임플랜트 지지 상부구조물에서 나사조임순서와 조임방법이 풀림토크값에 미치는 영향 (THE EFFECT OF SCREW TIGHTENING SEQUENCE AND TIGHTENING METHOD ON THE DETORQUE VALUE IN IMPLANT-SUPPORTED SUPERSTRUCTURE)

  • Statement of problem: The screw detorque value is a measure of the preload remaining in the screw just before detorquing. Purpose: This study evaluated the effect of different screw tightening sequences and tightening methods on detorque values for a well-fitting implant superstructure. Material and method: An implant superstructure that connected directly to four implants (Astra Tech) was fabricated on a fully edentulous mandibular acrylic resin model. Six well-fitting dental stone casts were made with a pickup impression of the superstructure from the acrylic resin model. To evaluate the effect of three screw tightening sequences (1-2-3-4, 2-4-3-1, and 2-3-1-4) and two tightening methods (2-step and 1-step) on the stability of screw joint, the detorque values for a well-fitting implant superstructure were measured twice after screw tightening using 20 Ncm. Detorque values were analyzed using multi-way analysis of variance and two-way analysis of variance at a .05 level of significance. Results: 1. The mean detorque values for three screw tightening sequences were 12.3 Ncm, 12.6 Ncm, and 12.0 Ncm, respectively. 2. The mean detorque values for two screw tightening methods were 12.0 Ncm, and 12.2 Ncm, respectively. 3. The mean of mimimum detorque values for three screw tightening sequences and for two tightening methods were 10.6 Ncm, 11.1 Ncm, 10.5 Ncm, and 9.8 Ncm, respectively. 4. No statistically significant differences among the variables of screw tightening sequence and tightening method were found (p>.05) for detorque values and for mimimum detorque values. Conclusion: Within the limitations of this study, the screw tightening sequence and tightening method did not have a significant effect on the detorque values for a well-fitting implant superstructure.

내부연결 임플란트 상부구조물에서 나사조임술식이 풀림토크값에 미치는 영향 (The effect of screw tightening techniques on the detorque value in internal connection implant superstructure)

  • 연구 목적: 본 연구는 잘 맞는 임플란트 상부구조물에서 서로 다른 나사조임순서와 조임방법이 나사의 풀림토크값에 미치는 영향을 평가하였다. 연구 재료 및 방법: 서로 수동적 적합 관계를 갖는 완전 무치악 하악 주모형과 네 개의 평행한 임플란트 (Astra Tech)에 직접 연결되는 금속구조물을 제작하였다. 금속구조물과 잘 맞는 실험모형을 위해 주모형에서 연결인상법을 이용하여 여섯 개의 경석고 모형을 얻었다. 20 Ncm로 조인 후 나사의 풀림토크값을 두 번 측정하였다. 세 가지 나사조임순서 (1-2-3-4, 2-4-3-1, 그리고 2-3-1-4)와 두 가지 나사조임방법 (two-step과 one-step)에 대한 나사의 풀림토크값과 최소풀림토크값을 유의수준 .05에서 각각 다원분산분석법 (multi-way ANOVA)과 이원분산분석법 (two-way ANOVA)을 이용하여 통계분석 하였다. 결과: 나사조임순서에 대한 나사의 평균 풀림토크값은 12.8 Ncm (2-4-3-1)에서 13.1 Ncm (2-3-1-4)의 값을 보였고, 나사조임방법에 대한 값은 13.1 Ncm (two-step)와 11.8 Ncm (one-step)였다. 나사조임순서에 대한 나사의 평균 최소풀림토크값은 11.1 Ncm (1-2-3-4)와 11.2 Ncm (2-4-3-1과 2-3-1-4)였고, 나사조임방법에 대한 값은 11.2 Ncm (two-step)와 9.9 Ncm (one-step)였다. 나사의 풀림토크값과 최소풀림토크값은 세 가지 나사조임순서 간에 통계학적으로 유의성 있는 차이가 없었다. 그러나 두 가지 나사조임방법 간에는 두 값 모두 통계학적으로 유의성 있는 차이가 있었다. 나사를 두 단계로 나누어 조이는 방법이 한 번에 조이는 방법보다 더 큰 풀림토크값 (P = .0003)과 더 큰 최소풀림토크 값 (P = .0035)을 보였다. 결론: 본 연구와 같은 조건하에서, 연결인상법으로 얻은 잘 맞는 임플란트 상부구조물에 있어서 나사조임순서는 나사의 풀림토크값에 영향을 미치는 중요한 요소는 아니었다. 그러나 나사를 두 단계로 나누어 조이는 방법은 한 번에 조이는 방법보다 더 큰 풀림토크값을 보였다.

