역행충전시 수복재와 와동 형태에 따른 변연누출의 정량적 분석 (QUANTITATIVE ANALYSIS OF MARGINAL MICROLEAKAGE IN VARIOUS RETROGRADE FILLING MATERIALS AND PREPARATION TYPES)

  • When conventional root canal treatment is failed or contraindicated, retrograde root canal filling following apicoectomy is a valuable procedure, aimed at hermetically sealing the root canal against leakage of irritants from the canal into the periapical tissue. In this in vitro investigation, to analyze apical microleakage electrochemically in teeth with different retrograde filling materials and preparation types, single - rooted tooth was cut 2mm from the apex and each Class I and Slot preparation was prepared. Experimental groups : Group 1. Amalgam filling with cavity varnish in Class I preparation Group 2. Scotchbond 2+Silux filling in Class I preparation Group 3. Gutta percha filling with ZOE cement in Class I preparation Group 4. Amalgam filling with cavity varnish in Slot preparation Group 5. Scotchbond 2+Silux filling in Slot preparation Each specimens was immersed in 1% solution of KCl, and applied a potential of 9V external power supply. Measurements of the current flow were obtained at 1, 2, 3, 7, 9, 12, 14, 18, 21, 25 and 28 day after immerson. Marginal microleakage were compared and evaluated. The results were as follows ; 1. The group filled with composite resin with dentin bonding agent shows lower apical microleakage value than the group filled with amalgam following varnish application (P<0.01). 2. In the group filled with gutta percha, apical microleakage value was the hightest 3. There was no significant difference between Class I cavity and Slot type cavity regardless of the used materials.

근관충전용 글라스 아이오노머 시멘트의 근단 폐쇄성과 치질 적합성에 관한 연구 (EVALUATION OF APICAL MICROLEAKAGE AND ADAPTATION TO THE DENTINAL WALL OF THE GLASS-IONOMER ROOT CANAL CEMENT)

  • The purpose of this study was to compare the apical leakage and adaptation to the dentinal wall of a glass ionomer root canal cement(Ketac-Endo) with those of AH-26 and Tubli-Seal. 102 single-rooted teeth were insrumented with step-back technique and randomly divided into four groups according to kinds of root canal cements: Ketac-Endo, AH-26, Tubli-Seal, and no root canal cement. Four experimental groups were as follows; Group 1 : Filling with gutta-percha and Ketac-Endo Group 2 : Filling with gutta-percha and AH-26 Group 3 : Filling with gutta-percha and Tubli-Seal Group 4 : Filling with gutta-percha without root canal cement All the specimens were obturated with lateral condensation technique, and stored in 100% humidity for 4 days. 20 teeth in each group were placed in centrifuge tube filled with India ink, and then centrifuged for 20 minutes at 3,000 rpm. And they were evaluated for linear dye leakage using a stereoscope and analyzed by one-way ANOVA. 22 teeth were split longitudinally and evaluated the adaptation to the dentinal wall using scanning electron microscope(4 teeth in each group) and stereoscope(2 teeth in 1, 2, 3 group). The obtained results were as follows ; 1. In case of not used root canal cement(Group 4), there was showed much degree of dye penetration comparerd with used root canal cements (Group 1, 2, 3)(P<0.01). 2. In the case of used root canal cements(Group 1, 2, 3), there was no significant difference in linear leakage in groups(P>0.01). 3. In the case of used root canal cements(Group 1, 2, 3), there was showed intimate adaptation to the dentinal wall independent on kinds of root canal cements. 4. Fractured surfaces indicated failure of cohesion in Ketac-Endo, and failure of adhesion to dentine in AH-26 and Tubli-Seal. 5. According to similar apical leakage and adaptability to the dentinal wall to another root canal cements, it is possible to be used glass-ionomer cement as root canal cement.

  • The purpose of this study was to evaluate the microleakage electrochemically using different retrograde filling materials, different root resection angle, and different cavity preparation instruments. 104 extracted single-rooted teeth were selected for this study. 100 teeth were used as experimental groups and four was used as controls. Anatomical crowns were resected, root canals were prepared, and the apical 2 mm of roots were removed. The experimental roots were randomly divided into five equal groups. Experimental groups : Group 1. no bevel, cavity preparation with ultrasonic instruments, amalgam filling Group 2. no bevel, cavity preparation with ultrasonic instruments, SuperEBA cement filling Group 3. no bevel, cavity preparation with ultrasonic instruments, desiccated ZOE filling Group 4. $45^{\circ}$ bevel, cavity preparation with ultrasonic instruments, amalgam filling Group 5. no bevel, cavity preparation with conventional bur, amalgam filling Microleakage was measured once a day for 30 days using electrochemical method and were analyzed statistically. The results were as follows : 1. The group with Super EBA cement filling showed the least marginal leakage from second to fourth day(p<0.05), there was no significant difference between the group with amalgam filling during eighth to eighteenth day(p>0.05), but after the nineteenth day here was a higher marginal leakage than the group with amalgam filling(p<0.05). 2. The group with desiccated ZOE filling demostrated that the highest marginal leakage, started on the eighth day(p<0.05). 3. The group using ultrasonic instrument showed lower marginal leakage than the group using bur until the nineteenth day(p<0.05), but there was no significant differnce with the group using bur after twentythird day(p>0.05). 4. The group without bevel showed lower marginal leakage than the group with bevel (p<0.05). 5. Whether bevel or nor had much more effect on marginal leakage than with cavity perparation instrument when the cavity was retrogrdefilled with amalgam(stepwise regression).

