Purpose: To evaluate the effect on clinical course of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. Materials and Methods: From June 2001 to May 2004, we found nine cases of heterotopic ossification of distal tibiofibular syndesmosis after ankle fractures. There were 8 male patients and 1 female patient; their mean age was 42 years old. There were 6 Weber type B and 2 Weber type C fractures, and there is 1 case with posterior malleolus fracture only. Among them, 8 ankle fractures were operated. Follow up period was averaged for 14 months. We were able to review radiographs at initial injury and to review clinical menifestation and radiographs at last follow up. We used an ankle-hindfoot scoring system of AOFAS which combined symptom, function and alignment with maximum score of 100 point. Results: In all cases ankle dorsiflexion and plantarflexion were not significantly different from that of the contralateral side. The patients who had developed heterotopic ossification in distal tibiofibular syndesmosis had a similar functional score. The mean ankle-hind foot score was 94 points. Conclusion: We concluded that the heterotopic ossification of distal tibiafibular syndesmosis after ankle fractures had little effect on clinical course and range of motion of ankle joint.
Purpose: To evaluate the results of arthroscopic repair of traumatic anterior shoulder instability with glenoid bone defect. Materials and Methods: Nineteen patients who had underwent arthroscopic repair for the shoulder with traumatic anterior instability and glenoid bone defect were retrospectively reviewed. Mean age was 24.6 years(range, 20 to 39) and mean follow-up was 23 months(range, 19 to 55). No glenoid bone defect was greater than 7mm in length and 20% of the glenoid. The results were evaluated according to stability, range of motion and function. Results: All patients obtained excellent-good results according to Rowe scoring system. Two patients(10.5%) had instability. The mean loss of external rotation was 15 degrees (range, 0 to 25). Functionally, 17 patients could participate in preinjured work or sports to the same level with or without mild discomfort. The remained 2 patients who had 25 degree loss of external rotation could not play sports. Conclusion: Though arthroscopic repair is a good treatment for traumatic anterior shoulder instability with small glenoid bone defect, it is possible to cause loss of external rotation

Correlation between the blood clearance half time and findings of liver scan using the colloidal radiogold in patients of liver cirrhosis is observed through the scoring system, in which the more changes in size, shape and density in the liver scan, the more points are given (table 1). Results: 1) Within the increase in severity of hepatocellular dysfunction in liver cirrhosis, the degree and frequency of following changes in liver scan (done with colloidal radiogold) were increased in order. a) generalized hepatomegaly b) enlargement of the left lobe & reduction of the right lobe c) relatively increased radiodensity in the left lobe and 4) visualization of spleen. 2) Frequency of the normal scan in liver cirrhosis was $12{\pm}3.56%$, frequency of normal value in blood clearance half time of the radiogold was $5.0{\pm}2.34%$ and frequency of normal scan & normal blood clearance rate in liver cirrhosis was $3.6{\pm}2.06%$.

