건강증진학교에서 초등학교와 중학교 학생들의 정신건강 실태 및 관련된 가정-사회적 요인들 (Mental Health of Elementary and Middle School Students, and Related Familial-Social Factors in Health Promoting School)

  • Purpose: This study was a part of preliminary survey for establishing Korean HPS(Health Promoting School) model which was originally suggested by WHO. Methods: Three elementary and middle schools were sampled in urban and rural area across three level of social-economic status, and survey was conducted on one class in each grade by random selection. To measure mental health and social and familial factors, self-esteem scale, self-efficacy scale, depression scale, family cohesion scale, parent-child communication scale, adolescent's mental health and problem behavior questionnaire(AMPQ), and other question items about family status and economic level were conducted. Students' mental health level was compared by region, sex, grade, and other familial and economic factors. Results: Familial and social factors such as economical and educational level of parents, number of family members were different between rural and urban area. Also, students of rural area got lower scores at self-esteem, self-efficacy, but higher scores at extroverted problem behavior than their urban counterparts. In pre-school students, high grade students' self-esteem, and family cohesion scores were lower than low grade students. And sexual differences were found across problem behavior domain and region in middle school students. Especially, the students of single-parent family and lower economic family got insistently low scores at various mental health related scales. Conclusion: These results should be considered, when the HPS model is designed and applied in Korea.

건강증진모델학교 분석: 읍면 지역 소규모 초등학교를 중심으로 (Analysis of Health Promoting Schools: Focusing on Small-sized Elementary School in Rural Area)

  • The purpose of the study was to present a model of WHO Health Promoting School easily applicable to small-sized schools in rural areas. Methods: The study analyzed data of 11 small-sized schools in rural areas selected from 85 health promoting schools, an initiative led by the Ministry of Education from 2012 to 2014. Results: Through the analysis, the study found out the operation process of health promoting schools consists of five stages: system development, needs survey & survey on current status, school health policy development, program development & execution, and evaluation. In addition, the study was able to discover three key factors in operating health promoting schools: connection with curriculum, connection with community, and consensus among members. While it turned out the schools were following operation manuals faithfully, the biggest problem with operation was that the stages of needs survey, priority setting, and core task development were not closely coordinated. Conclusion: Although the model suggested in the study fails to reflect the characteristics of small-sized schools in rural areas, it is close to a universal model which could serve as a guideline when regular schools adopt the system of health promoting schools.

농촌건강장수마을 사업의 성과와 과제 (A Study on the Achievements and Issues of the Senior Well-being Villages Project)

  • In order to improve the quality of life and support the successful living of the elderly people in rural community, the Senior Well-being Villages Project has been carried out by Rural Development Administration since 2005 in Korea. This timely project, however, produces good results with 582 villages so far, we must endeavor to upgrade it more effectively. This study is to find complementary way of improving the project, and the results of the study are summarized as follows. First of all, the sustainable and practical strategies of the health care for the rural elderly must be adopted. The elderly people's mutual care system can be a good option for improving the existing project. Secondly, the cooperative heath care system must combine with this project. In this regards, health cooperative federation system can be also a challenging option. The beneficially activity of rural elderly people, which is a component of this project, must be linked with rural social enterprise or community venture. The local experts or capable men of various fields including family, the aged problem, social welfare, and regional development, must be appointed, and the support system enables them to do their jobs actively in the Senior Well-being Villages Project as well as other community works.

A sampling and estimation method for monitoring poultry red mite (Dermanyssus gallinae) infestation on caged-layer poultry farms

  • Background: The poultry red mite, Dermanyssus gallinae, is a serious problem in the laying hen industry worldwide. Currently, the foremost control method for D. gallinae is the implementation of integrated pest management, the effective application of which necessitates a precise monitoring method. Objectives: The aim of the study was to propose an accurate monitoring method with a reliable protocol for caged-layer poultry farms, and to suggest an objective classification for assessing D. gallinae infestation on caged-layer poultry farms according to the number of mites collected using the developed monitoring method. Methods: We compared the numbers of mites collected from corrugated cardboard traps, regarding with length of sampling periods, sampling sites on cage, and sampling positions in farm buildings. The study also compared the mean numbers of mites collected by the developed method with the infestation levels using by the conventional monitoring methods in 37 caged-layer farm buildings. Results: The statistical validation provided the suitable monitoring method that the traps were installed for 2 days on feed boxes at 27 sampling points which included three vertical levels across nine equally divided zones of farms. Using this monitoring method, the D. gallinae infestation level can be assessed objectively on caged-layer poultry farms. Moreover, the method is more sensitive than the conventional method in detecting very small populations of mites. Conclusions: This method can be used to identify the initial stages of D. gallinae infestation in the caged-layer poultry farms, and therefore, will contribute to establishment of effective control strategies for this mite.