A micro-computed tomographic evaluation of root canal filling with a single gutta-percha cone and calcium silicate sealer

  • Objectives: The purpose of this study was to evaluate the void of root canal filling over time when a calcium silicate sealer was used in the single gutta-percha cone technique. Materials and Methods: Twenty-four J-shaped simulated root canals and twenty-four palatal root canals from extracted human maxillary molars were instrumented with ProFile Ni-Ti rotary instruments up to size 35/0.06 or size 40/0.06, respectively. Half of the canals were filled with Endoseal MTA and the other half were with AH Plus Jet using the single gutta-percha cone technique. Immediately after and 4 weeks after the root canal filling, the samples were scanned using micro-computed tomography at a resolution of 12.8 ㎛. The scanned images were reconstructed using the NRecon software and the void percentages were calculated using the CTan software, and statistically analyzed by 1-way analysis of variance, paired t-test and Tukey post hoc test. Results: After 4 weeks, there were no significant changes in the void percentages at all levels in both material groups (p > 0.05), except at the apical level of the AH Plus Jet group (p < 0.05) in the simulated root canal showing more void percentage compared to other groups. Immediately after filling the extracted human root canals, the Endoseal MTA group showed significantly less void percentage compared to the AH Plus Jet group (p < 0.05). Conclusions: Under the limitations of this study, the Endoseal MTA does not seem to reduce the voids over time.

Prevalence of apical periodontitis and quality of root canal treatment in an adult Kuwaiti sub-population: a cross-sectional study

  • Objectives: This cross-sectional study evaluated the prevalence of apical periodontitis (AP) and the technical quality of root canal fillings in an adult Kuwaiti subpopulation using cone-beam computed tomography (CBCT) images. Materials and Methods: Two experienced examiners analyzed 250 CBCT images obtained from Kuwaiti patients aged 15-65 years who attended government dental specialist clinics between January 2019 and September 2020. The assessment followed the radiographic scoring criteria proposed by De Moor for periapical status and the technical quality of root canal filling. Chi-square and Fisher's exact tests were used for statistical analysis, with significance level set at p < 0.05. Results: Among the 2,762 examined teeth, 191 (6.91%) exhibited radiographic signs of AP, and 176 (6.37%) had undergone root canal filling. AP prevalence in root canal-treated teeth was 32.38%, with a significant difference between males and females. Most of the endodontically treated teeth exhibited adequate root canal filling (71.5%). Conclusions: The study demonstrated a comparable prevalence of AP and satisfactory execution of root canal treatment compared to similar studies in different countries.

Effectiveness and safety of rotary and reciprocating kinematics for retreatment of curved root canals: a systematic review of in vitro studies

  • Objectives: This systematic review (register-osf.io/wg7ba) compared the efficacy and safety of rotary and reciprocating kinematics in the removal of filling material from curved root canals. Materials and Methods: Only in vitro studies evaluating both kinematics during retreatment were included. A systematic search (PubMed/MEDLINE, Scopus, and other databases, until January 2021), data extraction, and risk of bias analysis (Joanna Briggs Institute checklist) were performed. Efficacy in filling removal was the primary outcome. Results: The search resulted in 2,795 studies, of which 15 were included. Efficacy was measured in terms of the remaining filling material and the time required for this. Nine studies evaluated filling material removal, of which 7 found no significant differences between rotary and reciprocating kinematics. Regarding the time for filling removal, 5 studies showed no difference between both kinematics, 2 studies showed faster results with rotary systems, and other 2 showed the opposite. No significant differences were found in apical transportation, centering ability, instrument failure, dentin removed and extruded debris. A low risk of bias was observed. Conclusions: This review suggests that the choice of rotary or reciprocating kinematics does not influence the efficacy of filling removal from curved root canals. Further studies are needed to compare the kinematics safety in curved root canals.

수종의 역충전 재료의 치근단 밀폐력 비교 (COMPARISON OF THE SEALING ABILITY OF VARIOUS RETROGRADE FILLING MAIERIALS)

