainment places were the most frequent place of the sexual assault (57%); and the assailant was a stranger in 72% of the DFSA cases. Drugs were identified in the blood or urine samples in 145 cases (26%) and sedative-hypnotics, such as benzodiazepines and zolpidem, were the most commonly detected, along with sedative antihistamines such as doxylamine and diphenhydramine. The frequent presence of sedative drugs in biological samples tends to implicate their use in chemical submission. However, interpreting the analytical results in terms of voluntary vs. surreptitious administration of drugs requires further detailed investigation and knowledge of the victim's health status and medication used at the time of event.

This study was performed to assess clinical signs, sedative effect and clinicschemical profile of a mixture of fentanyl-azapemne-xylazine(Fentazine$^{(R)}$) in formed elk Twelve male elk(Cervus canadensis) were immobilized with Fentazine, and blood samples were taken of femoral venous blood. Samples were analyzed in the conditions of 10- and 30-minute after administration of the drug. Heart rates, respiratory rates, and body temperatures were in normal ranges during Fentazine anesthesia. After iqiection of Fentazine, most of elk were recumbency and did not respond to needle prick In young adult(3.5.4.5 years old) elk a high dose(>3.0 ml/head) of Fentazine does not result in more sedation, but it does prolong the duration of sedative effect. Fentazine induced sufficient analgesia far velvet antler removal and hoof trimming in elk Salivation, urination, intermittent apnea and mild bloat were observed in elk Globulin, alanine aminotransferase, aspartate aminotransferase, blood urea nitrogen, calcium, magnesium and phosphorus values were in normal ranges for at least 30 minutes after Fentazine administration. Total protein, albumins cortisol and prothrombin values were slightly increased during sedation(p<0.05). It was concluded that Fentazine is effective analgesic drug being useful for velvet antler removal and hoof trimming.

Chloral hydrate is the oldest and most common sedative drug used in moderate sedation for pediatric dental patients. Hence, the purpose of this article is to review the safety and possible adverse events of this drug when used for pediatric dental treatment. A bibliographic search in PubMed, MEDLINE, Cochrane Library and KMbase, KISS, DBpia, KoreaMed, and RISS databases was performed. Using the keywords "dental sedation," "chloral hydrate," and "children or adolescent," 512 scientific articles were found. Subsequently, 183 studies were individually assessed for their suitability for inclusion in this literature review. Altogether, 24 studies were selected. They included 12 cases of death before, during, or after chloral hydrate sedation for dental treatment, majorly due to dosing error and use of multiple sedatives. Additionally, intraoperative adverse events were mostly respiratory problems such as hypoxia and apnea, but most events were temporary. After treatment, prolonged sedation, including excessive sleep and less activity were the most common postoperative adverse events, and even death cases were reported. Despite the wide acceptance of chloral hydrate as a sedative-hypnotic agent, the risk of adverse events and adequate dose should be of great concern when using it for pediatric dental sedation.

Acanthopanax cortex (Acanthopanax sessiliflorum, Araliaceae, KP VIII), an important herbal drug, has been used as tonic, antistress and immuno-enhancing drugs in Korea. And Peiplocae cortex(Periploca sepium, Asclepiadaceae, CP 2000) has been used as cardiotonic, anti-inflammatory, and sedative effect in china. These are called “Namogapi” of Acanthopanax cortex and “Bukogapi” of Periplocae cortex in Chinese herbal market. These herbal medicines are sometimes circulated as same herbal medicine “Ogapi”.
