Factors Related to Suicidal Ideation and Depression according to Gender among Korean Adults (성인의 성별에 따른 자살생각과 우울경험 관련요인)

  • Purpose: This study was conducted in order to identify factors that predict suicidal ideation and depression according to gender among Korean adults. Method: From the data base of the fifth Korean National Health and Nutritional Examination Survey(KNHANESV-1, 2010), data on 4,894 adults aged 19~64 were used. Data were analyzed by application of the Rao-Scott Chi-Square test and logistic analysis with SAS 9.2. Results: Overall, 8.5% of men and 18.1% of women had suicidal ideation during the last year; 7.5% of men and 17.0% of women had depression during the last two weeks. Risk factors for suicidal ideation include perceived high stress(OR male 3.649, female 2.941), depression(OR male 10.109, female 6.033), poor cognition of health status(OR male 8.115, female 6.664), low economic status(OR male 1.898), and low educational status(OR female 1.631). Risk factors for depression include perceived high stress(OR male 2.678, female 3.686), suicidal ideation(OR male 9.925, female 6.022), non-employed(OR female 1.525), married(OR female 2.392), and physical discomfort(OR female 1.545). Conclusions: Based on the above mentioned results, risk factors of suicidal ideation and depression differed according to gender. Development of various policies and appropriate intervention programs according to gender, that lessen the risk of suicidal ideation and depression for adults is needed.

Angiographic Hemorrhagic Risk Factors of Cerebral Arteriovenous Malformations (뇌동정맥기형의 혈관조영 검사상 출혈위험 인자)

  • Objective : The authors reviewed 280 cases with intracranial arteriovenous malformations(AVMs) to identify risk factors of hemorrhage. Patients and Methods : From 1983 to 1997, a total 280 patients with AVMs were treated. Among them, 64% had a history of hemorrhage. Angiograms were retrospectively analyzed with particular attention to the size of the AVM, venous drainage, the location of the AVM and presence of associated aneurysm or varix. These characteristics were statistically analysed in relation to occurrence of hemorrhage. Results : A single variate analysis demonstrated that small size(p=0.0003), deep venous drainage(p=0.025) and periventricular location(p<0.0001) had a strong positive correlation. Associated aneurysms and varices were not found as hemorrhagic risk factors. A multivariate analysis revealed that the size of the AVM was most significant hemorrhagic factor(p=0.0003) followed by deep venous drainage(p=0.025). AVMs with small size and deep venous drainage bled more frequently regardless of their locations. Conclusion : These data would be useful in identifying patients at higher risk for developing hemorrhage of intracranial AVMs.

Scoring System and Management Algorithm Assessing the Role of Survivin Expression in Predicting Progressivity of HPV Infections in Precancerous Cervical Lesions

  • Background: To identify the risk factors and assess the role of survivin in predicting progessivity precancerous cervical lesions. Materials and Methods: This case-control study was conducted from October 2009 until May 2010. We obtained 74 samples, classified according to the degree of cervical intraepithelial neoplasia (CIN): 19 samples for CIN 1, 18 samples for CIN 2, 18 samples for CIN 3, and 19 samples as controls. Demographic profiles and risk factors assesment, histopathologic examination, HPV DNA tests, immunocytochemistry (ICC) and immunohistochemistry (IHC) staining for survivin expression were performed on all samples. Data was analyzed with bivariate and multivariate analysis. Results: Multivariate analysis revealed significant risk factors for developing precancerous cervical lesions are age <41 years, women with ${\geq}2$ sexual partners, course of education ${\geq}13$ years, use of oral contraceptives, positive high-risk HPV DNA, and high survivin expression by ICC or IHC staining. These factors were fit to a prediction model and we obtained a scoring system to predict the progressivity of CIN lesions. Conclusions: Determination of survivin expression by immunocytochemistry staining, along with other significant risk factors, can be used in a scoring system to predict the progressivity of CIN lesions. Application of this scoring system may be beneficial in determining the action of therapy towards the patient.