  • This study was performed to evaluate the sealing ability of various retrograde filling materials by using bacterial penetration and dye penetration test. One hundred and forty extracted human teeth with single, straight canals and mature apiece were collected and used for this study. All canals were instrumented using an engine driven Ni-Ti file (ProFile). After removing 3mm from the apex of tooth, a standardized 3mm root end cavity was prepared using an ultrasonic instrument. The 70 teeth were randomly divided into 7 groups : 6 groups for retrograde filling using Super-EBA, ZOE, Chelon-Silver, IRM, ZPC and amalgam. The 7th group was used as a negative control. Nail varnish was applied to all external root surfaces to the level of the reseated root ends to prevent lateral microleakages. The specimens were then sterilized in an ethylene oxide sterilizer for 24 hours. 2 mm of the reseated root was immersed in a culture chamber containing a Tripticase Soy Broth with a phenol red indicator. The coronal access of each specimen was inoculated every 72 hours with suspension of Proteus vulgaris. The culture media were observed every 24hours for color change indicating bacterial contamination. The specimens were observed for 4weeks. The remaining 70 teeth were submitted to a dye penetration test. The canals of all teeth were first sealed with AH26 and obturated using an Obtura II system. Root resection, root end preparation and retrograde filling was performed as above. All specimens were suspended in 2% methylene blue dye for 72 hours before being ion gitudinally split. The degree of dye penetration was then measured using a stereomicroscope at 10 magnification and evaluated. The results were as floows : 1. In the bacterial penetration, the degree of leakage was the lowest in the Super-EBA, followed by, in ascending order, ZOE, Chelon-Silver IRM and ZPC. The amalgam showed highest bacterial leakage of all(p<0.01). 2. In the dye penetration, the degree of microleakage was the lowest in the Chelon-Silver and Super-EBA, followed by, in ascending order, IRM, ZPC. The ZOE and amalgam showed the highest microleakage of all (p<0.05). These results suggested that the eugenol based cement, Super-EBA, have excellent sealing ability as a retrograde filling material.

Effects of Root on Bulk Density of Soils Tested by Volume Check Apparatus through Water-filling

  • Soil bulk density is a key parameter for soil physical property. Much root placed in rhizosphere soil lump, especially in grassland and orchard, makes it difficult to measure soil bulk density. This experiment was carried out to countermeasure the above drawbacks. Volume check apparatus using water-filling method was made of acryl for higher accuracy in bulk density measurement. 10 types of land cover, including bare, tall fescue, rye, and soybean, were used for determining the relationships between root and bulk density. In this study, higher root volume resulted in higher differences in bulk density between in-situ core soil and root-ridded core soil, which indicated the volume check apparatus through water-filling could be useful for increasing the accuracy of bulk density of soils with much root.

만곡된 근관에서 Gutta-Percha 제거 시 스테인리스스틸 수기구와 회전식 NiTi 재치료용 기구의 효과 비교 (A Comparison of the Effectiveness of Stainless-Steel Hand Instrument and Rotary NiTi Retreatment Instrument to Remove Gutta-Percha in Curved Root Canals)

  • 비외과적 재근관치료의 주된 목표는 치근주변의 치유를 위하여 근관 공간을 완전히 멸균하는 것이며, 이 목표를 달성하기 위해서는 이전 충전물질의 완전한 제거가 요구된다. 전통적으로 근관충전물질의 제거는 스테인리스스틸 수기구를 통해서 이루어져 왔으며 이는 시간과 노력이 많이 소요되는 작업이다. 최근 보다 효율적인 근관충전물질의 제거를 위해 재치료용 회전식 NiTi 기구가 소개되었다. 이 연구에서는 만곡된 근관에서의 스테인리스스틸 수기구, ProTaper Universal Retreatment (Dentsply Maillefer, Ballaigues, Switzerland), Mtwo retreatment (Sweden & Martina, Padova, Italy)의 gutta-percha 제거 효율을 비교하였다. 재치료 후 근관 내 남겨진 gutta-percha의 양은 실험군 사이에 유의한 차이가 발견되지 않았다. 하지만 재치료에 소요된 시간은 재치료용 회전식 NiTi 기구를 이용한 실험군에서 더 적게 소요되는 것으로 나타났다.

분광광도법을 사용한 수종 역충전재의 근단부 폐쇄효과에 관한 연구 (SPECTROPHOTOMETRIC EVALUATION OF SEALING EFFECTS OF SEVERAL ROOT-END FILLING MATERIALS)

  • The purpose of this study is to evaluate the sealing effect of several root-end filling materials using spectrophotometric analysis. 180 single root teeth with one canal were instrumented and canal filled. Root resected and root end preparation was made. Teeth were randomly classified to S experimental group(MTA, EBA, IRM, TCP, ZOE) and 1 control group according to root-end filling material MTA group used PRO ROOT MT A, EBA group used Super EBA. TCP group used NEW APATITE LINER TYPE II main component of which is ${\alpha}-tricalcium$ phosphate (TCP). According to manufacture's instruction experimental material was mixed and retrfilled. After 2% methylene blue solution penetration absorbance for each test sample was measured with spectrophotometer (JASCO UV-530, Japan). The mean absorbance of control and experimental group was as follows; MTA: 0.092, IRM: 0.226, Super EBA: 0.255, ZOE: 0.374, Control: 0.425, TCP: 0.501 and the result analyzed by Turkey test at P=0.05 level. Conclusions of this study are as follows; 1. The absorbance increase in following sequence MTA, IRM, Super EBA, ZOE, Control. TCP. 2. MTA showed the least leakage but was not significant with IRM or Super EBA and was significant with control or TCP(p<0.05). 3. TCP had the most leakage and was not significant with control group.