  • Abutment screw loosening of dental implant is a major problem in the treatment of dental implant. The purpose of this study was to examine the effect on the prevention of screw loosening according to tightening method and using of washer. In the first experiment, using a implant fixture and precisely fabricated temporary cylinder, the loosening torque was measured by tightening the screw 5 times repeatedly. In the second experiment, the fabricated crown and implant fixture was tightened by the screw that is attatched washer and then the loosening torque was measured after loading it 1800, 12600 times under the force of 150N. The results were as follows. 1. The highest loosening torque appeared after tightening 3 times repeatedly(p<0.01). 2. When the washer was used, the loosening torque was increased significantly under the loading 1800 times(p<0.01), 3. When the washer was used, the loosening torque was increaed under the loading 12600 times, but it is statistically not significant(p<0.01). The first experiment results mean that 3 times repeated tightening screw is most effective clinically. The second results mean that washer is effective in prevention screw loosening, but the effect is decreased with increasing load.

치과용 임플란트 지대주나사의 조임체결력에 따른 지지골과 지대주나사의 유한요소법 응력 분석 (A finite element stress analysis on the supporting bone and abutment screw by tightening torque of dental implant abutment screw)

  • Purpose: A study analysed the stress distribution of abutment screw and supporting bone of fixture by the tightening torque force of the abutment screw within clinical treatment situation for the stability of the dental implant prosthesis. Methods: The finite element analysis was targeted to the mandibular molar crown model, and the implant was internal type 4.0 mm diameter, 10.0 mm length fixture and abutment screw and supporting bone. The occlusal surface was modeled in 4 cusps and loaded 100 N to the buccal cusps. The connection between the abutment and the fixture was achieved by combining three abutment tightening torque forces of 20, 25, and 30 Ncm. Results: The results showed that the maximum stress value of the supporting bone was found in the buccal cortical bone region of the fixture in all models. The von Mises stress value of each model showed 184.5 MPa at the 20 Ncm model, 195.3 MPa in the 25 Ncm model, and 216.5 MPa in the 30 Ncm model. The contact stress between the abutment and the abutment screw showed the stress value in the 20 Ncm model was 201.2 MPa, and the 245.5 MPa in the 25 Ncm model and 314.0 MPa in the 30 Ncm model. Conclusion: The increase of tightening force within the clinical range of the abutment screw of the implant dental prosthesis was found to have no problem with the stability of the supporting bone and the abutment screw.

임플란트 고정체와 지대주의 체결방식에 따른 스크류 풀림토크에 관한연구 (A Study on the Screw Loosening Torque According to the Type of Tightening the Implant Fixture and Abutment)

  • Purpose: In this study, the loosening torque test was conducted with three implant products that are produced, approved and sold in Korea, which are manufactured in different fixture and abutment tightening methods (internal submerged type, internal morse taper type, and external type) to examine the loosening torque of the screw according to the method of tightening the implant fixture and abutment. Methods: In the loosening torque test, the three types of fixtures and abutments with different tightening methods were tightened by rotating them clockwise with a $30N{\cdot}cm$ force using a driver equipped with an electric torque meter. The results of the test are as follows. Results: The loosening torque values of the internal submerged type, internal morse taper type and external type implants were $24.10{\pm}0.742N{\cdot}cm$, $29.10{\pm}1.710N{\cdot}cm$, and $26.60{\times}1.636N{\cdot}cm$, respectively. Conclusion: The screw loosening torque values of the three fixture and abutment tightening methods were analyzed via Kruskal Wallis test layout, and they were significantly different (p<0.05).

다른 연결 시스템을 갖는 임플랜트 상부 구조물에서 조임술식에 따른 지대주 나사의 풀림 토크값에 대한 연구 (Removal Torque Values of Retaining Screws Tightened to Implant-Supported Prosthesis with Different Connection Systems by Various Tightening Technique)

