Comparison of Case Management between Tele Care Regions and General Care Regions in Korean Medicaid (의료급여 수급자의 건강관리 및 의료이용에 대한 텔레케어 사례관리의 효과)

  • Purpose: The purpose of the study was to compare recipients' health behavior, attitude to using medicaid, medication compliance, and the changes in hospital cost and visit-day of in-patient and out-patient care between tele-care regions (TCR) and general care regions (GCR) in Korean medicaid. Method: The design of the study was ex-post facto comparing recipients in TCR and GCR. The sample included 625 persons in TCR and 410 persons in GCR. To collect materials, the case manager interviewed recipients of medicaid and filled out questionnaires which were analyzed through SAS/PC 9.1. Results: In studying health behavior and medication, compliance was not significant. However, the attitude to using medicaid was significantly more positive in TCR than in GCR. In out-patients, the change of hospital visit-day was not significant between TCR and GCR, but TCR showed a reduction in hospital cost compared to GCR. For in-patient recipients, GCR showed a greater reduction in changes in hospital cost and visit-day compared to TCR. Conclusions: The results of the study show that attitudes to using medicaid via telephone are positive and results are more effective than hospital visit consultation, and the cost of out-patient care could be reduced.

An Analysis of Nurse Staffing Level and Nursing Performance in the General Unit (일반병동 간호인력 확보수준과 간호업무성과 분석)

  • Purpose: To examine the relationship between nurse staffing level and nurse perception of nursing performance. Method: The subjects of this study were 724 nurses from 70 medical or surgical wards in 23 hospitals from February to March 2006. A self-reporting questionnaire, which was developed by the researcher through a preceding study, was used. In data analysis, SAS program was utilized for descriptive statistics, t-test, ANOVA, and Tukey test. Results: The mean score of the nursing performance was 53.3. The mean of bed-to nurse ratio was 3.39:1. The study revealed a negative correlation between bed-to nurse ratio and the nurses' perception of nursing performance. There was a significant relationship between above 3.5:1 and below 2.5:1(F=4.59, p=0.010). Conclusion: We found that the higher the nurse staffing, the better the nursing performance. It is recommended that future research measures nurses' performance and nursing organizational performance using objective measurement tools including clinical indicators.

The Relationship of Stress, Social Support and Depression in the Elderly (노인의 스트레스, 사회적 지지와 우울간의 관계)

  • Purpose: This study was to identify the relationship between stress, social support and depression in the elderly. Method: The subjects were 283 elderly adults over 60 in Seoul. Data was collected by questionnaire surveys using convenience sampling. The instruments used in this study are the ELSI developed by Aldwin(1990), the Interpersonal Support Evaluation List developed by Cohen & Hoberman (1983), and the Geriatric Depression Scale by Yesavage & Brink(l982). Data was analyzed by the SAS program, using descriptive statistics, Pearson Correlation Coefficient, t-test, ANOVA and progressed Multiple Regression. Result: The relationship between stress and depression had a positive correlation (r=0.33), but the relationship between stress and social support had no significant correlations. The relationship between social support and depression had a negative correlation (r=-0.38). The most powerful predictor of depression was the economic status and then a combination of stress, and social support account for 39% of the variance in depression in the elderly. Conclusion: These results suggested that stress and social support deficits can be potential risk factors in old age depression. Therefore, these findings give useful information for constructing an intervention program focused on depression in the elderly.

  • A piperazinylalkylisoxazole library containing 86 compounds was constructed and evaluated for the binding affinities to dopamine (D3) and serotonin (5-HT2A/2C) receptor to develop antipsychotics. Dopamine antagonists (DA) showing selectivity for D3 receptor over the D2 receptor, serotonin antagonists (SA), and serotonin-dopamine dual antagonists (SDA) were identified based on their binding affinity and selectivity. The analogues were divided into three groups of 7 DAs (D3), 33 SAs (5-HT2A/2C), and 46 SDAs (D3 and 5-HT2A/2C). A classification model was generated for identifying structural characteristics of those antagonists with different affinity profiles. On the basis of the results from our previous study, we conducted the generation of the decision trees by the recursive-partitioning (RP) method using Cerius2 2D descriptors, and identified and interpreted the descriptors that discriminate in-house antipsychotic compounds.