A ribonucleic acid (RNA) is one of the two types of nucleic acids found in living organisms. An RNA molecule represents a long chain of monomers called nucleotides. The sequence of nucleotides of an RNA molecule constitutes its primary structure, and the pattern of pairing between nucleotides determines the secondary structure of an RNA. Non-coding RNA genes produce transcripts that exert their function without ever producing proteins. Predicting the secondary structure of non-coding RNAs is very important for understanding their functions. We focus on Nussinov's algorithm as useful techniques for predicting RNA secondary structures. We introduce a new traceback matrix and scoring table to improve above algorithm. And the improved prediction algorithm provides better levels of performance than the originals.
Objective: We conducted this study to report the clinical manifestations of integrative treatment with Korean and conventional medicines in a patient with acute hepatitis A after conventional medicine therapy for chronic obstructive pulmonary disease and pneumonia. Methods: A patient was treated with herbal medicine, western medicine, and moxibustion. The liver function tests (GOT, GPT, ALP, ${\gamma}-GT$), total bilirubin, and 4-scale scoring (none, mild, moderate, and severe) of symptoms were used to assess any improvement in symptoms. Results and Conclusion: The symptoms at the time of admission were improved. The levels of GOT, GPT, ALP, ${\gamma}-GT$, and total bilirubin were decreased in laboratory tests. We obtained positive effects for the integrative treatment of acute hepatitis A.
The analgesic effects of epidural morphine were evaluated on various types of cancer-related pain in forty-eight adult patients. Epidural morphine injections were given via an epidural catheter introduced to an epidural level corresponding to the pain area. Pain relief was classified as excellent, fair, or poor by subjective scoring and by the subsequent need for systemic analgesics. Thirty-two patients of all the patients became pain-free. In sixteen patients, pain relief was complete only for one or two of various types of pain with a certain dose of epidural morphine, The best result was obtained when the pain was continuous and originated from deep somatic structures. Based on the results, the ranking order of different types of cancer pain with regard to their susceptibility to epidural morphine was as follows: 1) Continuous somatic pain 2) Continuous visceral pain 3) Intermittent somatic pain 4) Intermittent visceral pain The differential effects of epidural morphine on cancer-related pain may suggest that various types of noxious stimuli involve different kinds of opioid receptors which differ in affinity to morphine, and that there are some pain-mediating systems which function independently of opioid mechanisms.
Bariatric surgery has evolved from a surgical measure for treating morbid obesity to an epochal remedy for treating metabolic syndrome as a whole, which is represented by type 2 diabetes mellitus. Numerous clinical trials have advocated bariatric or metabolic surgery over nonsurgical interventions because of markedly superior metabolic outcomes in morbidly obese patients who satisfy traditional criteria for bariatric surgery (body mass index [BMI] >$35kg/m^2$) and in less obese or simply overweight patients. Nevertheless, not all diabetes patients achieve the most desirable outcomes; i.e., diabetes remission after metabolic surgery. Thus, candidates for metabolic surgery should be carefully selected based on comprehensive preoperative assessments of the risk-benefit ratio. Predictors for diabetes remission after metabolic surgery may be classified into two groups based on mechanism of action. The first is indices for preserved pancreatic beta-cell function, including younger age, shorter duration of diabetes, and higher C-peptide level. The second is the potential for an insulin resistance reduction, including higher baseline BMI and visceral fat area. Several prediction models for diabetes remission have been suggested by merging these two to guide the joint decision-making process between clinicians and patients. Three such models, DiaRem, ABCD, and individualized metabolic surgery scores, provide an intuitive scoring system and have been validated in an independent external cohort and can be utilized in routine clinical practice. These prediction models need further validation in various ethnicities to ensure universal applicability.
Objectives: This study investigated the relationship between clock-drawing test (CDT) performance and neuropsychological functions in patients with chronic schizophrenia. Methods: Thirty-one patients with schizophrenia and 30 healthy controls participated in this study. The CDT was administered in three conditions and analyzed using both quantitative and qualitative scoring systems. Comprehensive neuropsychological tests were administered. Results: The results of the quantitative analysis showed that the schizophrenia group performed significantly worse in all three conditions of the CDT compared with the control group. However, no significant differences were observed between the two groups, when the IQ and educational level were controlled. The qualitative analysis showed that the schizophrenia group exhibited significantly more errors in "graphic difficulty" compared with the control group. In addition, CDT quantitative scores were significantly correlated with visuospatial function, memory, attention and executive functions in patients with schizophrenia. Conversely, each qualitative error type was correlated with specific cognitive domains. Furthermore, "graphic difficulty" and "spatial/planning deficit" were identified as predictors of depression symptoms in patients with schizophrenia. Conclusion: The present study demonstrated that the CDT is useful for assessing cognitive dysfunctions in patients with schizophrenia, while qualitative analyses provide more specific information about cognitive deficits compared with quantitative analyses.
Purpose: In this study, 100 stroke patients and 205 physical therapists were surveyed to determine the essential functions needed in the rehabilitation process of stroke patients. Methods: This study involved 100 stroke patients and 205 physical therapists. Sixteen functions suggested in the previous study as necessary in the rehabilitation process of stroke patients were selected, and a revised questionnaire was prepared and distributed to several institutions. A frequency analysis of the collected data was conducted to aggregate the functions required in rehabilitation, and a scoring process was used to determine their ranking among the 16 functions. Results: The functions required in the rehabilitation process, as selected by stroke patients, were ranked as follows: walking, toileting, eating, using products and technology for communication, and washing oneself. The functions required in the rehabilitation process, as selected by physical therapists, were ranked as follows: muscle power functions, maintaining body position, muscle tone functions, attention functions, and walking. Conclusion: The results of the study confirm the importance of an agreed goal between the stroke patient and the therapist regarding the functions required for the rehabilitation. This understanding plays a significant role in achieving the patient's expectations and the therapist's predicted performance, thereby providing reassurance and confidence in the impact of the research.

Recently, there has been a growing emphasis on identifying both known and unknown diseases in plant disease recognition. In this task, a model trained only on images of known classes is required to classify an input image into either one of the known classes or into an unknown class. Consequently, the capability to recognize unknown diseases is critical for model deployment. To enhance this capability, we are considering three factors. Firstly, we propose a new logits-based scoring function for unknown scores. Secondly, initial experiments indicate that a compact feature space is crucial for the effectiveness of logits-based methods, leading us to employ the AM-Softmax loss instead of Cross-entropy loss during training. Thirdly, drawing inspiration from the efficacy of transfer learning, we utilize a large plant-relevant dataset, PlantCLEF2022, for pre-training a model. The experimental results suggest that our method outperforms current algorithms. Specifically, our method achieved a performance of 97.90 CSA, 91.77 AUROC, and 90.63 OSCR with the ResNet50 model and a performance of 98.28 CSA, 92.05 AUROC, and 91.12 OSCR with the ConvNext base model. We believe that our study will contribute to the community.
