A Measurement of Exposure Dose for Patient Transporter (환자 이송원의 피폭선량 측정)

  • The medical institutions use radiation generating devices and radioactive isotopes to diagnose and treat patients. The patient transporter performs work in an environment that is more likely to be exposed to radiation when compared with the general public, such as inevitably entering the radiation management area for patient transfer, or transferring the isotope-administered patient at a short distance. For this reason, we conducted a study to determine the degree of exposure of the patient transporter. The 12 patient transporters working at Incheon A General Hospital are eligible. From April 1, 2019 to April 30, 2019, the dosimeter was used in the chest for one month and the accumulated dose was measured. The dosimeter used was a Optically Stimulated Luminescence Dosimetry (OSLD) and the dose reading was OSLD Microstar Reading System. As a result of cumulative dose measurement for one month, the average of the deep dose was 0.13 mSv and the surface dose was 0.13 mSv, and the cumulative dose for one month was multiplied by 12 to estimate the cumulative dose expectation As a result, the average of the deep dose and the surface dose were 1.52 mSv and 1.51 mSv, respectively. It is necessary to classify the patient transporter as a frequent visitor in order to measure and manage the exposure dose, increase the knowledge of protection against radiation through education and training, and prevent radiation trouble through medical examination.

Measurement of Spatial Scattered Dose Distribution According to Presence or Absence of Radiation Shielding in the Operating Room (수술실에서 방사선 차폐기 사용 유무에 따른 공간산란선량분포의 측정)

  • This study compared the spatial scattered dose distribution according to whether the recently developed radiation shielding is used or not in order to understand the spatial scattered dose distribution of C-arm. The horizontal side distribution increased by $30^{\circ}$ in the interval of the radius 50 cm on the height of 95 cm based on the head of the patient, and it was measured by increasing $30^{\circ}$ with the interval of 50 cm in the vertical side of each horizontal side. In the same method, the radiation shielding was installed and measured. The result of measurement shows that the horizontal side of 50 cm distance was $0^{\circ}$, $90^{\circ}$ and $180^{\circ}$, was $1.77{\pm}0.12$, $1.90{\pm}0.13$, $2.12{\pm}0.14$, and $2.69{\pm}0.15mSv/h$ in the $270^{\circ}$ direction, and was $1.59{\pm}0.12$, $0.99{\pm}0.09$, $1.47{\pm}0.11$, and $1.37{\pm}0.11mSv/h$ after the use of the radiation shielding. In addition, the vertical distribution in horizontal direction $90^{\circ}$ with 50 cm distance was $30^{\circ}$, $60^{\circ}$, $120^{\circ}$, was $3.85{\pm}0.18$, $9.15{\pm}0.28$, $10.82{\pm}0.31$, and $5.40{\pm}0.22mSv/h$ in $150^{\circ}$, and was $2.03{\pm}0.13$, $4.32{\pm}0.19$, $2.76{\pm}0.16$, and $1.92{\pm}0.13mSv/h\;mR/h$ after the use of the radiation shielding. Both direction showed decrease according to the use of the radiation shielding. Therefore, radiation related workers who work in operating rooms should recognize the spatial scattered dose distribution exactly and need to try to prevent the risk of radiation exposure with proper protective measures.

The Analysis of Radiation Exposure of Hospital Radiation Workers (병원 방사선 작업 종사자의 방사선 피폭 분석 현황)

  • Purpose : This investigation was peformed in order to improve the health care of radiation workers, to predict a risk, to minimize the radiation exposure hazard to them and for them to realize radiation exposure danger when they work in radiation area in hospital. Methods and Materials : The documentations checked regularly for personal radiation exposure in four university hospitals in Pusan city in Korea between January 1, 1993 and December 31, 1997 were analyzed. There were 458 persons in this documented but 111 persons who worked less then one year were excluded and only 347 persons were included in this study. Results : The average of yearly radiation exposure of 347 persons was 1.52$\pm$1.35 mSv. Though it was less than 50mSv, the limitaion of radiation in law but 125 (36%) people received higher radiation exposure than non-radiation workers. Radiation workers under 30 year old have received radiation exposure of mean 1.87$\pm$1.01 mSv/year, mean 1.22$\pm$0.69 mSv between 31 and 40 year old and mean 0.97$\pm$0.43 mSv/year over 41year old (p<0.001). Men received mean 1.67$\pm$1.54 mSv/year were higher than women who received mean 1.13$\pm$0.61 mSv/year (p<0.01). Radiation exposure in the department of nuclear modicine department in spite of low energy sources is higher than other departments that use radiations in hospital (p<0.05). And the workers who received mean 3.59$\pm$1.81 msv/year in parts of management of radiation sources and injection of sources to patient receive high radiation exposure in nuclear medicine department (p<0.01). In department of diagnostic radiology high radiation exposure is in barium enema rooms where workers received mean 3.74$\pm$1.74 mSv/year and other parts where they all use fluoroscopy such as angiography room of mean 1.17$\pm$0.35 mSv/year and upper gastrointestinal room of mean 1.74$\pm$1.34 mSv/year represented higher radiation exposure than average radiation exposure in diagnostic radiology (p<0.01). Doctors and radiation technologists received higher radiation exposure of each mean 1.75$\pm$1.17 mSv/year and mean 1.50$\pm$1.39 mSv/year than other people who work in radiation area in hospital (p<0.05). Especially young doctors and technologists have the high opportunity to receive higher radiation exposure. Conclusions : The training and education of radiation workers for radiation exposure risks are important and it is necessary to rotate worker in short period in high risk area. The hospital management has to concern health of radiation workers more and to put an effort to reduce radiation exposure as low as possible in radiation areas in hospital.