  • 임플랜트 치료가 보편화되고, 다양한 형태와 재료의 상부 구조물이 보급되었다. 그리고 실패에 대한 보고도 다양하며, 그 중 지대나사의 풀림현상이 가장 흔하다고 지적되고 있다. 본 연구는 외부연결구조와 내부연결구조 임플랜트에 의해 지지되는 상부 구조물을 나사로 연결할 때, 세 가지 나사조임순서와 두 가지 나사조임방법에 따른 나사의 풀림 토크값을 비교하여 나사조임순서와 나사조임방법이 서로 다른 연결구조에 따라 나사풀림현상에 미치는 영향을 알아보고자 하였다. 완전 무치악 하악모형을 자가중합형 아크릴릭 레진으로 제작하고 이공간 부위에 중심간 거리가 약 15 mm 되도록 네 개의 임플랜트 고정체 유사체를 고정한 후 바 타입으로 납형 형성하여 상부 구조물을 주조 제작하였다. 상부 구조물과 정확한 적합이 되는 주모형을 제작한 후 연결 인상법으로 외부연결구조, 내부연결구조를 가지는 연구모형을 각각 5개씩 제작하였다. 각 모형에서 각 나사의 풀림 토크값을 가장 왼쪽에 조여지는 나사를 1번 나사로 하고 가장 오른쪽의 나사를 4번 나사로 명명하였다. 먼저, 나사조임순서의 영향을 알아보기 위해 1-2-3-4, 2-3-1-4, 2-4-3-1의 순서로 15 Ncm의 힘으로 조이고, 다시 같은 순서로 최종 조임토크값인 30 Ncm까지 조인 후 (2-step 방법) 각 나사의 풀림 토크값을 측정하였다. 또한 나사조임방법의 영향을 알아보기 위해 2-3-1-4의 순서로 한 번에 최종 조임토크값인 30 Ncm까지 조인 후 (1-step 방법) 각 나사의 풀림 토크값을 측정하여 같은 순서 (2-3-1-4)의 2-step 방법과 비교하였다. 세 가지 나사조임순서에 따른 나사의 풀림 토크값은 외부연결구조에서 2-3-1-4군이 2-4-3-1군보다 유의하게 낮았다 (p<0.05). 그리고 내부연결구조에서도 2-3-1-4군이 2-4-3-1군과 1-2-3-4군보다 유의하게 낮았다 (p<0.05). 또한 나사조임순서와 무관하게 몇 번째 조여진 나사인지에 따른 풀림 토크값을 비교해 본 결과, 외부연결구조에서는 처음에 조여진 나사가 두 번째 조여진 나사보다 유의하게 높았으나 (p<0.05), 내부연결구조에서는 처음에 조여진 나사에서부터 마지막에 조여진 나사 간에 차이가 없었다. 그리고 두 가지 나사조임방법 간에는 외부연결구조와 내부연결구조 모두에서 통계학적으로 유의한 차이가 없었다. 연결구조에 대한 비교에서 외부연결구조와 내부연결구조의 풀림 토크값은 각각 16.27 Ncm, 14.25 Ncm 였으며, 통계적으로 유의한 차이가 나타났다 (p<0.05). 다수 임플랜트에 의해 지지되는 상부 구조물을 나사로 연결할 때, 나사조임순서에 따라 차이가 있었으며, 가운데부터 조인 경우에서 풀림 토크값이 낮았고, 연결구조 간에도 차이가 있었다. 풀림 토크값에 미치는 요인에 대해 좀 더 상세히 분석할 필요가 있으며, 하중 조건에서의 부가적인 연구가 필요할 것으로 사료된다.

수종의 임플랜트 시스템의 나사풀림에 관한 연구 (Screw Loosening of Various Implant Systems)

  • Dental implant systems have shown many post-surgical problems and One of the most frequent problem is screw loosening. To reduce screw loosening, a number of methods have been tried and recently fundamental modification of fixture-abutment connection structure was developed and used the most frequently. Former implant system structure, such as Br${\aa}$nemark, had external hex with the height of 0.7 mm and later, fixture with external hex of 1.0 mm height and internal hex structure were developed. In addition, the method of morse taper application was introduced to reduce screw loosening. In this study, the level of screw loosening of each implant systems was compared based on the vibration loosening measurement of abutment screw of each implant systems. Analysis of measured value was performed using 3 kinds of methods, (i) Percentage of average of initial 3 times loosening-torque value(initial loosening value) to tightening-torque of 30 Ncm, (ii) Percentage of loosening-torque value after 200 N strength loaded(experimental value) to initial loosening value and (iii) Percentage of experimental value to 30 Ncm of tightening-torque. Each result of analyses shows the value of initial loosening, loosening by repetitive load and final loosening level. The results of this study were as follows. (1) Percentage of initial loosening value to tightening-torque was increased in order of 0.7 mm external hex, 1.0 mm external hex, internal hex and internal taper and all values between each groups showed statistical significance (p<0.05). (2) Percentage of experimental value to initial loosening value was increased in order of internal hex, 0.7 mm external hex, 1.0 mm external hex and internal taper. Value of internal taper showed significant difference with that of 0.7 mm external hex and internal hex (p<0.05). (3) Percentage of experimental value to tightening torque was increased in order of 0.7 mm external hex, 1.0 mm external hex, internal hex and internal taper. Values of all groups showed statistical significance (p<0.05) except between the groups of 1.0 mm external hex and internal hex. Based on those results, there was no significant difference of loosening-torque by repetitive loading except internal taper. It is supposed that implant system with high resistant capability against initial loosening could be recommended for clinical use. In addition, in case of single implant restoration, 1.0 mm external hex or internal hex could be recommended rather than 0.7 mm external hex, and the use of internal taper would be the most useful way to reduce screw loosening.

