A Study on Operation-Effectiveness Analysis of Clinical Nurse Specialists : Focused on Patient Satisfaction and Expected Profit (임상 전문간호사 운영 효과 분석)

  • Purpose: The aim of this study was to evaluate patient satisfaction and indirect marketing effects (recommendation, hospital re-visit intention) and additionally to calculate the expected incomes by clinical nurse specialists (CNS). Methods: Sixty-six patients who had lower varicose vein removal surgery performed at two general hospitals from August 1 to November 30, 2008, were recruited. The effects were evaluated by degree of patient satisfaction, the number of personal recommendations given, rate of hospital re-visit intention, and indirect marketing effects. Indirect marketing effects were calculated as the total cost of surgery multiplied by the score of other variables. Results: The patient satisfaction score was 0.82 for CNS and 0.43 for general nurses (GN). The number of personal recommendations given was 2.5 for CNS and 1.1 for GN. The rate of hospital re-visit intention was 0.31 for CNS and 0.21 fot GN. The ratio of expected incomes between CNS and GN is 2.07 to 1. Conclusion: We found that when a CNS serves a patient, satisfaction, number of personal recommendations, hospital re-visit intention, and indirect marketing effects were higher than those of GN. These results show that CNS have dominant roles and functions and that the effects of CNS employment can be measured using economics parameters.

An Analysis of the Factors Related to the Degree of the Role Conflict and the Likelihood of Exits of the Labor Force of Working Wives (취업주부의 역할갈등과 취업중단의사 관련 변수에 대한 연구)

  • The purposes of this study are to identify the factors associated with the degree of role conflict and the likelihood of exits of the labor force of working wives and to investigate the relationship between the role conflict and job quitting behavior of working wives. The major results of this study are as follows. First, the ratio of reservation wage relative to wife's earnings, wife's earnings, husband's behavioral support, the presence of child under age of 6, the type of occupation, and job satisfaction are associated with the role conflict of working wives. Second, there is a significant difference in the job quitting behavior of working wives according to the degree of role conflict of working wives. Third, the level of role conflict, educational level, the ratio of resonation wage relative to wife's earnings, husband's behavioral support, the presence of child under age of 6, the type of occupation, and job satisfaction are associated with the likelihood of job quitting of working wives.

A Study of the Job Satisfaction of Clinical Nurses Related to Nurse Staffing (간호등급별 병원 간호사 직무만족 조사)

  • Purpose : The objective of this research is to explore the job satisfaction of clinical nurses by the rank of nurse staffing in order to provide effective management for nurses. Method : The research has been conducted on three hundred twenty nurses working at tertiary eight hospitals which were from 2nd rank of nurse staffing to 5th. rank of nurse staffing in Seoul, from August 1 to September 30 of 2003, through survey. For the experimental tools, used Park-Yoon's job satisfaction for nurses(1992) which was modified Stamp's job satisfaction test(1978). The acquired data were analyzed through SPSS program using descriptive method, $x^2$-test, ANCOVA, and LSD. Results : Overall job satisfaction of nurses showed fairly high level of 3.17; in the order of high score, 3.84 for interaction, 3.00 for autonomy, 2.63 for administration. Analysis based of the rank of nurse staffing showed that hospitals of 2nd rank and 3rd. rank of nurse staffing which were higher ratio of patient vs nurse were more satisfied with nurses' job satisfaction than other nurses who were 4th. rank and 5th. rank of nurse staffing. Conclusion : The result of this study revealed that hospital which was higher the rank of nurse staffing was more influenced of nurses' job satisfaction and especially interaction, administration and autonomy which were sub-category of job satisfaction were different among the ranks of nurse staffing.

  • This paper is an exploratory study to improve customer satisfaction survey for resolving practical problems. It is natural phenomenon that, as the level of customer satisfaction index increases, the ratio of satisfied customers increases too. However, the effectiveness of practical application of customer satisfaction survey for improvement of customer satisfaction decreases due to its structural limitation on its data analysis system. In order to cope with these problems, we compares the three satisfied customer reclassification methods such as attribute complex scores, satisfaction/dissatisfaction dimension and latent class analysis models. The case study results show that satisfied customer reclassification methods have merits and demerits and are expected to play the role as the groundwork for the revitalization of customer satisfaction survey as well as improving customer satisfaction management.

