Recognition and Attitude to Implement at ion of Service Area Assigned System of Public Health Programs among the Health Officer (공공보건사업의 지역담당제 실시에 관한 보건기관 근무 공무원의 인식과 태도)

  • Since medical clients and the community they live in are expected to be center of future public health and medical care system, new service programs must be developed with patients focused on in line with widening public access of information and social participation. Patients- focused service shall mean the area- oriented provision of public health service. In this study, health officers working at public health centers, public health sub- centers and medical offices in Jeonbuk- do area were taken for population in order to investigate their attitudes toward and knowledge about the service area assigning system under the public health programs. Findings from the survey to 260 health officers, divided by general category, are as follows : Government officers at public health organizations appeared to have high grade of understanding to the service area assigning system and also great appreciation for the necessity of it. Regarding the timing for the system to be introduced, they support the gradual implementation and, as for the type of service to be provided, they preferred home nursing and treatment of chronic diseases. Highly positive responses were centered on the health classes under the health promotion projects, and as far as health projects for the old are concerned, services for home nursing, for the disabled and for home- alone people are favored most. On the other hand, budgeting, manpower and reorganization are rated as prerequisite to establishment of the service area assigning system. From the viewpoint of system side, the improvement of working conditions is rendered as most urgent, while the information system for establishing the service area assigning system is conceived far from satisfactory. Proper assignment of specialists was noted as mostly important to establish the delivery system for medical service through the service area assigning system by team. As merits of the service area assigning system, it is pointed out that, through the system, health clients can better be managed and the nursing quality will be improved thank to the enhanced specialization. It is also perceived that the district health service is not well prepared to respond to the increased and diversified needs of community people and, furthermore, service programs of health centers have not been fully developed. The most serious problem standing in the way to expansion of health projects is, it is noted, uniformity (formality) of the project. Based on the results of the survey which suggest time has ripen to introduce the service area assigning system, following strategies are proposed to anchor down the system as soon as possible: First, we should introduce the system gradually, starting from the area selected, and in consideration of area specialities, refraining from the hitherto stereotyped way of providing health service. Second, we should seek to properly assign the specialists and improve the working conditions of the assigned officers by securing sufficient budget, since it is a most urgent step to lay foundation for the service area assigning system. Third, best service program should be developed to meet the satisfaction of community people by responding to their needs and solidifying the management of medical clients. Fourth, wide scope of study should further be conducted in order to help this system take roots in the central living of community residents since pilot project on the experimental base attended by specialists only can not win popularity among the masses.

Behaviors of hand washing practice Korean adolescents, 2011-2013: The Korea Youth Risk Behavior Web-based Survey (청소년의 손 씻기 실천 행태 분석; 청소년 건강행태 온라인 조사 2011-2013년도를 중심으로)

  • The purpose of this assessment was to provide the basic data for setting up education in terms of 'Hand wash' as one of the health plan & education programs for adolescents. The task analyzed the behavior of students regarding hand washing, which were ranged from middle school to high school. The SPSS 18.0 statistical program, frequency-test and cross-analysis were used for data analysis by 2011, 2012 and 2013, which were the recent 3 years, the Korea Youth Risk Behavior Web-based Survey data. In the data, the response of "Never washed" from students before having a meal accounted for 29.4% in 2011, 30.5% in 2012 and 18.5% in 2013, respectively. Unlike other subjects, these facts suggest that this kind of behavior should be considered significant under the assessment. By tracking the trend over three years, some facts were confirmed in that students living in the metropolitan and medium-sized cities were less likely to wash their hands than students in small- sized towns. In terms of gender, female students were less likely to wash their hands than male students. Regarding the type of school, more students in the public middle & high schools had a tendency to respond "Never hand wash" than the students in the special-purpose high schools. Furthermore, as the grade was increased in middle school and high school, students were less likely to wash their hands before meals in school. Therefore, Health promotion and health education for students should be conducted more carefully with more emphasis on this point.