Construction of Health-related Quality of Life Model in Acquired People with Physical Disabilities (후천성 지체장애인의 건강관련 삶의 질 모형구축)

  • Purpose: This study was designed to construct a structural model for explaining model health-related quality of life in acquired people with physical disabilities. Method: The hypothetical model of this study was consisted of 6 latent variables and 14 observed variables. Exogenous variables included in this model were physical status and economical level. Endogenous variables were social attitudes, family function, self-esteem, and health-related quality of life. Data were collected from 226 acquired people with physical disabilities residing in Seoul and Kyunggi-do from January to February, 2005. The collected data were analyzed using SAS 8.2 version and LISREL 8.32 version program. Results: The results of the fitness test of the modified model were follow as; ${\chi}^2=67.479$ (df=50, p=.05), GFI=.959, AGFI=.914, SRMR=.049, NFI=.961, NNFI=.979, CN=249.244. Health-related quality of life was influenced directly by physical status, economic level, and social attitudes and accounted for 88.8% of the variance by these factors. Conclusion: These results suggest that physical status is the most significant effect on health-related quality of life, and social attitudes and economic level are important factors having influences on health- related quality of life. Therefore improving physical status and economic level, and modifying negative attitudes are necessary to increase health-related quality of life of acquired people with acquired physical disabilities.

Ego-Resilience, Stress Coping Styles and Disposition to Delinquency in Middle School Students (중학생의 자아탄력성과 스트레스 대처방식 및 비행성향)

  • Purpose: The purpose of this study was to investigate the relationships of ego-resilience, stress coping styles and disposition to delinquency in middle school students. Method: The data were collected from October 15 to October 30, 2005. The subjects were 223 middle school students in 2nd grade in G city. To determine the most effective variable of ego-resilience to stress coping styles and disposition to delinquency, the dada were analyzed using Canonical correlation with SAS 9.1 TS, and Pearson's correlation with percentages, means, standard deviations with SPSS 14.0 K. Result: Ego-resilience showed a significant positive correlation with active coping styles and a significant negative correlation with passive coping styles, and disposition to delinquency. Canonical correlation analysis of 5 sub-domains of ego-resilience showed that curiosity made the highest contribution to predicting stress coping styles and vitality made the highest contribution in predicting disposition to delinquency in middle school students. Conclusions: The results of this study suggest that the development of ego-resilience for adolescents is important to improve stress coping strategies and decrease delinquency. Therefore, to increase ego-resilience, Stress Coping Programs should include curiosity and Delinquency Intervention Programs should include vitality.

A Comparative Study on Health-Promoting Behavior, Life Satisfaction and Self-esteem of the Young and Old Old (노년전기와 후기 노인의 건강증진행위, 생활만족도 및 자아존중감의 차이)

  • Purpose: To compare the differences of health-promoting behavior. life satisfaction and self-esteem between the young old and the old old. Method: The subjects were a volunteer sample of 200 elderly in K city. The instruments for this study were Health Promoting Lifestyle Profile (47 items). Life Satisfaction Scale(20 items) and Self-Esteem Scale(10 items). Frequency, percentage, t-test and Pearson's correlation coefficient with SAS program were used to analyze the data. Result: 1) There was a statistical significance(t=2.479. p<05) in health-promoting behavior between the young old and the old old which showed, on an average. 3.306 points in the young old and 2.872 points in the old old. 2) There was a statistical significance ( t = 1. 530. p<05) in self-esteem between the young old and the old old which showed. on an average. 3.091 points in the young old and 2.981 points in the old old. Conclusion: The old old is less the level of health-promoting behavior and self-esteem than the young old. It is necessary to develop comprehensive health-promoting program in order to improve a healthy lifestyle for the old old.

