Displacement pattern of the anterior segment using antero-posterior lingual retractor combined with a palatal plate

  • Objective: To evaluate and compare the effects of two appliances on the en masse retraction of the anterior teeth anchored by temporary skeletal anchorage devices (TSADs). Methods: The sample comprised 46 nongrowing hyperdivergent adult patients who planned to undergo upper first premolar extraction using lingual retractors. They were divided into three groups, based on the lingual appliance used: the C-lingual retractor (CLR) group (group 1, n = 16) and two antero-posterior lingual retractor (APLR) groups (n = 30, groups 2 and 3). The APLR group was divided by the posterior tube angulation; posterior tube parallel to the occlusal plane (group 2, n = 15) and distally tipped tube (group 3, n = 15). A retrospective clinical investigation of the skeletal, dental, and soft tissue relationships was performed using lateral cephalometric radiographs obtained pretreatment and post en masse retraction of the anterior teeth. Results: All groups achieved significant incisor and canine retraction. The upper posterior teeth did not drift significantly during the retraction period. The APLR group had less angulation change in the anterior dentition, compared to the CLR group. By changing the tube angulation in the APLR, the intrusive force significantly increased in the distally tipped tube of group 3 patients and remarkably reduced the occlusal plane angle. Conclusions: Compared to the CLR, the APLR provides better anterior torque control and canine tipping while achieving bodily translation. Furthermore, changing the tube angulation will affect the amount of incisor intrusion, even in patients with similar palatal vault depth, without the need for additional TSADs.

Retrospective study of single vitrified-warmed blastocyst transfer cycles according to the presence of morphokinetic variables

  • This study retrospectively assessed whether time-lapse data relating to developmental timing and morphology were associated with clinical outcomes, with the eventual goal of using morphokinetic variables to select embryos prospectively for cryopreservation. In this study, we examined the clinical outcomes of single vitrified-warmed blastocyst transfer cycles that were cultured in a time-lapse incubation system. The morphokinetic variables included uneven pronuclei, an uneven blastomere, multinucleation, and direct, rapid, and irregular division. A total of 164 single vitrified-warmed blastocyst transfer cycles were analyzed (102 cycles of regularly developed blastocysts and 62 cycles of blastocysts with morphokinetic variables). No significant differences in the age of females or the standard blastocyst morphology were found between these two groups. The regularly developed blastocysts showed significantly higher implantation and clinical pregnancy rates than the blastocysts exhibiting morphokinetic variables (30.4% vs. 9.7% and 37.3% vs. 14.5%, respectively; p< 0.01). The blastocysts that exhibited morphokinetic variables showed different mean development times compared with the regularly developed blastocysts. Although morphokinetic variables are known to have fatal impacts on embryonic development, a considerable number of embryos developed to the blastocyst stage. Morphokinetic variables had negative effects on the implantation and clinical pregnancy rates in vitrified-warmed blastocyst transfer cycles. These findings suggest that blastocysts cultured in a time-lapse incubation system should be considered for selective cryopreservation according to morphokinetic variables.

Related Factors and their Effects on Acute Renal Failure Resulting from Rhabdomyolysis after Pesticide Intoxication (횡문근 융해증이 발생한 농약중독 환자에서 급성 신부전 발생의 연관요인)

  • Purpose: Rhabdomyolysis is one of the most important complications of pesticide intoxication. It affects a patient's clinical prognosis and can cause acute renal failure. It is important that patients diagnosed with pesticide intoxication receive an accurate initial diagnosis and proper treatment to prevent significant complications. This study's objective was to investigate and confirm related factors causing acute renal failure by verifying clinical observations and laboratory findings collected following pesticide intoxication. Methods: A retrospective analysis was made of 734 patients who presenting to our emergency medical center after ingesting pesticides between January 2006 and December 2008, Of these, 513 patients were selected for the study. Two hundred and twenty-one patients were excluded because of paraquat intoxication, age (if under 18), or chronic renal failure. Seventy-four patients were diagnosed with rhabdomyolysis, based on serum creatinine phosphokinase levels were > 1,000U/L. Acute renal failure was diagnosed when creatinine levels were > 2.0 mg/dL. Results: Among the 74 patients diagnosed with rhabdomyolysis, 26 (35.1%) experienced acute renal failure. The most meaningful related factor in the prediction of acute renal failure was initial arterial pH and creatinine level. Conclusion: Initial arterial pH and creatinine level are predictors of complications such as acute renal failure in patients with rhabdomyolysis.