Risk Factors of Colon Polyps in Colonoscopy Examinee (대장내시경 피검진자의 대장용종 위험요인 분석)

  • Purpose: The purpose of this study was to identify the prevalence and risk factors of colon polyps. Methods: The subjects were 180 adults enrolled from examinees who underwent colonoscopy at the health promotion center and OPD in Ulsan University Hospital. The risk factors were collected by a self-reporting standardized questionnaire and medical records. Data analysis involved chi-square test, and multiple logistic regression. Results: The prevalence of colon polyps was 41.6%. The risk factors for colon polyps were significantly increased by the cases of subjects more than 50 years old (OR, 9.213; CI, 3.291-25.789), subjects who didn't have any regular high-intensity exercise (OR, 4.762; CI, 1.387-16.351), subjects whose intake of fruits was less than once per a week (OR, 5.938; CI, 1.069-32.985) and subjects who showed irregular defecation (OR, 3.066; CI, 1.178-7.980). Conclusion: This study suggests that eating habits and proper exercise, which promote a normal habit of defecation so as to be helpful in preventing colon polyps.

Comparison of Prevalence for Osteoarthritis and Its Risk Factors between Age 60-74 and 75 and Over (한국 전기노인과 후기노인의 골관절염 위험요인 비교)

  • Purpose: The purpose of this study was to identify the prevalence of osteoarthritis and its risk factors among a community dwelling population age 60-74 and 75 and over (young-old vs. old-old) in Korea. Methods: A total of 1,389 of age 60-74 and 397 of age 75 and over from the 2010 Korean National Health and Nutrition Examination Survey (KNHANES) were selected. The data analysis was performed using the complex sampling function of SPSS version 19.0. Results: The prevalence of osteoarthritis was greater for old-old people (35.50%) compared to young-old people (18.20%). Univariate logistic regression analysis shows that the difference of risk factors for osteoarthritis between young-old and old-old elderly people. The prevalence of osteoarthritis in young-old people was associated with lower education levels; performance of moderate intensity physical activity; higher waist circumference, whereas old-old people was most affected in females; depression and experience. Conclusion: The finding of differences in the prevalence of osteoarthritis and its' risk factors between young-old and old-old people is expected to promote the screening strategy for the aged at risk of osteoarthritis in the Korean community.

The Impact of Patient Satisfaction With Nursing Care Services, Switching Costs and Perceived Risk on Intention of Reuse in the Emergency Medical Center (응급의료센터 내원환자의 간호서비스 만족도, 전환비용 및 지각된 위험이 재이용 의도에 미치는 영향)

  • Purpose: The purpose of this study was to identify the factors influencing the intention of the reuse in patients admitted in university hospital emergency medical center. Method: The participants were 253 patients admitted to a niversity hospital emergency medical center. Data were collected with self-administrated questionnaires and analyzed by hierarchical multiple regression. Results: Patient satisfaction with nursing care service and switching cost were positively correlated with reuse by patients while the perceived risk was negatively correlated. As levels of satisfaction with nursing care services and switching cost increase, intention of reuse increases. Satisfaction with nursing care service, switching cost and perceived risk in emergency medical center influence intention to reuse and explain 68.8% of total variation of intention to reuse. Conclusion: Findings provide strong empirical evidence for importance of atient satisfaction with nursing care service, the switching costs and the perceived risk in explaining the intention of reuse an emergency medical center.

Suicidal Ideation and Risk Factors among the Elderly in Korea (노인의 자살 생각과 위험요인)

  • Purpose: This study aimed to identify the incidence and risk factors of suicidal ideation among the elderly in Korea. Methods: This cross-sectional study used secondary data of the 2005 Korean National Health and Nutrition Examination Survey. The participants were 1,097 elderly Koreans who responded to the health survey. Sociodemographics, health related characteristics, and the prevalence of suicidal ideation were obtained through face-to-face interviews. Risk factors of the suicidal ideation were also examined. Descriptive statistics and multivariate logistic regression were used for data analysis. Results: Of the 1,097 respondents, 30.5% had suicidal ideation during the past year. Depression and stress were the important risk factors; others included mobility, pain/discomfort, chronic illness, smoking, sleep, and leisure time physical activity. Sociodemographic characteristics such as gender, education, and marital status affected suicidal ideation. Conclusion: The high frequency of suicidal ideation in the elderly suggests the necessity of community-based interventions directed at depression and stress.