노인 건강증진프로그램이 농촌 노인의 자기효능감, 건강문제, 농부증 및 삶의 질에 미치는 효과 (The Effect of Health Promotion Program on Self Efficacy, Health Problems, Farmers Syndrome and Quality of Life of the Rural Elderly)

  • Purpose: The purpose of this study was to identity the effects of a health promotion program for rural elderly people on self efficacy, health problems, farmers syndrome and quality of life. Method: The study was a nonequivalent control group pre-post experimental design. The data collection was performed from April 12th, 2003 to August 2nd, 2003. The subjects were selected at Mari Myun Geochang Gun in Korea. Fortyfour elders were included in the experimental group and 45 elders were included in the control group. The 16-week health promotion program was given to the experimental group. Data were analyzed by descriptive statistics, $X^2-test$, t-test, paired t-test with SPSS/Win 10.0 program. Result: The experimental group showed a lower scores of health problems and farmers syndrome than the control group, and higher scores of quality of life than the control group. There were no significant differences in self efficacy. Conclusion: From the above results, it can be concluded that the health promotion program for rural elderly is an effective intervention for improving the rural elderly's quality of life and reducing health problems and farmer's syndrome. Therefore, development of strategy that can spread the health promotion program for rural elderly to the elderly of other rural areas are needed.

간호대학생의 당뇨병 방문건강관리 관련 지식, 문제해결과정, 자기효능감이 임상수행능력에 미치는 영향 (Knowledge, Problem Solving Process, and Self-Efficacy on Clinical Competency Related to Home Health Nursing Management for Diabetes Mellitus Management by Nursing Students)

  • Purpose: This study aimed to explore the influence of knowledge, problem-solving processes, and self-efficacy on the clinical competency of nursing students in the home health nursing management of diabetes mellitus (DM). Methods: The subjects of this study were 136 nursing students. Data were collected from April 18 to April 29, 2022, and analyzed using the SPSS 23.0 program. Results: The total mean scores of nursing students' knowledge, problem-solving process, self-efficacy, and clinical competency in DM home health nursing management were 71.24, 3.92, 7.47, and 4.09, respectively. Clinical competency was significantly and positively correlated with the problem-solving process (r=.60, p<.001) and self-efficacy (r=.48, p<.001) but not with knowledge (r=.09, p=.311). The problem-solving process was also positively correlated with self-efficacy (r=.41, p<.001). Regression analysis revealed a 41.4% variance in the nursing student's clinical competency with problem-solving process (β=.47, p<.001) and self-efficacy (β=.28, p<.001). Conclusion: The results of this study provide valuable evidence for the development of educational interventions aimed at enhancing the clinical competency of nursing students in relation to home-visit healthcare services for DM management.

농촌 치유관광의 당면과제 (An Urgent Problem and Challenge of Rural Healing Tourism)

  • Recently rural healing tourism has been attracting tourists' interest in relieving mental stress and restoring physical daily life with nature-friendly experience. The purpose of this study is to explore the urgent problems and challenges of rural healing tourism by conducting focus group interview(FGI) with experts. Fourteen experts were interviewed with a semi-structured questionnaire. From the interview data, six themes were identified for direction of healing tourism development. The results of the study are as follows; The area of rural healing tourism can be explained within the framework of health tourism. As for the difference between rural tourism and healing tourism, rural tourism is expected to enhance physical and mental relaxation, daily restoration, and health promotion if rural tourism expects the understanding and experience of agricultural and rural life, and educational effect. The core components of rural healing tourism were operators' expertise, program with rurality, ecological environment, and storytelling. The findings of this paper implicate the underlying dimension of rural healing tourism from a experts' perspective.

의사인력의 지역간 분포양상 및 공중보건의사의 영향 (Geographical Distribution of Physician Manpower under the Influence of Public Health Physician)