The Relations among Health-promoting Behaviors, Self-esteem and Depression in the Vulnerable Elderly of Urban Areas (도시 취약계층 노인의 건강증진행위, 자아존중감과 우울간의 관계)

  • Purpose: This study was to identify the relations among health-promoting behaviors. self-esteem and depression in the vulnerable elderly in an urban community. Method: The subjects were 185 vulnerable elderly adults aged over 65 in D city, who were selected through convenience sampling. Data were collected by a questionnaire survey. The tools used in this study are the HPLP developed by Walker(1987), the Self-esteem Scale developed by Rosenberg (1965), and the Geriatric Depression Scale developed by Yesavage & Brink(1983). Collected data were analyzed with the SAS program using descriptive statistics. Pearson's correlation coefficient, t-test, ANOVA, Duncan's multiple-range test and stepwise multiple regression. Result: Self-esteem and depression was in a negative correlation with each other (r=-0.21), but no significant correlation was observed between HPB and depression. HPB and self-esteem were in a positive correlation with each other (r=0.38). The most powerful predictor of depression was self-esteem and, next, leisure activity and the type of family living together. The three factors accounted for 41.1% of the variance in depression in the vulnerable elderly. Conclusion: These results suggest that self-esteem, leisure activity, and the type of family living together can be potential risk factors for old age depression. These findings may give useful information for developing visiting nursing service programs focused on depression in the vulnerable elderly.

The Influence of Job Characteristics and Professionalism on Empowerment Perceived by Nurses (임상 간호사의 직무특성과 전문직업성이 임파워먼트에 미치는 영향)

  • Purpose: This study aimed to understand the empowerment in clinical nurses and to analyze the influences of job characteristics and professionalism on empowerment. Method: Subjects were 386 nurses in 8 general hospitals at G city and Seoul. Data were collected with self-administered questionnaires and analyzed using SAS 8.1 by applying ttest, ANOVA, Pearson correlation coefficient, and Stepwise multiple regression. Results: Empowerment was $3.58{\pm}0.63$ and among the subscale scores meaningfulness $(3.83{\pm}0.74)$ was the highest and Impact $(3.23{\pm}0.86)$ was the lowest. The level of empowerment was different significantly by age, marital state, education, clinical careers, position, job satisfaction(p<.00). Job characteristic score was $3.60{\pm}0.45$ and Professionalism score was $3.65{\pm}0.52$. Empowerment was positively correlated with job characteristics and professionalism(r= .63, .81, p<.01). Major factors that affect to empowerment in clinical nurses was professionalism, which had an explanation of 66.9%. Conclusions: The more clinical nurses took the professionalism, the higher nurses perceived empowerment. Therefore, empowerment and professionalism in nurses should be generally screened and other factors should be looked into. And if the specified nursing intervention program which increase their empowerment would be developed, it may contribute to qualify the nursing services.

Validity Testing Study for Related Factors and Characteristics of Nursing Diagnosis (간호진단별 관련요인 및 특성에 관한 타당도 연구)

  • This study was conducted to test validity of related factors and characteristics of 98 Nursing Diagnosis identified in a previous study by the Korean Nurses Association. Data for this study was collected from 892 nurses in eight teaching hospitals located in Seoul using a cross sectional survey method. Each participating hospital was asked to produce at least 10 cases for every nursing diagnosis. There were 7,422 responses out of a possible 7,840. Out of the 7,422 responses 26 were discarded due to incompleteness. Data were analyzed using SAS. The result of the study shows that most of the related factors and characteristics for each of the 98 nursing diagnosis were ranked at more than 3.5 point out of 5 point Likert scale in terms of significance. Through this study the related factors and characteristics of the 98 nursing diagnosis identificance. Through this study the related factors and characteristics of the 98 nursing diagnosis identified through literature review were validated by experts in nursing diagnosis. These validated related factors and characteristics will be utilized for computerization of the nursing diagnosis process.