A Clinical Survey of 9 dogs with Naturally Occurring Hypoadrenocorticism (자연발생 부신겉질저하증에 걸린 9마리 개에 대한 고찰)

  • A retrospective study was performed with 9 cases of spontaneous canine hypoadrenocorticism presented to the Veterinary Medical Teaching Hospital of Seoul National University. The disease occurred mostly in young to middle­aged dogs. Most dogs had chronic nonspecific signs such as episodic anorexia, lethargy and vomiting, but 3 dogs were brought to the hospital with an acute adrenal crisis. Serum biochemical analysis revealed azotemia, abnormal concentration of sodium, potassium, and chloride in almost all the patients. Diagnoses were made based on lack of adrenocortical response to exogenous adrenocorticotropic hormone(ACTH). Most dogs have been responded well with mineralocorticoid and glucocorticoid replacement except for a dog which died during treatment for adrenal crisis. Three dogs treated with fludrocortisone acetate(final median dosage, $31.0{\mu}g/kg/day$) for mineralocorticoid supplementation showed some adverse effects, such as excessive appetite and polyuria/polydipsia. In this study, it was suggested that clinicians should also consider hypoadrenocorticism when they diagnose a patient showing nonspecific gastrointestinal signs with unknown cause or laboratory findings which mimic primary renal diseases.

Case Series Study about the Effect of Jageum-Jung on Atopic Dermatitis (자금정의 아토피피부염 치료 효과에 대한 환자군 연구)

  • Objectives The purpose of this study is to analyze the clinical data of atopic dermatitis patients and to report the therapeutic effect of Jageum-Jung. Methods We conducted a retrospective study on the effects of treatment on atopic dermatitis patients who took Jageum-Jung from August 1, 2017 to December 20, 2017. The patients visited three times during the treatment period. The SCORAD Index score was measured and analyzed. Results Atopic dermatitis patients were 2 adults and 5 children. Both 2 adults were male (100%). Among the children, 1 child was a male (20%) and 4 children were females (80%). For adults, average age was 29.5 years, average weight was 72.75 kg, average height was 1.76 m and average BMI was $23.49kg/m^2$. For children, average age was 6.2 years, average weight was 23.92 kg, average height was 1.2 m and average BMI was $16.08kg/m^2$. The atopic dermatitis patients' average SCORAD Index score was 18.04 at first visit, 11.46 at second visit, and 7.64 at third visit. The scores lowered significantly after every visit. Conclusions The SCORAD Index in atopic dermatitis patients who took Jageum-Jung significantly decreased and it is expected that Jageum-Jung will be clinically effective in the treatment of atopic dermatitis.

Prediction of nonresponsiveness to mediumdose intravenous immunoglobulin (1 g/kg) treatment: an effective and safe schedule of acute treatment for Kawasaki disease

  • Purpose: Medium-dose (1 g/kg) intravenous immunoglobulin (IVIG) is effective in the majority of patients with Kawasaki disease (KD) but some patients who do not respond to medium-dose IVIG are at high risk for the development of coronary artery lesions (CALs). The purpose of this study was to identify the clinical predictors associated with unresponsiveness to medium-dose IVIG and the development of CALs. Methods: A retrospective study was performed in 91 children with KD who were treated with mediumdose IVIG at our institution from January 2004 to December 2013. We classified the patients into responders (group 1; n=68) and nonresponders (group 2; n=23). We compared demographic, laboratory, and echocardiographic data between the 2 groups. Results: Multivariate logistic regression analysis identified 6 variables as predictors for resistance to medium-dose IVIG. We generated a predictive scoring system assigning 1 point each for percentage of neutrophils ${\geq}65%$, C-reactive protein ${\geq}100mg/L$, aspartate aminotransferase ${\geq}100IU/L$, and alanine aminotransferase ${\geq}100IU/L$, as well as 2 points for less than 5 days of illness, and serum sodium level ${\leq}136mmol/L$. Using a cutoff point of ${\geq}4$ with this scoring system, we could predict nonresponsiveness to medium-dose IVIG with 74% sensitivity and 71% specificity. Conclusion: If a patient has a low-risk score in this system, medium-dose IVIG can be recommended as the initial treatment. Through this process, we can minimize the adverse effects of high-dose IVIG and incidence of CALs.

A Retrospective Statistical Analysis of Miniscalpel Needle Therapy for Herniated Intervertebral Disc or Spinal Stenosis

  • Background: This study examined the characteristics and prognosis of patients admitted to the Dunsan Korean medicine hospital for treatment of herniated intervertebral disc (HIVD) or spinal stenosis with Miniscalpel needle therapy (MSN). Methods: Patients were admitted to the Dunsan Korean medicine hospital from January 01, 2016 to September 30, 2017 for the treatment of HIVD or spinal stenosis with MSN. Crossover analysis, Independent sample t test, one-way ANOVA, multiple linear regression analysis, and binary logistic regression analysis were performed. Results: Crossover analysis showed statistically significant differences in treatment methods according to gender, current pain according to the disease duration, satisfaction of MSN according to disease duration, treatment methods, and intention of re-treatment with MSN according to treatment methods. Independent t test and one-way ANOVA showed that there was a difference in current Numeric Rating Scale (NRS) according to disease duration, and difference between discharge and current NRS, and number of MSN according to disease. Multiple linear regression analysis showed that age, disease duration, and number of MSN affect discharge NRS, disease duration, and number of MSN affect current NRS, and Western medical treatment after MSN, discharge NRS, and current NRS affect satisfaction of MSN. Binary logistic regression analysis showed that discharge NRS affects current pain, and gender, discharge NRS, and treatment methods affect intention of re-treatment with MSN. Conclusion: Characteristics, prognosis, satisfaction and variables affecting prognosis of MSN were statistically significant, indicating that more systematic studies are required to further examine the effects of MSN on HIVD or spinal stenosis.