A Study on Inhabitant's Mental and Residential Environment Improvement of Old Residential Area in Pohang (포항시 노후 주거지의 거주자 의식과 주거환경정비방향)

  • This study examines the characteristics and the degree of inhabitant's mental against to their residential environment which was developed within the boundary of Haedo Dong in Pohang City during the 1970s and the early of 1980s and currently designated as programed site for Urban Maintenance Project. Findings from the study are as follows; First, the numbers of household occupied by lessee were greater than the numbers of household occupied by homeowner. Second, most of the lessee was living in single family household. Third, although the degree of satisfaction for their residential environment was not so good in general perspectives the degree of satisfaction of the long term residents for their residential environment was slightly higher than the others. Fourth, the degree of satisfaction for their neighborhood was the most while the degree of satisfaction for public facilities such as parks was the least. Fifth, Urban maintenance project shall be proceeded to raise re-settlement ratio of residents, to enforce a business by steps, to lead positively participation of a resident and to enlarge role of administration.

  • Patients' satisfaction while receiving medical service is affected by whether or not their preferences can be met, including time and physician preference. Due to scarcity of medical resource in China, efficient use of available resources is urgently required. To guarantee the utilization ratio, the scheduling decisions are made after all booking information is received. Two integer models with different objectives are formulated separately, maximizing the degree of satisfaction and revenue. The optimal value of the two models can be considered as the bound of corresponding objectives. However, it is improper to implement any of the extreme policies. Because revenue is a key element to keep the hospital running and satisfaction degree is related to the hospital's reputation, neither the revenue nor the satisfaction can be missed. Therefore, hospitals should make a balance. An integrated model is developed to find out the tradeoff between the two objectives. The whole degree of mismatching that is related to patient satisfaction and other separate mismatching degree are considered. Through a computational study, it is concluded that based on the proposed model hospitals can make their decisions according to service requirement.

Influencing Factors on Sleep Satisfaction among Korean Adolescents with Atopic dermatitis (아토피 피부염 청소년의 수면만족감 영향요인)

  • Purpose: This study investigated sleep satisfaction and influencing factors in Korean adolescents with atopic dermatitis. Methods: Data were obtained from 4,570 subjects who completed the 2017 13th Korea Youth Risk Behavior Web-based Survey. For data analysis, descriptive statistics and multiple-sample logistic regression were used. Results: The results of this study were as follows. Gender, academic achievement, economic status, perceived stress, happiness, health status, and depression in the subjects showed a significant difference in terms of sleep satisfaction in atopic adolescents. Multiple-sample logistic regression analysis showed that male adolescents (odds ratio (OR)=0.48), academic achievement (low OR=0.67, middle OR=0.78), economic status (low OR=0.50, middle OR=0.64), lack of depression (OR=0.63), perceived stress (very OR=4.65, a little OR=2.25), perceived happiness (unhappy OR=1.99, moderate OR=1.22), perceived health status (unhealthy OR=2.17, ordinary OR=1.35), alcohol nondrinking (OR=0.75), and nonsmoking (OR=0.73) were not associated with sleep satisfaction. Conclusion: It is necessary to develop interventions that can relieve stress management and depression and develop positive emotions in atopic adolescents. In addition, there is a need to improve the environment to avoid health risks such as alcohol consumption and smoking and to minimize the effects of passive smoking.

Development of Performance Indicators Based on Balanced Score Card for School Food Service Facilities (균형성과표(BSC)개념을 적응한 학교급식 운영성과 측정지표 개발)