Quality of Life in Malay and Chinese Women Newly Diagnosed with Breast Cancer in Kelantan, Malaysia

  • Background: Breast cancer is the leading cause of cancer-related death among women in Malaysia. A diagnosis is very stressful for women, affecting all aspects of their being and quality of life. As such, there is little information on quality of life of women with breast cancer across the different ethnic groups in Malaysia. The purpose of this study was to examine the quality of life in Malay and Chinese women newly diagnosed with breast cancer in Kelantan. Materials and Methods: A descriptive study involved 58 Malays and 15 Chinese women newly diagnosed with breast cancer prior to treatment. Quality of life was measured using the Malay version of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) and its breast-specific module (QLQ-BR23). Socio-demographic and clinical data were also collected. All the data were analyzed using SPSS version 20.0. Results: Most of the women were married with at least a secondary education and were in late stages of breast cancer. The Malay women had lower incomes (p=0.046) and more children (p=0.001) when compared to the Chinese women. Generally, both the Malay and Chinese women had good functioning quality-of-life scores [mean score range: 60.3-84.8 (Malays); 65.0-91.1 (Chinese)] and global quality of life [mean score 60.3, SD 22.2 (Malays); mean score 65.0, SD 26.6 (Chinese)]. The Malay women experienced more symptoms such as nausea and vomiting (p=0.002), dyspnoea (p=0.004), constipation (p<0.001) and breast-specific symptoms (p=0.041) when compared to the Chinese. Conclusions: Quality of life was satisfactory in both Malays and Chinese women newly diagnosed with breast cancer in Kelantan. However, Malay women had a lower quality of life due to high general as well as breast-specific symptoms. This study finding underlined the importance of measuring quality of life in the newly diagnosed breast cancer patient, as it will provide a broader picture on how a cancer diagnosis impacts multi-ethnic patients. Once health care professionals understand this, they might then be able to determine how to best support and improve the quality of life of these women during the difficult times of their disease and on-going cancer treatments.

Factors Associated with Sexual Debut among Korean Middle School Students (중학생의 성경험 영향요인)

  • Purpose: The purpose of this study was to investigate factors associated with sexual debut among Korean middle school students. Methods: From the database of the Eighth Korea Youth Risk Behavior Web-based Survey (KYRBS), the researchers selected 37,297 middle school students aged 12-15 years. Data were analyzed using $x^2$ test and multiple logistic regression with the SPSS WIN 18.0 program. Results: The proportion of middle school students who had a sexual debut was 2.1% of boys and 1.5% of girls. In multiple regression analysis, economic status, experience of part time work, smoking, drug use and depression were associated with an increased proportion of sexual intercourse for both boys and girls. Living with parents, drinking alcohol and suicidal ideation were associated with sexual debut for boys only. By contrast, formal sexual education was associated with a reduced risk of sexual debut. Conclusion: The results of this study show the factors associated with sexual debut among Korean middle school students. Gender-related interventions should be taken into consideration in school sexual education programmes, because of differences in related factors according to gender.

A Study on Health Aspects of Daily Life of Elementary School Children in an Urban Area (일부도시국민학교취학아동의 보건생활에 관한 실태조사연구)