Risk Factors Associated with Vitamin D Deficiency among Women Office Workers (여성 사무직 근로자의 혈중 비타민 D 결핍에 영향을 미치는 위험요인)

  • Purpose: Vitamin D is a fundamental element for bone metabolism. Recently vitamin D deficiency has been implicated in various diseases such as a cardiovascular disease, diabetes, and cancers. The aim of this study was to identify the risk factors associated with serum vitamin D deficiency among women office workers. Methods: We selected 369 women office workers using the secondary data of the 5th National Health & Nutrition Examination Survey 2010-2012. Data was analyzed by logistic regression of complex sampling design. Results: Women office workers with vitamin D deficiency, defined serum 25-hydroxyvitamin D concentration < 10ng/mL, were 12.5%. The risk factors for vitamin D deficiency were 20s aged group, married state and more than 40 working hours a week. The risk of vitamin D deficiency was decreased in those with alcohol drinking 1 to 4 times a month. The education level, income, region, smoking, physical activity and sun exposure time did not affect the risk of vitamin D deficiency significantly. Conclusion: Development of vitamin D deficiency prevention educational programs are required for women office workers who more than 40 hours a week in 20s. It should be considered health education including sun exposure duration and behavior.

Metabolic Syndrome Risk Factors related to Severity of Coronary Artery Diseases in Patients with Acute Myocardial Infarction (한국인 급성 심근경색증 환자의 관상동맥 중증도에 영향을 미치는 대사증후군 위험요인)

  • Purpose: This study was conducted to identify the clinical characteristics and risk factors on the occurrence of metabolic syndrome (MS), and to examine factors affecting the severity of coronary artery diseases in patients with acute myocardial infarction (AMI). Methods: A total of 894 patients who had admitted C national university hospital from 2008 to 2010 participated in this study. Collected data were lipid profiles, abdominal circumference, blood pressure, fasting blood sugar (FBS) level, participants' demographic data and other risk factors by interview, measurement, and review of participants' medical records. MS was defined according to modified National Cholesterol Education Program Adult Treatment Panel III and Asia-Pacific Criteria. Results: The participants' mean age was 64.7 (${\pm}11.0$) years and 65% was male patients. The participants' with MS was 37.6% in men and 71.4% in women. According to binary logistic regression analysis, high FBS (95% CI 1.7-2.0) and lower high-density lipoprotein (HDL) cholesterol (95% CI 1.1-1.9) were independent predictors of severe coronary artery disease. Conclusion: These risk factors of severe coronary artery disease will be utilized as an important basic data in part of management, education, and countermeasure of patients with both MS and AMI.

Breastfeeding and the Risk of Childhood Hodgkin Lymphoma: A Systematic Review and Meta-analysis

  • Purpose: Numerous observational epidemiological studies have evaluated associations between breastfeeding and the risk of childhood Hodgkin lymphoma; however, the existing results are inconsistent. We therefore conducted a systematic review and meta-analysis. Methods: Medical literature was searched in the Pubmed and Embase databases to identify all English-language relevant studies up to April 10, 2013. Reference lists were thereafter hand-searched for additional articles. Studies that reported relative risk ratios (RRs) or odds ratios (ORs) with 95% confidence intervals (CIs) were included. This meta-analysis was conducted in accordance with the guidelines for the meta-analysis of observational studies in epidemiology. Results: We finally included 10 case-control studies in our meta-analysis, involving 1,618 childhood Hodgkin lymphoma cases and 8,181 controls. Overall, we did found a borderline significant association between breastfeeding and reduced risk of childhood Hodgkin lymphoma comparing ever breastfed children to never breastfed children (pooled OR =0.79; 95%CI, 0.58-1.08; P=0.13), with limited evidence for between-study heterogeneity (P =0.12, $I^2$ = 35.70%). Conclusion: There is limited evidence for an inverse association between breastfeeding and risk of childhood Hodgkin lymphoma.