  • This study raised the necessity of developing performance indicators for measuring the management efficiency and effectiveness of school food service, and as a means of helping its implementation, a balanced score card (BSC) approach developed by Norton and Kaplan was adopted. This study established BSC in seven phases through literature: Phase 1 Defining a school food service and the scope of working activities, Phase 2 Establishing the vision of a school food service, Phase 3 Setting strategic goals, Phase 4 Identifying critical success factors (CSFs), Phase 5 Developing Key Performance Indicators (KPIs), Phase 6 Extracting cause and effect relationship, and Phase 7 Completing a preliminary BSC. The preliminary BSC was fumed into a survey, which was administered to food service related people working at the Office of Education and School Food Service including 16 offices,209 dietitians, 48 school administrators both from self-operated and contract-managed, and 9 experts in areas related to school food service. They were asked questions about strategies from 4 different perspectives,12 CSFs, 39 KPls, and the cause and effect relationships among them. As a result, among the CSFs based on 4 different perspectives, all factors other than ' zero sum on profit/loss ' from the financial perspective turned out to be valid. In terms of KPIs, manufacturing cost percentages, casualty loss count/reduction rates, school foodervice participation rates, and sales goal achievement rates were found to be valid from the financial perspective, while student satisfaction index, faculty satisfaction index, leftover ratio, nutrition educational performance count, index of evaluating nutrition education, customer claim count/reduction rate, handling customer claim count/reduction rate, and parent satisfaction index were found to be valid from the customers' perspective. Besides, nutritional requirement sufficient ratio, nutritional management score, food poisoning outbreak count, employee safety accident count, sanitary inspection assessment index, meals per labor hour (productivity index), computerization ratio, operational management index, and purchase management assessment index were also found to be valid from the perspective of internal business processes. From the perspective of innovation and learning, employee turnover ratio/rate of absenteeism, annual education and training count, employee satisfaction index, human resource management assessment index, annual menu-related customer feedback, food service information index for employees and parents/schools were also found to be valid. The significance of this study is to present indices for measuring overall performance of school lunch food service operations without putting any limitation on types of school food service management, and to help correctly assess the contribution of the current types of school food service management to schools and students. Comparative Study on the Building Outline Simplification Algorithms for the Conversion of Construction Drawings to GIS data (건설도면의 GIS 데이터 변환을 위한 건물외곽선 단순화기법 비교 연구)

  • Recently needs for the convergence of CAD and GIS data are increasing, and many studies on converting two systems to each other are being carried out. In this study, to revise and update the building data of digital map using CAD data for construction, the outline of building is abstracted from the CAD data and the outline is generalized to the same level of detail with the building data of digital map. Several line simplification algorithms to generalize the outline are adopted and compared, especially at the view of satisfaction to the drawing rule for digital map. Douglas-Peucker algorithm, Lang's algorithm, Reumann-Witkam algorithm, and Opheim algorithm are applied as the line simplification method. To evaluate the results of these algorithms, visual assessment and variation ratio of the number of points, total length of lines, the area of polygon, and satisfaction ratio to the drawing rule of digital map are analyzed. The result of Lang algorithm and Douglas-Peucker algorithm show superior satisfaction ratio. But general satisfaction ratio is 50~60% for all algorithm. Therefore there seems to be a limit to use these algorithms for the simplification method to update the building data in digital map and it is necessary to develop line simplification algorithm which satisfy the drawing rule well.

A Study on the Use Realities and Satisfaction with Transport Services in 119 Emergency Medical Service System and Private Transport Agent in Some Areas (일부지역 119 구급대와 사설이송기관의 이송 서비스 이용 실태와 만족도에 관한 연구)

  • Purpose : Centering on users(patients) who are offered the patient transport service by the emergency medical service system in our country, the use and satisfaction are analyzed with the transport service in 119 Emergency Medical Service System and Private transport agent. Results : 1. As for personnel in ambulance cars, 119 emergency staff showed a higher boarding rate of the first-class emergency medical technicians than private transportation centers : 48.4% and 17.7%, respectively. 2. Private transportation centers showed higher user satisfaction with transportation service than 119 emergency staff, which was not statistically significant. 3. As for the case that needs to receive the transport service in the future, the ratio with the will to use 119 Emergency Medical Service System(70.9%) was indicated to be much higher than the ratio of the private transport agent(29.1%). Suggestions : First, Centers transporting first-aid patients should essentially secure a sufficient number of first-class emergency medical technicians as professional emergency medical staff to reinforce qualitative improvement in emergency medical service. Second, It is necessary to establish a systematic monitoring system and develop educational programs in order to enhance satisfaction with the use of 119 emergency staff. Third, the government or the local government needs to positively support and guide the private transport agent, which is in charge of the public medical service.
