Dental Treatment of a Patient with Cerebral Palsy under General Anesthesia (뇌성마비 환자의 전신마취 하 치과치료)

  • Background: Cerebral palsy (CP) is non-progressive disorder of motion and posture. In CP patient, there are difficulties in dental treatment because of uncontrolled movement of limb and head, and conjoined disabilities such as cognitive impairment, sensory loss, seizures, communication and behavioral disturbances. It is reported that CP patients have high incidence in caries and a higher prevalence of periodontal disease. But, despite the need for oro-dental care, these patients often are unlikely to receive adequate treatment without sedation or general anesthesia because of uncontrolled movements of the trunk or head. Methods: We reviewed the 58 cases of 56 patients with CP who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul National University Dental Hospital. Results: The mean age was 19 (2-54) years. The number of male patient was 40 and that of female was 18. They all had severe spastic cerebral palsy and 22 had sever mental retardation, 15 epilepsy, 8 organic brain disorder, 1 blindness, 2 deafness and cleft palate. For anesthesia induction, 14 cases was needed physical restriction who had sever mental retardation and cooperation difficulty, but 44 cases showed good or moderate cooperation. Drugs used for anesthesia induction were thiopental (37 cases), sevoflurane (14 cases), ketamine (3 cases ) and propofol (4 cases). All patients except one were done nasotracheal intubation for airway management and 4 cases were needed difficult airway management and 1 patient already had tracheostomy tube. Mean total anesthetic time was $174{\pm}56$ min and staying time at PACU was $88{\pm}39$ min. There was no death or long term hospitalization because of severe complications. Conclusion: If general anesthesia is needed, pertinent diagnostic tests and workup about anomaly, and appropriate anesthetic planning are essential for safety.

  • Objectives: The purpose of this study is to analyze the relationship between dental caries and the frequency of dental cariesrelated dietary intake among primary, middle, and high school students. It is intended to be used as a basic data for adolescent dietary guidance. Methods: The data were analyzed using the health questionnaire survey and oral examination data from the raw data of the 2019 student health test sample statistics conducted by the Korean Ministry of Education. A total of 59,370 primary, middle, and high school students were selected. Results: For middle and high school students, the more frequent the intake of sodas, the more cases of dental caries were noted. The rate of dental caries was increased in comparison to the group which did not consume dairy products or consumed these once or twice a week. Conclusions: For middle and high school students, dietary guidance should be employed so that consumption of sodas will decrease or consumption will be reduced to less than twice a week.

Views about the degree of recognition of dental hygiene of elementary school teachers - The object of study: elementary school teachers in city of Daegu - (대구지역 일부 초등학교 교사의 구강보건인식도에 대한 견해)

  • In order to establish basic data before deciding on the ways to manage school dental hygiene project, this study polled 165 elementary school teachers. Collected data was used SPSS 12.0 statistics program and got the technical statistics and This research and analysis came to the following conclusion by using Fridmen rank test, Pearson's correlation. 1. To the question about how they acquire the knowledge of dental hygiene, 60.6 percent of elementary school teachers turned out to learn about it through mass media(TV, Internet, magazines). 2. Regarding the purpose to maintain dental hygiene, "the prevention of dental diseases" ranked highest, and next "the prevention of cavity" and "the cure of halitosis" appeared at the similar level, and "to become a good model of children" and "to serve the esthetic purpose" ranked lowest. 3. As to the ways to prevent cavity, regular visits to dental clinic and daily brushing turned out to be more effective preventation method. 4. As for the degree of recognition of fluoride, the correct answer about fluoride that is recovered slight caries is appeared 11.8%, that was lowest correct answer ratio.

Quantitative and qualitative analysis of microorganisms in dental unit water (치과용수 미생물의 정량적 및 정성적 분석)

  • Objectives: The purpose of this study was to investigate the pathogenicity of microorganisms by quantitative and qualitative analysis of microorganisms before and after flushing of dental unit water. Methods: This study was conducted on the supply of high-speed handpieces, ultrasonic scalers, and air-water syringes, which sterilized from 10 dental unit chairs at a dental clinic in South Gyeongsang Province. The number of bacterial communities was calculated by collection before and after flushing (2, 4, and 6 minutes). Results: The mean number of bacteria in the handpiece water before flushing was $27,208CFU/m{\ell}$; 2 minutes after flushing, $2,180CFU/m{\ell}$; 4 minutes after flushing, $900CFU/m{\ell}$; and 6 minutes after flushing, $412CFU/m{\ell}$. Conclusions: To minimize the risk of cross-infection and intra-clinic infection in dental clinics, education and water quality monitoring may be needed.

  • General anesthesia or sedation is an essential for a successful dental treatment for the disabled. The aim of this study is to assess the patients who had dental management under general anesthesia or sedation at Chungnam dental clinic for the disabled, who received dental treatment under general anesthesia, sevoflurane sedation and intravenous sedation from January, 2011 to September 2012. Of the 426 patients studied, 389 patients received dental care under general anesthesia, 20 patients received dental care under sevoflurane sedation and 17 patients received dental care under intravenous sedation. The Rate of general anesthesia was higher than that of sevoflurane sedation, intravenous sedation. Sevoflurane sedation is a useful method for short time treatment such as traumatic pediatric patients. Intravenous sedation is an option for patients who had anxiety and fear such as dentally disabled patients. Sevoflurane or intravenous sedation not only gradually reduces the use of general anesthesia but also useful methods themselves for the dentally disabled.

