A Development of Pulse Oximeter module for Measurement of $SpO_2$ (산소포화도 측정을 위한 모듈형 펄스 옥시메터 개발)

  • Pulse oximetry is a well established non-invasive optical technique for monitoring the $SpO_2$ during anaesthesia, recovery and intensive care. Pulse oximeters determine the oxygen saturation level of blood by measuring the light absorption of arterial blood. In the measurement of the hemoglobin oxygen saturation, conventional method has required the technique of filtering of remove the noise, and of complex signal processing algorithm. So much time have been required to signal processing. In this research, we separate AC signal and DC signal in the stage of signal detection. Therefore we simplify the calculation algorithm for $SpO_2$. The implemented system have the high performance such an accuracy and a processing time than the traditional method.

SEDATIVE EFFECT OF INTRANASAL ADMINISTRATION WITH MIDAZOLAM IN SEDATING PEDIATRIC DENTAL PATIENTS (Midazolam의 비강내 투여시 의식진정효과에 관한 연구)

  • The purpose of this study was to assess the sedative effect of intranasal spray with midazolam for management of the uncooperative 20 children aged from 24 months to 92 months who required extensive treatment. The patients were given randomly a dose of 0.2mg/kg of intranasal placebo, intranasal spray with midazolam, and intranasal drop with midazolam. All the children were restrained in a pediwrap and were monitored with pulse oximeter for assessing the pulse rate and peripheral oxygen saturation. According to Fukuta's behavior rating scale, behavior was checked for evaluation of the clinical sedative effect. The obtained results were as follows: 1. Behavior score of intranasal spray with midazolam was lower than intranasal drop with midazolam(P<0.01). 2. Pulse rate was a significant change as a function of dental procedure(P<0.001), however the peripheral oxygen saturation was not influenced significantly by either adiministration route of drug or dental procedure. Clinically, intranasal spray with midazolam were safe and effective sedation in young children undergoing pediatric dental procedures.

Effects of Distraction using Operating Doll on Preschool Children's Pain during an IV Catheter Insertion (작동인형을 이용한 관심전환이 정맥주사 시 학령전기 아동의 통증에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effect on pain of distraction with an operating doll in preschool children(3-6 years old) during an IV catheter insertion. Method: The research design was a nonequivalent control group posttest design, and the participants were 22 children in the experimental group, and 23 children in the control group. The children were hospitalized for enteritis and pharyngitis in a university hospital. The subjective pain was measured using FACES Pain Rating Scale by self report, and the pain behavior responses were scored using Procedure Behavior Check List by observing a film of the entire procedure, and changes in pulse and oxygen saturation as the physiological pain responses were measured by pulse oximeter. Data were analyzed with the SPSS Win 10.0 program. Result: The degree of subjective pain, pain behavior responses, and changes in pulse during IV catheter insertion were significantly lower in the experimental group. But there was no significant difference in $O_2$ saturation between the two groups. Conclusion: Distraction using an operating doll was effective in decreasing pain during inserting the IV catheter in preschool children.

  • A conventional pulse oximeter has high power consumption; thus, its mobility is severely limited. In this paper, we discuss the drawbacks of the existing pulse oximeters and propose a new ultra-low-power pulse oximeter that supports wireless data transmission for remotely monitoring vital signs, such as peripheral capillary oxygen saturation (SpO2) and beats per minute (BPM). We could notably reduce power consumption by using a low-frequency single clock in all well-customized modules. Also, our device is publicly certified, and thus, possibly engaged in clinical trials for commercial use.

INTRAVENOUS SEDATION FOR UNCOOPERATIVE DENTAL CHILD PATIENTS (비협조적인 아동의 치과 치료를 위한 정주 진정요법)

  • The purpose of this study is to evaluate the intravenous (IV) sedation technique with midazolam and ketamine for uncooperative dental child patients. Three child patients between the ages of 4.4 years and 5.6 years who required sedation for dental treatment were given midazolam and ketamine. Initial dosage was combined of $0.03{\sim}0.06mg/kg$ midazolam and 1.0mg/kg ketamine. Additional dosages of ketamine (0.33mg/kg) were given as needed to maintain deep sedation. Pulse rate, respiratory rate, blood pressure, oxygen saturation, adverse reactions, postoperative recovery and behavior were monitored with saturation pulse oximetry, ECG, NIBP and Cardiocap. In conclusion, the combination of midazolam and ketamine is efficacious in deep sedation for painful dental treatment. However, a significant drop in oxygen saturation was observed as a common adverse side effect. Therefore advanced airway management proficiency is required in order to achieve a successful IV sedation.