Clinical Outcomes of Antiplatelets Combined with Statins in Patients with Ischemic Heart Disease (허혈성 심장질환 환자에서 스타틴과 병용하는 항혈소판제 종류에 따른 임상결과)

  • Background: Patients with cardiovascular risks are recommended to use statins and antiplatelet agents to prevent major cerebro-cardiovascular events (MACCE). Antiplatelet agents also possess anti-inflammatory and antioxidant effects, in addition to their inhibitory activity on platelets. The differences in clinical outcomes in ischemic heart disease (IHD) based on the type of antiplatelet therapy combined with statin treatment were investigated in this study. Methods: We conducted a retrospective cohort study using electronic medical records of IHD patients from January 2010 to December 2014 at Ajou University Hospital. Patients on combination therapy of antiplatelet drugs and statins were grouped based on antiplatelet drug types: clopidogrel, cilostazol, or sarpogrelate. Propensity score matching was applied to balance the baseline of the groups of clopidogrel vs. cilostazol and the groups of clopidogrel vs. sarpogrelate. The incidence and risk of MACCE as primary outcomes were assessed between the groups of antiplatelet drugs. Results: Among the approximately 128,500 patients with IHD, 1,049 patients had taken a combination therapy of statin and antiplatelet agents. The cohorts of patients administered clopidogrel, cilostazol, or sarpogrelate were 906, 79, and 64, respectively. The incidence of MACCE was not significantly different among the cohorts (p=0.58), and there were no differences between clopidogrel vs. cilostazol (p=0.72) or clopidogrel vs. sarpogrelate (p=1.00) after propensity score matching. Conclusion: There was no difference in the incidence of MACCE based on the type of antiplatelet drug (clopidogrel, cilostazol, or sarpogrelate) in combination with a statin in patients with IHD.

The Retrospective Study on Modified Fasting Therapy in Inpatients (절식요법을 시행한 입원환자의 후향적 연구)

  • Objectives : The purpose of this study was to investigate the effects of modified fasting therapy on body composition, blood cell count and blood chemistry in inpatients. Methods : The modified fasting therapy program consists of three phases, a period of reducing food intakes, fasting and refeeding. Body weight, body composition, and the degree of obesity were estimated in before fasting therapy and after refeeding. Blood cell count and blood chemistry were estimated in before fasting therapy, before refeeding and after refeeding. Results : After modified fasting therapy, body weight, body fat mass, percent body fat, body mass index(BMI), body matabolic rate(BMR) and muscle mass was decreased significantly. WBC count and platelet count decreased significantly, RBC count and hemoglobin increased significantly and hematocrit was not changed significantly but within normal limit. AST and ALT increased significantly, especially patients group over BMI 25 within normal liver function. This study was not founded correlation between decreased muscle mass and increased liver function test. ALP, BUN, Total-cholesterol, and triglyceride decreased significantly. Creatinine increased significantly, but within normal limit. Sodium was not changed significantly, potassium decreased significantly but within normal limit. Conclusions : These results suggest that modified fasting therapy will be beneficial if several measures complement.

Factors Affecting the Postoperative Pain and Length of Hospital Stay of Liver Transplantation Donors (간이식 공여자의 수술 후 통증 특성 및 재원기간에 대한 영향 요인)

  • Purpose: This study was performed to investigate the factors affecting postoperative pain and length of hospital stay of liver transplantation donors. Methods: This is a retrospective study using the Electronic Medical Records (EMR) of 91 patients operated on at a tertiary hospital in Seoul, Korea in 2016. The collected data were analyzed using descriptive statistics, t-test, Mann-Whitney U test and Kruskal-Wallis test, Spearman's rank correlation, and multiple regression analysis. Results: The average age of the donors was $35.7{\pm}12.2$ years, and all donors were family members. PCA was applied for control pain in all patients, and 40.7% of PCA-related side effects were observed. The average length of hospital stay was $9.24{\pm}2.52$ days. The factors influencing the length of hospital stay were operative methods, pain control methods, and postoperative complications. The length of hospital stay was 1.29 days shorter if donors had no complication, 1.43 days shorter when only PCA was used, and 1.19 days shorter when laparoscopic resection was performed (Adjusted $R^2=0.17$, F=4.67, p<.05). Conclusion: The results of this study can be used as basic data for practical and effective postoperative nursing education and intervention of living liver donors.