  • This study was carried out for the objectives to collect the basic informations on the health behaviors of the elementary school children in an urban area in Korea. Seven hundred students were drawn to fill in the designed questionnaire which carries variety of Questions on health re-lated behaviors in general, eating habits, disease history, mental health, and sex education. Questionnaire were filled in by their parents. Major findings are as follows: ① 55.7% had habits of washing the hands before eating whereas 59.8% trashing their hands after toilet. The others had no idea of washing hands before eating and after toilet. ② 26,5% had habits of brushing the teeth twice a day 54.7% only once in the morning, and 2.6% once only in the evening. Thus, the idea of prevention from decayed teeth seems to be lacking among the school children. ③ Bathing habits were also inquired to get 40.3% of bathing more than once a week, 43.1% once every two weeks, and the rest of 16.6% once every one to three months. ④ 41.7% keep the regular bedding time whereas 58.3% irregular. Physical exercises were con-ducted by 76.6% on the ground while 23.5% did not practice any physical exercises at all. Of those physical exercises, rope skipping occupied 37.5%, and the other 66.9% consisted of 14 different kinds of individual type physical exercises such as gymnastic exercise. The main reasons for not enjoying exercises were different by sex; boys largely complained the inadequacy and lack of gymnastic facilities and girls felt in short of friends who could join the exercises. ⑤ 31.9% of the school children had been taking not much of food while 28.3% had unbalanced diets. Of these unbalanced diets, meat occupied 33.2% to be the priority to have an order of the following items such as vegetables, bread or noodle, and fishes as next to each. For eating habits, 88.5% take simple snack such as bread (38.4%, cookies, fruits, and candies in order. 25.8% of the children were provided such snacks or their parents regularly. Breakfast was sufficiently taken by 45.0% whereas 8.4% had never sufficiently. As to the lunch, 63.6% had sufficiently while 16.8% insufficiently. 70.6% take breakfast with all family members together and 30.4% separately. Correlation of sufficient taking of breakfast and eating together of tile family member's seems to be significant when we compare 72.5% of sufficient takers who enjoy breakfast together with the family members with 55.6% of insufficient takers who enjoy it with the family. This finding allows the investigator to point out the importance of table circumstances for children's eating. ⑥ The most common disease was catching a cold (38.8%), and the second was stomach trouble to be followed by the frequency of car sickness, headache, and skin infection. Doctors are consulted only by 23.9% when they are sick whereas 59.7% resorted to the drug stores. The lower the educational attainment of the parents, the lower the rate of visiting clinics. ⑦ 36.7% of their parents pointed out the problems of personality guidance as the most difficult thing at home 71.3% of their parents worried about and unsatisfied with their children's personality traits. Of these complains of the parents, impatience stood at the top to be tabulated at 24.1%, and 21.1% indicated narrow-mindedness. In line with this primary socialization at home, the most crucial problem seems to be related with the lack or recognition of the parents'own role when we find only 43.1% of the parents understood the importance of their own role for the home education of children; the latter group attributed tile responsibility of personality formation to the children themselves. ⑧ As to the sex educational aspects, 30.9% of children have ever asked about the physiology of reproduction or sexual matters to their parents, of those parents only 17.0% could give the constructive responses to the inquiries of the children. In companies on with these data, 25.6% recognized their own role in sex education for their own children while the large segments of the parents (51.1%) attributed the responsibility of sex education to tile low level of 38.3% who recognized the importance of sex education in the school curriculum and 25.1% of the parents insisted to wait until they get to know naturally about sex. 38.1% of the parents said they had some knowledge on sex from books while 16.9% through mass media. The next groups had common senses of sex from their own parents, school friends and other sources.

An Influencing Health Promoting Behavior of Perceived Health Status and Self-Efficacy according to major of college students (대학생의 전공에 따른 건강상태 지각과 자아효능감이 건강증진행위에 미치는 영향)

  • The purpose of this study was to identify the influencing health promoting behavior of perceived health status and self-efficacy according to major of college students to develop health promoting interventions of college students. The subjects of this study were 592 college students in C city in Korea. Health promoting behavior was significantly higher(p=.014) in health related major students than none health related major students. There were significantly difference in "an time to school(p=.007)", "sleeping hours(p=.004)", "drinking(p=0.028)", "exercise(p<.001)", "exercise frequency(p=.012)" between health related major and none health related major. perceived health status(r=0.381, p<.001) and self efficacy(r=0.620, p<.001) were significantly positive relation to health promoting behavior. The other sub factor in self efficacy were confidence(p<.001), effort(p=.008), will(p=.001) and sub factor in perceived health status were present health(p<.001), health anxiety(p=.002), resistibility and sensitivity(p=.002) in none health related major students($R^2$=0.432, F=38.620). We suggested that the results of this study be considered in developing health promoting programs through appropriate education and various activity of young adults.