  • The purpose of this research is to assess the geographical distribution of physicians and dentists and the degree of maldistribution of the physician. Data were obtained form the Korean Medical Association's report on physicians registry and census for 1990. To assess the degree of disparity in the rural-urban distribution of physician manpower and to identify changes in the distribution pattern, the Gini index of concentration was used. Major findings are as follows; 1. Urban-rural disparity in the distribution of physician manpower exists in all categories of manpower, i.e. physician, dentist, oriental medical doctor, general practitioner, medical specialist, practitioner, public health physician and public health dentist. Urban area which had 74.4% of nation's population, accounted for over 90% of all physician manpower. 2. In terms of the ration of physician manpower per 10, 000 population, in urban area, they were 8.2 physicians, 2.7 general practitioners, 5.5 specialists, 3.0 practitioners, 1.8 dentists and 1.3 oriental medical doctors. In rural area, the ratios were 1.4 physicians, 0.6 general practitioners, 0.9 specialists, 1.0 practitioners, 0.4 dentists and 0.4 oriental medical doctors. 3. Gini indicies computed to measure inequality of physician manpower distribution were 0. 3675 for physicians, 0.3372 for general practitioners, 0.3338 for specialists, 0.2263 for practitioners, 0.3132 for dentists and 0.3293 for oriental medical doctors. 4. Inspite of increase in the number of physician manpower, urban concentration of physician manpower intensified from 1980 to 1990. However, the Gini index for all physician manpower fell by 18.3~36.7% from 1980 to 1990, indicating more even distribution. 5. In rural area, the public health physicians and dentists had increased the ratios of physicians, general practitioners, practitioners and dentists per 10, 000 population remarkebly, and had decreased the Gini indicies of physicians, general practitioners, practitioners and dentists. Thus, public health physicians and dentists contributed to improve the distribution of physician manpower in rural area. Based on the results of this study, long-term and rational manpower policies should be developed to solve the problem of geographical maldistribution of physician manpower as well as short-term policy for inducing physicians to the rural areas.

  • This paper deals with analysis on complex and dynamic Primary Health Care (PHC) Systems in rural community to increase understanding of the nature of PHC feedback systems. Because Industrial Dynamics can be very useful for the analysis of such complex and dynamic systems. We used that as a basic tool of Modelling and simulation running. Even If PHC system-models require many assumptions, simulations based on these models can lead decision makers to a better way of problem solving.

농촌주민의 건강증진행위, 건강문제, 지각된 건강상태 및 농부증 (Health Promotion Behavior, Health Problems, Perceived Health Status and Farmers' Syndrome of Rural Residents)

  • 이 연구는 농촌주민들의 건강증진행위, 건강문제, 지각된 건강상태, 농부증에 대해 알아봄으로써 농촌주민을 위한 맞춤형 건강증진프로그램 개발에 기초자료를 제공하기 위해 시도되었다. 연구대상자는 전국 보건진료소 1,875개소 중 지역별 임의표출한 19개 보건진료소의 농촌지역에 거주자 637명을 대상으로 이루어졌으며, 2006년 2월 6일부터 2월 27일까지 자료수집을 실시하였다. 자료 분석 방법은 SPSS 12.0을 이용하여 서술적 통계, 평균과 표준편차, Pearson Correlation Coefficient로 분석하였다. 이 연구결과는 다음과 같다. 농촌주민의 건강증진행위 평균평점은 2.23점으로 나타났으며, 하위영역별로 영양이 2.42점, 영적성장 2.33점, 대인관계 2.30점, 스트레스관리 2.19점, 건강책임이 2.17점, 신체활동이 2.07점으로 가장 적게 수행하는 것으로 나타났다. 농촌주민의 신체계통별 건강문제에서는 근골격계에 대한 건강문제가 가장 많은 것으로 나타났으며, 그 다음이 피로, 눈 귀, 심장혈관계, 소화기계, 신경계, 비뇨생식기계, 호흡기계, 피부의 순으로 나타났고, 농부증 양성율이 98.4%로 나타났으며, 여성이 남성보다 농부증 점수가 더 높은 것으로 나타났다. 농촌주민의 지각된 건강상태는 보통, 1년 전과 비교 시 마찬가지이다, 하고 싶은 것이 있으나 건강상태에 약간의 문제가 있다, 동년배와 비교했을 때 건강상태가 '비슷하다'에 응답율이 가장 높았고 지각된 건강상태의 평균점수는 8.54점으로 나타났다. 농촌주민의 건강증진행위, 건강문제, 지각된 건강상태와 농부증 간의 상관관계에서는 건강증진 행위와 지각된 건강상태 간에 유의한 양의 상관관계가 있었고, 건강증진행위와 농부증 간에 유의한 음의 상관관계가 있었다. 건강문제와 지각된 건강상태 간에 유의한 음의 상관관계, 건강문제와 농부증과도 유의한 양의 상관관계가 있는 것으로 나타났다. 지각된 건강상태와 농부증간에 유의한 음의 상관관계가 있는 것으로 나타났다. 결론적으로 농촌주민의 다양한 건강문제와 농부증 발생률을 감소시키고 건강증진행위를 증진시키기 위해서는 지역 환경적인 요소를 고려한 농촌주민을 위한 맞춤형 건강증진프로그램의 개발 및 실시가 필요할 것으로 생각된다.