Comparison of Arterial Oxygen Saturation Measured by Pulse Oximetry at Different Sensor Sites in Neurocritical Patients (신경계 중환자의 측정부위별 맥박 산소포화도의 비교)

  • Purpose : This study aimed to compare peripheral pulse oxygen saturation (SpO2) values, measured at different monitoring sites, and arterial oxygen saturation (SaO2) of neurocritical patients. Methods : The study included 110 patients admitted to the neurosurgical intensive care unit of a university hospital. The patients' SpO2 values were measured in their index fingers, both second toes, both earlobes, and foreheads, using the patient monitoring system. These values were compared with the standard value of SaO2 measured using a blood gas analyzer. Data were analyzed using descriptive values, Pearson's correlation coefficients, Lin's concordance correlation coefficients (CCC), and Bland-Altman plots. Result : Regardless of the measuring site, SpO2 was correlated with the paired measurements of SaO2 (r=.40~.60, p<.001, CCC range=.40~.58). No significant bias in paired measurements of SpO2 and SaO2 was observed at all sites (-0.06~0.19%, p>.05). SpO2 values at the left finger and right earlobe had the narrowest range, with a 95% limits of agreement (LOA) (left finger -3.04~2.93% and right earlobe -3.18~2.79%). SpO2 at the index finger, on the side without an arterial catheter, had a narrower range of 95% LOA than that of the opposing finger (-3.00~2.97% vs. -3.73~3.26%). Conclusion : SpO2 at the finger without an arterial catheter had the highest level of precision. This study suggests using the index finger, on the side without an arterial catheter, for pulse oximetry in neurocritical patients.

Artifacts Improvement by using the Echo Planar Imaging and Pre-Saturation Pulse Band techniques of Reduced Field-Of-View in Breast Magnetic Resonance Imaging Examination (유방 자기공명영상검사에서 감소된 영상영역의 에코평면영상기법과 사전포화기법 사용에 의한 인공물 개선)

  • This study was conducted in reducing the involuntary motion artifacts because of lungs and heart movements as well as the aliasing artifacts generated during the use of the reduced-FOV EPI technique while performing breast MRI. Performed on a total of 38 obesity female subjects who visited the clinic for pre-examination before surgery within the period from August 1 to November 30, 2014. The 3.0T MRI scanner equipped with a breast scanning coil. Qualitative and quantitative analyses were each used for the evaluation of the acquired images while an Paired T-test and Wilcoxon rank test were performed to check the statistical significance. The variation ratio rose by 15.69% with the additional application of a pre-saturation pulse in the lesion, by 13.72% near the lesion, and 20.63% in the fat and the contrast-to-noise ratio rose by 10.58% in and near the lesion and by 12.03% in the lesion and fat, respectively. there were increases of 22.05% and 21.42% at 0 and 1000 respectively in qulitative evaluation and growth of 16.10% in apparent diffusion coefficient. it showed a statistically significant result(p<0.05) in signal to noise ratio, contrast to noise ratio, diffusion slope coefficient and apparent diffusion coefficient. The involuntary movements artifacts that occur in the phase encoding direction and the aliasing artifacts are considered to be reduced to obtain the best image in the additional use of the pre-saturation pulse as DWI is acquired.

  • Traditionally, EPT and thermal tests were used as diagnostic methods for pulp vitality test. The thermal and electrical stimulation tests are the methods to determine the vitality of a tooth based on its neuronal response. These have certain limitations, one of them is the difficulty of approaching the correct result in case of treatment of children. The reason is management problem caused by the unpleasant stimulation. Also, the response from patients are not objective, and false positive or false negative could be happened. Recently, laser doppler flowmetry and pulse oximeter which evaluate vascular integrity are introduced in an effort of overcoming to limitation of traditional methods. The principle of pulse oximeter is to and out level of oxygen saturation by ratio of the two pulses between emitted light and detected light penetrating them to the termination of body, such as ears or fingers. From this point of view, it can be applied to a tooth to determine its vitality. The objective of this study lies mainly on varifying pulse oximeter as a method of determining tooth vitality and providing basic data of its clinical implementation. The result of the research showed that level of oxygen saturation in vital teeth was average of 96.3% and 0.0% in pulpless teeth. As a comprehensive result, pulse oximeter could be an useful diagnostic equipment in determining of tooth vitality.

The Effect of Line Dance Program on Oxygen Saturation and Happiness of Elderly (라인댄스 프로그램이 노인의 산소포화도와 행복정도에 미치는 영향)

  • Purpose: The research was to investigate the effect of line dance programs on oxygen saturation level and happiness of elderlies. Methods: Researchers had conducted a series of test to measure peripheral blood vessel oxygen saturation level, pulse and happiness level of 47 elderlies from the D district of G city. Collected data were analyzed with ${\chi}^2$-test and independent t-test, equipped with SPSS WIN 17.0. Results: As a result of serial tests, Pre-test and Post-test of line dance performance showed that oxygen saturation of the Experimental group had significant increase following the program and happiness of the Experimental group also had significant increase following the program. Conclusion: In conclusion, with many advantages such as its easiness to access and emulate as well as its ubiquitousness as an aerobic exercise, line dance cannot only be very beneficial to cardiovascular function and relation of elderlies, but also to emotional and psychological effect, considered as a great breakthrough to improve comprehensive well-being in both physical and mental aspects.

An Optimization of the 3D $^{1}H-^{15}N-^{1}H$ TOCSY-HSQC and NOESY-HSQC Experiments Using Sensitivity Enhancement with Gradient Selection

  • Proper pulse sequences and experimental optimization for the 3D 15N edited TOCSY and NOESY spectra were described. Using sensitivity enhancement approach with coherent selection by pulsed field gradients described by Kay and co-workers, an considerable gain in sensitivity was achieved. The sensitivity was also improved by minimal water saturation using water flip-back pulse. Among the three types of TOCSY mixing pulse, named MLEV-17, DIPSI-2rc, DIPSI-2rc sequence gave the most sensitive spectrum. These results suggest an appropriate pulse sequence for for those 3D experiments for large proteins.