Prognosis after treatment with multiple dental implants under general anesthesia and sedation in a cerebral palsy patient with mental retardation: A case report

  • Cerebral palsy is a non-progressive disorder resulting from central nervous system damage caused by multiple factors. Almost all cerebral palsy patients have a movement disorder that makes dental treatment difficult. Oral hygiene management is difficult and the risks for periodontitis, dental caries and loss of multiple teeth are high. Placement of dental implants for multiple missing teeth in cerebral palsy patients needs multiple rounds of general anesthesia, and the prognosis is poor despite the expense. Therefore, making the decision to perform multiple dental implant treatments on cerebral palsy patients is difficult. A 33-year-old female patient with cerebral palsy and mental retardation was scheduled for multiple implant treatments. She underwent computed tomography (CT) under sedation and the operation of nine dental implants under general anesthesia. Implant-supported fixed prosthesis treatment was completed. During follow-up, she had the anterior incisors extracted and underwent the surgery of 3 additional dental implants, completing the prosthetic treatment. Although oral parafunctions existed due to cerebral palsy, no implant failure was observed 9 years after the first implant surgery.

A pilot study using machine learning methods about factors influencing prognosis of dental implants

  • PURPOSE. This study tried to find the most significant factors predicting implant prognosis using machine learning methods. MATERIALS AND METHODS. The data used in this study was based on a systematic search of chart files at Seoul National University Bundang Hospital for one year. In this period, oral and maxillofacial surgeons inserted 667 implants in 198 patients after consultation with a prosthodontist. The traditional statistical methods were inappropriate in this study, which analyzed the data of a small sample size to find a factor affecting the prognosis. The machine learning methods were used in this study, since these methods have analyzing power for a small sample size and are able to find a new factor that has been unknown to have an effect on the result. A decision tree model and a support vector machine were used for the analysis. RESULTS. The results identified mesio-distal position of the inserted implant as the most significant factor determining its prognosis. Both of the machine learning methods, the decision tree model and support vector machine, yielded the similar results. CONCLUSION. Dental clinicians should be careful in locating implants in the patient's mouths, especially mesio-distally, to minimize the negative complications against implant survival.

The Analysis of Industrial Concentration in Medical Clinic, Dental Clinic, and Oriental Clinic (의원, 치과의원, 한의원에 대한 산업집중도 분석)

  • This study was conducted to analyze the industrial concentration of the districts, cities and counties among clinics, dental clinics, and Oriental clinics. For this purpose, the sales from 1996 to 2015 in five-year intervals were analyzed by the Heifindal-Hirschmann index (HHI). The results of this study showed that the competition index represented by the HHI tended to intensify as a whole. Especially in the county area, HHI tended to be in comparison with other regions, but decreased with statistically significance. On average, the HHI was higher in the county than in the district and city areas. In the case of the clinic, the HHI of the county area tended to decrease. In the case of the dental clinic, the HHI tended to decrease in the city and county, while in the Oriental clinic the HHI tended to decrease in the district, city and county. Overall, monopolies were present in the county, but competition strengthened over time. In the case of clinics, competition is intensifying in country. area. In the case of dental clinics, competition in the city and country areas has not yet reached saturation. In the case of Oriental clinics, competition in all regions has not yet been saturated, and competition in the distinct market has been most intense.

A study on quality of handicapped inpatients' service satisfaction at special dental clinic and their transformed perceptions toward to the dental treatment services (일부 복지관 장애인치과 내원 환자들의 서비스 만족도 및 진료 후 변화 인식도 연구)

  • Objectives : The purpose of this study is about discovering the basic references to find the ways to vitalize handicapped dental clinics. Methods : The study was analyzed by the satisfactions of those patients who took the advantages of using the dental care at B-welfare center and also their transformed perceptions after the services are influenced. The analysis was the questionnaire consisting of 100 items and survey data. Results : 1. The satisfaction of the dental service for the disabled was respectively high which was the average of 4.49. 2. Their satisfactions of receiving kindness services by volunteers and employees were the highest in the entire research of successful handicapped dental services and it was 4.78 overall. 3. The satisfaction of using handicapped dental clinic has the higher range of female users than male's. In the mean time treatment details of the dental care and the satisfaction towards to the volunteers and faculty at the center show the statistical significance gap. 4. The oral care service after experiencing the dental clinic for handicapped relived their discomforts of using the regular dental clinic which shows their highest satisfaction as it is the point of 4.75. 5. the change perception after dental treatment for handicapped has the higher range of females than men's and solving the problems of mouth reference and discomfort of using regular clinics show the statistical significance gap. 6. In the change perception after having dental treatment for handicapped the thought of the possibility of periodical dental care shows the highest perception when the number of visiting is usually shorter and it shows the statistical significance. Conclusions : According to the satisfaction of those inpatients who use free dental care services that belong to dental clinics for handicapped in a part of Seoul welfare centers human services were appeared as the most important factor due to their advantages of taking services from volunteers and staff members. On the other hand to enhance the medical treatment information and environment which showed the weakest factors each inpatient should be specifically specialized for their needs and also further study on plans which enhance their perceptions toward to a better quality of oral-related life is required after using dental treatment service.

  • Recently, there has been an increasing interest in the regulation of medical & dental profession in South Korea due to various medical scandals & exacerbated commercialism. Consequently, the voice asking for strengthening the license management of medical & dental profession is rising. However, there is an absolutely lacking discussion on self-regulation of the Korean dentist community. This study investigated International Society of Dental Regulators and dental regulatory authorities in the U.K., Ontario in Canada, California in the U.S. and Australia. In addition, this study examined what situations Japan was in, which was similar to Korea in terms of systems. In the U.K., the U.S., Canada and Australia, there are independent dental regulatory authorities, which place emphasis on lay personnel participation. In addition, the organizations prepared very specific and detailed ethics, standards, and punishment guidelines to be followed by professionals. And, various efforts are being made to secure transparency and trust. As a result of this study, self-regulation in Korea seems to require an open approach that embraces civil society, and it is considered that dentist should lead social discussion more positively.