  • Statement of problem: One of common problems associated with dental implant is the loosening of abutment screws that retain the implants. Purpose : This study was performed to investigate the influence of abutment screw length and repeated tightening on screw loosening in dental implant. Material and method: Forty nine Hexplants (13mm length, 4.3mm diameter, Ti grade IV, Warantec. Co. Ltd. Seongnam, Korea) and cementation type abutments(straight abutment) and abutment screws (0.4mm/pitch) were divided into 7 groups, depending on abutment screw length. Each implant and abutment was tightened to 30Ncm by torque controller(MGT50, MARK-10 Inc., USA) and the removal torque values were measured during 10 consecutive closure/opening trials. Results and Conclusion: The results of comparing the removal torque value are as follows : 1. There is no significant difference in the removal torque value between groups in 10 consecutive closure/opening trials (p = 0.97). 2. If the fractured abutment screw is engaged in longer than 2.425 thread length, there is no significant difference in the preload between the fractured abutment screw and the new abutment screw when both are equally tightened to 30 Ncm. 3. The removal torque value in the 1st trial(24.510 Ncm) was lower than that in the 2nd, 3rd, 4th, 5th, 6th, 7th trials and the removal torque value in the 2nd trials(25.551 Ncm) was maximum and was decreased in 1311owing trials. The removal torque value in the 1st trial was significantly lower than that in the 2nd, 3rd, 4th trials and was significantly higher than that in the 8th, 9th, l0th trials(p<0.05). 4. In the 2nd, 3rd, 4th, 5th, 6th, 7th trials, the abutment screw was mainly influenced by settling effect and the higher preload was obtained In the 8th, 9th, l0th trials, the abutment screw was mainly influenced by adhesive wear and the progressively lower preload was obtained.

A STUDY ON MECHANICAL PROPERTIES OF TiN, ZrN AND WC COATED FILM ON THE TITANIUM ALLOY SURFACE

  • Statement of problems. In an attempt to reduce screw loosening, dry lubricant coatings such as pure gold or tefron have been applied to the abutment screw. However, under repeated tightening and loosening procedures, low wear resistance and adhesion strength of coating material produced free particles on the surface of abutment screw and increased frictional resistance resulting in screw tightening problems. Purpose. The aim of this study was to compare friction coefficient, adhesion strength, vickers hardness and evaluate coating surface of titanium alloy specimens coated with TiN(titanium nitride), ZrN(zirconium nitride) and WC(tungsten carbide). Material and method. Titanium alloy(Ti-6Al-4V) discs of 12mm in diameter and 1mm in thickness divided into 4 groups. TiN, ZrN and WC was coated for the specimens of 3 groups respectively, and those of 1 group were not coated. Each group was made up of 4 specimens. In this study, sputtering method was used among the PVD(Physical Vapor Deposition) techniques available for TiN, ZrN and WC coatings. Friction coefficient, adhesion strength, vickers hardness and coating surface of 4 groups were measured. Results. 1. For all three coating conditions, friction coefficient was significantly decreased. Especially, ZrN coated surface showed the lowest value. $TiN(0.39{\pm}0.02)$, $ZrN(0.24{\pm}0.01)$, $WC(0.31{\pm}0.03)$. 2. TiN coating showed the highest adhesion strength, however ZrN coating had the lowest value. $TiN(25.3N{\pm}1.6)$, $ZrN(14.8N{\pm}0.6)$, $ WC(18.4N{\pm}0.7)$. 3. Vickers hardness of all three coatings was remarkably increased as compared with that of none coated specimen. TiN coating had the highest Vickers hardness, however WC coating showed the lowest value. $TiN(1865.2{\pm}33.8)$, $ZrN(1814.4{\pm}18.6)$, $WC(1008.5{\pm}35.9)$. 4. The ZrN or WC coated specimen showed a homogeneous and smooth surface, however the rough surface with defects was observed for TiN coating. Conclusions. When TiN, ZrN and WC coating applied to the abutment screw, frictional resistance would be reduced, as a result, the greater preload and prevention of the screw loosening could be expected.

  • 잔류변형이나 초기 미끄러짐(slip)이 발생하지 않는 나사산 밀착방식의 철근 이음장치(커플러)를 개발하였다. 철근 단부에 형성된 사다리꼴 전조나사와 커플러 내부의 암나사는 나사골 끝단이 접하지 않은 상태에서 나사산과 나사골 양옆의 경사면이 강하게 밀착되도록 하였다. 커플러 시공성을 향상시키기 위하여 개폐식 렌치를 개발하여 적용하였고, 체결력의 확인은 육안 검사로 간단히 할 수 있는 메커니즘을 채택하였다.