On the Seasonal Transports of Freshwater and Salt in the Tropical Atlantic Ocean (열대 대서양에서의 계절별 담수 및 염분의 수송)

  • The transports of the seasonal freshwater and salt from surface to 500 m depth in the tropical Atlantic Ocean are derived from the equations of the continuity and saltconservation, respectively. The freshwater transport is obtained by southward integration of the divergence of surface freshwater flux, using climatological freshwater(i. e. precipitation, evaporation, and river discharge) data. The annual freshwater transport is northward, ranging from 0 Sv near the equator to 0.3 Sv at $12^{\circ}{\;}N{\;}and{\;}20^{\circ}{\;}S$. The seasonal meridional transport amounts of freshwater range from 1.35 Sv to-0.45 Sv. The strong northward freshwater transports prevail for the intraseasonal period summer to fall. This seasonal cycle is caused by the shifts of the ITCZ as well as the changes in the local freshwater storage. Annual and seasonal salt transports are calculated from objectively analyzed historical (1900-86) salinity observations. The annual salt flux in the ocean zero, showing that the salt flux by horizontal advection balances the flux by horizontal diffusion. The salt flux due to the diffusion is northward, and has a maximum of $5{\;}{\times}{\;}10^6kg/s$ at 15oN. Seasonal transport amounts of salt range from $30{\;}{\times}{\;}10^6kg/s{\;}to{\;}-35{\;}{\times}10^6kg/s$. The direction of the seasonal salt transports is northward except for the intraseasonal period summer to fall.

Reconstruction of Radiation Dose Received by Diagnostic Radiologic Technologists in Korea

  • Objectives: Diagnostic medical radiation workers in Korea have been officially monitored for their occupational radiation doses since 1996. The purpose of this study was to design models for reconstructing unknown individual radiation doses to which diagnostic radiation technologists were exposed before 1996. Methods: Radiation dose reconstruction models were developed by using cross-sectional survey data and the personal badge doses of 8167 radiologic technologists. The models included calendar year and age as predictors, and the participants were grouped into six categories according to their sex and facility type. The annual doses between 1971 and 1995 for those who were employed before 1996 were estimated using these models. Results: The calendar year and age were inversely related to the estimated radiation doses in the models of all six groups. The annual median estimated doses decreased from 9.45 mSv in 1971 to 1.26 mSv in 1995, and the associated dose variation also decreased with time. The estimated median badge doses from 1996 (1.22 mSv) to 2011 (0.30 mSv) were similar to the measured doses (1.68 mSv to 0.21 mSv) for the same years. Similar results were observed for all six groups. Conclusions: The reconstruction models developed in this study may be useful for estimating historical occupational radiation doses received by medical radiologic technologists in Korea.

How Effective Are Radiation Reducing Gloves in C-arm Fluoroscopy-guided Pain Interventions?