Comparative Study on Dietary Life and Nutrient Intakes of Elderly Persons at Nursing Home or Their Home in Chungnam (충남지역 일부 시설노인과 재가노인의 식생활 및 영양섭취상태 비교)

  • The purpose of this study was to compare the dietary life and nutrient intakes among elderly persons residing at nursing homes and their homes in order to provide fundamental data for improved foodservice and dietary management. We recruited 150 subjects at a nursing home stay and 150 home stay elderly persons for a survey. The results were as follows. There was no significant difference in frequency of napping between the two groups. The frequency of going out was higher in the home stay subjects, but regular exercise was higher in nursing home stay subjects. Subjects possessed the following diseases: dementia and digestive disorders for nursing home stay elderly men, blood vessel disorder and diabetes for home stay elderly men, digestive and blood vessel disorders for nursing home stay elderly women, and blood vessel and heart disease for home stay elderly women. In a health-related assessment, smoking, drinking, and teeth condition of nursing home stay subjects were not higher than those of home stay subjects. The ratio of subjects who ate meals everyday was higher in nursing home stay elderly women than in home stay women. While most nursing home subjects ate adequate amount of meals, home stay subjects ate until they were full. The daily energy intakes of men and women were 2,229.9 kcal and 2,302.1 kcal for nursing home subjects and 1,894.0 kcal and 1,885.9 kcal for home subjects, respectively. Nutrient intake was also higher in nursing home than home stay subjects. In summary, the dietary life of elderly persons residing in a nursing home was higher than that of elderly persons residing at their home from the view point of meal frequency, meal intake, and nutrient intakes. Therefore, systematic assessment and management of nutrition for elderly staying at a nursing home or at home alone should be conducted continually.

Convergence Relationships among Body-esteem, Obesity Stress, Self-esteem, Weight Control Behavior in High School Girls (여고생의 신체존중감, 비만스트레스, 자아존중감, 체중조절행위의 융복합적 관계 연구)

  • The purpose of this study was to examine relationships among body-esteem, obesity stress, self-esteem and weight control behavior in high school girls. The descriptive correlational study design was used. The data were collected by Self-reported questionnaires from 135 high school girls in the two high school located B and J city. There was except for none weight control behavior or actual BMI was identified with a low body weight. The collected data were analyzed using t-test, one way ANOVA, Pearson's correlation coefficient in SPSS statistics 20.0 program. As results of examining the correlation, body-esteem(r=-.18, p<.05), obesity stress(r=-.39, p<.001), self-esteem(r=-.32, p<.001) were related to their unhealthy weight control behavior in high school girls. Based on the results of this study, it is necessary to be considered body-esteem, obesity stress, self-esteem variables during development convergence programs to attract the healthy weight control behaviors in high school girls.

  • The fractional anisotropy value of the basal ganglia fibers in the brain gray matter region was analyzed by Tract-Based Spatial Statics(TBSS) method after acquiring the diffusion tensor image to identify the presence or absence of brain white matter damage to smoking in male. As a result of measurement analysis, the fractional anisotropy measurement value was lower in smokers than non-smokers in all areas, and the FA value was statistically significant. smoking significantly affects all the anatomic micro structural changes in the brain gray matter and damages the nerve fiber tract. As a result, it can affects functional abnormalities related to the minute changes of the brain due to smoking.

Effects of integrated obesity management program on obese elementary school children (초등학교 비만아동을 위한 통합비만관리프로그램의 효과)

  • The purpose of this study was to develop a integrated obesity management program and measure the effects of the program on obesity, life style, dietary self-efficacy and exercise self-efficacy among obese elementary school children. The integrated obesity management program was provided for the experimental group of 4~6th grade elementary school students for 8 weeks. The program included obesity related health educations based on behavior modification and self-efficacy once a week, exercise classes three times a week. Method: There were 30 children with obesity in the experimental and 28 in the control groups. The collected data were analyzed by the 𝑥2-test, independent t-test and repeated measure ANOVA. Results: After 8weeks of participation in the program, the experimental group showed significantly lower body mass index (p<.01), percent body fat(p<.05) and obesity index(p<.01), significantly higher life style (p<.05)), dietary self-efficacy(p<.001), exercise self-efficacy(p<.05) than those in the control group. Conclusion: This study provides evidence that integrated obesity management program is effective nursing intervention for managing child obesity.