  • Background: The physician's hands are close to the X-ray field in C-arm fluoroscopy-guided pain interventions. We prospectively investigated the radiation attenuation of Proguard RR-2 gloves. Methods: In 100 cases, the effective doses (EDs) of two dosimeters without a radiation-reducing glove were collected. EDs from the two dosimeters-one dosimeter wrapped with a glove and the other dosimeter without a glove-were also measured at the side of the table (Group 1, 140 cases) and at a location 20 cm away from the side of the table (Group 2, 120 cases). Mean differences such as age, height, weight, radiation absorbed dose (RAD), exposure time, ED, and ratio of EDs were analyzed. Results: In the EDs of two dosimeters without gloves, there were no significant differences ($39.0{\pm}36.3{\mu}Sv$ vs. $38.8{\pm}36.4{\mu}Sv$) (P = 0.578). The RAD ($192.0{\pm}182.0radcm^2$) in Group 2 was higher than that ($132.3{\pm}103.5radcm^2$) in Group 1 (P = 0.002). The ED ($33.3{\pm}30.9{\mu}Sv$) of the dosimeter without a glove in Group 1 was higher than that ($12.3{\pm}8.8{\mu}Sv$) in Group 2 (P < 0.001). The ED ($24.4{\pm}22.4{\mu}Sv$) of the dosimeter wrapped with a glove in Group 1 was higher than that ($9.2{\pm}6.8{\mu}Sv$) in Group 2 (P < 0.001). No significant differences were noted in the ratio of EDs ($73.5{\pm}6.7%$ vs. $74.2{\pm}9.3%$, P = 0.469) between Group 1 and Group 2. Conclusions: Proguard RR-2 gloves have a radiation attenuation effect of 25.8-26.5%. The radiation attenuation is not significantly different by intensity of scatter radiation or the different RADs of C-arm fluoroscopy.

Detection of Magnetic Nanoparticles and Fe-hemoglobin inside Red Blood Cells by Using a Highly Sensitive Spin Valve Device

  • A highly sensitive, giant magnetoresistance-spin valve (GMR-SV) biosensing device with high linearity and very low hysteresis was fabricated by photolithography. The detection of magnetic nanoparticles and Fe-hemoglobin inside red blood cells using the GMR-SV biosensing device was investigated. When a sensing current of 1 mA was applied to the current electrode in the patterned active devices with an area of $2{\times}6{\mu}m^2$, the output signals were about 13.35 mV. The signal from even one drop of human blood and nanoparticles in distilled water was sufficient for their detection and analysis.

A Study of the Patient Dose in Chest Radiography (흉부(胸部) X선검사시(線檢査時) 환자(患者)의 피폭선양(被曝線量)에 대(對)한 연구(硏究))

  • A study was carried out to investigate the technical factors and the patient dose (entrance and absorbed dose) in chest P-A radiography based on the 86 hospitals in Seoul from July 1 to July 30, 1989. As a result of this study, main finding were as follow : 1. 51.2% of the surveyed hospitals made use of $60{\sim}69\;kVp$ as tube voltage in chest radiography 2. The majority of the surveyed(88.3%) have the use of $6{\sim}20\;mAs$ as tube current-time. 3. Percentage absorbed doses in patient were showed more than 90 percent in every tube voltage. 4. Object densities were all much the same in all tube voltages. 5. 48.8% of surveyed entrance doses ranged from $100\;{\mu}Sv$ to $190\;{mu}Sv$, and the mean dose was $158\;{\mu}Sv$.

  • Newborn Armenian and Chinese hamsters inoculated with adenovirus type 12 developed undifferentiated small cell tumors as early as 27 days after inoculation in the Chinese hamsters and within 30$\\sim$45 days in the Armenian hamsters. These tumors were transplantable and epithelial-like cell in morphology. Cultures of 6 tumors underwent spontaneous reversion to fibroblast-like morphology during the 14$\\sim$20 in vitro passages in the absence of chromosomal disturbances. While epithelial-like tumor derivatives were oncogenic and positive for the T-antigen, fibroblast-like revertants were non-oncogenic and negative for the T-antigen. Two other tumor derivatives reverted to fibroblast-like forms, immediately following exposure to SV40. These lacked the adenovirus T-antigen but were positive for the SV40 T-antigen and formed sarcomas in animals.

Assessment of Absorbed Dose of by Organ according to Thyroidal Uptake of Radioactive Iodine for Adult Korean Males (한국성인 남성을 대상으로 한 방사성옥소의 갑상선 섭취율에 따른 각 장기별 흡수선량 평가)

  • In an effort to assess the internal absorbed dose of radionuclides that is suitable to Koreans' physiological characteristics, we asked 28 male Koreans to take $^{131}|$ orally, determined the thyroidal uptake and daily urination ratio, and assessed the absorbed dose by organ. As a result, first, 24 hours after administering, the average thyroidal uptake and the daily urination ratio registered 19.70% and 71.12%, respectively. Second, the whole body effective dose according to the thyroidal uptake calculated herein and the existing ICRP-suggested thyroidal uptake of 30% offered 1.464E-08 Sv and 2.189E-08 Sv, respectively, showing a 1.5 times difference. To evaluate the quantity of the absorbed dose of radioactive iodine, we can better reduce the error in assessing the body exposure dose by conducting measurement according to human races rather than depending on the existing ICRP data.
